5.Cryopreservation of Day 3 Mouse Embryos by Vitrification.

The use of hormonal stimulation in human in vitro fertilization and embryo transfer (IVF-ET) leads to increased production of embryos for ET. So to avoid high pregnancies and to allow conception in future, unstimulated cycles, cryopreservation of spare embryos is desirable. One of the improvement of cryopreservation methods is vitrification. We cryopreserved mouse day 3 embryos by vitrification using the three different vitrification solution (EFS40, VSll and VS3a). EFS40 solution is consisted of 40% (v/v) ethylene glycol, Ficol170 30% (w/v) and 0.5M sucrose and VSll is 6.0M ethylene glycol and 1.8M glycerol. And VS3a is 6.5M glycerol and 6% (w/v) BSA (bovine serum albumin). First we tested the toxicity of three vitrification solution by exposure to these solution during 3 min. After washing by thawing solution, the survival rates of each groups are 95.5%, 90.9% and 84.4% (EFS40, VS11 and VS3a). High percentages of them developed to expanded blastocyst and hatching embryos in culture 48hrs 94.2%, 97.7%, 100% and 97.4% (no treatment group, EFS40, VS11 and VS3a). So there is no significant differences among the each group. Second, after thawing of vitirfied embryos, the survival rates of each groups are 96.8% (slow freeze), 94.1% (EFS40), 85.5% (VS11) and 80.0% (VS3a, P vs. no freeze or EFS40 is 0.01). Vitrified embryos exhibited a high rate of development in vitro after 48hrs culture. The percentages of each group to blastocyst and hatching embryos are 88.7% (no freeze), 91.8% (slow freeze), 93.4% (EFS40), 87.7% (VS11) and 73.0% (VS3a, P vs. other group is 0.01). The results suggest that there is no significant differences in exposure of various vitrification solution and day 3 mouse embryos can be vitrified in solution EFS40 and VS11 by simple procedure.

6.Tarsal Tunnel Syndrome: A Case Report

Tarsal tunnel syndrom is characterized by pain, paresthesias in the foot in either the entire distribution of the tibial nerve or the distribution of one or two of its major branches and caused by compression of the posterior tibial nerve as it passes posterior and inferior to the medial malleolus. This syndrome is not recognized as readily as its counterpart in the upper extremity, the carpal tunnel syndrome. Anatomically unlike the transv erse carpal ligament the laciniate ligament(flexor retinaculum) has several deep fibrous septa which blend with the periosteum covering the medial side of the calcaneus. The neurovascular bundle in the tarsal tunnel is often attached to some of these septa, rendering itself more liable to minor degrees of traction on movements of the foot. A case of tarsal tunnel syndrome, the patient 24 years old policeman, treated succesfully with surgical release of laciniate ligament is reported.

8.Urachal Adenocarcinoma -Report of Two Cases-.

Urachal carcinoma is an uncommon neoplasm associated with a poor prognosis. We have encountered two another cases of urachal adenocarcinoma recently 5 years after our first report. The common sign of urachal cancer was painless hematuria. We diagnosed the specific site of origin preoperatively by means of computerized tomography. One patient was performed partial cystectomy and another was total cystectomy with double barrel ureterocutaneostomy. A brief review of the literature of urachal adenocarcinoma was also made.

9.Pulmonary Cavernous Hemangioma: A case report.

The pulmonary cavernous hemangioma is usually from birth and there may be without symptoms until adulthood. Larger or multiple pulmonary angiomata with considerable pulmonary arteriovenous shunts may cause cyanosis, finger clubbing, dyspnea and frequently accompanyingbruit. Recently, we experienced a case of cavernous hemangioma of the lung. A 34-year-old woman was admitted to our hospital for surgical evaluation of a 4 cm solitary, round nodule in the right upper lobe on the chest X-ray and CT scan. She had no symptoms. Laboratory findings are within normal limits except for elevated glucose levels. At surgery, the mass was well encapsulated and easily excised from the peripheral portion of the posterior segment of the right upper lobe. Grossly, it consisted of a 4 cm in diameter, round, soft, sponge-like, hemorrhagic, slightly lobulated mass with a smooth external surface. Microscopically, the mass was composed of vessels, which were thin walled, dilated and filled with blood. The wall of the abnormal vessels was thin and composed of endothelium and fibrous connective tissue with only a little smooth muscle. Immunohistochemically, the wall of the dilated abnormal vessesls showed negative reaction for cytokeratin(low and high) and epithelial membrane antigen but weakly positive reaction for UEA-1 in focal areas.

10.A Case of Human Immunodeficiency Virus Infection Associated with Syphilis.

We report a case of HIV, infection associated with syphilis. A 40-year-old heterosexual man presented with headache, malaise, and a cutaneous lesion consisting of a whitish patch and petechiae on the hard palate. The serum VDRL titer wss positive at 1:64 and the cerebrospinal fluid VDRL titer was negative. Anti-HIV antibodies were detected in the patient sera by particle agglutination test and confirmed by Western blot test.
