3.The Genetic and Clinical Studies of Xeroderma Pigmentosum.

Genetic and clinical observations were recorded on 16 patients with xeroderma pigmentosum, 14 of them having visited the Department of Dermatology, Chonnam National University Hospital, Kwangju, Korea from 1968 through 1982, and 3 of them having visited the Department of Dermatology, Chosun University Hospital located in the same city during same period(one patient was duplicated). Xeroderma pigmentosum was found to occur with a frequency higher than one in 183,000 births in Chonnam province, Korea. It waa recognized that xeroderma pigmentosum is a disease of autosomal recessive inheritance. The sex ratio of xeroderma pigmentosum was male to female l. 7 to 1. The age of onset was between 5 months and 10 years, and the age when the patients visited the hospital for the first time was 6 months and 25 years. The skin malignancies were observed in 9 patients, among which squamous cell carcinomas occurred in 6, basal cell carcinomas in 2, and actinic keratosis in l. The associated ocular abnormalities were photophobia in 4 patients, chalazion in 1, blepharoconjunctivitis in 1, pterygium in 1 and squamous cell carcinomas in 1. Only cerebral galsy as a neurological abnormality was observed in 1 patient.

4.Artificial prosthesis of the knee joint combining with homogenous bone transplantation

Here, we report a case of artificial prosthesis of the knee joint combining with homogenous bone transplantation for the treatment of giant cell tumor of the right distal femur. At one year follow up, the patient complains almost no pain nor tenderness at operative site and ambulates with partial weight bearing with crutches. The X-ray finding shows satisfactory bony union with progressive bony remodelling.

5.Anesthetic Management for Carotid Endarterectomy: case report.

We have experienced an anesthetic management of 56-year-old male patient with carotid stenosis. Carotid endarterectomy was performed with processed electroencephalography (compressed spectral array, CSA) monitoring. Carotid endarterectomy has significant risk of perioperative stroke and myocardial infarction. For brain protection during carotid cross clamp, body temperature of patient was maintained at 33.5oC and thiopental was infused continuously to induce burst suppression. To maintain blood pressure during cross clamp, dopamine was infused continuously and phenylephrine was injected intermittently. CSA was performed to monitor thiopental-induced burst suppression and ischemic brain insult. After releasing carotid cross clamp, body temperature was raised to 35oC.

9.The relationship of common reasons for encounter and major diagnoses of patients admitted to a hospital.

BACKGROUNDS: It is important to know the contents of health problems in patients in a primary care setting. The aim of this study was to explore the main chief complaints and major diagnoses of patients, who were admitted to a hospital using the ICD-10 and to observe difference according to each department and admission route and how diagnoses were made. METHODS: A total of 18,560 patients who were admitted to a hospital located in Chungnam Province from 1 Jan 1998 to 31 Dec 1998. Main chief complaints and major diagnoses made through the admissions departments were chosen as subjects (medical vs. surgical) and by admission route (emergency vs. OPD)were analyzed. How the diagnoses were derived from the most common chief complaints among medical and surgical departments were analyzed by admissions departments and by admission route. RESULTS: The most common 10 chief complaints revealed no significant difference by admission route in medical departments, but there was some difference in surgical departments. There was some difference in the most common 10 major diagnoses by admission routes in both medical and surgical departments. Abdominal and pelvic pain, which was the most common chief complaints, became a significantly different diagnosis by admission route in both departments. CONCLUSION: Main chief complaints were similar regardless of admission routes, but the diagnoses were different. ICD-10 classification may be useful to classify the diagnoses, but have limitations to classify chief complaints or reasons for encounter. It is necessary to introduce a new classification such as ICPC-2 for reasons for encounter in order to explore the dimension of health problems.

10.Single breath-hold MR imaging of liver.

Single breath-hold gradient echo images with Small Tip. Angle Gradient Echo(STAGE) were evaluated in the study of liver in 16 patients(4 normal liver, 5 hepatoma, 5 cholangiocarcinoma, 1 hemangioma, 1 cavernous transformation of portal vein). We obtained one slice of gradient echo during single breath-hold at the level of pathology defined on conventional spin echo inage. Single breath-hold gradient echo images were compared with spin echo images for image quality and artifacts. Single breath-hold gradient image showed improved resolution of vascular detail and excellent contrast between lesion and adjacent normal liver in hepatoma. Cholangiocarcinoma showed decreased contrast between lesion and biliary trees but improved contrast between lesion and blood vessel. Cavernous transformation of porta vein was noted as tortuous vessel of high signal intensity. Single breath-hold gradient echo scan increased vascular artifact, but decreased respiratory artifact leading to improved image quality. Single breath-hold technique can reduce can reduce imaging time and improve image quality and may be used as complementary method to the spin echo scan.
