1.Awareness, Attitude and Impact of Perceived Depression in the Workplace in Korea.

OBJECTIVES: Depression is a common disabling psychiatric illness in the workplace and is associated with high burden to both employer and employee. Little is known about the awareness, attitude, and impact of depression in the Korean workplace. This study investigates how workers perceive the symptomatology, disability, and management of depression. METHODS: This study is a cross-sectional web-based survey of 1000 workers recruited from online sites. Selected panel members were invited to participate in the survey through Ipsos MORI (www.ipsos-mori.com/) between 10th and 19th February 2014. The participants were Korean workers, aged 16-64, who are either currently working or had worked within the past year. Subjects were encouraged to respond to a 13-item questionnaire on the awareness, attitude, and impact of depression. RESULTS: According to the results, 7.4% of the subjects responded that they had ever been diagnosed as having depression from medical professionals. One third of workers with depression who had taken time off from work did not provide the actual reason to their employers, and the mean number of days off work was 9.8 days. The most common symptoms were depressed mood (76.7%), loss of interests (71.5%), sleep problem (69.8%), and cognitive dysfunction (47.0%). Cognitive symptoms (72.2%) were prominent among the workers who continued to work under the influence of depression. CONCLUSION: Korean workers reported a low rate of depression. Those who have been diagnosed with depression reported shorter number of days taken off from work compared to other countries. These results suggest that depression is poorly recognized and disclosed to employers in the Korean workplace. Presenteeism rather than absenteeism may be a major loss of productivity in Korea. An employee assistance program for depressed workers and new therapeutic strategies for treatment of cognitive symptoms of depression should be provided in Korea.

2.Association between Media Reporting and a Lower Suicide Rate in 2012.

OBJECTIVES: A recent study conducted by the Korea National Statistical Office (KNSO) reported that the suicide rate in Korea decreased in 2012. The aim of this study was to investigate the association between the decrease in suicide rate and media reporting of suicide events. METHODS: Data on suicide rates were obtained from the KNSO. Correlation between suicide rate and celebrity suicide events, number of media reports on suicide, and suicide searches was compared. Data were analyzed using Pearson's correlation coefficient. RESULTS: The number of celebrity suicide events and recognition of celebrity decreased in 2012. Pearson's correlation test showed a significant positive correlation between the monthly number of media reports on suicide and the suicide rate (daily newspaper r=0.619, p<0.001 ; broadcast r=0.393, p<0.001). The number of suicide searches showed association with celebrity suicide events. CONCLUSION: The suicide rate may be influenced by the number of media reports on suicide events. Establishing guidelines for media reporting on these events could help to decrease the suicide rates.

3.Korean farm animal veterinarians’ perception and practice of prudent use of antimicrobials

Antimicrobial (AM) resistance is a growing threat in human and veterinary medicine, spreading across species and perceived as One-Health issue. Prudent use of AM products is essential in mitigating this risk in both human and veterinary medicine. Farm veterinarians, responsible for prescribing appropriate AM and offering advice to farmers on their proper usage, are regarded as key players in the livestock industry. An online survey of farm veterinarians (n = 1,531) was conducted to assess their educational experience, beliefs, current status of prescription, practical behavior, and self-efficacy regarding the prudent use of antimicrobials (PUA). The data from 170 respondents were analyzed using descriptive statistics, regression and mediation analysis. Participants chose AM based on their experiences, academic resources, and peer veterinarians. Approximately 77% did not routinely conduct antibiotic-susceptibility-test. Most participants believed in the importance of PUA and the role of veterinarians to reduce AM resistance, but they reported a lack of awareness among farmers and insufficient national support interfere with their practical behavior with regard to PUA. Half of the participants had PUA-education, and 78.6% reported that education had a positive impact on their behavior with PUA. Self-efficacy partly mediates the relationship between belief and behavior for PUA, which accounted for 23.77% of the total effect that beliefs have on behavior for PUA. The findings of the study show that enhancing veterinarians' abilities to practice and their self-efficacy through education tailored to the current status of farms can contribute to the reduction of AMs in the livestock sector.
4.Efficacy of salivary versus subgingival bacterial sampling for the detection and quantification of periodontal pathogens

Purpose:
The aim of this study was to investigate the efficacy and validity of subgingival bacterial sampling using a retraction cord, and to evaluate how well this sampling method reflected changes in periodontal conditions after periodontal therapy.
Methods:
Based on clinical examinations, 87 subjects were divided into a healthy group (n=40) and a periodontitis group (n=47). Clinical measurements were obtained from all subjects including periodontal probing depth (PD), bleeding on probing (BOP), clinical attachment loss (CAL), and the plaque index. Saliva and gingival crevicular fluid (GCF) as a subgingival bacterial sample were sampled before and 3 months after periodontal therapy. The salivary and subgingival bacterial samples were analyzed by reverse-transcription polymerase chain reaction to quantify the following 11 periodontal pathogens: Aggregatibacter actinomycetemcomitans (Aa), Porphyromonas gingivalis (Pg), Tannerella forsythus (Tf), Treponema denticola (Td), Prevotella intermedia (Pi), Fusobacterium nucleatum (Fn), Pavimonas micra (Pm), Campylobacter rectus (Cr), Prevotella nigrescens (Pn), Eikenella corrodens (Ec), and Eubacterium nodatum (En).
Results:
Non-surgical periodontal therapy resulted in significant decreases in PD (P<0.01), CAL (P<0.01), and BOP (P<0.05) after 3 months. Four species (Pg, Tf, Pi, and Pm) were significantly more abundant in both types of samples in the periodontitis group than in the healthy group. After periodontal therapy, Cr was the only bacterium that showed a statistically significant decrease in saliva, whereas statistically significant decreases in Cr, Pg, and Pn were found in GCF.
Conclusions
Salivary and subgingival bacterial sampling with a gingival retraction cord were found to be equivalent in terms of their accuracy for differentiating periodontitis, but GCF reflected changes in bacterial abundance after periodontal therapy more sensitively than saliva.
5.Chronic Epstein-Barr virus infection causing both benign and malignant lymphoproliferative disorders.

The Epstein-Barr virus (EBV) is oncogenic and can transform B cells from a benign to a malignant phenotype. EBV infection is also associated with lymphoid interstitial pneumonia (LIP). Here, we report the case of a 14-year-old boy who was diagnosed with a latent EBV infection and underlying LIP, without any associated immunodeficiency. He had been EBV-seropositive for 8 years. The first clinical presentations were chronic respiratory symptoms and recurrent pneumonia. The symptoms worsened in the following 2 years. The results of in situ hybridization were positive for EBV, which led to a diagnosis of LIP. The diagnosis was confirmed by the results of a thoracoscopic lung biopsy. The EBV titer of the bronchoalveolar lavage specimens obtained after acyclovir treatment was found to be fluctuating. The patient had latent EBV infection for 8 years, until presented at the hospital with intermittent abdominal pain and distension. Physical examination and pelvic computed tomography revealed a large mesenteric mass. A biopsy of the excised mass led to a diagnosis of Burkitt's lymphoma (BL). The patient received combination chemotherapy for 4 months, consisting of vincristine, methotrexate, cyclophosphamide, doxorubicin, and prednisolone. He is now tumor-free, with the LIP under control, and is being followed-up at the outpatient clinic. This is the first report of a Korean case of chronic latent EBV infection that developed into LIP and BL in a nonimmunocompromised child.

6.Clinical Validation of a Deep Learning-Based Hybrid (Greulich-Pyle and Modified Tanner-Whitehouse) Method for Bone Age Assessment

Objective:
To evaluate the accuracy and clinical efficacy of a hybrid Greulich-Pyle (GP) and modified Tanner-Whitehouse (TW) artificial intelligence (AI) model for bone age assessment.
Materials and Methods:
A deep learning-based model was trained on an open dataset of multiple ethnicities. A total of 102 hand radiographs (51 male and 51 female; mean age ± standard deviation = 10.95 ± 2.37 years) from a single institution were selected for external validation. Three human experts performed bone age assessments based on the GP atlas to develop a reference standard. Two study radiologists performed bone age assessments with and without AI model assistance in two separate sessions, for which the reading time was recorded. The performance of the AI software was assessed by comparing the mean absolute difference between the AI-calculated bone age and the reference standard. The reading time was compared between reading with and without AI using a paired t test. Furthermore, the reliability between the two study radiologists’ bone age assessments was assessed using intraclass correlation coefficients (ICCs), and the results were compared between reading with and without AI.
Results:
The bone ages assessed by the experts and the AI model were not significantly different (11.39 ± 2.74 years and 11.35 ± 2.76 years, respectively, p = 0.31). The mean absolute difference was 0.39 years (95% confidence interval, 0.33– 0.45 years) between the automated AI assessment and the reference standard. The mean reading time of the two study radiologists was reduced from 54.29 to 35.37 seconds with AI model assistance (p < 0.001). The ICC of the two study radiologists slightly increased with AI model assistance (from 0.945 to 0.990).
Conclusion
The proposed AI model was accurate for assessing bone age. Furthermore, this model appeared to enhance the clinical efficacy by reducing the reading time and improving the inter-observer reliability.
7.Complete denture rehabilitation utilizing digital process: A case report

Complete dentures are one of the most basic treatment methods for the treatment of edentulous patients. The manufacturing process of traditional complete dentures goes through the steps of taking primary impressions, secondary impressions, jaw relation record, trying in wax denture, and final denture insertion. Multiple visits and complex manufacturing procedures are required, and errors may occur in each step. With the development of digital technology, manufacturing steps have been reduced by introducing digital technology to the denture treatment process. In the process of manufacturing dentures by introducing a digital process, a more precise work is possible using Computer-Aided Design, and it is possible to shorten the period of labor and reduce the number of visits. In this case, the anterior teeth arrangement of the patient’s existing dentures was transferred to the final dentures using a digital method. After taking impression, try-in dentures were digitally fabricated and tried in the oral cavity to evaluate their retention in the oral cavity. Final dentures were manufactured by milling process. The number of visits was reduced, satisfactory retention and stability of dentures were obtained, and aesthetic recovery was achieved.
8.Modulation of Vascular Smooth Muscle Cell Phenotype by High Mobility Group AT-Hook 1

Objective:
The purpose of this study is to examine the effect of high mobility group AT-hook 1 (HMGA1) on the phenotyptic change of vascular smooth muscle cells (VSMCs).
Methods:
Gene silencing and overexpression of HMGA1 were introduced to evaluate the effect of HMGA1 expression on the phenotypic change of VSMCs. Marker gene expression of VSMCs was measured by promoter assay, quantitative polymerase chain reaction, and western blot analysis. Common left carotid artery ligation model was used to establish in vivo neointima formation.
Results:
HMGA1 was expressed strongly in the synthetic type of VSMCs and significantly downregulated during the differentiation of VSMCs. Silencing of HMGA1 in the synthetic type of VSMCs enhanced the expression of contractile marker genes thereby enhanced angiotensin II (Ang II)-dependent contraction, however, significantly suppressed proliferation and migration. Stimulation of contractile VSMCs with platelet-derived growth factor (PDGF) enhanced HMGA1 expression concomitant with the downregulation of marker gene expression which was blocked significantly by the silencing of HMGA1. Silencing of HMGA1 retained the Ang II-dependent contractile function, which was curtailed by PDGF stimulation, however, overexpression of HMGA1 in the contractile type of VSMCs suppressed marker gene expression. Proliferation and migration were enhanced significantly by the overexpression of HMGA1. Furthermore, the Ang II-dependent contraction was reduced significantly by the overexpression of HMGA1. Finally, the expression of HMGA1 was enhanced significantly in the ligated artery, especially in the neointima area.
Conclusion
HMGA1 plays an essential role in the phenotypic modulation of VSMCs.Therefore, paracrine factors such as PDGF may affect vascular remodeling through the regulation of HMGA1.

Artificial intelligence (AI) in radiology is a rapidly developing field with several prospective clinical studies demonstrating its benefits in clinical practice. In 2022, the Korean Society of Radiology held a forum to discuss the challenges and drawbacks in AI development and implementation. Various barriers hinder the successful application and widespread adoption of AI in radiology, such as limited annotated data, data privacy and security, data heterogeneity, imbalanced data, model interpretability, overfitting, and integration with clinical workflows. In this review, some of the various possible solutions to these challenges are presented and discussed; these include training with longitudinal and multimodal datasets, dense training with multitask learning and multimodal learning, self-supervised contrastive learning, various image modifications and syntheses using generative models, explainable AI, causal learning, federated learning with large data models, and digital twins.
10.A Report on a Nationwide Surveillance System for Pediatric Acute Hepatitis of Unknown Etiology in Korea

Background:
Several cases of pediatric acute hepatitis of unknown etiology related to adenoviral infections have been reported in Europe since January 2022. The aim of this study was to compare the incidence, severity, possible etiology, and prognosis of the disease with those in the past in Korea.
Methods:
The surveillance group collected data between May and November 2022 using a surveillance system. Acute hepatitis of unknown etiology was defined in patients aged < 16 years with a serum transaminase level > 500 IU/L, not due to hepatitis A-E or other underlying causes. For comparison, data from 18 university hospitals were retrospectively collected as a control group between January 2021 and April 2022.
Results:
We enrolled 270 patients (mean age, 5 years). The most common symptom was fever. However, the incidence was similar between 2021 and 2022. Liver function test results, number of patients with acute liver failure (ALF), liver transplantation (LT), death, and adenovirus detection rates did not differ between the two groups. None of the adenoviruspositive patients in either group experienced ALF, LT, or death. In the surveillance group, adenovirus-associated virus-2 was detected in four patients, one of whom underwent LT. Patients with an unknown etiology showed significantly higher bilirubin levels, a lower platelet count, and a higher LT rate than patients with a possible etiology.
Conclusion
The incidence of pediatric acute hepatitis of unknown etiology and adenovirus detection rate have not increased in Korea.
