Authors analysed 19 cases with Cavernous Hemangiomas of Skeletal Muscles in Extremities. Those were confirmed clinicopathologically for the period of 2 years from October 1976 to September 1978, and following results were obtained. 1. The age of initial visit varied from 6 months to 42 years of age with the average of 17.7 years. Seventeen patients(89.5%) were under 30 years of age. 2. The age of initial onset of symptoms varied from birth to 36 years of age with the average of 13.1 years. Seventeen patients(89.5%) were under 20 years of age. 3. The duration of disease varied from 6 months to 17 years with the average of 4.6 years. 4. The ratio of male and female was 9:10. 5. Chief complaints were a swelling or mass in 17 cases(89.5%), pain in 13 cases(68.4%), tenderness in 6 cases(31.6%), and impairment of function in 5 cases(26.3%). 6 The size of a mass varied from a nut size to an adult fist one, and its shape appeared oval with the average 8.1 by 4.6 cm in 12 cases, and round with the average 5.5 cm in 7 cases. 7. The site of involvement distributed to lower extremities in 10 cases(52.7%), upper ones in 9 cases(47.4%). Among extremities(thigh, leg, arm and forearm) the most common site was forearm. 8. Considering the number of involed muscles, most frequently they developed in a single muscle with 11 cases(57.9%). 9. By history, The symptoms were initiated or aggravated by trauma in 3 cases(15.8%). 10. Plain roentgenograms showed the swelling of soft tissue in 9 cases(47.4%) and phleboliths in 8 cases(42.1%). 11. Considering the methods of treatment, complete resection was possible in 17 cases(85.7%), in the remaining 2 cases only incomplete resection could be done.

PURPOSE: WHO (World Health Office) and WADEM (World Association of Disaster Emergency Medicine)have recommended that all countries should complete their preparedness for CBRNE disasters (chemical, biologic, radiological, nuclear, and explosive disaster) within a few years. To establish whether a biologic or chemical disaster plan is most appropriate in a given situation, essential information regarding biologic materials and chemicals should be provided to emergency personnel as rapidly as possible. METHODS: We prospectively investigated information systems operated by government bodies in Korea. We obtained data about toxic material information systems of Korea by contacting individuals in charge of toxic material information systems at seven government agencies between April and July 2007. RESULTS: There was no single toxic information system designated as a repository for all toxic material data in Korea. Instead, data were distributed among 9 institutes in 6 government cabinets. Moreover, redundant data were independently stored at four different cabinets or ministries the Ministry of Commerce, Industry and Energy; the Ministry of Labor; the National Emergency Management Agency; and the Ministry of Environment. Because each ministry or cabinet handled toxic material information system according to its own laws or legal guidelines, there were effectively eight sets of rules for toxic materials. CONCLUSION: Nine institutes in six government bodies were found to be operating computerized information systems for toxic materials independently, and four government bodies had redundant chemical information. In order to establish a central toxic material information center, a uniform set of guidelines for toxic materials should be established. Doing so, would result in financial savings, and improve the quality of information.

PURPOSE: We describe clinical assessment of visual internal urethrotomy (VIU) as primary treatment modality in 69 men who presented with complete or partial obstructed urethral stricture. MATERIALS AND METHODS: The 63 patients with partial obstructed urethral stricture were treated by typical VIU. The 6 men with complete obstructed urethral stricture were treated by VIU using sound passed through previously placed suprapubic tract as guide. RESULTS: The procedure was completed successfully in 57 of the 63 men with partial obstructed (90.5%) and 3 of 6 patients with complete obstructed urethral stricture (50%). CONCLUSIONS: VIU is a safe and effective as primary treatment plan when urethral stricture is partial obstructed and the impassable urethral segment is shorter than 1.0cm.

Purpose: The accessibility of medical facilities for cancer patients affects both their comfort and survival. Patients in rural areas have a higher socioeconomic burden and are more vulnerable to emergency situations than urban dwellers. This study examined the feasibility and effectiveness of a cancer care model integrating a regional cancer center (RCC) and public health center (PHC). Methods: This study analyzed the construction of a safety care network for cancer patients that integrated an RCC and PHC. Two public health institutions (an RCC in Gyeongnam and a PHC in Geochang County) collaborated on the development of the community care model. The study lasted 13 months beginning in February 2019 to February 2020. Results: The RCC developed the protocol for evaluating and measuring 27 cancer-related symptoms, conducted education for PHC nurses, and administered case counseling. The staff at the PHC registered, evaluated, and routinely monitored patients through home visits. A smartphone application and regular video conferences were incorporated to facilitate mutual communication. In total, 177 patients (mean age: 70.9 years; men: 59%) were enrolled from February 2019 to February 2020. Patients’ greatest unmet need was the presence of a nearby cancer treatment hospital (83%). In total, 28 (33%) and 44 (52%) participants answered that the care model was very helpful or helpful, respectively. Conclusion We confirmed that a combined RCC-PHC program for cancer patients in rural areas is feasible and can bring satisfaction to patients as a safety care network. This program could mitigate health inequalities caused by accessibility issues.

Limbic encephalitis is a rare paraneoplastic syndrome characterized by prominent memory impairment, affective and behavioral disturbances, and progressive intellectual deterioration. Among many different neoplasm known to cause limbic encephalitis, a small cell lung cancer is certainly the most frequently reported. We experienced a 64-years old male patient presented with clinical features consistent with limbic encephalitis. He was subsequently found to have small cell lung cancer and showed characteristic evolutional MRI features ; increased signal intensity on T2 weighted image in the bilateral medial temporal regions during early stage and progressive severe atrophy in the late stage.

Purpose: This study was performed to investigate the association of high age-adjusted shock index (AASI) with mortality in Korean children with trauma. Methods: The data of children (aged < 15 years) with trauma who visited an university hospital in Korea from 2010 through 2018 were reviewed. High AASI was defined by age groups as follows: < 12 months, ≥ 2.7; 12-23 months, ≥ 2.1; 2-4 years, ≥ 1.9; 5-11 years, ≥ 1.5; and 12-14 years, ≥ 1.1. Age, sex, transfer status, injury mechanism, hypotension, tachycardia, base deficit, hemoglobin concentration, trauma scores, hemorrhage-related procedures (transfusion and surgical interventions), and severe traumatic brain injury were compared according to high AASI and in-hospital mortality. The association of high AASI with the mortality was analyzed using logistic regression. Results: Of the 363 enrolled children, 29 (8.0%) had high AASI and 24 (6.6%) died. The children with high AASI showed worse trauma scores and underwent hemorrhage-related procedures more frequently, without a difference in the rate of the traumatic brain injury. High AASI was associated with in-hospital mortality (survivors, 6.5% vs. non-survivors, 29.2%; P = 0.001). This association remained significant after adjustment (adjusted odds ratio, 6.42; 95% confidence interval, 1.38-29.82). The other predictors were Glasgow Coma Scale (for increment of 1 point; 0.62; 0.53-0.72) and age (for increment of 1 year; 0.84; 0.73-0.97). High AASI showed a 29.2% sensitivity and 93.5% specificity for the mortality. Conclusion High AASI is associated with mortality, and have a high specificity but low sensitivity in Korean children with trauma. This predictor of mortality can be used prior to obtaining the results of laboratory markers of shock.

Various options have been introduced for the selection of implant overdenture attachments. Attachment wear due to the repeated insertion and removal of dentures has caused problems such as decreased retention and the requirement for suprastructure remanufacturing. In these cases, a Locator bar system was applied using the drill and tapping technique to achieve total retrievability. In a 55-year-old female patient who showed three degrees of mobility in most of her teeth due to severe alveolar bone loss, a complete denture in the maxilla and an implant supported type overdenture in the mandible were planned after extracting all the remaining teeth. Six implants were placed from canine region to the distal molar region, and the locator was connected to the milled bar using the drill and tapping technique. For a 61-year-old female edentulous patient who complained of poor retention with old denture, a complete denture in the maxilla and an implant-tissue supported type overdenture in the mandible were planned. Four implants were placed in front of mental foramen, and the Locator was also connected to the Hader bar using the drill and tapping technique. With this technique, female parts can be easily replaced, and retention can be continuously maintained.
