2.Observations in the Recovery Room according to Post-Anesthetic Recovery Score .

A postanesthetic recovery score to provide objective information on the phsical condition of patients arriving in the recovery room after anesthesia. The status of the patients was judged after arriving in the recovery room and every 30minutes thereafter, until their discharge to the ward or Intensive Care Unit. To study this method, 324 patients were selected at random to include as many variants of anesthesia as possible. The patients were transported to the recovery room within 10 minutes of the completion of the anesthesia; Once there, the first score was made by the anesthesiologist. Thereafter, the evaluation was made by the same anesthesiologist. Depending on the length of stay in recovery room, this evaluation was repeated 30, 60, and 90 minutes after the patients arrival. The various signs were evaluated. The independent variables such as age, sex, physical status, anesthetic agents or technics, type of surgery, use of muscle relaxants, duration of anesthesia were correlated with the proportional number of patients receiving top or safe score 10, 9 or 8, and those given 7 or less, which were considered low or dangerous.

3.Postirradiation Malignant Mixed Mesodermal Tumor of the Uterus: A case report.

A case of malignant mixed mesodermal tumor (MMMT) developed after radiation therapy for a uterine cervix cancer is described. The patient was a 62-year-old female at the time of diagnosis of stage Ib squamous cell carcinoma of the cervix and a total of 12,000 rads of x-ray was administered on the pelvic area. Five years later she manifested vaginal spotting and rectal pain. Endometial curettage and biopsy revealed carcinosarcoma. Radical hysterectomy was done and a 5x3x2 cm sized polypoid mass was noted in the uterine cavity. Microscopically, the tumor showed intimate admixture of adenocarcinomatous and sarcomatous areas. The sarcomatous stroma was composed of compactly arranged atypical spindle cells with frequent mitoses, merging into a loosely textured reticular areas and abundant amount of heterologous elements such as skeletal muscle and cartilage. The rhabdomyosarcomatous element was confirmed by PTAH staining and immunohistochemical staining for myoglobin and desmin. Multiple metastases to the liver, lung, and lymph nodes appeared within one year of total abdominal hysterectomy and bilateral salpingo-oophorectomy. In spite of palliative radiotherapy, she expired one month later.

4.Nutritional and Health Status of Korean Elderly from Low-income, Urban Area and Improving Effect of Meal Service on Nutritional and Health Status - V. The Effect of Meal Service for One Year on Nutritional and Health Status.

This study was performed to assess the effect of one year's of meal service for home-staying urban elderly with low incole on their nutritional status. One hundred and eighty three subjects, who had already completed the first nutritional survey, were assigned to two group : meal served(served) and non-meal served(non-served). A meal containing approximately on half of the RDA for energy, protein, calcium and iron was served as lunch everyday to served group. After on year of meal service, follow-up-nutritional survey was done and changes of parameters were analyzed with paired t-test. Served female showed signficantly increased intake of riboflavin and calcium, while non-served female showed significantly decreased intake of calcium. Serum total protein, serum albumin and serum cholesterol were significantly increased in female regardless of meal service. Served remale was observed with significantly elevated LDL-cholesterol, whereas non-served female showed singnificantly lowered HDL-cholesterol. Significantly decreased serum iron, serum transferrin saturaion and significantly increased TIBC were observed for female regardless of meal service. But the proportion of anemic elderly according to Hb or serum iron was decreased more in served group. Female showed significantly increased serum zinc and copper regardless of meal service, whereas only served male showed significantly increased serum copper.

5.The Effects of Sexual Rehabilitation Education on the Knowledge of Sexual Activities and Satisfaction of Sexual Activities of Spinal Cord Injury Clients.

The purpose of this study is to identify the effects of sexual rehabilitation education on the knowledge of sexual activities and satisfaction of sexual activities of spinal cord injury clients by the method of Nonequivalent control group pretest-posttest group. The study period was between Jan. 6th and Apr. 11th, 1999. The experimental group was 21 spinal cord injury clients from one general hospital, and the control group was 21 spinal cord injury clients from three University hospital. Sexual rehabilitation education was done with manuals and video-tapes. Education time was 90 minutes a week for 4 weeks. The study tool was 20 questionnaires about sexual knowledge, designed Song, Chung Sook(1990) and 10 questionnaires from Derogatis's Derogatis Sexual Function Inventory for satisfaction of sexual activities(1979) translated by Jang, Soon Boke(1989). Analysis of data was done by x2 -test, t-test, repeated measures ANOVA and simple main effect with SAS/PC. The results are as follows. The 1st. hypothesis, "The group educated about the sexual rehabilitation has more sexual activities knowledge than those of the uneducated group." is significant statistically(F=12.06, p=0.001). The 2nd. hypotheses, "The group educated about the sexual rehabilitation has more satisfaction of sexual activities than those of the uneducated group." is significant statistically(F=5.04, p=0.030). According to the results, sexual rehabilitation education helps spinal cord injury clients to gain knowledge of sexual activities and satisfaction of sexual activities.

6.The Frequency and Distribution of the ABO Subgroups in Korean Blood Donors.

BACKGROUND: ABO subgroups are obstacles in blood typing and cross matching in the process of pre-transfusion. Correct ABO typing is important for ensuring safety of a transfusion. The ABO subgroups contain a lesser amount of antigen on red cells and this is a common cause of discrepancy between the results of cell and serum tests. This study was performed to analyze the frequency and distribution of the ABO subgroups in Korean blood donors. METHODS: ABO blood grouping was performed by an automated analyzer (PK7200/7300, Olympus, Japan) for the 3,397,983 donors from January 2007 to December 2009. The 1,868 donor samples that were suspected to have ABO subgroups were tested with adsorption/elution techniques. The results of the tests were analyzed and we received approval of the Institutional Review Board of the Korean Red Cross to publish this study. RESULTS: 1,771 donors out of the 1,868 donors who underwent the tests were finally identified as having ABO subgroups. The kinds of identified ABO subgroups were as follows; 118 donors with the A subgroup (A2, A3, Ax, Am, Ael), 237 donors with the B subgroup (B3, Bx, Bm, Bel) and 1,416 donors with the AB subgroup (A2B, A2B3, A1B3, AmB, A1Bm, A1Bel, AxB, A1Bx). The most frequently observed subgroups were A2B (496/1,868, 26.55%) and then A2B3 (457/1,868, 24.46%). The subgroups of 97 donors (5.19%) were not identified. CONCLUSION: This study was meaningful to provide data on the frequency and distribution of the ABO subgroups of Korean blood donors. A more sensitive technique such as genotyping could be useful to resolve cases with an unidentified subgroup.

8.The Effect of Corticosteroid Therpy in the Very Premature Infant.

PURPOSE: Our purpose was to deterrnine the efficacy of maternal corticosteroid therapy on the morbidity of premature infants between 26 and 31 weeks' gestation. METHOD: A total of 62 premature infants between 26 and 31 weeks gestation admitted to pediatric department of National Medical Center from Nov, 1990 to June 1996 were analyzed to evaluate the efficacy of prenatal corticosteroid therapy on the morbidity such as hyaline membrane disease, intreventricular hernorrhage, necrotizing enterocolitis, sepsis, neonatal death, days on ventilation and hospital days. RESULT: Among 62 women who delivered premature infants between 26 and 31 weeks, 22 received betamethasone before delivery and 40 did not. 1) The rate of hyaline membrane disease was less in the betamethasone group (41% vs. 70%, P<0.05). 2) The rate of intracranial hemorrhage was less in the betamethasone group (0 vs. 20%, P<0.05). 3) The days of ventilator care was less in the betamethasone group among survival cases (27+/-3.2 vs. 5.2+/-4.6, P<05). 4) Arnong 14 women who delivered at 26 to 28 weeks, 4 received betamethasone before delivery and 10 did not. The rate of neonatal death was less in the betamethasone group (o% vs. 80%, P<0.05). 5) Among 48 women who delivered at 29 to 31 weeks, 18 received betamethasone before delivery and 30 did not. The rate of hyaline membrane disease was less in the betamethasone group (39% vs. 73%, P<0.05). CONCLUSION: Betamethasone appears to reduce hyaline membrane disease, intraven- tricular hemorrhage, neonatal death and the morbidity significantly in premature infants between 26 and 31 weeks' gestation.

9.Effects of Cryocautery on the Recurrent Pterygium.

The results of cryotherapy for 12 cases with the tendency of recurrent pterygium treated with Thio-tepa or strontium 90 after the surgical removal are presented. There appeared the rupture of vessels. and edema following cryocautery for: 3~15 days. No special complication or any sequelae in the eyeball is observed. The irritatini signs such as foreign body sensation, lacrimation or photophobia are easily relieved with systemic treatment of Celestamin tablet.
