1.The Effect of Intra-articular Injection of Steroid on Lysozyme Activity in Patient with Rheumatoid Arthritis

The lysozyme activities of knee joint fluid in normal Korean people, patients with rheumatoid arthritis, and patients with degenerative arthritis have been reported by Lee in 1974. This time I have studied the chronologic changes of lysozyme activity of knee joint fluid in patients with rheumatoid arthritis following intra-articular injection of steroid preparation (6-methylprednisolone acetate). The results obtained were as follows. 1. The synovial concentration of lysozyme in patients with rheumatoid arthritis was found to be 10.88 mcg/ml which is higher than normal value of 8.69 mcg/ml. 2. The lysozyme activity of joint fluid obtained 5 days after intra-articular injection of steroid prepatation was variable ranging from 7.40 mcg/ml to 10.0mcg/ml with the average of 8.97 mcg/ml, which corresponded to the relief of clinical manifestations. 3. The lysozyme activity of joint fluid obtained 10 days after intra-articular injection of steroid preparation decreased to 8.36 mcg/ml on average with the range from 7.60 to 9.30 mcg/ml which was in accordance with much relief of clinical manifestations. 4. The lysozyme activity of joint fluid obtained 20 days after intra-articular steroid injection was 9.55 mcg/ml on average with the range from 8.25 to 11.25 mcg/ml which was in accordance with the aggravations of clinical manifestations.

2.Study on the trematode parasites of the birds in Korea.

Authors carried out an investigation of the helminthic parasites of the wild birds in Korea. Most wild birds were collected from Kyoung Sang Do, Kyoung Kee Do and Seoul area from September 1970 to March 1973. A total of 70 wild birds, 19 species were examined, which comprised as follows; 10 Anas platyrhynchus, 10 Anas fulicata, 4 Anas poeciolorhyncha zonorhyncha, 8 Anser albifrone albifrone, 5 Anser cygonopsis cygnoid, 10 Aythya fuligula, 6 Gallicrex cinerea, 2 Porgana fusca, 2 Phisianum calichius korponri, 4 Sterptopilla orientalis, Podiceps cristatus, Larus crasirotatus, Malantta fusca, Garis artcus, Buetes buteo, Egretta alba, Starmus philpiens, Bombycilla granulus. Trematode parasites found from six species of the wild birds. Echinostomatidae has been found from intestine of the Anas platyrhynchus platyrhynchus, Anas poeciolorhyncha zonorhyncha, Anas fulcata. Notocatylus attenuatis found from intestine of the Podiceps cristatus, Anas fulcata. Catylurus japonicus was found from rectum of the Anas platyrhynchus platyrhynchus, Anas poeciolorhyncha zonorhyncha.

5.Abdominal Cerebrospinal Pseudocyst: a Complication of Ventriculoperitoneal Shunt in a Child.

Abdominal cerebrospinal fluid pseudocyst is an uncommon complication of ventriculoperitoneal shunt (VPS) performed for hydrocephalus. The incidence of VPS complications in children is higher than in adults. There are controversies and difficulties in the treatment of the abdominal pseudocyst. We report a case of abdominal pseudocyst complicating VPS in a boy. Partial excision of pseudocyst and replacement of the VP shunt were effective during a followup of 18 months postoperatively with no recurrence.

6.Angiosarcoma of the Distal end of the Clavicle: Report of a Case

Angiosarcoma is a rare tumor, and it is highly unusual for it to appear in bone as a primary lesion. Review of literature revealed that 38 such lesions have bsen recorded since 1925, when Codman wrote that they had as yet registered no case which could be considered a typical angiosarcoma prior to that date. Recently we encountered one tumor developed in the distal end of a clavicle which was confirmed as angiosarcoma by pathologic study. This case is presented with a review of literature.

7.Four Cases of Morning Glory Syndrome.

Morning glory syndrome is a rare congenital clinical entity that results from abnormal optic nerve development. In 1970, Kindler reported on ten patients having this anomaly and coined the term "morning glory syndrome" because of its similarity to the flower. The ophthalmoscopic picture is characterized by an enlarged pink excavated optic disk containing a mass of white tissue at its center, surrounding the disk is a wide, grey, elevated annulus of chorioretinal pigment disturbance. The retinal vessels appear as multiple radially oriented branches near the edge of the disk. Vision is usually markedly reduced. We experienced four cases of morning glory syndrome demonstrating various ophthalmoscopic and fluorescein angiographic features.

8.Total Gastrectomy for Gastric Cancer Involving the Cardia.

BACKGROUND: Gastric cardia cancers are relatively rare neoplasms althougth it is reported that their relative incidence among gastric cancers is increasing. At the time of diagnosis, gastric cardia cancers in many cases are already in a more advanced stage in terms of depth of invasion or metastasis to lymph nodes. The purpose of this study was to evaluate the effect of the proximal resection margin on the survival rates for cardia cancer patients who underwent a total gastrectomy without a thoracotomy. METHODS: We retrospectively analyzed 87 patients who had undergone operations for gastric cardia cancers at Eulji College Hospital during the 8 years from Jan. 1988 to Dec. 1995. RESULTS: The most prevalent age group was in the 6th decade (34.5%) and the male-to-female ratio was 2.2:1.0. The TNM classification showed stage I (13.8%), II (10.3%), III (54%) and IV (21.8%). The overall 5-year survival rate was 35.6%, and the survival rates according to the stage were 83.3% in stage I, 87.5% in stage II, 27.1% in stage III, and 0% in stage IV. The 5-year survival rates according to the length of the proximal resection margin (PRM) for all patients were as follows: 22.0% for PRM< or =2 cm (N=34) and 44.0% for PRM>2 cm (N=52) 44.0% (p=0.0783). The 5-year survival rates according to the length of the PRM for stage III cancers were as follows: 19.7% for PRM< or =2 cm (N=18) and 31.7% for PRM>2 cm (N=28)(p=0.4090). CONCLUSIONS: These results suggest that the length of the proximal resection margin is not significant as a prognostic factor. We believe a total gastrectomy without a thoracotomy is a reasonable and safe alternative to a thoracotomy approach to the dissection of the mediastinal lymph node in cases of gastric cardia cancer.

9.Infantile Littre's Hernia: A Case Report.

The term "Littre's hernia" was originally defined by Reinke in 1841 as "the presence of a Meckel's diverticulum in any hernia sac." Littre's hernia is a very rare disease, which accounts for less than 1% of all Meckel's diverticula. We report a case of Littre's hernia experienced in a 45 day-old infant.

10.A Preliminary Report of Laparoscopic Hernia Repair in Children.

Minimally invasive techniques for pediatric inguinal hernia repair have been evolving in recent years. We applied the laparoscopic method to repair pediatric inguinal hernia using the techniques of sac transection and intra-corporeal ligation. Between November 2008 and August 2010, 67 pediatric patients (47 boys and 20 girls) with inguinal hernias were included in this study. Postoperative activities, pain, and complication were checked prospectively at regular follow-up. One patient presented with clinically bilateral hernia, and three patients had metachronous hernias. Thirty-two cases out of 63 patients with unilateral hernias had a patent processus vaginalis on the contralateral side. Mean operation time was 35+/-11.4 minutes for unilateral hernias and 43+/-11 minutes for bilateral hernias. There were no intra-operative complications. One patient had a small hematoma on the groin postoperatively, which subsided spontaneously in a week. Recurrence and metachronous hernia were not found at follow up. In summary, laparoscopic inguinal repair in children is safe, easy to perform and has an additional advantage of contralateral exploration. Further studies should include long term follow-up.
