1.A case of Idiopathic Neonatal Chylothorax Treated with Thoracostomy and Medium Chain Triglyceride - Containing Diet.

Chylothorax is the most common cause of pleural effusion in the neonatal period and is defined as an effusion of lymph in the pleural cavity. We report a case of chylothorax in a 13-day-old male who was admitted due to respiratory difficulty. Chest AP showed pleural effusion of the left lung and milky yellow fluid was aspirated via thoracentesis upon which laboratory and lipoprotein electrophoresis of pleural fluid revealed findings compatible with that of chylothorax. Due to reaccumulation of chyle after daily thoracentesis, chest tube was inserted at the 4th hospital day, and MCT containing diet was given. He was discharged on the 21st hospital day in good health, and revealed no recurrence for 3 months during outpatient follow up visits.

2.Self-esteem and Social Support as a factor for Suicidal Tendency for College Students in Pusan.

BACKGROUND: Suicide is a self-destructive behavior that can occur throughout life. Recently the suicide rate has gradually increased in Korea. Suicide is the third most common cause of death in adolescents. The most important factOrs closely related to suicide were depression and hopelessness. This study aimed to analyze first, relationships between suicidal ideation, depression, hopelessness, social support and self-esteem, second, the differences between the demographic factors and suicidal ideation, depression, hopelessness, social support and self-esteem. METHODS: This study was surveyed with a questionaire to 220 students of colleges located in Pusan. The instruments were used in this study were Harters(1988) Self-Perception Profile for College Students, Harter and Robinsons(1988) Approval Support Scale for Adolescents, Becks(1961) Beck Depression Inventory, Becks(1974) Beck Hopelessness Scale and Reynolds(1987) Suicidal ideation Questionaire. RESULTS: Self-esteem had a relatively strong positive correlation with social support. Suicidal ideation had a strong positive correlation with depression. Depression had a negative correlation with self-esteem and social support. Suicidal ideation had a negative correlation with self-esteem and social support. CONCLUSIONS: According to this study, the major factors related to suicidal tendency of adolescents were depression, low self-esteem, and low social support. Then the strategy to improve self-esteem and to encourage social support should be implemented for adolescents to prevent suicide.

4.A Case of Sarcoidosis Associated with Non-tuberculous Mycobacteria

Sarcoidosis is a multisystem granulomatous disease of unknown etiology. A range of factors including tuberculous infection, beryllium exposure, and cold climate have been implicated in the pathogenesis of the condition. A 45-year-old woman presented with a 4-month history of an asymptomatic eyelid swelling and multiple erythematous papules on the neck. Histological examination of the neck, orbital soft tissue, and lacrimal gland was consistent with sarcoidosis. Ziehl−Neelsen stains and polymerase chain reactions (PCRs) for tuberculosis on the skin biopsy specimens were negative. However, the orbital soft tissue specimen was positive for non-tuberculous mycobacteria (NTM) PCR. The patient was finally diagnosed with sarcoidosis associated with NTM. Treatment with systemic steroid and hydroxychloroquine was started, resulting in an improvement of skin lesions. We herein report a case of sarcoidosis associated with NTM infection with review of the literature, as only little is known regarding the role of mycobacteria in sarcoidosis.

6.Organizational Socialization and Intention to Leave in Operating Room Nurses Working at Secondary General University Hospitals.

PURPOSE: To evaluate the relationship between organizational socialization and intention to leave, and to identify factors affecting on intention to leave in operating room nurses. METHODS: This study used a descriptive design. Seventy operating room nurses recruited from three hospitals under the same management style, in-service educational system, and working conditions agreed to participate in this study, and completed self-administered questionnaires of the organizational socialization and the intention to leave questionnaire. Respondents were classified by career as advanced beginner, competent practitioner, proficient practitioner and expert practitioner by Benner's stages of nursing proficiency. Data were analyzed with t-test, ANOVA with multiple comparisons, Spearman correlation, and multiple regression analysis. RESULTS: Nurses unsatisfied with the current in-service continuous education had lower organizational socialization and higher intention to leave. In advanced beginners, job performance was lowest and organizational commitment was highest. In expert practitioners, intention to leave was highest. Organizational commitment, interpersonal relationship, identity and burnout were associated with intention to leave. Mutual trust, burnout, and interpersonal relationship were predictors of intention to leave explained 20.8% of variance. CONCLUSION: The career ladder program to enhance nurses' organizational socialization and intention to stay should be established and expanded for well-experienced operating room nurses.

7.Insertion and removal torques according to orthodontic mini-screw design.

OBJECTIVE: This study was designed to analyze the primary and secondary stability characteristics of orthodontic mini-screws of tapered design when compared with the cylinder mini-screw. METHODS: A total of 48 mini-screws were placed into the buccal alveolar bone of the mandible in 6 male beagle dogs. Comparison was made between tapered and cylinder type mini-screws (Biomaterials Korea, Seoul, Korea). Maximum insertion torque (MIT) was measured using a torque sensor (Mark-10, MGT 50, USA) during installation, and maximum removal torque (MRT) was recorded after 3 and 12 weeks of loading. RESULTS: Taper mini-screws showed a higher MIT value of 22.3 Ncm compared with cylinder mini-screw showing 13.6 Ncm (p < 0.001). The MRT of the taper mini-screw showed a significantly higher value of 9.1 Ncm than those of cylinder mini-screw of 5.7 Ncm at 3-weeks after installation (p < 0.05). However, there was no difference in the MRT value between the taper and cylinder mini-screws at 12 weeks of loading. CONCLUSIONS: These results showed that the high insertion torque of the taper mini-screw design increases initial stability until 3 weeks of loading, but does not have any effect on the secondary stability at 12 weeks of loading.

8.Computed Tomography and Magnetic Resonance Images of Adrenocortical Oncocytoma Cases.

We present two cases of adrenocortical oncocytomas that were well-delineated on multi-detector computed tomography and magnetic resonance imaging. The images showed a well-enhanced large mass with multiple stippled calcifications in a 10-yr-old girl who was consulted due to precocious puberty. A well-enhanced solid mass with necrotic components was incidentally noticed in a 54-yr-old man. These lesions were resected and diagnosed as adrenocortical oncocytomas through immunohistochemical studies and electron microscopy. Adrenocortical oncocytomas are rare disease entities, therefore, we report these interesting, rare adrenocortical oncocytomas here with radiologic findings, and discuss differential diagnosis.

10.Clinical Characteristics and Antibiotic Susceptibility of Staphylococcus lugdunensis Isolated from Skin Diseases: A Retrospective Single-Center Study

Background:
Staphylococcus lugdunensis is an emerging pathogen in skin and soft tissue infections. However, until recently, the role of S. lugdunensis as a skin pathogen remained underrated.
Objective:
The purpose of this study was to investigate the clinical characteristics and antibiotic susceptibility of S.lugdunensis isolated from skin diseases.
Methods:
This is a retrospective chart review of 48 cases in which S. lugdunensis was isolated from bacterial skin cultures between 2016 and 2021.
Results:
Among the 48 patients, the mean age was 41.0 years, 54.2% were male, and 12.5% of the patients had immunosuppressive diseases. Nearly 87.4% of infections occurred below the waist and the dominant sources of isolation were epidermal cyst (31.3%) and hidradenitis suppurativa (29.2%). The rate of resistance of S. lugdunensis against β-lactam antibiotics were as follows: penicillin, 83.3%; oxacillin, 37.5%; and methicillin, 29.2%.
Conclusion
S. lugdunensis is emerging as one of the primary pathogens of skin infections. When S. lugdunensis is isolated, dermatologists should consider that it is not just a component of the skin flora but can also be a pathogen. If S. lugdunensis is identified as the cause of infection, it should be managed with appropriate antibiotic therapy.
