1.A Study on the Sourcees of Postoperative Wound Infection in Patients with Open Fracture

The purpose of this paper is to define the bacterial sources of wound infection after surgery including debridement for open fractures. Total number of patients were 123 during surgery July 1975 to Oct. 1976 at Hangang Sacred Heart Hospital. Out of 123 cases 17 were infected postoperatively. Serial bacterial cultures were performed for specimens taken from the contaminated wound in emergency room and by exposing the Petri-dishes in the operating room. Additional cultures were carried out for 17 specimens taken from the infected wounds postoperatively. Positive bacterial cultures from the initial wounds were obtained in 71 cases (57.7%) out of 123 contaminated wounds in emergency room. Setting monitoring of air contamination in conventional operating room, 51 colonies of bacterial growth per 1 hour exposure were noted and common organisms were colliform bacilli, streptococci and staphylococci in order. Postoperative wound infection rate was 13. 8%. Common pathogens were staphylococcus aureus, coliform bacilli and pseudomonas and kanamycin was most effective antibiotics. The retrospective bacteriological studies for 17 infected cases elucidated following results: 1) Organisms of same species; from initial wound and infected wound postoperatively 7 cases 2) Organisms of same species; from air contamination in the operating room and infectected wound postoperatiyely 5 cases 3) Organism of same species; from air contamination in the operating room, initial wound and infected wound postoperatively 1 case 4) Others (contagious or endogenous origin) 4 cases The results reveal that the initial contaminated open fracture and air contamination during surgery are the main sources of postoperative infection. Four cases which the infectious sources were not determined included in contagious origin dogmaticaIly.

2.Clinical Evaluation of Stable Thoracolumbar Vertebral Fractures

One hundred and fifteen thoracolumbar vertebrel fractures Out. of 85 patients were avaluated in this study. In 97 cases out of 65 patients, angular deformity and loss of height of the fractured vertebral bodies were measured, and for 6. 3 months in average follow up study was made in 23 cases out of 18 patients. The angular deformity became increased from 18.4 degrees to 21. 0 degrees wheroas loss of height has also increased from 38.6 to 40.8%, showing little statical value. Incidence was higher in male than in female without seasonal differences. Age incidence ranged from 16 to 78, and most frequent in third and fourth decades. Causes of injury were industrial accident, traffic accident, home accident and fall from height. Location of injury was mainly twelfth thoracic and first lumbar vertebrae. As associated injury, fracture of the rib, ulna, malleolus and calcaneus in order. Conservative treatment was a method of choice, while operstive treatment was performed in 7%.

3.A Clinical Analysis of Open Fractures of Long Bone

This is to present the clinical analysis of 197 (165 Patients) open fractures of long bones which were treated at Han Gang Sacred Heart Hospital from December 1971 June 1975. The results obtained as follows: 1. The causes of injury were traffic accident 91, industrial accident 63, home acident 8 and other 3. 2. In this study, there were 139 males and 26 females. The main age incidence ranged 11–40 years. 3. Among the 197 cases, Tibia and fibula were affected mostly(54.8%) 4. Associated injuries included closed fracture of long bone 35, cerebral contusion 15, skull fracture 10, maxillofascial inury 9, joint dislocation 6 and other 21. 5. The method of treatment was chiefly debridement and primary closure of open wound with 16 internal fixation, followed by cast immobilization 161, skeletal traction 15 and pin & plaster. 5. 6. Consequent incidence of infection was 10.7% (deep 5.1%, superficial 5.6%) 7. The causative strains were coliform bacillus 28, pseudomonas 16, staphylococcus 6, streptococcus 1, and bacillus subtilis 1, 8. Death occured in 3 cases: the causes were sepsis due to pseudomonas 2 and anuria due to renal shut down 1.

4.Treatment of Displaced Fractures of the Femoral Neck by Muscle-Pedicle Bone Graft and Internal Fixation

The major blood supply to the femoral head is probably damaged by the fracture of the femoral neck especially when it is a displaced fracture. Complications such as delayed or nonuion of the fracture and avascular necrosis of the femoral head are common. Revascularization after reduction and internal fixation appears to be a slow process and in many patients late segmental collapse occurs. Various techniques have been proposed to restore the blood supply or to improve the circulation to the femoral head. Hereby we performed internal fixation and an autogenous muscle-bone pedicle graft composed of the quadriceps femoris muscle in 3 cases of old, neglected displaced femoral neck fracture. The results seemed to be good in regard to bony union in all 3 cases but it would be needed further follow up study for developing of any avascular necrosis.

6.A Clinical Study on Transscleral Suture Fixation of Posterior Chamber Intraocular Lens.

Posterior chamber intraocular lenses were implanted into 40 eyes with inadequate capsular or zonular support by securing both haptics of the lenses to the sclera at ciliary sulcus with 10-0 prolene sutures. Our follow-up of 36 eyes with scleral fixated posterior chamber intraocular lenses ranged from 2 months to 8 months(average 4.6 months). At the last follow-up visit. 31 eyes(86%) had a vision of 0.5 or better. Mean refractive error was -1.15 +/- 1.13D. which was -0.67 +/- 0.99D more myopic than predicted (p<0.05). Measurement of anterior chamber depth showed deeper than the control group of posterior chamber intraocular lenses without scleral fixation suture(4.18 +/- 0.42mm, 3.96 +/- 0.29mm, respectively), but which was not statistically significant(p>0.05). Additionally, the difference of postoperative axial length between two groups was not statistically significant(23.39 +/- 0.87mm, 23.42 +/- 0.75mm, respectively). In 4 eyes(ll %), minor intraoperative bleeding oceured but was absorbed spontaneously without any sequelae. One eye(3%) of minimal decentration of PCL, one eye(3%) of subluxation of PCL, and one eye(3%) of exposed scleral fixation suture were noted, but no serious postoperative complication, such as corneal edema, glaucoma, persistent uveitis, or retinal detachment was detected.

7.Clinical Observation of Supracondylar Fracture in Children

Supracondylar fracture is the most freqent injury of the elbow and frequently accompany complications in children. This injuries require meticulous care during first week period for the prevention of complications. Sixty cases of supracondylar fracture in children treated and managed at the Orthopedic Surgery, Taegu Presbyterian Hospital during the period from 1962 to 1970 The most cases were treated conservatively and various method of treatment were adapted according to the condition of the fracture. All obtained satisfactory bone union but six cases of them resulted gun-stock deformity (reversed carrying angle) of the elbow, Two of these underwent supracondyIar wedge osteotomy and gained satisfactory corrections.

9.Measuring the Burden of Major Cancers due to Premature Death in Korea.

OBJECTIVE: To estimate the burden of diseases in Korea especially caused by major cancers using the YLL(years of life lost due to premature death) measurement. METHODS: First, we determined the parameters: such as age-specific standard life expectancy, age on death, sex, cause of death by analyzing the national death certificate data and life table collected during 1996 provided by the National Statistical Office. Secondly, we estimated the age group-specific YLL by employing standard expected years of life lost(SEYLL). Thirdly, final burden of disease due to premature death was estimated by using YLLs measurement which developed by global burden of disease study group. RESULTS: The burden of premature death by cancer for male was attributed mainly to liver cancer(514.5 person-year), stomach cancer(436.4 person-year), and lung cancer(367.7 person-year). Each of these cancers was responsible for the loss of over 100 person-year based on our YLL measurement. The burden of premature death by cancer for female was attributed mainly to liver cancer(135.1 person-year), stomach cancer(252.1 person-year), and lung cancer(121.8 person-year). Each of these cancers was responsible for the loss of over 100 person year based on our YLL measurement. CONCLUSION: We found the YLL method employed in this study was appropriate to quantify the burden of premature death. Thereby, it would provide a rational bases to plan a national health policy regarding premature death caused by cancer.

10.Two Cases of Tattoo Reaction.

Tattooing has been performed the for cosmetic purpose by many beautician in the beauty parlors without having been any medicalknowledge. This has resulted in various serious complications. One patient is a 56-year-old female, who had been developed pruritic erythernstous indurated plaques on the tattooing sites about 3 months after getting tattoo. Skin biopsy findings revealed granulomatous inflamrnation and patch test showed positive reaction to potsssiurn dichromate. The other patient is a 55-year-old female, who had been developed erythematous papules on the both lips of tattooing sites about 1 year after getting tattoo. Skin biopsy findings revealed lichenoid resction. Patch test showed positive reactions to reornycin eulfate and nickel sulfate. Patch test of as is revealed positive reactions to red color.
