1.Antiviral Effect of Antisense Phosphorothioate Oligonucleotides Targeted to Herpes Simplex Virus.

To search the effective antisense oligonucleotide that inhibit the growth of Herpes simplex virus type 1 (HSV-1), six kinds of phosphorothioate oligodeoxynucleotides (S-ODNs) were synthesized and the antiviral activity was assessed by measuring cytopathic effect in Vero cells infected with HSV-1. Of the three dodecamer S-ODNs, cornplernentary to the translation initiation site of IE2 (AS2) and scrambled S-ODN (AS1) showed more significant antiherpetic activity than AS4 complementary to the IE4. Accordingly, the antiviral effect of dodecamer S-ODN was not specific. In contrast to the no inhibitory effect of sense strand S-ODN of ICP8 (AS6), two S-ODNs complementary to the translation initiation site of ICP8 (AS3) and that of IE1 (AS5) showed potent antiherpetic activity assessed in vitro HSV-1 virus yield assay. Antiherpetic effect of AS3 was decreased in proportion to the addition of AS6. The synthesis of viral protein ICPS and IE1 were inhibited in AS3 and AS5 treated HSV-1 infected Vero cells, respectively. These findings suggest that antiherpetic effect of AS3 is specifically mediated by targeting ICPS. S-ODNs had no effect on Vero cell viability. The data suggest that the 19-mer S-ODNs may be effective in antiviral chemotherapy.

2.Argon Laser Therapy of Nevus Flammeus.

Nevus flammeus is benign but cosmetically devastating congenital vascular malformation whose past treatments have been marred by unacceptable complication or by unacceptable ineffectiveness. The argon laser is a therapeutic device which has been newly applied to this condition. Blue-green argon light is absorbed by the intraluminal red hemoglobin molecule, leading to the destruction of blood vessels. This study was undertaken to evaluate the therapeutic effect, to look for complications caused by the argon laser on nevus flammeus and to investigate the prognostic values of the color, the degree of blanching and the histological pattern of lesion after laser treatment. All patients were treated with Coherent Radiation Model No. 1,000 Laser with a spot size of 1mm diameter. The power range was 1.8W (irradiance of 229W/cm2), the pulse duration was 0.4 second and the repetition rate was 120 impulses per minute. The result was evaluated according to the criteria of Gilchrest(Table 1). Among the 109 patients who have been treated between November, 1981 and July, 1982 at the Department of Dermatology, Seoul National University Hospital, data from the 49 patients who could be followed up was evaluated and analyzed. 4.A Case of Lichen Scrofulosorum.

Lichen scrofulosorum, also known as Tuberculosis cutis lichenoides, is thought as a tuberculid such as Erythema induratum, Papulonecrotic tuberculid, Tuberculosis miliaris disseminata and Lupus miliaris disseminatus faciei. The lesions of Lichen scrofulosorum are pin-head sized or smaller, normal skin or reddish brown colored, arranged in nummular groups, usually on the trunk, where they persist unchanged or undergo spontaneous involution. As a rule, they appear in children who have tuberculosis of the bones or the lymphnodes. We report a case of Lichen scrofulosorum, 13-year old girl, who showed clinically and histologically typical lesions on the trunk. She was highly sensitive to Mantoux test and had tuberculous lymphadenitis of left cervical lymphnodes, but the chest was negative. Many challenges to the tuberculous origin of the tuberculids including Lichen scrofulosorum have prevailed for the last decade, Several studies and reports indicated that healing responses of Erythema induratum and other tuberculids to corticosteroids were more definite than to anti-tuberculosis drugs. But more recently, some reports recommended that anti-tuberculosis drugs were eftective than corticosteroids (in the treatment of Erythema induratum and other tuberculids). In this case anti-tuberculosis treatment with PAS 10gm., INH 400 mg., and streptomycin 1.0 gm. daily for the first one month and then 1.0gm. twice a week has remarkably improved the clinical manifestation in follow-up of three months. While the follow-up period was no enough, we strongly suggest the tuberculous origin of Lichen scrofulosorum.

5.Influence of Dimethyl sulfoxide on the Effect of Ultraviolet Irradiation.

Various concentrations, including 100%, 75%, 50%, and 25% of dimethyl sulfoxide (DMSO) were applied on the back of albino rats, followed by ultraviolet irradiation after 30 minutes. Biopsy specimens were taken 6, 12, 24, 36, and 48 hours afters ultraviolet irradiation without local anesthesia. The macroscopic and histologic findings were as follows. 1. Mild degree of erytheme appeared only on the site of the highest concentration (100%) of DMSO. 2. Vacuolization of the prickle cells appeared slightly later on the sites of higher concentrations (75% and 100%) than those of lower concentrations and control. 3. After 48 hours post-irradiation, there was no detectable vacuolization on the sites of higher concentrations, whereas marked vacuolization still remained on the other sites. 4. More pronounced epidermal thickening could be observed on the sites of higher concentrations than the sites of lower concentrations and control. 5. The dermal edema was more completely disappeared on the sites of higher concentrations than the sites of lower concentrations and control, 48 hours after ultraviolet irradiation.

6.Quantitation of T Lymphocytes and T Subsets Peripheral Blood and Cutaneous Lesion in Patients with Atopic Dermatitis.

This study was undertaken to elucidate any quantitative abnormalities of T cells and T subsets(helper/inducer T cell: T cell and suppressor/cytotoxic. T cell; Tsc cell) in the peripheral blood and cutaneous lesion in patients with atopic dermatitis by using monoclonal antibodies, and to ascertain whether there are any relations bctwcen such immunologic abnormalities and the severity of the disease. Thirty seven patints(19 male, 18 female) were entered in this study, who vistited the Departement of Dermatology of Seoul National University Hospital frnrn February 1985 to November 1985. The control group comprised fifteen healthy children (8 male, 7 female). The peripheral blood samples of 30 patients and the skin biopsy specirnens of 29 patients were examinecl. Both were examined in 22 patients. The results showed that there was a quantitative abnormality in pcripheral blood Tsc cells in patients with atopic dermatitis and it could be related to the severity of the disease. The predominance of T cells seemed to be relatecl to the pathogenesis of atopic dematitis.

7.Studies on Causative Microorganisms of Pyodermas.

Clinical and bacteriological studies, including seasitivity tests, of 85 patients in pyodermas such as impetigo contagiosa, pustular acne vulgaris, hair follicle and sweat gland infections, and secondary pyogenic infections superimposed on primary dermatoses were carried out during 4 months period, from June 1970 to Sept.1970, at department of dermatology, schooI of medicine, Seoul national university. The results were as follows; 1) Predominant age group of impetigo was the preschool. ages (87%). 2) Causative agents of impetigo in our 30 cases were coagulase positive staphylococcus aureus only in 27, streptococcus only in 0 and both organisms in 3 (one case wasmixed infection with s. aureus and b-hemolytic streptococcus, and two were mixed infections with s. aureus and a-hemolytic streptococcus). 3) Culturing from l2 cases of pustular acne vulgaris, we found coagulase negative staphylococcus albus in 9, coagulase positive stsphylococcus aureus in 1 and no growth in 2. 4) In 35 cases of hair follicle and sweat gland infections, we isolated coagulase positivestaphylococcus aureus only in 32, coagulase negative staphylococcus albus only in 2, and mixed infection with staphylococcus aureus and b-hemolytic streptococcus in l. 5) Isolated organisms in 8 cases of secondary pyogenic infections superimposed on primary dermatoses were coagulase positive staphylococcus aureus only in 2, coagulase negative staphylococcus albus only in 1, b-hemolytic streptococcus only in 1, a- hemolytic streptococcus only in 1, and mixed infections with various combination in 3. 6) Results of sensitivity tests with isolated organisms in the above diseases groups were as follows; (a) Total 66 strains of coagulase positive staphylococcus aureus showed resistance to ampicillin in 97.0%, penicillin in 78.8%, terramycin in 72.7%, rizynomycin in 71.2%, streptomycin in 24.1%, chloramphenicol in 16.7%, neomycin in 12.1%, kanamycin in 4.5% and leukomycin in 1.5%, (b) Total 8 strains of hemolytic streptococci were sensitive to chloramphenicol, leukomycin and penicillin.

8.An Experimental Study on the Passive Transfer of Dermpgaphism.

Dermographism is an exaggerated triple response of Lewis and appears as a lineat wheal with a flare at a site in which skin is briskIy stroked with a firm object. The etiology and pathogenesis of dermographism are uncertain but passive transfer tests are sometimes poeitive. The active serum factor has been characteiized as Ig E in some patients. In this experimental study, we tried passive transfer of dermographism with the sera of the patients with dermographism. Twelve dermographic patients' sera were tested to fourteen normal healthy persons and two asymptomatic dermographic persons, total number of thirty nine cases. Only two recipients were passively transferred but they showed positive responses to all sera tested, two and three respectively. This result suggests that not only serum factor but also recipient's factor(s) may be concerned in passive transfer of dermographism.

9.A Case of Persistent Grover's Disease.

We herein presented a case of persistent variant of Grovers disease in a 83-year old male. He had a rather extensive eruption of erythematous scaly papules, which had been profusely grouped but discretely distributed over 2 years or more. The histopathologic findings of two lesions in the same patient revealed mixture of various histologic patterns, patterns resembling Dariers disease, pemphigus vulgaris, Hailey-Hailey disease and spongiosis. The ultrastructural findings of Darier pattern should be essentially similar to those of Dariers disease and also be common to those observed in the early stages of other causes of focal acantholytic dyskeratosis. The patient had been treated with topical and intralesional steroid and oral 13-cis-isotretinoin, however, the lesions had been persisted only with partial improvement of clinical appearance and reduction of pruritus.

10.Serologic Survey for Syphilis in Seoul.

The prevalence of syphilis was sharply decreased throughout the world since the introduction of penicillin into the treatment of syphilis in 1943, however recent in formations seemed to be increased in incidence of syphilis since about 1960. In Korea, recent trends of syphilis can hardly be assesed because there are few reports of it. In order to know the current incidence of syphilis in Seoul we carried out S.T.S. in 1046 healthy young men living in Seoul. At that we classified the man as syphilitic who has positive serologic test on both VDRL and RPCF and has not recieved regular antisyplulitic treatment in the past. The result reveala that the incidence of syphilitic infection is 1.72%, which is slightiy decreased statistical figure than in 1960s. There are no much differences in incidence between each group of age or educational degree. Among the 653 unmarried men, 62.3% of them had experiences of sexual contact and their sex partners were business girls, students and prostitutes in order of frequency.
