1.Autoradiographic studies on some parasitic helminth.

Direct contact method of autoradiography was utilized in studying the distribution of exogenous C(14)-proline in Clonorchis sinensis, Eurytrema pancreaticum, Hymenolepis diminuta and Dipylidium caninum. The most distinct autoradiogram were obtained after 60 min of incubation, corresponding to the maximal absorption of C(14)-proline in these parasite. The radioactivitity of this labeled amino acid was chiefly concentrated in reproductive organs, especially egg-containing uterine tubules.

2.Hazardous Drinking and Its Association With Smoking and Ego-Resiliency in Medical Students

Objectives:
The aim of this study was to investigate the prevalence of hazardous drinking (HD) and its association with sociodemographic factors, smoking, social anxiety, self-esteem, and ego-resiliency in medical students.
Methods:
A total of 408 medical students were included in this study. Data were collected by self-administered questionnaire, and HD was assessed by Alcohol Use Disorder Identification Test (AUDIT). The HD and non-HD group were defined using the AUDIT score 10 for male and 6 for female as a cut-off, respectively. Multiple logistic regression analyses were carried out to examine the impact of sociodemographic factors, smoking, social anxiety, self-esteem, and ego-resiliency on HD.
Results:
A total of 192 participants (47.1%) were identified as having HD. Frequency of smoking and the severity of social anxiety were significantly higher in HD group than non-HD group. Meanwhile, the levels of self-esteem and egoresiliency were significantly lower than HD group than non-HD group. In multiple logistic regression, smoking was associated with a higher risk of HD, whereas ego-resiliency and age were associated with a lower risk of HD. Compared to the high level of subjective socioeconomic status, both middle and low level were associated with a lower risk of HD.
Conclusions
HD among medical students was quite common and associated with smoking, ego-resiliency, age, and level of subjective socioeconomic status. Strong efforts to improve smoking cessation and ego-resiliency, as well as education for HD in medical schools, is needed to reduce the harmful effects of alcohol.

5.A Case of Acute Anterior Uveitis Associated with HLA-B27 Positivity after Intravitreal Injection of Bevacizumab.

PURPOSE: Intravitreal injection of bevacizumab (Avastin(R)) can lead to several intraocular complications including endophthalmitis, hemorrhage and inflammation. We present one case of acute anterior uveitis with hypopyon in an HLA-B27(+) patient following intravitreal injection of bevacizumab. CASE SUMMARY: A 40-year-old male with known central retinal vein occlusion presented with redness and decreased visual acuity in the left eye. Symptoms had developed 4 days earlier after the sixth intravitreal injection of bevacizumab in that eye. The patient had a marked anterior chamber reaction with hypopyon and posterior synechiae. Nine days after onset, similar symptoms in the left eye occurred in the uninjected right eye. Through examination and clinical manifestations, we diagnosed acute anterior uveitis associated with HLA-B27 positivity. The patient was treated with 1% prednisolone acetate, 0.5% moxifloxacin and 1% atropine in both eyes and all symptoms had resolved after 6 weeks.

6.Parasitologial studies of Korean Forces in South Vietnam II. A comparative study on the incidences of intestinal parasites.

A survey of intestinal parasites was performed in South Vietnam. Samples were collected from 717 Vietnameses, 1,933 U.S. Forces, 433 stool specimens of Korean Troops in South Vietnam. And 114 of Korean Army patients in home land were also examined as a control. Findings were discussed with others and summarized as follows: A parasitic rate, incidence in general, was 44.8 %. Koreans in South Vietnam showed 82.4 % of infection rate, Vietnamese 64.6% and U.S. Forces 26.1 %. Korean home patients revealed 93.9 %. Following 10 species of helminths ova and 5 kinds of protozoa were found: Ascaris lumbricoides, Hookworm, Trichocephalus trichiurus, Strongyloides stercoralis, Trichostrongylus orientalis, Enterobius vermicularis, Metagonimus yokogawai, Clonorchis sinensis, Hymenolepis nana, Taenia sp., Entamoeba histolytica, Entamoeba coli, Endolimax nana, Giardia lamblia and Trichomonas hominis. The incidence of Ascaris lumbricoides among Koreans in South Vietnam was 15.9 %, much less than Korean home patients. The infection rate of A. lumbricoides in Saigon residents was 47.2% but in Chulai 36.8% and in Quinhon 35.8 %. Trichocephalus trichiurus was most frequently found in Koreans. Quinhon residents showed 78.7 % positive in hookworn infection and Chulai 24.7 %. In Saigon, on the contrary, infection rate was only 3.1 %. No Strongyloides stercoralis, but two cases of Trichomonas hominis were found among Koreans in South Vietnam. U.S. Forces showed higher incidences than Koreans and Vietnameses in the incidences of Entamoeba histolytica and Endolimax nana. Most of the U.S. Forces revealed single infection. Korean home patients showed the highest per centage of polyparasitism. Non-autochthonous infections in Korea could not be found among Koreans in this survey.

7.Parasitological studies of Korean forces in South Vietnam I. Examination of blood films on malaria patients.

A parasitological study was performed with 452 malaria patients evacuated from South Vietnam by examinations of their peripheral blood. Results were as follows: The peripheral blood examinations revealed that 52.0% of the examined have parasitemia, of which 95.3% was P. falciparum, one case of P. vivax and the other 10 patients were mixed infected. Neither P. malaria nor P. ovale were found. A total of 1,500 thick and thin blood films was prepared and 707 slides of them (47.1%) showed positive. In P. falciparum, ring forms were found most frequently and the next was gametocytes. Eighty slides (50%) showed mixed together with both ring form and gametocytes. All of the erythrocytic stages were seen in three slides of P. vivax. Weekly periodical examinations showed 233.8 parasite density every 1,000 W.B.C count in average, while occasional at fever attacks 531.7.Size of gametocytes in P. falciparum was 9.31(+/-0.89) by 2.16(+/-0.53) in macrogametocyte and 6.61(+/-0.82) by 2.51(+/-0.35) in microgametocyte. Their sex ratio was 100 : 92. Repeated blood examinations showed increased detection rates. The positive rate of parasitemia was 52.0% in single examination, increasing in succession with repetitions.

9.Management of Sequellae of Pyogenic Osteomyelilis

After discovery of Penicillin by Fleming in 1939, a great improvement in the treatment of osteomyelitis has been made and the mortality rate in acute stage was markedly decreased. But due to misplacement of antibiotics, and the improper treatment of the osteomyelitis, the tendency to chronicity and various sequellae of the osteomyelitis are still persist. So, it will be of importance to reconsider the management of various sequellae of pyogenic osteomyelilis. We reviewed 3 cases of sepsis, 10 cases of pathologic fracture, 6 cases of sepsis, 6 cases of malignant turnor developed in an old draining sinus, 2 cases of bone defect, 3 cases of joint contracture and 4 cases of deformity and growth disturbance 604 cases of osteomyelitis treated at National Medical Center between 1972 to 1983. The results were as follows: 1. Established sepsis was noted in 3 cases, and one of these patient dead. We treat these patients with high dose of the antibiotics accompanied by decompression of the lesion. 2. Pathologic fracture developed in 10 cases, which was treated with either conservative or operative treatment. All cases obtained good union except one. 3. Spuamous cell carcinoma developed in 6 cases around the old draining sinus. All cases were treated with amputation. Theres no recurrence except one. 4. Bone defect developed in 2 cases. A case of bone defect in humeral shaft was treated with vascularized fibular graft and obtained firm union and good function. 5. Joint contracture was treated with soft tissue release and even joint fusion in severe case. The results were not gratifying. 6. Deformity and growth disturbance of limbs were treated with soft tissue release, osteotomy and stapling. Most of the cases obtained good results, but limb length discrepancy was not recovered satisfactorily.

10.Clinical Results of Bipolar Endoprosthesis

In 1974, bipolar endoprosthesis was first introduced by Bateman & Giliberty. It has theoretical advantages including minimizing acetabular wear, possible reduction of incidence of stem loosening, lessening of dislocation and easy revision procedure. Various methods for the treatment of fracture of the femur neck have been developed & performed, but it is still called “the unsolved fracture” in certain situation. But the role of it as primary treatment of fracture of the femur neck continues to be controversial. Current clinical results encourage its continued use and expansion of indication. Authors reviewed and analysed 21 cases of bicentric endoprosthesis and 1 case of Giliberty endoprosthesis those have been operated at the Department of Orthopaedic Surgery, National Medical Center from 1981 to 1983. Following results were obtained, l. According to disorder distribution, 14 cases were neglected fracture of femur neck, 2 old fracture, 1 idiopathic femoral head necrosis and 2 avascular necrosis & 1 non-union complicated by femoral neck fracture. 2. Among 20 cases, 8 cases were operated within 1/2 1 month after injury and 6 cases within 1 3months 3. Harris lateral appmach was used in 16 cases (80%) and Modified Gibson approach in 4 cases (20%). There were no significant differences in morbidity during operation and postoperative complication. 4. We used femoral stems those head diameter was 22 and 32mm(Charnley & Miiller type) and head pieces those diameter was 48-51 mm in male and 44–48 mm in Ifemale. 5. 3 cases were stiff in inner bearing without acetabular erosion. 6. In 6 cases more than 2 years followup 1 degree CE angle of Wiberg was increased in 2 cases of youngage respectively without associated hip joint pain. 7. Although inclination of head piece was vertical in 8 cases, fracture and dislocation were not found. 8. The circumference of head piece and opposite femoral head was measured and compared. There were 12 cases (mean 12.6 mm) that of head piece larger than opposite femoral head and 3 cases smaller than opposite femoral head. 9. There was 4 cases of complication (3 patients) and still no reoperation and revision converted to T.H.R. 10. With regard to average 18 months of follow-up, 7 cases of 81–90 Harris hip-rating score, 6 of 91–100, 4 of 71–80 and 3 of 61–70.
