1.Studies on Pseudomonas Aeruginosa Isolated from Chronic Wounds

49 strains of Pseudomonas aeruginosa were isolated from chronic wounds associated with orthopedic surgical field at Taegu armed forces general hospital. In the present report, authors have studied the Pyocin type of isolated strains by Gillies and Govans method. Also observed various interrelationship in associaton with Pyocin type, Pyocyanine formation, Oxidase reaction and antibiotic sensitivity. The results were obtained as follows; 1) Among the 49 strains tested in term of pyocin type, 41 strains(81.6%) were found to be typable and 8 strains (16.4%) to be untypable. Among the pyocin type of 41 typables, unclassified type was the most prominent type 17 strains (41.5%) and followed by type I(11 strains), 10 (3 strains), 5 (2 strains) and other type. 2) In the relationship among pyocyanine formation, oxidase reaction and pyocin type, no definite conclusion can be reached due to the paucity of tested strains except that more strains belong to the pyocin type I of the Pyocyanine negative strains. 3) In antibiotic sensitivity of isolated strains tested by plate dilution method, most of all strains were resistant to Kanamycin and Neomycin but few strains were moderate sensitive to Colistin, Polymyxin B, and Terramycin. 4) The Pyocin type of isolated strains were not related with antibiotic sensitivity to Colistin, Polymyxin B and Terramycin substantially.

2.Three Cases of Neutrophilic Eccrine Hidradenitis.

Neutrophilic eccrine hidradenitis(NEH) was originally described in 1982 by Harrist et al. in a patient with myelogenous leukemia receiving chemotherapy. Clinically NEH represents various cutaneous manifestations with or without tenderness and pruritus. Histologic examination demonstrates a neutrophilic infiltrate within and around the eccrine gland and degeneration of the eccrine gland structures. Although the pathogenesis and possible cause of NEH remain unknown, it is probably an unusual cutaneous reaction to chemotherapeutic agents. A few cases of infection associated eccrine hidradenitis are found in the literature. We report three cases of neutrophilic eccrine hidradenitis. Two cases were associated with hematologic malignancy. The third case was associated with an infection of Vibrio vulnificus.

4.Recurrent Transitional Cell Carcinoma of the Urethra in Men having Cystectomy and Ileal Conduit for Bladder Cancer.

15 male patients who received total cystectomy with ileal conduit for bladder tumor were reviewed in relation to the incidence of urethral involvement. Of 15 patients six had recurrent transitional cell carcinoma in the urethral remnant among them one (1/15) received subsequent therapeutic urethrectomy and remaining 5 patients underwent delayed therapeutic urethrectomy after confirmation of urethral tumor but 9 of the 15 had only received total cystectomy. We concluded that the remaining urethra might constitute a potential hazard for further tumor formation. We believe that prophylactic urethrectomy is the way to prevent the recurrence of the malignancy who have multiple tumors, concurrent upper tract tumors, and involvement of the trigone or prostatic urethra.

7.Detection of Antibody to Candida albicans Proteinase in Sera from C. albicans - Cultured Patients and Healthy Controls.

To investigate whether anti-Candida proteinase antibody could be a diagnostic marker, we examined seroreactivity to proteinase in sera from 90 healthy controls and 8 of C. albicans culture-positive patients. Previously we purified proteinases of C. albicans, C. tropicalis, and C. parapsilosis using a series of chromatographic steps consisting of DEAE- Sepharose, Sephacryl S-200, and size-exclusion HPLC. ELISA and Western blot technique were adopted to examine seroreactivity of C. albicans proteinase with sera. On ELISA, the seroreactivities of healthy controls and C. albicans-cultured patients were 0.601 +- 0.014 (mean+SEM), and 0.695 +- 0.079, respectively (P=0.084, t-test). In C. albicans-cultured patients, the positive rate was 62.5% (5/8) and the positive rate of healthy controls was 39% (35/90). On Western blot analysis, C. albicans proteinase molecule was blotted by all sera tested. But the intensity of blotted band was different with the same dilution of sera; the intensity of C. albicans proteinase molecule band blotted by 2 sera of 3 healthy control's sera was distinctively lower than that by C. albicans-cultured patients sera. However, all sera including C. albicans-cultured patient's sera did not blot the proteinase secreted by C. tropicalis and C. parapsilosis. It is necessary to collect sequential sera of patients with candidiasis and to establish a cut-off value for ELISA or serum dilution for Western blot analysis that will give reliable test sensitivity and specificity.

8.Cervical Lymph Node Metastasis of Squamous Cell Carcinoma of the Oral Tongue and Floor of Mouth.

BACKGROUND AND OBJECTIVES: Squamous cell carcinoma of the oral cavity is a well known tumor with frequent nodal metastases, but reports on detailed prevalence of nodal metastases by primary subunits and neck level are lacking. PATIENTS AND METHOD: A retrospective review of 63 previously untreated patients (who visited the hospital from 1992 to 2001) with primary squamous cell carcinoma of the oral tongue and floor of the mouth, were undertaken to ascertain the prevalence of ipsilateral neck node metastases by neck level; of elective dissection in cN0 neck (n=43), and of therapeutic dissection in cN+ neck (n=20). RESULTS: Overall incidence of neck metastases, cancer of the oral tongue, and cancer of the floor of the mouth were 44.4%, 50% (24/48) and 26.7% (4/15), retrospectively. False negative and false positive rates were 27.9% and 20%, respectively. Neck metastasis was predominantly revealed in levels I, II, and III. Metastases at level IV and V were exclusively accompanied with metastases to other levels. CONCLUSION: Squamous cell carcinomas of the oral tongue and floor of the mouth metastasized to the neck frequently and neck metastasis correlated to T-stage. Elective treatment is needed to ipsilateral neck level I, II, and III. However, comprehensive neck dissection is needed for cN+ neck.

10.Percutaneous Nephrolithotripsy.

Recently, the modern surgical practice for the renal stone is being increasingly supplemented through the development of new technique. For example, hypothermic renal surgery, partial nephrectomy and extended pyelolithotomy were carried out by the ""therapy with the knife"". The percutaneous Nephrolithotripsy is one of the minimal invasive procedure to the renal stone surgery all over the world recently. This method is known as a safe, simple and reliable procedure for renal stone surgery, and they realize this technique as enormous advantage for patients; short hospital stay, minimal morbidity and rapid return to work etc. Three cases of renal stone through percutaneous nephrolithotripsy were carried out with much favour the first procedure in Korea. The first and third case were excellent result, but the second case was perforated on the uretero-pelvic junction of the kidney. There after, on the fifth postoperative day pyeloureteroplasty was performed with the splint of double J catheter. All the Patients were performed as the one session puncture, dilatation with telescopic dilator and lithotripsy with ultrasound lithotriptor. The two patients had 6th and 7th hospital day postoperatively.
