Recently, we experienced a patient with a B cell leukemia which could not be classified as chronic lymphocytic leukemia (CLL), prolymphocytic leukemia (PL) or follicular lymphoma in leukemic phase. He is a 58-year-old male and suffered from progressive lymphadenopathy at both inguinal and cervical areas. His blood film showed lymphocytosis (79%), and the cells were heterogeneous in size and shape. Twenty-five percent of the lymphocytes had nuclear irregularities or cleavages unlike the lymphocytes of CLL In which the cells often shows monomorphic features, small size with scanty cytoplasm and round nucleus. The bone marrow trephine biopsy specimen showed a diffuse pattern of infiltration of atypical Iymphoid cells. The lymph node histology showed atypical lymphoid colls proliferated as wide mantles around non-neoplastic appearing germinal centers. The immunophenotype of circulating lymphocytes in peripheral blood showed strong reactivity with CD5, CDl9 and CD20 without expression of CD2, CD3, CD7, CD10, CD22 and CD23. Although this case resembled CLL, the laboratory features showed major differences, notably in the Peripheral blood morphology, histology Patterns and the membrane Phenotype. By combining these data, we diagnosed this case as a leukemic phase of mantle cell lymphoma.

Journal of the Korean Radiological Society 1998;38(4):723-729
PURPOSE: To assess the utility of shoulder MRI by using ROC (receiver operating characteristic) curve in theevaluation of rotator cuff tear, anterior labral tear, superior labral tear and Hill-Sachs Lesion. MATERIALS AND METHODS: We evaluated 38 arthroscopically confirmed patients who had undergone shoulder MRI. According to thesignal intensity of the rotator cuff, as seen on T2 and proton density imaging, a five-stage grading system wasdevised. Labral tears were graded according to the separation of the labium; this was based on gleaned, morphologic and signal intensity changes of the labrum : six grades of anterior labral tear and three grades ofsuperior labral tear. Hill-sachs lesion was also classified into four grades according to morphologic and signalchanges of the humeral head. These findings were reviewed by two musculoskeletal radiologists and ROC curves andareas under the curve (Az) was abtained. RESULTS: The ratio of area under the curve of rotator cuff tear, anterior labral tear, superior labral tear and Hill-Sachs lesion were 82.4%, 88.%%, 62.4% and 91.3%, respectively. CONCLUSION: The Accuracy of shoulder MRI using ROC curve was relatively high in rotator cuff tear, anteriorlabral tear and Hill-Sachs lesion, but low in superior labral tear.

OBJECTIVE: The involvement of IGF system in hyperandrogenism and abnormal follicular development is controversial. This study is to assess whether IGF system contribute to it in the women with polycystic ovary(PCO). METHODS: Baseline serum levels of luteinizing hormone (LH), follicle stimulating hormone (FSH), estradiol (E2), testosterone (T), androstenedione (ADD), prolactin, thyroid stimulating hormone (TSH), free insulin-like growth factor(IGF)-I, free IGF-II, insulin-like growth factor binding protein(IGFBP)-1, and IGFBP-3 were measured in twelve healthy regularly cycling volunteers and forty-two women with PCO then, the changes of baseline serum levels were evaluated after laparoscopic ovarian electrocauterization in nine PCO patients. In addition, the expression pattern of IGF-I and IGF-II was examined in the ovary of control and PCO group. RESULTS: Baseline levels of LH, ADD, free IGF-II, and IGFBP-3 were significantly higher in PCO group. However, there were no significant differences in the levels of free IGF-I and IGFBP-1, although free IGF-I showed decreasing tendency in PCO group. And there was a significant positive correlation between the LH and free IGF-II level in the PCO(P=0.011, r2=0.3899), but not in the control. After ovarian electrocauterization, LH, T, and ADD levels decreased, and free IGF-I and IGFBP-3 level increased. While free IGF-II and IGFBP-1 level showed no significant changes. In the ovary, expression of both IGFs showed similar pattern in normal and PCO ovaries. CONCLUSIONS: The elevated IGFBP-3 level may alter the bioavailability of IGF(s) in the PCO. The change in IGF-I level and resumption of ovulation after electrocauterization, suggest a possible role of IGF system in the impairment of follicular development in the PCO.
