In order to reduce the test time in real ear hearing-aid fitting for children, the validity of applying the average real ear to coupler differences (RECDs) in prefitting procedure using a 2cc coupler measurement system was evaluated by checking whether the majority of people's RECDs might occur within 5 dB of the average RECDs (N = 116) in each test frequency and age group. The percentages of occurrence were around 90% in test subjects' RECDs in saturation sound pressure levels (SSPLs) and around 70% in gain in each important test frequency. Appropriate test frequencies in prefitting are 500, 1000, 1500 and 2000 Hz.

There is much controversy concerning the method of treatment on the tibial fracture because of frequent complications, such as non-union, delayed union and infection. Authors reviewed and analyzed 110 tibial fractures which had been treated at the Department of Orthopedic Surgery, Sung Ae General Hospital from June 1983 to March 1987 by the groups, conservative treatment, open reduction and internal fixation with compression plate and closed flexible intramedullary nailing. The results were as follows :1. In regard to the fracture location, there was no difference in the healing time among the treatment methods. The I.M. nailing group healed 1.5 weeks earlier than other groups in each location. 2. In minor and moderate severity by Ellis, the I.M. nailing group revealed better result, and in major one, the compression plate group showed better result. More complications occured in major severity. 3. Union time among the trestment methods was not affected by the fibular fractures, but most of the complications occured in the concomitant fibular fracture case. 4. In open fracture, the healing time showed no difference among the groups, and was prolonged about 2 weeks than closed fracture. In closed case, the I.M. nailing group showed good result. 5. The cases, which were reduced earlier in the conservative group, and treated within 3 days in the I.M. nailing group revesled better result. 6. The radiological union time from the definite treatment was 15.80 weeks in the conservative treatment group, 15.18 weeks in the compression plate group and 13.92 weeks in the I.M. nailing group.

lmpedance audiometry requires physical modifications during the test, which might influence retest data. Therefore, in Order to interprete retest data meaningfully, the range of variation should be identified in each measure of impedence audiometry. The present study obtained data on the retest variation of peak pressure, acoustic reflex threshold, static compliance and earcanal volume in impedance audiometry. ln addition, the authors wanted to know whether or not impedance data would assist otolaryngologists in the detection of conductive hearing impairment. The variation of the retest data was not clinically nor statistically significant in the measurement except for those of ear canal volume. The data on ear canal volume also suggested that the ear canal increases in size during the teenage period and that male ear canals are larger than those of females in ears over 20 years of age. PURPOSE: The normal flora, which was suggested to prevent infection, is acquired first from the birth canal and develops by dietary factors. Here presents a case-control study, aimed to evaluate the postnatal acquisition factors relating to the achievement of the normal flora in infants with urinary tract infection (UTI). METHODS: 115 UTI infants, admitted at Ewha Womans University Mokdong Hospital from 2004 to 2005 and 116 age-matched control infants, who had visited well baby clinic, were evaluated. The suggested postnatal acquisition factors of the normal flora such as vaginal delivery, breast feeding, probiotics and yogurt intake and their relationship with UTI were evaluated. RESULTS: The rate of vaginal delivery was 50%(58/115) in UTI infants, which was not significantly different from 60%(69/116) in control infants(P>0.05). In the infants with UTI, the feeding pattern(breast milk 19%, mixed 26%, formula 55%) was significantly different from that(44%, 19%, 37%) in control infants(P<0.05). This significant difference was shown only in infants less than 6 months of age, but was not in infants over 6 months of age. The rate of probiotics intake in UTI infants was 4%(4/115), which was significantly lower than 27% (32/116) in control infants(P<0.05). The rate of regular intake of yogurt in UTI infants over 6 months of age was 27%(6/23), which was not significantly different from 35%(8/23) in control infants(P>0.05). The odds ratios of breast feeding and probiotics intake against UTI were significantly low as 0.30 (95% CI 0.17-0.55)(P<0.01) and 0.03 (95% CI 0.01-0.07)(P<0.01). CONCLUSION: The significantly lower rates of breast feeding and probiotics intake in UTI infants suggest that these dietary factors might have preventive effects in infants with UTI.

We experienced a complication of brachial plexus palsy secondary to operative position during thoracoscopic thoracic sympathectomies. His general health was excellent and no previous histories vulnerable to peripheral nerve systems were observed. The thoracic sympathectomies were done under general anesthesia. The patient was placed left lateral position with his right arm abduced 150o on padded arm board. An operation was lasted 2 hours and 30 minutes at this position because of severe right apical lung adhesion. The controlateral side was performed same procedure and lasted 20 minutes. After the patient recovered from the anesthesia, the patient had a complete paralysis of right arm. There was also slightly diminished sensation to pinprick on the arm and hand. Neurologic examination and EMG study revealed brachial plexus palsy. Nerve blocks and physiotherapy were performed to treat brachial plexus injuries. His motor functions were improved day by day and he was discharged with a complete range of motion against gravity on 14th. postoperation day. However, there were loss of muscle powers against some resistances and tingling sensations of fingertips. Two months later, he was recovered completely and there was no residual disabilities.

PURPOSE: To investigate clinical characteristics of post-traumatic intraocular foreign body (IOFB), which occurred between 2006 and 2010, and prognostic factors associated with final visual outcome. METHODS: A retrospective chart review was performed of patients with IOFB who visited our clinic from January 1, 2006 to December 31, 2010, and who were followed up for more than 6 months. Cross tabulation and correlation analyses were conducted to evaluate the predictive factors related to final visual acuity. RESULTS: A total of 36 eyes (35 patients) were included in this study. The mean age was 44.2 years, and 33 patients (94.3%) were male. The most common cause of IOFB was lawnmower-related activity (12 patients; 33.3%) and hammering-related activity (11 patients; 30.1%). Among analyzed prognostic factors, only initial visual acuity was significantly correlated with visual outcomes (p < 0.001). CONCLUSIONS: Compared with previous studies, lawnmower and hammering-related activities were still the most common causes of IOFB, however, the incidence of occurrence in the 40's and 50's was relatively higher than in previous studies. Considering the poor visual outcome of IOFB even after proper surgical treatment, using proper eye protection should be emphasized in a dangerous work environment to prevent ocular trauma.
