2.A case report on correction of Angle's Class III malocclusion with macroglossia.

This is case report of true class III malocclusion with macroglossia is corrected by glossectomy in 13 years female patient. After orthodontic treatment, the patient is bound to glossectomy because the corrected condition is relapsed to the previous condition due to relatively enlarged tongue compared with the original dental arch. By the interpretation of the cephalogram and model analysis, it is approved that the growth pattern and direction are normal range and mandible is located anterioly to the cranium. The results are follows: 1. We could treat the true Cl III malocclusion. 2. We could prevent the relapse of the treated condition by the surgical intervention, such as partial glossectomy. 3. Sensory, speech, swallowing and so other functions after the operation have been with in normal limit without any serious complications or seguellae.

4.Effects of Foot Reflexzone Massage on State-Anxiety and Discomfort in Ovarian Cancer Patients Receiving Chemotherapy.

PURPOSE: This study was conducted to investigate the effects of foot reflexzone massage on state - anxiety and discomfort of ovarian cancer patients receiving chemotherapy. METHOD: A quasi experimental design with a non equivalent control group and non synchronized design was used. The experiment was conducted from October 1, 2003 to September 30, 2004. The subjects consisted of 40 patients admitted to C University Hospital. Twenty subjects were assigned to the experimental group and received foot reflexzone massage everyday for 3 days. The other 20 subjects were in the control group and received routine care. Foot reflexzone massage was done on both feet of the subjects for 30 minutes for 3 days using standard protocol. The 'State-Anxiety Inventory' developed by Spielberger was used to measure the degree of state-anxiety. Discomfort was assessed using the 'Symptom Distress Scale' of McCorkle and Young. Data was analyzed by a SAS program using t-test, Wilcoxon signed rank sum test and Wilcoxon rank sum test. RESULTS: State-anxiety and discomfort of subjects receiving foot reflexzone massage were significantly lower than those of the control group. CONCLUSION: These findings indicate that foot reflexzone massage could be an effective nursing intervention for relieving state-anxiety and discomfort in ovarian cancer patients.

5.Changing pattern of serum leptin concentration in women undergoing clomiphene citrate challenge test or controlled ovarian hyperstimulation.

OBJECTIVE: To evaluate the changing pattern of serum leptin level and the correlation between estradiol level during the clomiphene citrate challenge test(CCCT) or controlled ovarian hyperstimulation(COH) cycle. METHODS: Twenty-seven women who underwent CCCT and fourteen women who underwent controlled ovarian hyperstimulation were recruited to measure the serum leptin level. After correction of serum concentration with body mass index(BMI), changes of leptin level and correlation with serum estradiol level during CCCT and COH cycle were analyzed. RESULTS: Circulating leptin levels were significantly correlated with BMI at each time point in CCCT cycle(P<0.01). In CCCT cycle, leptin/BMI level was significantly increased at midluteal phase compared to that of menstrual cycle day 3 and 10(p<0.05). In women with regular menstruation, leptin/BMI level at midluteal phase was significantly higher than that of menstrual day 3 and 10, but this difference was not seen in women with irregular menstruation. The leptin/BMI level in COH cycle showed increasing tendency throughout ovarian stimulation. But there was no significant correlation between leptin/BMI and estradiol level in CCCT and COH cycle. CONCLUSION: There is a significant correlation between BMI and circulating leptin level. Midluteal leptin level is significantly higher than that of follicular phase in CCCT cycle, and there is an increasing tendency in leptin level after ovarian stimulation in CCCT and COH cycle without statistical significance. These findings suggest that circulating estradiol concentration has no major influence on circulating leptin level.

6.Clinical Efficacy and Safety of Gepefrin in the Treatment of Hypotension.

BACKGROUND: The proper management of the patients with hypotention remains a difficult clinical problem. This study was designed to evaluate the efficacy and safety of gepefrin in the treament of symptomatic hypotensive patients in outpatient clinic of university hospital. METHODS: Thirty milligrams of Gepefrin was given twice daily for 4 weeks in 30 patients with hypotension(6 males, 24 females ; 23-60 years of age). Blood pressure, heart rate, subjective symptoms and side effects were checked before medication and at the end of 1st, 2nd, 3rd, 4th week of medication. RESULTS: 1) Grpefrin increased sitting systolic blood pressure significantly(p<0.05) by the end of 1st week and thereafter. 2) Hypotensive symptoms were improved in 76% of hypotensive patients and overall rate of usefulness was 70%. 3) Heart rate did not change significantly. 4) One patient complained of loose bowel movement and another one, of anorexia among 30 study patients and all laboratory parameters were within normal limits before and after drug treatment, thus the rate of safety was 93%. CONCLUSION: Results from this study suggests that gepefrin 60mg daily increases the blood pressure mildly and improves the symptoms moderately in symptomatic hypotensive patients.

7.A Study of Case-Based Adult Advanced Cardiac Life Support(ACLS) course in Korea.

BACKGROUND: To describe the fast experience of case-leased advanced cardiac life support(ACLC) course in Korea. METHODS: We have given case-based ACLC course to 13 nurses(6 emergency nurses, 4 coronary care unit muses, 3 professors) and 17 physicians(4 emergency physicians, 12 emergency residents, 1 intem). We performed the case-based ACLS course according to 1992 American Heart Association guidelines and recommendations for advanced cardiac life support by american ACLS instructors(1 pulmonologist, 4 critical care nurses). We performed final theoretical written test and 2 times written survey (immediate and 100th day after the course) about the course. RESULTS: On final written test, all practitioners answered at leasts 70% of the questions correctly. There was no significant difference between nurses and physicians(86.2+/-3.6 of physicians and 82.5+/-6.8 of nurses, p=0.06). 90%of participants considered that case-based advanced cardiac life support was acceptable. 100th day after the course, 93%of participants answered that ACLS course have been helpful on his/her job and also want retraining of ACLS course. 70%of participants considered that ideal ACLS training committee in Korea is the Korean Society of Emergency Medicine. CONCLUSION: Case-based ACLS course is a useful educational method far physicians and nurses in Korea. In the future, we should organize Korean resuscitation committee and then make guidelines for ACLS, and then continuously educate physicians and nurses.

8.Two Cases of Psoriatic Arthritis.

A psoriatic patient may have rheumatoid arthritis, psoriatic arthritis(or both), osteoarthritis or gout. In so far as possible, each of these must be distinguished on clinical grounds with some help from laboratory tests. Psoriatic arthritis is very similar to rheumatoid arthritis but clinically, it is regarded as a unique disease entity, which is found in 1% to 32% of psoriatic individuals. We herein report two cases of psoriatic arthritis that are thought to be distal type and arthritis mutilans on the basis of clinical, serological and radiological features.

9.The Current Status and the Perspectives of Nutrition Survey.

The Korea National Health and Nutrition Examination Survey is a nationwide health survey in Korea. This survey is composed of three parts: the Health Interview Survey, the Health Examination Survey, and the Nutrition Survey. The aim of this study was to refine the nutrition survey system in the Korean National Health and Nutrition Examination Survey (KNHANES). Based on the literature review, we summarized international survey systems and the limitations of the current system in Korea and proposed an improved system. We suggested the following: 1) A government agency which will be exclusively responsible for KNHANES overall is needed. 2) The current short-term nutrition survey should be a year-round survey. 3) The objectives of this survey should be written with more clarity and detail. 4) Biochemical measures in relation to nutritional status should be added. 5) The budget to set up various databases in the nutrition survey should be increased. 6) Various other issues should be addressed, including the definition of raw data (i.e., whether nutrient intake data per day is raw data), ethical issues in survey techniques, and strategies to produce evidence for the public using this survey data. Not all suggestions could be adopted in the short-term, but should be integrated into long-term goals for survey system improvement.

10.A Clinical Study on Softening E.C.T. & Comparison of Propofol and Pentothal as Anaesthetic Agents on Seizure Duration.

The authors performed this preliminary study to investigate the effect of softening E.C.T. and propofol was compared to pentothal for induction of anaesthesia for E.C.T on seizure duration. The results were follows ' 1) E.C.T. was performed in 60 psychiatric inpatients who were admitted during the study period. Of them 51.7% were diagnosed as schizophrenia, 21.6% as major depressive disorder, 16.7% as bipolar I disorder, manic and 10% of others. 2) Mean number of E.C.T. was 12.2 times a patient. 3) The most common target symptoms were persecutory delusion in schizophrenia, psychomotor retardation or agitation in major depressive disorder, and violent aggressive behavior in bipolar I disorder, manic. 4) Pre-ECT medication usually used were atropine 0.0093mg kg(-1), pentothal 2.76mg kg(-1) or propofol 1.42mg kg(-1). 5) The duration of seizure, as measured clinically, was reduced with propofol(20.5 sec) in comparison with pentothal(35.7 sec)(p<0.001). This suggest the possibility that additional treatment may be needed for the same clinical effect in psychiatric illness when propofol is used as the induction agent.
