4.Experimental Reproduction of Polymorphous Light Eruption: By Potent UVA Irradiating Instrument , Metal Halide Mercury Lamp.

We report a case of experimental reproduction of the skin lesion of polymorphous light eruption by potent UVA irradiating instrument, metal halide mercury lamp. A 20-year-old female had complained skin lesion at sun-exposed area since childhood. The skin lesion was reproduced after repetitive exposure of high dose of UVA, and the histologic feaure of the lesion was consistent with polymorphous light eruption. Metal halide mercury lamp used in this patient has hight irradiance and exclusive of UVB and UVC. It can be convenient and precise one in phototest using high doses of UVA.

5.Role of Inducibility of Superoxide Dismutases and Metallothionein of Mouse Lungs by Paraquat in Aging.

BACKGROUND: The aging process may be induced, at least in part, by reactive oxygen species(ROS). It has been though that the lung could be a good source of ROS because it has a high oxygen tension. In the present study, we invetigated the inducibility of the first and last lines against oxidative stress, superoxide dismutases (Cu/Zn-SOD and Mn-SOD) as a scavenger of O2- and metallothionein(MT) as a scavenger of OH·, respectively, in mouse lungs with age. METHODS: Oxidative stress was induced by paraquat, an intracellular superoxide generator, at 1, 4, 8, and 12 months of age and then SODs and MT mRNAs were determined by RT-PCR method. RESULTS: The steady-state level of Mn-SOD mRNA increased from 1 to 8 months but decreased thereafter. However, Mn-SOD mRNA was not induced by paraquat after 1 month. On the other hand, there was no change in the steady-state level of Cu/Zn/-SOD mRNA, which decrease abruptly at 12 months of age. Additionally, Cu/Zn/-SOD mRNA was not induced by paraquat at any age. There was no change in the steady-state level of MT mRNA with age whereas its inducibility by paraquat was intact at all ages. CONCLUSION: These results indicate that lack of induction of SODs with age may be one of the causative factors in the aging process while induction of MT may play an important role in the defense against oxidative stress. It is therefore implicated that the tissue antioxidant/proxidant balance could be one determinants of meal life span.

6.A Case of Atropie Blanche Accompanied by Cryoglobulinemia.

We report a case of atrophie blanche which was accompanied by cryoglobulinemia. The patient, 17-year-old female, have had recurrent painful ulcerations and ivory-white atrophic scars with telangiectases surrounded by hyperpigmentation. On histological examination, vessel wall thickening and hyalinization of the intima, partial vascular occlusion, vascular proliferation and mild perivascular chronic inflammatory cells infiltration and hemorrhage in the upper dermis were noted. The patient had been treated with dipyridarnol(Persantin) and acetylsalicylic acid hut the lesions had been waxed and waned.

7.Changes in Blood Glucose and Electrolyte During Open Heart Surgery.

This study deals with the changes in the concentrations of blood glucose and electrolytes during open heart surgery. Blood glucose and electrolytes in connection with age, disease and anesthetic period were measured in 25 patients about to undergo heart surgery which were performed between June 1986 and August 1986 in Yeungnam University Hospital. Because glucose solution is commonly used as priming solution, and the priming solution is hyperglycemic and hyperosmolar, glucose level of priming solution in this study was adjusted to 100-200 mg% level to minimize hyperglycemic and hyperosmolar effect. The following results were obtained. 1. Glucose level of priming solution before extracorporeal circulation was 151.6+31.3 mg%. 2. With body cooing, blood glucose level was elevated. As duration of extracorporeal circulation is prolonged, blood glucose level was elevated more, but no difference between age and diseases were observed. On warning, blood glucose level was progressively lowered. 3. Despite the low serum potassium level during by-pass, the potassium level was progressively elevated following by pass, cut the serum potassium level was low compared to control values. Elevated calcium level was maintained during by pass.

8.Expression of Prostatic Carcinoma Oncogene PTI - 1 in Prostatic Carcinoma , Prostatic Intraepithelial Neoplasia and Benign Prostatic Hyperplasia Using in situ PCR .

PURPOSE: Prostatic tumor induced gene-1 (PTI-1) is a mutated human EF-la and putative prostatic carcinoma tumor-inducing oncogene, that is differently expressed in prostatic cancer and benign prostatic hyperplasia. And, it is more sensitive marker than prostate- specific antigen (PSA) for detecting human prostate cancer in the bloodstream. This study invastigated the expression of PTI-1 in paraffin embedded tissue of prostatic carcinoma, prostatic intraepithelial neoplasia, and benign prostatic hyperplasia using in situ PCR. MATERIALS AND METHODS: we evaluated expression of PTI-1 in prostatic carcinoma with prostatic intraepithelial neoplasia (PIN) of 32 cases, benign hyperplasia of 20 cases, high grade transitional cell carcinoma of 10 cases and colon cancer of 10 cases for control group. Also, the immunohistochemical staining for PSA was performed to comparison with clinical value of PSA. RESULTS: The serum level of PSA was closely related to stage and Gleason score (p < 0.05). However, the results of immunohistochemical stains were variable to stage and Gleason score. PTI-1 using in situ PCR expressed in 50% of prostatic carcinoma, 41% of prostatic intraepithelial neoplasia, 10% of benign hyperplasia and colon cancer (p < 0.05). No expression is observed in transitional cell carcinoma. In prostatic carcinoma, PTI-1 expressed in 43.8% (7/16) of stage II, 50.0% (5/10) of stage III, and 66.7% (4/6) of stage IV (p<0.05). In PIN, expression of PTI-1 was similar to prostatic carcinoma (p<0.05). CONCLUSION: PTI-1 represented a relatively sensitive marker for prostatic carcinoma and PIN, indicator of prostatic carcinoma progression.

10.Computed tomography in the staging of esophageal carcinoma

CT was found to be highly accurate in predicting tumor size and assessing invasion of the surroundingstructures and distant metastasis. Also CT played an important role for determination of operability of esophagealcarcinoma. The CT findings with barium esophagogram in 21 patients with histologically proven esophageal carciomawere reviewed from Feb. 1985 to Feb. 1986 at the department of Radiology, Yonsei University ,College of Medicine.The results were as follows: 1. Number of patients in each stages were: 2 in stage 1, 6 in stage 2, 4 in stage 3,and 9 in stage 4. 2. Peak age distribution was in its 6th decasedes as 9 patiens (42.9%). Overall mean age was 60.8 years. Number of male patients were 19 and 2 of female. 3. Histologic types of esohageal carcinoma were 19cases of epidermoid (90.5%) and 2 cases of adenocarcinoma(9.5%). 4. The tumor location was 1 case in upper, 14cases(66.7%) in middle and 6 cases in lower one-third. 5. Various types of esophageal carcinoma were as follows: 3cases of fungating, 4 cases of infiltrating, 5 cases of ulcerofungating, and 9 cases of ulceroinfiltrating type.6. Average length of involvement in each stages were 4cm in stage 1, 5.5cm in stage 2, 8.8cm in stage 3, and 8.3cmin stage 4. The involved length was longer in advanced cases. In 11 cases(52.4%), the involved length was between4 and 8cm. 7. Angle of periaortic fat plane obliteration of the aortic circumference were as follows: Below 45degrees( 7 cases 33.3%), 45degrees-90degrees (3 cases 14.3%), over 90 degrees(11 cases, 52.4%). 8. Method oftreatment of esophageal carcinoma were as follows: Only radiotherapy in 11 cases(52.4%), radiotherapy withoperation in 5 cases, only operation in 1 cases, and no treatment in 4 cases. 9. Distant metastatic sites were:brain in 1, pericardium in 5, liver in 5, trachea in 2, bronchus in 9, and distant lymph node in 5 cases.
