3.Clinical Studies on Spondylolisthesis

Spondylolisthesis is characterized by forward displacement of the vertebral body with bilateral defects of the partes interarticulares. Because the progression of slipping occurs in young adults, difficult problems are present in the treatment of this condition. Watkins(1953) described the technique of posterolateral fusion between transverse processes, which was accepted in the dynamics of spine motion. In this procedure, grafted bone is placed close to the center of axis of the spine motion and so, bony fusion is not affected by flexion motion but distracted by a Iateral shearing force. The role played by the Norton-Brown back brace is in the restriction of lateral bending. However it pormits somewhat flexion motion of the spine. It is suggested that early ambulation may be possible without the interference of bony union after the posterolateral fusion operation when this brace is applied instead of longterm cast immobilization. In clinical analyses and follow-up studies after operative treatments in 55 patients with spondylolisthesis, the following results were obtained: 1. 30 patients (54.6%) were distributed between 21 and 40 years of age. 2. Males (52.7%) were slightly more than females (47.3%). 3. Among clinical symptoms and signs, low back pain was most common (87.3%). Sciatica was present in 45.5% 4. Duration of symptoms at admission was variable. 5. 15 patients (27.3%) had a previous history of trauma. 6. 5th lumbar vertebra was most commonly involved (74.5%) followed by the 4th lumbar vertebra(23.6%). There was a case(1.9%) in which the 5th and 4th lumbar vertebrae were involved. 7. Degrees of slipping were divided into 4 grades (Grade I–IV) by Meyerdings method. Grade 1 was most common (72.2%), Grade II 22.2% and Grade III 5.6%. 8. In follow-up studies of the operative treatment, laminectomy & H-graft were satisfactory in 84.9% and posterolateral fuion in aII cases (13 patients). 9. Early ambulation was preferred without healng disturbance in a case in which the Norton-Brown back brace was applied after posteroIateraI fusion and also, other advantages were expected as follws: a) Elimination of prolonged cast immobiIization b) Financial aid c) Early return to work d) Mental comfort.

5.Medullary carcinoma of the breast: Imaging findings characteristics vs histologic classification.

It is well known that the medullary carcinoma of the breast is one of the special types of breast carcinoma with a good prognosis. At present, the medullary carcinoma of the breast is subclassified into 3 types: typical medullary, atypical medullary and nonmedullary carcinoma. Among them, the former has the best prognosis. We reviewed the film mammographic and ultrasonomammographic findings of 13 patients according to the reevaluated histopathologic diagnosis. Typical medullary carcinoma shows a well circumscribed mass with surrounding halo on film mammogram, and well defined mass with central intermediate echogenicity and peripheral low echogenicity and posterior acoustic enhancement on ultrasonomammogram. Atypical medullary carcinoma shows relatively well circumscribed mass with partial marginal obliteration on film mammogram, and irregular bordered mass with inhomogeneous echogenicity due to focal necrosis in the mass and associated findings of thick boundary, asymetrical lateral shadowing on ultrasonomammogram. Nonmedullary carcinoma shows lobulated mass with surrounding parenchymal distortion and skin thickening on film mammogram, and relatively well defined lobulating mass with surrounding parenchymal distortion and marked heterogeneous internal echogenicity on ultrasonomammogram. Therefore, differentiation between typical medullary carcinoma with good prognosis and atypical medulary or nonmedullary carcinoma with poor prognosis, may be possible by various diagnostic imaging modalities preoperatively. But further collective study shall be needed in near future.

6.A calpain inhibitor protects against fractalkine production in lipopolysaccharide-treated endothelial cells.

BACKGROUND: Fractalkine (CX3CL1) is a chemokine with a unique CX3C motif and is produced by endothelial cells stimulated with lipopolysaccharide (LPS), tumor necrosis factor (TNF)-α, interleukin (IL)-1, and interferon-γ. There have been several reports that the caspase/calpain system is activated in endotoxemia, which leads to cellular apoptosis and acute inflammatory processes. We aimed to determine the role of the caspase/calpain system in cell viability and regulation of fractalkine production in LPS-treated endothelial cells. METHODS: Human umbilical vein endothelial cells (HUVECs) were stimulated with 0.01–100 μg/mL of LPS to determine cell viability. The changes of CX3CL1 expression were compared in control, LPS (1 μg/mL)-, IL-1α (1 μg/mL)-, and IL-1β (1 μg/mL)-treated HUVECs. Cell viability and CX3CL1 production were compared with 50 μM of inhibitors of caspase-1, caspase-3, caspase-9, and calpain in LPS-treated HUVECs. RESULTS: Cell viability was significantly decreased from 1 to 100 μg/mL of LPS. Cell viability was significantly restored with inhibitors of caspase-1, caspase-3, caspase-9, and calpain in LPS-treated HUVECs. The expression of CX3CL1 was highest in IL-1β-treated HUVECs. CX3CL1 production was highly inhibited with a calpain inhibitor and significantly decreased with the individual inhibitors of caspase-1, caspase-3, and caspase-9. CONCLUSION: The caspase/calpain system is an important modulator of cell viability and CX3CL1 production in LPS-treated endothelial cells.

7.Experimental Study on Representation of Flow on the Bifurcated Carotid Arterial Phantoms Using Magnetic Resonance Angiography.

PURPOSE: A common finding of carotid artery on magnetic resonance angiograms(MRAs) is a signal dropout along the posterior wall of carotid bulb due to reverse flow. The purpose of this study is to evaluate variable flow patterns on bifurcated carotid arterial phantoms using steady-state flow. MATERIALS AND METHODS: We designed phantoms of a bifurcated carotid artery with acrylic materials. Flow patterns were evaluated with axial and coronal imaging of MRA(2D-TOF, 3D-TOF), color Doppler imaging, and computational fluid dynamics (CFD) within the phantoms constructed of an automated closed-type circulatory system filled with 4% sugar solution. These findings were compared with findings obtained from normal volunteers. RESULTS: Axial 3D-TOF MRA images exhibited closer resemblance to the contour of the inner wall of phantoms when compared to coronal 2D-TOF MRA imaging. However, 2D-TOF MRA showed good contrast difference of signal intensities between forward flow area and reverse flow area. Dark zones with reduced signal intensities due to reversed flow were separated from the outer wall of the internal and external carotid arteries by a thin layer of forward flow along the wall on the source slice image of MRA. The general hemodynamics of the phantoms on MRA were identical to hemodynamics on color Doppler imaging and CFD. The results obtained with the phantoms matched the findings on normal volunteers. CONCLUSION: Although representations of bifurcated carotid arterial phantoms on axial 3D-TOF MRA were excellent if ideally desinged, the zone of reversed flow could be a significant factor in creating distorted image when the zone of reversed flow contacted directly with curved or deformed arterial wall.

8.Study on the trematode parasites of the birds in Korea.

Authors carried out an investigation of the helminthic parasites of the wild birds in Korea. Most wild birds were collected from Kyoung Sang Do, Kyoung Kee Do and Seoul area from September 1970 to March 1973. A total of 70 wild birds, 19 species were examined, which comprised as follows; 10 Anas platyrhynchus, 10 Anas fulicata, 4 Anas poeciolorhyncha zonorhyncha, 8 Anser albifrone albifrone, 5 Anser cygonopsis cygnoid, 10 Aythya fuligula, 6 Gallicrex cinerea, 2 Porgana fusca, 2 Phisianum calichius korponri, 4 Sterptopilla orientalis, Podiceps cristatus, Larus crasirotatus, Malantta fusca, Garis artcus, Buetes buteo, Egretta alba, Starmus philpiens, Bombycilla granulus. Trematode parasites found from six species of the wild birds. Echinostomatidae has been found from intestine of the Anas platyrhynchus platyrhynchus, Anas poeciolorhyncha zonorhyncha, Anas fulcata. Notocatylus attenuatis found from intestine of the Podiceps cristatus, Anas fulcata. Catylurus japonicus was found from rectum of the Anas platyrhynchus platyrhynchus, Anas poeciolorhyncha zonorhyncha.
