2.Clinical Studies on the Supracondylar Fractures of the Humerus in Chilldren

Supracondylar fractures of the humerus is the most common elbow fractures in children. The fractures so designated are in the supracondylar area of the humerus just proximal to the elbow, and do not directly involve the joint or the distal humeral epiphysis. Unfortunately, it can also be one of the most difficult fractures to treat. And the catastrophic picture of Volkmann's ischemic contracture and nerve injuries associated with these fractures, as after successful reduction, the late complications of loss of elbow motion, the chage in carrying angle, myositis ossifgicans, and delayed ulnar nerve palsy still lie ahead. For the period of 4 years and 10 months from May 1979 to March 1985, 47 patients who had been treated for supracondylar fractures of the humerus at the Department of Orthopaedic Surgery, college of medicine, Soon Chun Hyang University Hospital, are presented, The results obtained are as follows; 1. Supracondylar fractures of the humerus frequently occured between the ages of 4 and 9 years (70.2%) and the sex ratio was 2.1: 1 in male to female. 2. Of all fratures, 89.4% were the extension type and the left side was predominent in 55.3% than right. 3. According to the Holmberg clasification, Group 1 was 6 cases(12.8%), 10 cases(21.3%) in Group 2, 11 cases(23.4%) in Group 3, and 20 cases(42.5%) in Group 4. 4. Closed reduction with percutaneous pinning showed little changes in carrying angle. 5. Limitation of motion was most frequent complication after closed reduction with plaster cast immobilization. 6. By the modified Mitchell and Adams' criteria for grading results, excellent were obtained in 55.3 %, good in 44.7%, and zero in unsatisfactory. 7. In the neurovascular injuries associated with fractue, there were 4 neural injuries, and 2 vascular injuries. In 4 traumatic neuropathies, median nerve was involved most commonly and the were recovered spontaneously within 5 weeks. 8. Average time from injury to reduction was 1.6 day and delayed reduction or operation made worse the prognosis in most cases.

4.A Clinical Study of Judet Plate Fixation for Subtrochanteric Fractures of the Femur

Fractures in the subtrochanteric region of the femur present more formidable problems in treatment than do other trochanteric fractures, because such severe stresses exit at the fracture site. Therefore, as a rule we prefer to treat subtrochanteric fractures by operative means if possible. Many internal fixation devices have been recommanded for use in subtrochanteric fractures and each of these devices have advantages in certain types of subtrochanteric fractures, and their selection should be based on the individual fracture anatomy. A total of 20 patients with fracture of the subtrochanteric region of the femur, who were treated by internal fixation with Judet plate between January 1982 and December 1988 have been reviewed. Our conclusions were as follows; 1. Average age was 38 years old and most common cause of injury was traffic accident. 2. Cast immobilization was done after Judet plate fixation. Partial weight bearing ambulation was enabled from POD 10 weeks and full weight bearing ambulation was enabled from POD 16 weeks. 3. 18 in 20 cases, primary bony union was happened and the mean duration of radiological bony union was 24 weeks. 4. 3 cases of complications, metal failure with refracture (2 cases) and screw loosening (1 case), were occured due to early weight bearing in the post operative course. 5. Judet plate fixation was gave satisfactory result in uncomminuted subtrochanteric fracture, especially Seinsheimer's type I, II-A,B,C or Fieldings type I, II. 6. From a consideration of these series, Judet plate showed easy to use, perfect adaptation and rigid fixation. We thought Judet plate is one of a good implants for rigid fixation of subtrochanteric fracture without severe medial cortical disruption.

In 1971, for purpose of reduction of peak stress occuring in cement and trabecular bone of the acetabulum, Harris reported a series of metal-backed acetabular components with removable polyethylene liners that could be replaced if excessive wear occured. Modular designs of hip prostheses have become popular recently, but they introduce the risk of disassembly of the components postoperatively. And introduce of loosening and infection of femoral or acetabular components by debris of polyethylene liner. We report a case of mechanical disassembly of polyethylene liner in cementless metal backed cup, and review previously reported articles.

6.A Clinical Study on Urinary Tract Infection in Infants and Children.

This is a report of clinical study on the 63 patients of symptomatic urinary tract infection who were hospitalized at the Department of Pediatrics in Kangnam General Hospital during the period between the May of 1989 and the September of 1992. The observation results were as follows: 1) The frequency of urinary tract infection (UTI) was the highest in children under the age of one year (66.7%). 35 patients were male, while the other 28 were female. The ratio of male and female patients was 1.25:1. 2) Fever was observed in most of the cases. Systemic nonspecific manifestation was predominant in the infancy and early childhood, whereas local symptom of UTI was predominant in the late childhood. Among the 35 male patients, 32 cases were phimoses. As for the other 3 cases, Phimoses were not identified. 3) E. coli had the most frequency (65.3%), followed by Klebsiella, Enterobacter, Pseudomonas in descending order. 4) In the vitro antimicrobial susceptibility test, Gram negative organisms such as E. coli Klebsiella, Enterobacter were sensitive to cefotaxime, amikacin, and ampicillin/sulbactam. Meanwhile, Gram positive organisms such as Staphylococcus, Enterococcus were sensitive to vancomycin and cephradine. 5) The renal ultrasonography test was performed for 59 patients, 13 cases of whom showed abnormal findings. 6 cases out of the 13 abnormal patients had hydronephrosis. 6) DMSA renal scan was performed for 40 patients within 2 months of an acute pyelonephritic attack. Signs of pyelonephritic change were found in 18 patients. DMSA scan was repeated 4~12 months later in 4 of these patients. This showed renal cortical scarring in all patients. 7) Voiding cystourethrographic findings in 38 patients showed vesicoureteral reflux in 13 patients. 8) We performed urine culture again after 48 hours from the beginning of therapy and 85.5% of the cases became sterile. 9) The recurrent percentage of UTI was 23.8% with the ratio of 1:1.5 between male and female. Most of the patients were clildren under the age of one year. 10) Operations were made on two cases showing the increase of reflux during the follow-up of unilateral Grade ll and Grade 3 reflux, respectively. Operations were also performed on other two cases with bilateral Grade IV reflux. All the cases were good after the operations. In the meantime, as for another two cases having the right reflux of Grade lland the bilateral reflux of Grade lll, the refluxes were able to be reduced by prophylaxis only, in the course of following up.

7.Change of Sexual Function after Castration in Patients with Advanced Prostatic Carcinoma.

PURPOSE: The limited information regarding preservation of erectile function following castration is based on self-reports by castrated patients. Therefore, we evaluated the erectile status in 38 patients with advanced prostatic cancer. MATERIALS AND METHODS: Castrations were achieved by bilateral orchiectomy, estrogen therapy of both. Patients answered the questionnaires regarding the medical status and erectile function before and after castration, and the blood levels of testosterone were assessed. Especially in seven patients, penile circumference and erection quality were monitored during the visual sexual stimulation. RESULTS: 11 patients(58%) out of 19 potent men achieved functional erection after castration. Mean serum testosterone level was 0.31 +/-0.19ng/ml in men who were potent after castration and 0.06+/- 0.04ng/m1 in those not potent(p<0.05). No statistically significant differences were noted in age, interval after castration, method of castration, degree of gynecomastia, stage of prostatic cancer and doing radiation therapy between the men who did and did not achieve erection(p>0.05). CONCLUSIONS: Following castration, sexual potency and libido decreased markedly in most cases, but 58% retained some degree of normal sexual potency. And, statistically significant difference was noted only in serum testosterone level between the men who did and did not achieve erection.

8.The Study on the Relations among Ego-identity, Stress, and Internet Addiction in High School Students.

PURPOSE: The Purpose of this study is to investigate th relations among ego identity, stress, and internet addiction in high school students. METHODS: In order to get the data by self-questionnaire, 750 subject were selected from Aug. 20 to Aug. 31, 2008. The data was analyzed by percentage, mean, standard deviation, t-test, one-way ANOVA, Pearson's correlation coefficient, and multiple regression using SPSS/WIN 12.0 program. RESULTS: Ego identity scores were 39.54, stress scores were 64.68, internet addiction scores were 65.34. There were significantly negative correlations between ego identity and stress, between ego identity and internet addiction. But there was significantly positive correlation between stress and internet addiction. Multiple regression analysis revealed that the most powerful predictor of internet addiction was stress related to individual. CONCLUSION: The findings of study suggest that broad internet addiction and stress program should be provided to prevent problems of internet addiction.

10.Cysticercosis at the Level of Spomdyloisthesis: A Case Repport

Cysticercosis is due to a larva of Taenia Solium. It is frequently reported in developing countries. It usually affects subcutaneous tissue, muscles and brain. But, spinal cysticercosis is rare and is usually diagnosed at surgery. Authors experienced a case of spinal cysticercosis accompanied with spondylolistheses at the same level. Space occupying lesion and arachnoiditis, confirmed by MMCT and MRI, were noted at the same level of spondylolistheses. So, it was very hard to make dicision whether we should open up the dura or not.
