Sirenomelia is a severe form of caudal regression syndrome that results in a fusion of the lower extremities, which is not compatible with life. A various spectrum of anomalies affecting primarily the musculoskeletal, genitourinary and gastrointestinal systems also can occur. This rare malformation has a reported incidence rate of approximately 1 in 60,000 births, with a range of 1 to 1 percent of all malformed infants. We experienced a sirenomelic case with combined anomalies of genitourinary, cardiovascular and gastrointestinal systems. Maternal obstetric history revealed oligohydramnios and intrauterine fetal growth retardation, and the baby was spontaneously delivered at 37 weeks of gestational age, but died I hour after birth.

26 patients with urethral injury treated during the period from January 1967 to May 1970 have been studied in particular reference to sexual ability following injury and the results are summarized as follows: 1. The incidence of urethral injury was 7.07% of the total inpatients and the average in age was 23 years, 17 cases (65.3%) were accompanied by pelvic fractures. 2. The incidence of impotence following injury was 38.44%, 26.92% temporary and 11.53% permanent. 3. Of 7 cases of temporary impotence, urethral injury was accompanied by pelvic fracture in 6 cases and all three cases of permanent impotence followed urethral injury accompanied by pelvic fracture. 4. Of 7 cases of temporary impotence, previous urethral injury was complete in 3 and incomplete in 4. Three cases of permanent impotence followed in 3 cases of complete urethral injury. 5. The average interval between the reception of trauma and the recognition of impotence was 6 months.

PURPOSE: To analyse unusual radiologic manifestations of colonic adenocarcinoma, and to correlate these with pathologic findings. MATERIALS AND METHODS: Radiologic findings of ten patients with atypical adenocarcinoma ofthe colon were retrospectively evaluated. The unusual radiologic findings were difined as terminal ileal involvement of the cecal mass, long segmental involvement of over 9cm, and exophytic tumor growth. radiologic and sonographic findings were compared with pathologic specimens obtained from surgical resection. RESULTS: Involvement of the terminal ileum was noted in three cases, long segmental involvement of over 11cm in five cases, and exophytic mass in two. Of three cases with thickening of the terminal ileum, two revealed the infiltration of cancer into the terminal ileum through the ileocecal valve, and the other revealed vascular congestion and edemaon microscopic examination. Five cases with long segmental involvement of over 11cm comprised one of cancer totally infiltrated through the submucosal and proper muscle layer, one of inflammatory thickening distal to thecancer, two of inflammatory change of pericolic fat and serosal adhesion and one of a large intraluminal fungatingmass. In the cases of exophytic mass, one with a larger extraluminal and a smaller intraluminal component revealed necrosis and abscess on pathologic examination, accounting for low attenuation on CT, whereas the other, with exophytic growth, disclosed abundant pools of mucin, resulting in low attenuation on CT. These two cases could notbe differentiated from submucosal tumors. CONCLUSION: Atypical colon cancer may have various manifestations, suchas thickening of the terminal ileum, involvement of a long segment, and an exophytically growing mass. An appreciation of the radiologic findings of this cancer may therefore help in differential diagnosis in cases simulating colitis or submucosal tumors of the colon, such as lymphoma or leiomyoma.

PURPOSE: To evaluate the extent of lesion by using ultrasound biomicroscopy in eyelid tumor. METHODS: We retrospectively analyzed 17 patients with eyelid tumor, who had undergone preoperative ultrasound biomicroscopy to access the extent of the lesion and the depth of involvement, excision and biopsy. These data were compared to the histopathologic findings. RESULTS: On ultrasound biomicroscopic examination, the eyelid tumor showed greater homogeneous hypoechoic density or heterogenous echo density than that of the surrounding tissue, leading to identifying the lateral margin of the lesion and depth of involvement. The mean maximum thickness of the lesion measured by ultrasound biomicroscopy was 2.26 +/- 0.49 mm and that of the histopathologic depth was 2.22 +/- 0.56 mm. Linear regression demonstrated that lesion depth measured by ultrasound biomicroscopy was correlated with histopathologic depth. CONCLUSIONS: In eyelid tumor, ultrasound biomicroscopy is helpful in assessing the extent of the lesion and the depth of involvement preoperatively, and in obtaining good postoperative results.

The differential diagnosis of benign tumors from other intra-ureteric lesions was difficult. But. recently. it is easy due to developed diagnostic procedures. such as ureteroscopy. We herein report a case of fibroepithelial polyp of the left ureteropelvic junction which was diagnosed with ureteroscopic finding and biopsy with brief review or literatures.

Volume deficiency and poorly defined cupid's bow of the upper lip are frequently encountered problems in secondary cleft lip deformities. In this report, we present the method of a periumbilical dermal graft for correcting secondary cleft lip deformity. A 17-year-old male patient presented with a poorly defined notching cupid's bow and volume deficiency of the upper lip. He had undergone cleft lip surgery when he was 2 years old. We planned to perform a periumbilical dermal graft. There were sufficient amount of periumbilical subdermis and fat tissue thickness to obtain abundant volume. The periumbilical contour was similar to the natural contour of the lip. Less scarring was expected. There was no need to change the posture during operation, making it easy to harvest. Postoperative evaluations showed vermilion symmetry. Ideal cupid's bow shape and position were achieved without color mismatch. As a result, we obtained an ideal volume of the upper lip. Therefore a periumbilical dermal graft is a good option for correcting volume deficiency of the lip with good aesthetic outcomes.

Scalp and calvarium defects are caused by trauma, burn, tumor resection, or congenital diseases. We experienced a few cases of severe electrical burn of scalp and calvarium, but fourth-degree contact burn of scalp and calvarium is a rare case. A 67 years old man was presented with a 25% total body surface area contact burn. A 20 cm×15 cm thick eschar on the patient's scalp was observed. Among various techniques for scalp reconstruction, we planned fasciocutaneous transposition flap with split thickness skin graft for coverage of large defect. Considering aesthetically satisfactory outcome, we designed a fasciocutaneous transposition flap including the hair-bearing areas. We additionally used skin graft for uncovered surrounding areas. There were no flap necrosis, graft loss, or any other surgical complications after the surgical flap and skin graft. At 6-month follow-up, the operation site was stable. The patient satisfied with functional and aesthetical outcomes, so we report this case.

Neurolytic or surgical lumbar sympathectomy are commonly used for the treatment of intractable pain, peripheral vascular insufficiency and hyperhydrosis. For the procedure, the patient sometimes has to suffer from complications such as major vascular perforation, subarachnoid injection, genitofemoral neuralgia, somatic neuralgia, disc perforation, ureteral stricture, inflammation, ejaculatory failure, chronic lumbago. The authors came across with a case of ejaculatory failure a week after both lumbar sympathetic block with 6 ml of 100% alcohol at L3 level under the control of fluoroscopy. Three months after the block the patient recovered without any treatment.
