2.Effect of Tending Diancibo Pu on Burn Treatment.

PURPOSE: Recently, in an experiment using animals, radiation therapy using a laser or Tending Diancibo Pu (TDP) has been shown to be effective in treating scars on the skin by increasing the production of fibroblast cell and collagen and by accelerating the process of epithelization. The purpose of this study is to examine the effectiveness of TDP radiation therapy in treating human burn injuries in terms of the frequency of treatment, the timing of eschar separation, and the duration of treatment. METHODS: In the treatment group, the burn area of the patients was first sterilized with saline solution and potadin solution and covered with one vaseline gauze. Then, the burn area was radiated every other day by using TDP for 20 minutes at distance of 20-25 cm, at radiant plate temperature of 250-280oC. In the cases of control group, the burn area of the patients was first sterilized with saline solution and potadin solution and covered with one vaseline gauze. Then, the area was covered with one burn gauze and bandaged. The treatment was conducted every other day. RESULTS: In cases of superficial second degree burn injuries, the difference between the treatment and the control groups was 1.34 in terms of the frequency of treatment. In the cases of both superficial and deep second degree burns, the differences between the treatment and the control groups were 3.47 in terms of the frequency of treatment, 0.63 weeks in terms of the timing of eschar separation, and 6.03 days in terms of the duration of treatment. All these differences were statistically significant. CONCLUSION: From the experiment, it can be concluded that TDP radiation therapy is more effective in treating human burn injuries than conventional treatment in terms of the of the frequency of treatment, the timing of eschar separation, and the duration of treatment.

3.Primary Chondrosarcoma of the Lung: A case report.

Primary chondrosarcoma of the lung is an extremely rare tumor, and classified into two types, tracheobronchial & lung variety. The tracheobronchial variety is usually localized and lacked lymph nodal or distant spread with good prognosis. The lung variety tends to be more invasive associated with mediastinal lymph node involvement and thoracic metastasis. Authors experienced a case of primary chondrosarcoma, lung variety, of the lung involving left lower lobe with thoracic metastasis, not yet reported in Korean literatures. The patient was 55-year-old Korean female and chief complaints were cough, dyspnea & chest pain. Pathologically, it was confused with the metastatic adenocarcinoma in the first pleural fluid cytology. But bronchoscopic biopsy revealed typical morphology of chondrosarcoma with concentric growth encircling the bronchial tree and partial destruction of the bronchiols, suggesting its primary origin. Later pleural biopsy and aspiration materials during ches tube insertion revealed also metastatic chondrosarcoma. Radiologic studies were also campatible findings to chondrosarcoma. After admision, the patient underwent progressively downhill course & expired at 31st hospital day.

4.Immunohistochemical Analysis of TGF-beta Expression and Angiogenesis in Infiltrating Duct Carcinoma of the Breast.

Forty cases of infiltrating duct carcinoma of the breast were examined immunohistochemically for expression of TGF-beta and angiogenesis in order to analyze significant correlation with prognostic parameters including tumor size, axillary lymph node metastasis, clinical stage, histologic grade, estrogen receptor and progesterone receptor status. The TGF-beta expression was observed in tumors center and advancing edges of tumors. To determine microvessel density for angiogenesis, we stained endothelial cells for Factor VIII related antigen and counted microvessel within tumor. The results were as follows: 1) The strong immunohistochemical expression of TGF-beta and higher counts of microvessels were observed in advancing edges of tumors (p<0.05). 2) The TGF-beta expression in the advancing edges of tumors was closely related to clinical stage and presence of axillary lymph node metastasis (p<0.05). 3) The mean microvessel counts were significantly higher in tumors from patients with axillary lymph node metastasis and increased with increasing clinical stage (p<0.05). 4) The TGF-beta expression was not related to histologic grade, estrogen receptor and progesterone receptor status(p>0.05). Therefore, the results suggested that the TGF-beta expression and angiogenesis in infiltrating duct carcinoma of the breast may play an important part in prognostic factors, closely related to the lymph node metastasis and clinical stage.

5.A Phenomenological Study on Experience of Clinical Supervision by Psychiatric Mental Health Nurse Practitioners.

PURPOSE: The main purpose of this study was to identify the essential meaning of clinical supervision for psychiatric mental health nurse practitioners. METHODS: Data were collected in 2015 through individual in-depth interviews using open-ended questions from 9 women psychiatric mental health nurse practitioners. Each interview lasted about 90 minutes. Verbatim transcripts were analyzed using the Giorgi's phenomenological analysis method. RESULTS: The results of the analysis showed the following four categories: ‘Barren mental health site alone’, ‘Enduring with reflective supervision’, ‘Professional competence enhanced by continuous supervision’, ‘Maintaining a healthy life balance as a psychiatric mental health nurse practitioner’. CONCLUSION: The findings in this study provide helpful insights for understanding the need for clinical supervision in the psychiatric mental health nurse practitioner course. The results also provide support for the psychiatric mental health nurse practitioner course and practice development for psychiatric mental health nurses at the work site.

8.Mutational and Expressional Analysis of ATG5 Gene in Non-Small Cell Lung Cancers.

PURPOSE: Several lines of evidence have indicated that perturbations of autophagy are involved in the development of many human diseases, including cancer. The autophagy-related genes (ATG) encode proteins that play important roles in autophagic processes. The aim of this study was to see whether alterations of the ATG5 protein expression and somatic mutations of the ATG5 gene are present in human non-small cell lung cancers (NSCLCs). MATERIALS AND METHODS: We analyzed the ATG5 somatic mutations in 45 NSCLCs by performing single-strand conformation polymorphism (SSCP). We examined the ATG5 protein expression in 45 NSCLCs by performing immunohistochemistry. RESULTS: The SSCP analysis revealed no evidence of somatic mutation in the DNA sequences encoding the ATG5 gene in the 45 NSCLCs. On the immunohistochemistry, ATG5 protein was expressed in the normal bronchial epithelial cells, while it was lost in 9 (20%) of the NSCLCs. CONCLUSION: Our data indicates that ATG5 is altered in NSCLC at the expressional level, but not at the mutational level. The data also suggests that the loss of expression of ATG5 might play a role in the pathogenesis of NSCLC by altering autophagic and apoptotic cell death.

10.A Clinical Study of Nulliparous Women Aged 40 Years and Older.

OBJECTIVE: Our purpose was to compare the pregnancy outcomes of nulliparous women aged 40 years and older with those of nulliparous women under 35 years of age. METHODS: From January 1989 to December 1998 total 57,563 deliveries were seen in Gachon Gil Medical Center. Among them we experienced 59 cases of nulliparas at 40 years and older. These women were compared with 188 young nulliparas under 35 years of age as the control group. The statistical analysis was performed using Chi-square tests, and statistical significance was defined as p<0.05. RESULTS: The incidence rate of elderly nulliparas aged 40 years and older was increased from 0.04% in 1989 to 0.30% in 1998. The age distribution was from 40 years to 45 years. The gravidity of eldery nulliparas was 2.2 in comparison with 1.6 in control group. The incidence of uterine myoma, gestational DM, IUGR and oligohydramnios was significantly high rate in elderly nulliparas. There was significantly high rate of cesarean section in elderly nulliparas(88.1%) in comparison with control group(40.4%). The reasons of cesarean section were her demand(39.0%), CPD(15.3%) and breech presentation(13.6%) in decreasing order. The preterm delivery rate was 10.2% in elderly nulliparas in comparison with 4.3% in the control group. Placenta accreta and uterine atony were significantly high in elderly nulliparas and mean estimated blood loss was also high. There was no difference in 5-minute Apgar score between both group, but there were more cases of neonatal intensive care unit admission in neonates of elderly nulliparas(6.8% vs 0%). CONCLUSION: The incidence of elderly nulliparas is continuously increasing. The elderly nulliparas and their babies are at greater risk than young women. Therefore all elderly nulliparas aged 40 years and older can be regarded as high risk patients and they must be managed with careful attention.
