2.Analysis of Histological Findings and Estrogen and Progesterone Receptor Status in Luteal Phase Defect Caused by Delayed Ovulation and Clomiphen Treatment.

Luteal phase defect (LPD) is an ovulatory disorder of considerable clinical importance that leads to delayed endometrial maturation and inadequate endometrium for blastocyst implantation. This disorder is implicated in infertility and recurrent spontaneous abortion. We analyzed the endometrial histological maturation, and the estrogen receptor(ER) and progesterone receptor(PR) status in patients with unexplained delayed ovulation, and in patients with ovulation induction by clomiphen citrate. The purpose of this study was to determine whether the length of the follicular phase influences the endometrial histological maturation and the hormonal receptor status, and to know the effect of clomiphen citrate on the endometrium. In the delayed ovulation group, the endometrium was characterized by an irregular outpouching and dilated lumina of the glands, a decreased secretory activity and predecidualization, and a decreased number of granulocytes in the stroma. In the clomiphen citrate-ovulation induction group, glandular proliferation was markedly diminished with poorly convoluted and narrow glands, secretory activity was decreased, stroma was undecidualized, and there was an absence of granulocytes. ER expression was increased in the glandular epithelium in the delayed ovulation group, and both ER and PR expressions were markedly decreased in the glandular epithelium and stromal cells in the ovulation induction group. Endometrial maturation and differentiation may be diminished by increased estrogenic and relatively deficient progesterone effects in patients with delayed ovulation. The hormonal receptor status might also be influenced by hormonal changes. Clomiphen citrate successfully induced ovulation in patients with delayed ovulation, however, endometrial proliferation and maturation were markedly suppressed. This might be related to the lower pregnancy rate and higher abortion rate in patients with clomiphen citrate induced ovulation.

4.Status of intestinal parasitic infections in a remote island, Yondo, Jeonranam-do(province).

This study was performed to observe the present status of intestinal parasitic infections in a remote island, Yondo, located in southern part of Jeonranam-do(province), Korea. In February and May 1988, total 1,011 individual stool samples were collected and examined for intestinal helminths and protozoa using formalin-ether centrifugal sedimentation technique. The results are summarized as follows: Of 1,011 ingabitants examined, 398(39.4%) were positive for intestinal parasites. Helminth positives were 372(36.8%), and protozoan cyst positives were 56(5.5%). Ten species of parasites were found. Trichuris trichiura revealed the highest infection rate of 27.5%, Ascaris lumbricoides 17.4%, Taenia sp. 5.8%, Entamoeba coli 3.3%, Giardia lamblia 1.5%, Endolimax nana 0.8%, Hymenolepis nana 0.4%, hookworm 0.2%, Trichostrongylus orientalis 0.2%, and Entamoeba histolytica 0.2%, respectively. The female group showed higher positive rate (44.0%) than males (34.7%). Also, higher positive rates were observed among adults as compared with the group younger than 10 years old. Average value of E.P.G. was 1,876(range 200-17,800) in A. lumbricoides positives, and 327(range 200-1,600) in T. trichiura positive cases. In helminth egg positive cases, single infection was 63.4%, double infection 34.7%, and triple infection 1.9%, respectively. Among protozoan cyst positives, single infection was 94.6%, and double infection was 5.4%. The present study revealed that the prevalence of intestinal parasites among inhabitants in Yondo island is still so high that special control measures should be performed.

6.Ultrastructural Findings of Cultured Ciliary Nerve.

PURPOSE: To investigate the cellular changes and cellular characteristics of myelinated nerve fiber, unmyelinated nerve fiber and Schwann cell under the culture conditions of the explant of the long ciliary nerve. METHODS: The ciliary nerve was obtained from the eyecup after removal of the cornea and intraocular contents following the division of the eyeball at the limbal zone. The explant of the ciliary nerve was cultured with DMEM medium containing nerve growth factor in an incubator with a 5% CO2 atmosphere. The cultured ciliary nerve was examined with phase contrast microscopy at 35 days after culture and stained in the culture dish with 1% methylene blue. The cultured ciliary nerve was also examined with transmission electron microscope. RESULTS: The phase contrast microscopy showed monolayered Schwann cells and cells with thin cytoplasmic processes. The ultrastructure showed myelinated nerve fiber, unmyelinated nerve fiber and Schwann cells entrapped in the nerve fiber. The axoplasm of the myelinated nerve fiber showed the degeneration of microorganelles whereas the unmyelinated nerve fiber showed well preserved microorganelles such as neurofilaments and mitochondria within the axoplasm. CONCLUSIONS: These findings suggest that the axoplasm of the unmyelinated nerve fiber under these culture conditions is relatively better preserved than that of the myelinated nerve fiber probably due to the absence of the myelin sheath. Further study will be required to investigate whether the myelin sheath of the ciliary nerve derived from peripheral neuropathy may affect the functional change of the anterior segment of the eye.

Hemophagocytic syndrome is a rare complication of a systemic infection that can have a fatal outcome. The skin manifestations can present as erythematous patches or nodules, purpura, ulcers, or edema. But the histopathologic features solely consist of infiltration of benign histiocytes and hemophagocytosis. We report a case of a 67 year-old male patient diagnosed with hemophagocytic syndrome caused by Pseudomonas aeruginosa infection.

9.Secondary Erythromelalgia: A Case Report.

Erythromelalgia is a rare neurovascular pain syndrome characterized by a triad of redness, increased temperature, and burning pain primarily in the extremities. Erythromelalgia can present as a primary or secondary form, and secondary erythromelalgia associated with a myeloproliferative disease such as essential thrombocythemia often responds dramatically to aspirin therapy, as in the present case. Herein, we describe a typical case of a 48-year-old woman with secondary erythromelalgia linked to essential thrombocythemia in the unilateral hand. As this case demonstrates, detecting and visualizing the hyperthermal area through infrared thermography of an erythromelalgic patient can assist in diagnosing the patient, assessing the therapeutic results, and understanding the disease course of erythromelalgia.
