3.A Clinical Study of Colles' Fracture Treated by Closed Reduction and Sugar Tong Splint

We analyze ninty-seven cases of Colles fractures treated with closed reduction and sugar tong splint immobilization and followed for more than 1 year at Seoul National University from January 1981 to Decmber 1985. The results of this study were as follows; l. All of 97 cases were closed fractures and the incidence was high in females who aged over 6th decades. The main cause of this injury was slipping down accident comprising of 68.0%. 2. 63 cases (64.9%) were simple extra-articular fractures, and 34 cases (35.1%) were comminuted intra-articular fractures. 3. In a group of simple extra-articular fracture the functional end result was satisfactory in 88.9%, so closed reduction and sugar tong splint were thought to be a good procedure in this group. 4. In a group of comminuted intra-articular fracture the functional end result was satisfactory in 58.9% only. In the cases which show unacceptable loss of position during first two weeks of routine sugar tong splint immobilization we feel another technique is indicated, such as.pin and plaster immobilization or percutaneous pinning. 5. The main cause of unsatisfactory result was malunion, which was produced by loss of reduction position.

4.A study of the characteristics of craniofacial skeleton on orthognathic surgical cases with skeletal Class III malocclusion.

The purpose of this study was to evaluate the characteristics of craniofacial skeleton on orthognathic surgical cases with skeletal Class III malocclusion. For this study, 74 students at the dental college of Chosun University volunteered as a normal occlusion group. They had well-balanced faces and good occlusions with acceptable Class I molar relationship. They had not received orthodontic treatment and had no signs or symptoms of temporomandibular joint dysfunction. 45 malocclusion patients enrolled for orthognathic surgical treatment with skeletal Class III malocclusion at the Department of Orthodontics, College of Dentistry, Chosun University. On the basis of this study, the results of this study were as follows: 1. Skeletal Class III malocclusion was largely due to the overgrowth of mandible in man and the undergrowth of maxilla in woman. 2. The mandible was antero-inferiorly overgrown by large MP-HP angle and large gonial angle in orthognathic surgical cases with skeletal Class Iii malocclusion. And also, upper incisors were severely labioversioned, but on the other hand lower incisors were linguoversioned. 3. In female, lower-third facial height was characteristically shortened in comparison with middle-third facial height and also, lower facial throat angle was sgrall in male.

5.Usefulness and Limitation of 24 Hour Reinjection Images to Assess Myocardial Viability in Patients with Acute Myocardial Infarction.

OBJECTIVE: The study was performed to evaluate whether thallium reinjection (RI) distinguishes viable from nonviable myocardium among myocardial segments which showed persistent perfusion defect (PD) in patients with acute myocardial infarction (AMI). MATERIALS AND METHODS: We studied 22 patients underwent PTCA after AMI. SPECT was performed in all patients using dipyridamole stress- 4 hour redistribution (RD) followed by 24 hour RI protocols. Dysfunctional segs were classified into 5 groups: 1) normal, 2) reversible, 3) mild to moderate PD, 4) severe PD and 5) reverse redistribution (RR). All patients underwent follow up echocardiography after 4 months to assess regional wall motion (WM) improvement such as a criteria of viable myocardium. RESULTS: A total of 127 segs with abnormal WM was analyzed. Of 74 segs with PD, 17 (23%) showed enhanced uptake after 24 hour RI. Five of 17 segs (29%) with PD that responded to RI with enhanced thallium uptake showed WM improvement. WM improvement were seen in the 24 of 57 segs (42%) not responding to RI. All four segs (100%) with RR that responded to RI showed improvement. WM improvement were not seen in the 5 of 8 segs (71%) with RR not responding to thallium RI. Eleven (73%) of 15 segs with mild-moderate PD after RI showed improvement, but 33% of segs with severe PD after RI did not showed improvement. Segs with mild-moderate PD after RI and fill in after RI showed improvement in comparison to segs with severe PD after RI(p<0.05). CONCLUSIONS: These data indicate that because only small proportion of PD showed further perfusion improvement after RI and predictive value by the uptake after RI was low, there was limited role of RI after myocardial infarction. Usefulness of RI could be found in segs showing RR responding to RI in AMI reflects viable myocardium.

6.Histologic study of chemical peel in guinea pig after pretreatment with tretinoin and tretinoin-combined pretreatment kit.

The purpose of this study was to evaluate the histologic difference that occured after trichloroacetic acid(TCA) chemical peel in an animal model that was pretreated with Tretinoin alone or Tretinoin-based combined pretreatment kit. Eight Hartley white guinea pigs were used in our study. The dorsal skin of the guinea pigs was divided into six equal squares(2x2 cm). Upper two areas of these six were not pretreated, middle two areas were pretreated for 4 weeks with Tretinoin alone and lower two areas were pretreated for 4 weeks with Tretinoin-based combined kit. Each guinea pig underwent chemical peel with 50% TCA. The wounded areas were biopsied at post-peeling 3 weeks and 6 weeks. The histology revealed that those animals pretreated with combined kit healed quicker than the animals pretreated with Tretinoin alone. This study implies that if patients are treated with Tretinoin-based-combined pretreatment kit before undergoing chemical peel, the pretreatment time will be shortened.

8.Comparisons of Early MRI Patterns with Arthroscopic Findings of Possible Acute ACL Tears.

Early MRI and arthroscopic findings were compared to evaluate the efficacy of the former for patients, who upon physical examination, were diagnosed with possihle acute ACL tears. MRI, taken between 1-21 days after injury, had to reveal complete ACL tears and arthroscopic surgery should have taken place 3-12 weeks after the injury for patients of this prospective study. Fifty such cases were initially catagorized according to five MRI patterns. Types I to V were separated by the following ACL tear characteristics: Enlarged and diffusely increased in signal, horizontally oriented, non-visualized, discontinuous and vertically oriented. The cases were then divided into two groups of arthroscopic findings; Group A included cases of complete ACL tearing and group B included those with intact or minimally torn ACLs. Of MRI types I through V, there were 31, 9, 5, 4, and I cases of each respectively. 37 cases resulted in complete ACL tears (21 type I, 6 type II, 5 type III, 4 type IV and 1 type V) and 13 cases were catagorized into group B, 10 of which had pattern type I and 3 of which had pattern type II. As result, types III, IV and V had a 100% Positive Predictive Value (PPV) whereas types I and II together had a 67% PPV vaiue. Division of the cases into the five pattern types in early MRI proved valuble for possible cases of acute ACL tears. MRI pattern types III, IV and V were correlated with definite complete ACL tears. However, caution must be taken for types I and II, the majority of the cases, for which there exists a significant possibility for misdiagnosis and consequently unnecessary arthroscopic surgery.

9.Operative Treatment for Nonunions of Distal Radial Fracture: A Report of 7 Cases.

PURPOSE: To report the treatment results of 7 cases of distal radius nonunion with a review of the relevant literature. MATERIALS AND METHODS: Seven patients treated with an autoiliac bone graft and rigid internal fixation for nonunion of the distal radius were analyzed retrospectively for the cause of injury, the factors affecting nonunion, radiological findings, treatment method and complications. The results were analyzed radiologically using Kreder's method and functionally using the Anderson' protocols. RESULTS: Union was achieved in all cases after a mean period of 19 weeks. The functional result of treatment at the last follow-up was excellent in 4 cases and satisfactory in 3 cases. The probable factors of nonunion were instability of the fracture site in three cases, type II or III open fracture in 2 cases, postoperative infection in one case and idiopathic in one case. CONCLUSION: Infection control using a stepwise operation, rigid internal fixation and autogenous iliac bone graft showed satisfactory results in distal radius nonunion, which had developed in those with severe open fractures, postoperative infection, instability on the fracture site and associated distal ulnar fracture.

10.Clinicopathologic Characteristics and Local Recurrence after Skin-sparing Mastectomy with Immediate Reconstruction.

PURPOSE: For those women with breast cancer who require mastectomy for surgical treatment, consideration should be given to optimize the cosmetic outcome. Skin-sparing mastectomy (SSM) is being used more frequently to treat many of these patients. Skin-sparing mastectomy can maximize breast skin preservation and facilitate immediate reconstruction, and so result in an excellent cosmetic appearance. The aim of this study was to access the local recurrence rate of breast cancer after SSM. METHODS: A retrospective review was conducted of 53 patients who underwent skin-sparing mastectomy with immediate reconstruction between January 1996 and February 2006. Immediate reconstruction was achieved via the TRAM flap or latissimus dorsi flap or artificial bag. The mean follow-up time was 34.6 months (range: 7~142 months). RESULTS: Local recurrence occurred in 1 (2.1%) of 53 patients. The time to local recurrence was 44 months. The patient with local recurrence was well controlled by wide excision and postoperative chemotherapy and radiotherapy. The patient with local recurrence is still free of disease. Distant metastasis occurred in 7 (14.9%) of 53 patients. Of the 7 patients with distant metastasis, 1 patient was died from brain involvement. CONCLUSION: The risk of local recurrence after skin-sparing mastrectomy was not different from that of conventional mastectomy. Local recurrence was effectively managed with surgical excision of the involved tissues and then administering chemotherapy or radiotherapy.
