1.The Effects of Perceived Stress on Cognition in the Community Elderly.

PURPOSE: The purpose of this study was to investigate the effects of perceived stress on cognition in the community elderly. METHODS: This was a descriptive study. Data were collected using individual-based interviews from 40 senior residents at the hall for the elderly in S City. The period of data collection was from June 19 to July 7, 2006. The tools of data collection were Mini-Mental State Examination (Folstein, Folstein, & McHugh, 1975) and Perceived Stress Scale (Cohen, Kamarck, & Mermelstein, 1983). RESULTS: Cognition showed significant differences according to gender, education, and regular exercise. Cognition was significantly correlated to stress and age. Stress was significantly correlated to orientation, recall, and attention/calculation. In the results of stepwise multiple regression, factors affecting cognition were stress, age, and gender. CONCLUSION: It is necessary to prepare health promotion programs that can reduce stress level in the community elderly.

2.Estimation of Joint Risks for Developing Uterine Cervix Cancer in Korea.

OBJECTIVE: This study was aiming at estimating the joint effects of various risk factors associated with uterine cervix cancer in Korea. METHODS: Data obtained from a case-control study were analyzed with a multiplicative model. RESULTS: After adjustment for age and husband's educational attainments, the family history of cervical cancer (OR=2.1, 95% CI=1.2-3.9), unstable marital status due to separation, by death or divorce, etc. (OR=2.8, 95% CI=1.7-4.6), and a large number of deliveries (> or = 3 vs. nulliparous OR=6.5, 95% CI=1.4-29.9) increased the risk of uterine cervix cancer. Conversely, first sexual intercourse at an older age (> or = 25 years vs. <19 years OR=0.4, 95% CI=0.2-0.6) and husband's circumcision (OR=0.7, 95% CI=0.5-1.0) decreased the risk. In the multiplicative model, the highest joint risk (OR=39.2, 95% CI 5.9-258.9) was observed in women with a family history of uterine cervical cancer, an unstable marital status, where the ex-husband was not circumcised, with 3 or more delivery experiences, and having her first sexual intercourse when younger than 19 years of age. However, women without a family history of uterine cervix cancer, married to a circumcised husband, having had her first sexual intercourse at 25 years or older, and nulliparous, showed the lowest joint effect (OR=0.3, 95% CI=0.1-0.5). CONCLUSION: As carcinogenesis is a complex action involving various factors, we consider a joint effects approach to be appropriate in an epidemiological study on risk factors for uterine cervix neoplasms.cervix neoplasm.

3.Effects of Topically Applied 0.1%Dexamethasone on Endothelial Healing and Aqueous Composition Following Experimental Corneal Alkali Wounds.

The effects of topical dexamethasone on the endothelial healing and the change of aqueous composition were examined in the repair process of experimental corneal alkali wounds. Corneal alkali wounds were induced, then the eyes were treated topically with either 0.1%dexamethasone or abalanced salt solution[BSS]4 times per day for 8 weeks. Endothelial wound morphometry was performed after alizarin red and trypan blue staining. The concentrations of ascorbic acid, glucose, and the ions, Na, K , Ca2 and Mg2 , were measured in the aqueous humor. Endothelial healing in control corneas showed a biphasic pattern of healing:an initial short-term healing for the first week and then a late long-term healing following a secondary endothelial breakdown. Topical administration of 0.1%dexamethasone deterred endothelial healing during the early period and prevented secondary endothelial breakdown. Total repair process of endothelium was accelerated by the dexamethasone treatment. Among the various components of the aqueous humor examined, ascorbic acid seemed most sensitive to change caused by the alkali injury and dexametha-sone treatment. The present data indicate that dexamethasone may have a therapeutic potential in the management of endothelial healing after corneal alkali injury.

4.The Effects of an Aerobic Exercise Program on Mobility, Fall Efficacy, Balance, and Stress in the Elderly at Senior Centers.

PURPOSE: This study was conducted in order to investigate the effects of an aerobic exercise program on mobility, fall efficacy, balance, and stress in the elderly at senior centers. METHODS: This research was conducted as a quasi-experimental pretest and posttest control and experimental comparison study. The subjects were 41 senior residents (Exp.=18, Cont.=23) who aged above 65 at senior centers in S City. The period of time for data collection and intervention was from August 25 to December 5, 2008. RESULTS: Mobility (t=-3.10, p<.01) in the experimental group were significantly higher than those in the control group without an aerobic exercise program. However, fall efficacy (t=1.28, p=.207), balance (t=-.53, p=.602; t=.36, p=.723), stress (t=-1.32, p=.199) in the experimental group was not significantly higher than that in the control group. CONCLUSION: These findings confirmed that aerobic exercise programs make a contribution to improving mobility in the elderly at senior centers.

5.Effects of Topically Applied Na-Hya luronan on Stromal and Endothelial Healing in Experimental Corneal Alkali Wounds.

The effects of topical sodium-hyaluronan on the stromal and endothelial healing were examined in the repair process of experimental corneal alkali wounds. Corneal alkali wounds were produced in one eye of each rabbit by applying a 5.5 mm round filter paper soaked in 1N NaOH onto the central cornea for 60 seconds. Then the eyes were treated topically with either 1% Na-HA(the treatment group)or a phosphate buffered saline(PBS)(the con-trol group)4 times per day for 3 weeks. Endothelial wound morphometry was performed after alizarin red and trypan blue staining. The stromal healing was assessed by counting polymorphonuclear leukocytes(PMNs)and keratocytes in the central and marginal wounds areas. The stroma treated with Na-HA had less PMNs than that of the control group during the early healing period. The defect area of the endothelium was significantly smaller in the Na-HA treated group than in the control corneas. The present findings indicate that topically applied 1%Na-HA affects stromal and endothelial healing during the early repair process after corneal alkali wounds.

6.Effect of Experimentally Induced Anisometropia on Binocular Vision.

We investigated the effect of experimentally induced anisometropia on binocular function. Unilateral myopia, hyperopia, or astigmatism(Axis 90degrees) was induced in eighteen adults with normal binocular function using softcontact lenses. Sensory status was assessed by measuring stereoacuity, Worth four dot test, and Bagolini lens test. Binocular function of all subjects decreased with increasing levels of anisometropia. In Worth four dot test, foveal suppression increased in proportion to the levels of anisometropia. Stereoacuity was similarly degraded by the induced anisometropia. Bagolinilens responses were normal in almost all subjects. Our study suggest that experimentally induced anisometropia cause significant abnormalities in binocular function in adults.

8.Serum Prealbumin Affected More by Serum CRP Than by Residual Burned Surface Area.

PURPOSE: Prealbumin is known as a biochemical marker for assessing nutritional status, and it is influenced by a systemic inflammatory condition. This study aims to find any correlation between patients' low serum prealbumin in electrical burn and unhealed burn surface area and insufficient nutritional support. METHODS: Data were collected by a review of the medical charts of patients admitted to Hanil General Hospital for electrical burn. Laboratory results such as prealbumin, albumin, total lymphocyte count (TLC), and C-reactive protein (CRP) were collected and tested every week. Residual burn surface area (residual BSA) during a specific period was calculated from the surgery record. Statistical analysis was conducted using Pearson's correlation and multiple regression analysis. RESULTS: A total of 30 subjects were selected, all male. Average total burn surface area was 20.9±14.9%, and patients were operated on about three times after admission. There was statistical significance among all variables in Pearson's correlation test, but in multiple regression analysis, albumin and CRP were significant compared with prealbumin. CONCLUSION: The results could indicate that burn causes a systemic inflammatory reaction, which could affect the serum prealbumin level. Further study concerning the biological plausibility of each variable is needed.

10.Effects of Halothane, Fentanyl, and Propofol-Fentanyl Anesthesia on Functional Recovery of Stunned Myocardium in Dogs.

BACKGROUND: Stunned myocardium may be mediated by intracellular Ca2+ overloading or oxygen derived-free radicals. Halothane and propofol have been shown to block Ca2+ channels. Propofol is also known to have antioxidant properties. The present study was aimed to investigate the effects of anesthetics on recovery of postischemic, reperfused myocardium in open-chest dogs. Incidence of ventricular arrhythmia upon ischemia and reperfusion was also determined. METHODS: Forty dogs were subjected to 15 min occlusion of left anterior descending coronary artery (LAD) followed by 3 hr reperfusion during halothane (n=10), fentanyl (n=12), or propofol plus fentanyl (n=11) anesthesia. Regional contractile function was assessed using percent systolic shortening (%SS), the preload recruitable stroke work slope (Mw), and peak systolic intramyocardial pressure (IMPs). Diastolic function was evaluated using time constant for isovolumic intramyocardial pressure decline of left ventricle (IMP-tau) and percent post-systolic shortening (%PSS). RESULTS: %SS in the halothane, fentanyl, and propofol-fentanyl groups was similar at 3 hours of reperfusion (58%, 60%, and 55% of baseline value, respectively). Moreover, Mw recovered to the baseline values in the early reperfusion period in all three groups. However, IMP-tau was significantly prolonged in the halothane group throughout the 3 hour reperfusion period, whereas it remained unchanged in the fentanyl and propofol-fentanyl groups. Coronary occlusion was associated with 9, 33, and 0% mortality rate due to ventricular fibrillation upon ischemia and reperfusion in the halothane, fentanyl, and propofol-fentanyl groups, respectively. CONCLUSION: These findings indicate that halothane, but not fentanyl and propofol- fentanyl, impairs myocardial relaxation, while recovery pattern of contractile function do not differ among three groups, and that halothane and propofol reduce reperfusion arrhythmia in the canine model of myocardial stunning.
