3.Vascular Injuries Associated with the Fractures About the Knee

Fractures about the knee including the distal femur and the proximal tibia not infrequently result in vaseular injuries of the popliteal artery and the distal part of the femoral artery. This is often followed by circulatory disturbance and jeopadizes the viability of the lower extremity. During the three and a half year period, from January 1973 to June 1976, 20 cases of vascular injuries complicated by fractures about the knee were treated at the Orthopedic Department of Busan National University Hospital. Review of the cases resulted in the following observations. 1. The lower extremity escaped amputation in 45% the cases (9 of the 20 cases) 2. Femoral arteriography showed that the site of injury was proximal to the Soleus insertion. in 17 cases. 3. Types of vascular injury were contusion in 9 cases, laceration in 7 and total disruption in 4. 4. 5 of the 6 extremities that were treated within 12 hours after injury survived, while only 2 out of t he 9 extremities treated after 24 hours following injury escaped amputation. 5. 6 of the 13 extermities (46%) tredted by interval fixation survived, while 1 out of 5 extremities (20%) treated by traction escaped amputation.

4.Congenital Hepatic Fibrosis with Caroli's Disease.

Congenital hepatic fibrosis is an inherited, congenital disorder of the liver, and is occasionally associated with cystic disease of the liver and kidney. We present a case of congenital hepatic fibrosis with Caroli's disease. A 21-year-old woman had suffered from an episodic fever with headaches for 3 years. In laboratory examination, the liver function test was within the normal limits. Esophageal varix was noted by an endoscopic examination. Hepatosplenomegaly and multiple dilated bile ducts were seen by abdominal CT scanning. An orthotopic whole liver transplantation was done. The liver was fibrotic and enlarged. Multiple cystically dilated intrahepatic ducts were noted. Microscopically, diffuse portal fibrosis and widening with proliferation of bile ductules were seen. Intrahepatic bile ducts were markedly dilated and tortuous. The liver cell cords were well preserved.

5.Review of Group Psychotherapy: For Its Activation of Practical Applications.

In Korea Mental Heath Law came into operation in 1997, and the management of the mentally ills has become an important issue and it is inevitable to train mental health workers for the management of the mentally ills. The purpose of the management seems not only to care the patients' symptoms but also to improve the quality of life. Nowadays, in the best approach of treatment and management it is difficult that one to one management is used because of the cost limitation and management efficacy. So it is highly recommended that the group approach should be used. Hence 'the activation of group psychotherapy' is very important for the effective treatment and efficient management of the mentally ills. And therapists should know group psychotherapy well and to use it properly. In these viewpoints, we first introduced various types of group psychotherapy generally, including Yalom's group psychotherapy which focus on the interpersonal relationship and is the representative type of group psychotherapy. And then we classified group psychotherapy simply, describe the concepts of practical application of group psychotherapeutic approach. And last on the basis of understanding group psychotherapy in this paper, we explored the necessity of its activation on the practical setting and the surrounding circumstances.

8.Urethral Trauma -Clinical Experience of Visual Urethrotomy on Urethral Stricture-.

A clinical evaluation has been undertaken of 41 patients with urethral stricture who have been treated with visual urethrotomy in the Department of Urology, Seoul National University Hospital during 3 years periods from 1983 to 1985. The over all success rate was 54%. The results were unrelated to the sites of the lesion, the causes of the stricture and the durations of the catheter drainage. However, the results of the obliterated strictures (12.5%) are more poor than the non-obliterated strictures (64%), and patients with nonobliterated stricture less than 2cm long had the best results (22%). The overall morbidity rate was 20%, six cases of false way, one case of incontinence, one case of penile hematoma. Twenty seven cases were followed up over 6 months and its success rate showed 44.4%.

9.Culture of Nail Matrix Cells.

BACKGROUND: In vitro epidermal keratinocytes(EKs) culture systems are widely used in studying skin biology and differentiation. Recently a monolayer culture method of nail matrix cells(NMCs) has been developed. OBJECTIVE: This study was designed to establish a method of culture of NMCs and compared its characteristics with those of EKs. METHODS: Human NMCs were isolated and cultured in a defined medium. Phase contrast microscopic examination, growth rate and ketatin expression were evaluated. RESULTS: In in vitro monolayer culture, NMCs had a similar growth pattern and morphological features as EKs. The synthesis of hard keratins was detected. CONCLUSION: These observations suggest that the NMCs culture model may be useful for the study of nail properties and the effects of drugs.

10.The Content and Process of Group Psychotherapy.

OBJECTIVES: The purpose of this study was to investigate important therapeutic factors in encounter group and compare these therapeutic factors with those in long-term outpatient group psychotherapy, to understand the content and process in real cases for facilitation of interpersonal learning which is important for long-term outpatient group psychotherapy, and to acquire basic data of long-term outpatient group psychotherapy on the basis of this understanding. METHODS: The subject was 7 encounter groups, composed of 41 persons. We used short form of Yalom's Q-sort to find important therapeutic factors in encounter group. We selected 2 cases for analyzing the process and content in these cases. RESULTS: Three of the most important therapeutic factors were existential factors','self understanding', and 'interpersonal teaming(input)'. CONCLUSION: We suggest that interpersonal learning is the most important therapeutic factor in encounter group.
