1.A Clinical Study of the Ankle Fractures

Ninety six patients treated at Seoul Red Cross Hospital from Jan. 1979 to Dec. 1982 were analized in clinical and radiological aspect. The results obtained from this study were as followings; 1. Among the 96 patients, male was 67 patients and female was 29 patients (M:F=2.5:1). The average age was 32. 2. The causes of the injury were traffic accident, slipping down, falling down and sports injuries in orders. 3. According to the classification of Lauge-Hansen, supination external rotation type (38.5%) was the most common type. 4. 65 cases (67.7%) were treated by open reduction and 31 cases (32.3%) by closed reduction. The better result was obtained by open reduction than closed reduction. 5. Accurate reduction and rigid internal fixation of the lateral malleolus was most important in treatment of the ankle fractures. 6. Classification of Lauge-Hansen was useful in diagnosis and treatment of the ankle fractures.

3.A Case of Reiter' s Syndrome Combined with Lung Cancer.

Reiters syndrome is an unusual disease characterized a triad of nongonococcal urethritis, conjunctivitis, and arthritis in association with the mircoutaneous lesions of keratoderma blenorrhagica and balarintis circinata. We present herein a case of Reiters syndome combined yiti lung cancer. A 39-year-old man has experienced naigrating polyarthralgia and high fever ilitermittently for about 20 years, Hyperkeratotic erythemnous patches and plaques, which dyeliped about 1 year ago, have aggravated and expanded to the whole body. Histopathologic sections from the plaque on the right forc ari showed characteristic findings including thickened parakeratotic horny layer and spongiform micropustules of Kogoji He was associated with HLA-B27. On the chest X-ray, a thumbtip size mass was incidently found. Chest CT and bone scan findings supported lung cancer and multiele metastases.

4.Trichobezoar dectected by ultrasonography: case report.

The authors experienced a case of gastroduodenojejunal trichobezoar detected by ultrasongraphic examination. We thought that tricholbezoar had relatively specific ultrasonographic findings such as a broad hyperechogenic superficial bandlike rim with a complete posteior sonic shadowing and multiple linear echogenic strands on its surface representing hairs, which were better seen after water ingestion. So we were able to get the correct diagnosis of trichobezoar prior to conventional barium studies in a patient who could not be suspected suspected clinically.

5.Proximal Half Corpectomy and Fusion of One Motion Segment in Denis Type B Burst Fracture of Thoracolumbar and Lumbar Spine.

STUDY DESIGN: The authors is to report the clinical and radiological results of proximal half corpectomy with one motion segment fusion in Denis type B burst fracture. OBJECTIVES: To evaluate the efficacy of proximal half corpectomy in Denis type B burst fracture of thethoracolumbar and lumbar spine. SUMMARY OF LITERATURE REVIEW: For the operative management of burst fracture, various mothods including posterior ligamentotaxis, posterolateral decompression, anterior decompression or combined were reported. Among the methods, anterior decompression by the corpectomy of fractured vertebral body and fusion with or without instrumentation is the one of the widely accepted method of treatment. However, anterior decompression by excision of whole vertebral body has the disadvantage of high complication rate due to the instability from the large defect and long length of bone graft. Moreover, two motion segments have to be sacrified, which is very important especially in thoracolumbar and lumbar area. Material and METHODS: 43 cases operated from 1989 to 1996 and the minimum follow up period was two years and compared with that of 48 cases who were treated by total corpectomy and two motion segment fashion from 1986 to 1989. RESULTS: Solid bony union was obtained in 43 cases within 6 months and no back pain was complained in 39 cases (93%) at last follow up. There was no significant difference between two groups in correction of anterior vertebral height and kyphotic angle. Length of bone graft was 3.0cm in half corpectomy group and was 6.3cm in total corpectomy group. Hardward breakage or graft collapse was not observed in proximal half corpectomy, while there were 5 cases in total corpectomy. CONCLUSION: Proximal half corpectomy and fusion of one motion segment in Denis type B burst fracture is believed to be a successful method which can minimize the fused level, increase the stability, preserve motion segment and reduce the complication.

7.A Case of Cutaneous Focal Mucinosis.

We report a case of cutaneous focal mucinosis in a 60 year-old male, who had asymptomatic localized small flesh colored papule and nodule on the left side of the neck. The histopathologic finding revealed fibroblastic proliferatior in the dermis and an increased prodact on of hyaluronic acid by these fibroblasts. The lesions were treated by surgical excision.

8.Treatment of Brachymetatatsia.

A total of 16 metatarsal bones were treated for ten with brachymetatarsia using two different methods: single staged lengthening by bone graft(2 cases) and gradual distraction lengthening using unilateral external fixator(14 cases). Three cases of first metatasal bone and thirteen cases of fourth metatarsal bone were operated. All of the cases of the brachymetatarsia were congenital. The average amount of lengthening was 16.1 mm in gradual distraction lengthening while 10.5mm in single staged lengthening. Average percentile increase was 38.8% in gradual distraction lengthening and 23% in single staged lengthening. The average healing index of gradual distraction lengthening was 1.7(months/cm). Although single staged lengthening has advantage of cosmetics, gradual distraction lengthening using unilateral external fixator was thought to be effective functionally for metatarsal lengthening.

9.Triscaphe Fusion with Radial Styloidectomy in Kienbock's Disease.

Neither the cause nor the correct treatment of Kienbocks disease has been clearly established, but its clinical and radiologic presentations have been distinctly defined. There are many controversies concerning therapeutic guidelines for the treatment of Kienbocks disease. The purpose of this study is to evaluate the clinical result of triscaphe fusion with radial styloidectomy of Kienbocks disease. Ten cases of stage III Kienbocks disease by Lichtmans classification were treated by triscaphe fusion with radial styloidectomy from September 1991 to March 1997. We followed up over 24 months and evaluated clinical results. In all cases, pain was relieved. The postoperative results according to Kuschners method revealed three excellent, six good and one fair. The triscaphe fusion and radial styloidectomy was considered as useful method of treatment which has clinical efficacy in Kienbocks disease.

10.Radiological Analysis of Aging Changes of the Lumbar Intervertebral Disc.

The present study was carried out to clarify the anatomical changes of lumbar intervertebral discs by aging. Anterior height, posterior height, anteroposterior diameter of intervertebral discs were measured on 512 normal plain lateral radiographs of lumbosacral spine. And the indices of disc wedging and relative disc height were calculated. There was a cephalocaudal gradient of increase in the indices of disc wedging in all age groups. The indices of relative disc height were constant at all lumbar levels. These suggest that the lower disc is more wedge shaped and the height of discs changes in the constant ratio with that of vertebral body.
