1.Immunotherapy of Warts with DNCB.

Fifty-one cases of various warts were treated with 1-cbloro-2,4-dmitrobenzene (DNCB), and divided these warts into two groups by the method of treatment. The first group was treated by challenge on normal skin a,fter sensitization and the second group was treated by challenge on wart lesion after sensitization. The number of first group was 27 consisted of 13 of verruca vulgaris, 12 of verruca plana and 2 of condyloma acuminatum. The number of second group was 24 consisted of 10 of verruca vulgaris, 11 of verruca plana and R of verruca plantaris. The results are summarized as follows; 1. Thirty-one of all 51 cases (60. 7%) were cured. 2. Fifteen of 27 cases (55.5%) were cured in first group. 3. Sixteen of 24 cases (66. 6%) were cured in second group. 4. All 3 cases of verruca planaris were cured in second group. 5. Eight cases were cured only by sensitization and consisted of 6 of verruca plana and of verruca vulgaris. 6. There was no significant difference in cure rates of warts between the group within 2 years of duration and the group over 2 years of duration. 7. There were no severe adverse reactions.

2.Studies on Side Effects of Various Soaps, Cosmetics and Topical Medicaments: I. Path Tests with Some Materials.

The routine closed patch tests were performe0 with 41 kinds of domestic soaps, detergents, cosm etics and topical medicaments (incIuding a adhesive plaster) on the intact skin of the back of 100 adult males without any dermatoses, to evaluate their side effects on the skin. 1, None of the materials showed no positive reaction, 2, The over all positive reactions were observed in 21.l% of my subjects, including a 1+reacti ons in,l6,4% and a 2+or 3, In 4.7% 3, In 12 kinds of soaps, Positive reaction wer observed in 19-33% positive of my subjects (average Z7.8%) 4, In 4 kinds of detergents, positive reations were observed in 6-19% of my subjects (average 9,7%), in which the highest reaction rate revealed was IVonderful of Dongsan fats and oils co.. 5. The positive reactiors in 3 kinds of toothpastes were observed in 37-61% of my subjects (average 50.7%). 6. The positive reactions to the adhesive plaster were observed in 25% of my subjects. Arnong 17 kinds of topial medicaments, positive reaction rates above 10 shown were Aroos oirtment of Sarn-A phaxmaceutical co. (49%), PM spirit of Kyungnam pharmaceutical co. (16%), Periio ointment of Namgang pharmaceutical co. (13%), Badional-Gel of Hanil pharm-aceutcal co. (10%) and Antiphlamine of Yuhan corporation (10%). 8. In 4 kinds of cosmetic creams, all showed higher positive reaction rates, i.e, "Masan Joongang cream of Yeinvonlang No I, 41%;No.2, 35%;N0.3, 32% and E.mong cream" of Daido phanna-ceutical co,44%.

3.A Case Repori of Lichen Amyloidosis.

A typical case of lichen amyloidosis is reported. This case is considered to be the first one in Korea. Intralesional injection of prednisolone proved to be effective in this case. The etiologic and therapeutic aspects of this disease are discussed.

5.Contact Urticaria - Relationship between Blocking Effect to Wheal Formation and Antiinflammatory Potencies of Some NSAID.

This study was undertaken to investigate the response of non-immunologic contact urticaria(NICU) test before and after ingestion of cyclo-oxygenase inhibitors such as naproxene, ibuprofen and mefenamic acid. Forty patients who showed positive reaction to 5% benzoic acid (BA) in petrolatum by 20 minutes closed patch test were chosen and divided into 3 groups. Group I was consisted of 13 patients who were taken naproxene 250mg bid, group II, 14 patients, taken ibuprofen 600mg bid, and group III, 13 patients, taken mefenamic acid 500mg bid. All the patients were tested with 5%, 2.5%, 1%, 0.5% and 0.1% BA in petrolatum using Finn chamber on Scanpor tape on the right arm before medication and next day on the left arm after medication of each day. Mefenamic acid did not show any significant differences before and after ingestion of drug. Naproxene reduced reaction about half of patients. Ibuprofen reduced reaction in almost all patients and blocked reaction completely in 9 of 13 patients. This results suggested that there was no correlation between blocking effect to BA induced contact urticaria and so called anti-inflammatory potencies of naproxene and ibuprofen, and that NICU by BA is partly mediated by prostaglandins(PG) or mediated by other mediators, which were potentiated by PG, except histamin.

6.Effect of suction blister fluid and IFN-gamma on HLA-A,B,C and HLA-DR espression in cultures human melanocytes.

HLA-A, B, C and HLA-DR are very important in the regulation of immune response and of transplant success or failure, but there have been a few reports about F3I A expression in melanocyt.es, a minor population of cpidermal cells. In the treatment of stable or segmental vitiligo, allogeneic melanocytes graft could t)e potentially useful if rejection does not. occur. The aim of this study was to examine the expression of HI.A-A, B, C and HI.A-DR in cultured hurnan melanocytes with and without. stimulation by INF-r and suction blister fliid. All strains of cultured me- lanocytes expressed HLA-A, B, C regardless of stmulation, but HLA-DR was expressed only following stimulation of INI-r. HLA expression in me]anocytes appears to be similar to that in keratinocytes.

8.DNCB and NBT test in Patients with Atopic Dermatitis.

The present study was undertaken to inveatigate whether contaet sensitivity was decreased or not in patients with atopic dermatitis, and to investigate phagocytic function of neutrophils in patients with atopic dermatitis. 1. Dinitrochlorobenzene (DNCB), a compound which uniformly induce hypersensitivity in normal individuals, had been used in the preaent study to investigate cutaneous anergy in 16 patients with atopic dermatitis. Sensitizing does of DNCB (2,000 microgram) was applied and examined at 7th and 14th day for a spontaneous flare. In the absence of spontaneous flare, challenge dose (50microgram) was reapplied and examined. Seven of the 16 cases (43.8%.) had positive reaction to DNCB. These result showed decreased contact sensitivity in patients with atopic dermatitis, which suggested some defect in the cell-mediated immunity in patients with atopic dermatitis. 2. There were some evidence that susceptibility to certain infection was increased in patients with atopic dermatitis. For investigating killing function in phagoeytosis of neutrophils in patients with atopic dermatitis, nitroblue tetrazolium dye reduction (NBT) and stimalated NBT test had performed in 35 cases of atopic dermatitis and 20 cases of healthy control. 1) There was no significant difference in the mean percentage and absolute number of NBT positive neutrophils between whole group of atopic dermatitis (8. 4+4. 9%, 301.2+225.6) and control group (6. 9+2. 9%, 246.2+143. 8). 2) The mean percentage and absolute number of stmulated NBT poeitive neutrophils in whole group of atopic dermatitis (19.6+7.3%, 702. 1+377. 2) did not differ significantly from control group (22.7+5.3%, 812.2+382.2). 3) There was no significant difference in the mean value of NBT aad stimulated NBT test in comparing the mild groud, moderate group and severe group with control group. These resuIts suggested that killing function in phagocytosis of neutrophils in patients with atopic dermatitis was not impaired.

9.Pseudoxanthoma Elasticum.

Two cases of pseudoxanthoma elasticum associated with angioid streaks in both eyes were presented and the literatures were reviewed. The affected sisters were 15 and 18 years female among the five siblings. They had pin head sized yellowish maculopapular eruptions on the sides of the neck and axillae, and also visual distubance of the both eyes. They seemed to be autosomal recessive inberitance in their family background. There were no other complications.

10.Argon Laser Therapy of Nevus Flammeus.

Nevus flammeus is benign but cosmetically devastating congenital vascular malformation whose past treatments have been marred by unacceptable complication or by unacceptable ineffectiveness. The argon laser is a therapeutic device which has been newly applied to this condition. Blue-green argon light is absorbed by the intraluminal red hemoglobin molecule, leading to the destruction of blood vessels. This study was undertaken to evaluate the therapeutic effect, to look for complications caused by the argon laser on nevus flammeus and to investigate the prognostic values of the color, the degree of blanching and the histological pattern of lesion after laser treatment. All patients were treated with Coherent Radiation Model No. 1,000 Laser with a spot size of 1mm diameter. The power range was 1.8W (irradiance of 229W/cm2), the pulse duration was 0.4 second and the repetition rate was 120 impulses per minute. The result was evaluated according to the criteria of Gilchrest(Table 1). Among the 109 patients who have been treated between November, 1981 and July, 1982 at the Department of Dermatology, Seoul National University Hospital, data from the 49 patients who could be followed up was evaluated and analyzed.
