5.Expression of TGF -beta I and II Ligands and Receptors at Epiphyseal Plate and Fracture Callus.

To understand the expression of hoth TGF-beta l and II ligands and the receptors, artificial fracture was made on rat femur. Fracture callus and epiphyseul plate were stained immunohistochemically on 3rd. 7th, 14th, 21st, 42nd and 56th day after trauma. Polyclonal antibody was used to stain TGF-beta I and II ligands and receptors. At epiphyseal plate, both ligand and receptor were expressed from each cell in proliferating and maturing zone. But there was no difference between type I and II except expression time. TGF-beta II ligand and receptor were expressed earlier: they were expressed mostly by the cells at the zone of proliferating cartilage but TGF-beta1 ligand and receptor were expressed mostly hy the cells at zone of maturing cartilage. At fracture site, TGF-beta expression was observed from 3rd day after trauma and it reached its maximum intensity at 2 weeks. It decreased thereafter and disappeared at 6 weeks after trauma. In enchondral ossification area, TGF-beta expressing cells were scattered throughout the enchondral mass. In intramembranous ossification area, the ligands and receptors were expressed from the osteohlasts just heneath the periosteum. ln summary, TGF-beta ligands and receptors were expressed at epiphyseal plate and fracture callus. There was no difference between TGF-beta 1 and 2 expres.ion except the appearance time at epiphyseal plate. We could not draw any conclusion about ligand and rcceptor mechanism with this immunohistochemical staining.

6.The Value of the Dermatomal Somatosensory Evoked Potentials in Patients with Lumbosacral Radiculopathy.

OBJECTIVE: The purposes of this study were to evaluate the diagnostic value of dermatomal somatosensory evoked potentials (DSEPs) in the unilevel/unilateral lumbosacral radiculopathies. METHOD: The study was performed on 41 patients with herniated lumbosacral disc which was confirmed by magnetic resonance imaging, and the patients with clinical lumbosacral radiculopathies (L5 radiculopathy in 33 cases and S1 radiculopathy in 8 cases). Stimulation sites were over the dorsum of the foot on the distal fifth metatarsal bone for the S1 dermatome and at the interdigital web space between first and second toe for the L5 dermatome. Recordings were made at Cz' and reference to Fz. Conventional nerve conduction study, needle EMG and H-reflex were also examined. RESULTS: While the needle EMG showed abnormalities in 32 patients (78.0%), the abnormalities of DSEPs were in 13 patients (31.7%): 33.3% for the L5 radiculopathy and 25.0% for the S1 radiculopathy, respectively. Moreover, there was no significant relationship between the abnormal findings of needle EMG and DSEPs (p>0.05). The H-reflexes were abnormal in 6 of 7 patients (85.7%). And then two of them were found abnormal in S1 DSEPs. CONCLUSION: The conventional needle EMG appears to be the more useful electrophysiological technique in the diagnosis of lumbosacral radiculopathies. The ultimate diagnostic value of DSEPs in lumbosacral radiculopathies is doubtful and controversial.

7.Propofol Infusion Associated Metabolic Acidosis in Patients Undergoing Neurosurgical Anesthesia: A Retrospective Study.

OBJECTIVE: Propofol and volatile anesthesia have been associated with metabolic acidosis induced by increased lactate. This study was designed to evaluate changes in pH, base excess (BE), and lactate in response to different anesthetic agents and to characterize propofol infusion-associated lactic acidosis. METHODS: The medical records of patients undergoing neurosurgical anesthesia between January 2005 and September 2012 were examined. Patients were divided into 2 groups : those who received propofol (total intravenous anesthesia, TIVA) and those who received sevoflurane (balanced inhalation anesthesia, BIA) anesthesia. Propensity analysis was performed (1 : 1 match, n=47), and the characteristics of the patients who developed severe acidosis were recorded. RESULTS: In the matched TIVA and BIA groups, the incidence of metabolic acidosis (11% vs. 13%, p=1) and base excess (p>0.05) were similar. All patients in the TIVA group who developed severe acidosis did so within 4 hours of the initiation of propofol infusion, and these patients improved when propofol was discontinued. CONCLUSIONS: The incidence of metabolic acidosis was similar during neurosurgical anesthesia with propofol or sevoflurane. In addition, severe acidosis associated with propofol infusion appears to be reversible when propofol is discontinued.
