1.Macrophage/dendritic Cell Marker Staining Characteristics of Langerhans cell Granulomatosis(Histiocytosis X).

Histiocytosis X is characterized by aggregates of Langerhans cells with other inflammatory cells. These Langerhans cells are antigen-presenting cells to T lymphocytes and identified by characteristic morphology, ultrastructural demonstration of Birbeck granules and immunologic reactivity with OKT-6 and HLA-DR antibodies. In this report, the tumor arising in a 2-years-old baby was examined byimmunostaining with several macrophage/dendritic cell markers. The main tumor cells showed cytoplasmic and nuclear staining with S-100 protein and ring-like surface and paranuclear staining with PNA. However, they were negative for follicular dendritic cell marker CD21, macrophage markers lysozyme, Mac 387, alpha-1 antitrypsin and CD68, and interdigitating reticulum cell marker ID4 and ID5. These observations demonstrate the usefulness of S-100 protein and PNA for the identification of Langerhans cells in paraffin-embedded tissue.
2.Congenital Hepatic Fibrosis with Caroli's Disease.

Congenital hepatic fibrosis is an inherited, congenital disorder of the liver, and is occasionally associated with cystic disease of the liver and kidney. We present a case of congenital hepatic fibrosis with Caroli's disease. A 21-year-old woman had suffered from an episodic fever with headaches for 3 years. In laboratory examination, the liver function test was within the normal limits. Esophageal varix was noted by an endoscopic examination. Hepatosplenomegaly and multiple dilated bile ducts were seen by abdominal CT scanning. An orthotopic whole liver transplantation was done. The liver was fibrotic and enlarged. Multiple cystically dilated intrahepatic ducts were noted. Microscopically, diffuse portal fibrosis and widening with proliferation of bile ductules were seen. Intrahepatic bile ducts were markedly dilated and tortuous. The liver cell cords were well preserved.

5.Factors Influencing Nurses’ Performance of Care in COVID-19 Wards

Purpose:
The purpose of this study was to identify factors influencing nurses’ performance of care in COVID-19 wards.
Methods:
The participants were 132 nurses who worked in COVID-19 wards at three hospitals, and were recruited from April 1 to May 31, 2021. Data were analyzed using descriptive statistics, t-test, ANOVA, and multiple regression analysis with SPSS/WIN 24.0 program.
Results:
Nursing performance was significantly and positively correlated with ethical sensitivity (r = .75, p < .001), nursing professionalism (r = .67, p < .001), and social support (r = .67, p < .001). Nursing professionalism was positively correlated with ethical sensitivity (r = .64, p < .001) and social support (r = .55, p < .001). Multiple regression analysis for nursing performance revealed that the most significant factor was ethical sensitivity (β = .47, p < .001). Ethical sensitivity, nursing professionalism, and social support explained 66.0% of total variance in nursing performance.
Conclusion
Ethical sensitiviy, nursing professionalism, and social support significantly influence nurses’ performace of care in COVID-19 wards. It suggests that intervention programs should be directed at improving nurses’ ethical sensitivity, bolstering social support, and enhancing nursing professionalism.
6.A Case Report of Mantle Cell Lymphoma in Leukemic Phase.

Recently, we experienced a patient with a B cell leukemia which could not be classified as chronic lymphocytic leukemia (CLL), prolymphocytic leukemia (PL) or follicular lymphoma in leukemic phase. He is a 58-year-old male and suffered from progressive lymphadenopathy at both inguinal and cervical areas. His blood film showed lymphocytosis (79%), and the cells were heterogeneous in size and shape. Twenty-five percent of the lymphocytes had nuclear irregularities or cleavages unlike the lymphocytes of CLL In which the cells often shows monomorphic features, small size with scanty cytoplasm and round nucleus. The bone marrow trephine biopsy specimen showed a diffuse pattern of infiltration of atypical Iymphoid cells. The lymph node histology showed atypical lymphoid colls proliferated as wide mantles around non-neoplastic appearing germinal centers. The immunophenotype of circulating lymphocytes in peripheral blood showed strong reactivity with CD5, CDl9 and CD20 without expression of CD2, CD3, CD7, CD10, CD22 and CD23. Although this case resembled CLL, the laboratory features showed major differences, notably in the Peripheral blood morphology, histology Patterns and the membrane Phenotype. By combining these data, we diagnosed this case as a leukemic phase of mantle cell lymphoma.

7.Divergence Paralysis.

A fifteen yrs old boy developed divergence paralysis following meningitic attack. The ocular examination revealed homonymous diplopia in all gazes when he fixed at further than 13 cm. He had 18 delta esotropia at distance and 7 delta esophoria at near in prism cover test. His stereopsis was excellent at near in House Fly Stereopsis Test. There wasn't much change in ocular examination in 90 days after initial examination.

8.Circumferential Buckling on Equator: II. Advantages of Circumferential Buckling on Equator with Conserved Scleral Implant.

As a contrast to the surgery described in part I, a lamellar undermining was performed in a conventional way. The sclera was lamellarly undermined on the retinal hole areas which extended at least 3 mm beyond the disc edge of the retinal breaks. If the scleral buckling with a circling element is indicated the location of the undermined area determines whether the circling band must follow the globe's equator or another great circle around the eye, The technique for decreasing a vitreoretinal traction used in this series of cases was that proposed by Schepens. Of 56 surgical procedures for retinal detachment, there was a case of an unsuccessful closure of the retinal break due to fishmouthing of the posterior edge of the break. The technique proposed by Schepens was sufficiently promising. But it has the following drawbacks; difficulties in exact hole localization in a high detached retina, anchoring sutures of the circling band behind the equator and problems to protect the vortex veins from the circling band etc. Our experience since 1978, led us to modify the conventional view, in favor of the circumferential buckling on the equator.

10.Normal predicted values of pulmonary function test in Korean school-aged children.

In recent years pulmonary function teste have had a wide application in clinical pulmonary diseases. By combining a number of different pulmonary function tests, abnormality present in a particular pa tient can be defined. This narrows the list of possible diagnosis and allows to determine accurately the degree of impairment and assess the pesponse to therapy. To assess the pulmonary function in a patient and label as abnormal, it becomes important to define the range of expected values for a normal population. These normal'predicted values'vary according to sex and prowth, i.e., age, height, and weight, particularly in children. In addition, race and geography have been consistently shown to bc an important determinant of lung function. In order to obtain normal perdicted values of pulmonary function tests in Korean school-aged children, we performed spirometry-based pulmonary function tests on the normal healthy 2022 children, age from 6 through 15, and analyzed the data, with the parameters such as sex, age, height, weight, and body surface area. We calculated each of their simple and complex linear and logarithmic regression equations setting the predicted values. The results are as follows: 1) Predicted values of pulmonary function test items were generally higher in boys than those of girls. 2) A correlation coefficient to the parameters examined was the highest in FEV1, then those of FVC, FEF25,PEFR, FEF50, MMEF, AND FEF75 follow in order. 3) Of the parameters examined, height had the highest correlation coefficient consistently in all pulmonary function test items. 4) As compared with the inland and overseas data, some differences were observed.
