1.CT findings of the hydatidiform mole

Even though ultrasonography is a primary tool for evaluation of gynecological pelvic mass, CT can be helpful when ultrasonography is suboptimal either because of abundant intestinal gas or marked obesity. Authors experienced 3 cases of hydatidiform mole demonstrated by CT. The findings of hydatidiform mole are enlargement of uterus, low density in the uterine cavity, irregular soft tissue mass with small multiple cysts, and strong contrast enhancement of the soft tissue mass within the uterine cavity. CT can be a useful diagnostic tool for differentiation of hydatidiform mole from choriocarcioma because of good visualization of uterine wall and pelvic structures.

2.Histopathologic Study of Erythema Nodosum: Licke Lesions in Behcet's Disease.

A histopathological study of erythema nodosum-like lesions in Behcet's disease was performed on 55 patients with incomplete and suspect types of Behcet's disease. Relatively common characteristics in histopathology of erythema nodosum-like lesions could be found in the patients with incomplete type of Behcet's disease. The histopathologic findings of erythema nodosum-like lesions in 21 patients with incomplete type of Behcet's disease were as follows : l. A moderate lymphocytic infiltration was found around the blood vessels and the sweat glands in the dermis. In the subcutaneous tissue, besides lymphohistiocytic infiltration, neutrophils were present in significant number in 9 cases (42.9%) 2. Vasculitis of small vessels was found within the periphery of fat lobules in 7 cases (81.0%), Five cases (23.8%) also showed thrombophlebitis. Seven cases (33.3%) showed histiocytic granuloma formation within the fat lobules. Microabscess formation in the subcutaneous tissue was seen in 5 cases (23.8 %), but it was not certain whether this was the charateristic feature.

3.Xanthoma of the Corneoscleral Limbus: A Case Report.

We describe a case of xanthoma on the corneoscleral limbus in a 21-year-old man. It was characterized by a localized, round, elevated, yellowish mass on the right superonasal limbus with 6mm in size. There was no correlation with systemic or metabolic diseases such as hyperlipidemia and the microscopic examination revealed scanty fibrous element.

5.Clinical and Histopathologic Study of Acral Melanoma.

A total of 68 primary melanorna cases diagnosed at Seoul National University Hospital for the past 10 years from 1978 to 1987 were included for this clinico pathological study. Among the 31 primary cutaneous melanomss, 19 cases (61.3%) occurred on hands or feet, especially on the plantar surfaces. The average age of patients with acral melanomas was 49.3 years at the tirne of diagnosis and the male to female ratio of 19 cases was 3.8 to 1. The clinical and histopathologic findings showed that most ecral melanomas, if not all, were the acral lentiginous type. A high incidence of metastasis (73.7%) was recognized in acral melanomas.

6.Photodensitometric tracing of mach bands and its significance

Mach bands, a visual phenomenon resulting from lateral inhibitory impulsess in the retina, are recognized aslucent or dense lines at the borders of different radiographic densities. A number of clinical situations have been described in which Mach bands may cause difficulty in radiographic diagnosis. Photodensitometric measurement of the film can differentiate the true change in film density from the Mach band which is an optical illusion. Authors present several examples of photodensitometric tracings of Mach bands, with the brief review of themechanism of their production.

7.Attitude, Beliefs, and Intentions to Care for SARS Patients among Korean Clinical Nurses: An Application of Theory of Planned Behavior.

PURPOSE: This study examined Korean clinical nurses' intentions to care for SARS patients and identify determinants of the intentions. Theory of planned behavior was the framework to explain the intentions of Korean nurses for SARS patients care. METHODS: A convenient sample of six hundreds and seventy nine clinical nurses from four university-affiliated hospitals located in Seoul and in Kyung-gi province was used. Self-administered (83-items) questionnaire was used to collect data. Intentions, attitude, subjective norm, perceive behavioral control, behavioral beliefs, normative beliefs, and control beliefs were the study variables. All items were measured using 7-point Likert scale (-3 to +3). Data were analyzed using descriptive statistics, Pearson correlation method, and stepwise multiple regression methods. RESULTS: Intentions and attitudes toward SARS patient care among Korean clinical nurses were moderate, but their subjective norm and perceive behavioral control of SARS patients care were negative. Stepwise multiple regression analysis indicated that attitude toward SARS patient care, perceived behavioral control, subjective norm were the determinants of the intentions for SARS patients care as theory proposed. Among the behavioral beliefs, "SARS-patient caring would be a new experience", "during SARS-patient caring, I should be apart from my family", "after completing SARS-patient caring, I would be proud of myself being able to cope with a stressful event" and "with my SARS-patient caring, patients could recover from SARS" were the significant determinants. Among the normative beliefs, colleague approval, spouse approval, and physician approval were significant determinants of the intentions. Among the control beliefs, "SARS-patient caring would be a challenge" "SARS-patient caring is a professional responsibility", "tension during the care of SARS patients" and "support from team members" were the significant determinants of the intentions. CONCLUSIONS: Korean clinical nurses in this study were not willing to care for SARS patients and showed negative attitude toward the care. They believed their friends and family were not approved their care for SARS patients. Nurses were in conflicts between professional responsibilities to care for SARS patients and personal safety. This study was the first to understand stress and burden of Korean clinical nurses who are in front line to care for newly developed communicable disease such as SARS. Under the circumstance where several fatal communicable diseases are predictable, conflicts between professional responsibility and their personal risks should be taken into considerations by nurses themselves and by nursing administrators in order to improve quality of care.

8.Infiltrating Orbital Granular Cell Tumor: A case report.

Granular cell tumor is an uncommon neoplasm with uncertain histogenesis that rarely occurs in the orbit. Although it is regarded as a benign lesion, one case of malignancy has been recorded. The authors present a case of recurred, infiltrating orbital granular cell tumor in a 39-year-old woman. On light microscopic examination, the tumor cells showed the characteristic findings of granular cell tumor, but there were hyperchromatic and pleomorphic nuclei with unusual mitotic figures, suggesting a malignant nature.

9.Ultrastructural Changes of the Bile Canaliculi after Common Bile Duct Ligation.

The purpose of this study was to investigate the morphologic changes of the bile canaliculi and its associated structures of the liver induced by common bile duct ligation(CBDL) in the rat. The canalicular surface and lateral surface of the dry-fractured hepatocytes was studied with scanning electron microscopy at 1~6 weeks post ligation. The first week after CBDL, the bile canaliculi were dilated. The microvilli were increased in number and the lumens contained granular materials After 2 weeks or more, the bile canaliculi were dilated to a variable degree, and with irregularity, measuring from 1.5 to 5 micrometer in diameter, and in the advanced stage, the canaliculi showed blunting and the disappearance of microvilli. Some canaliculi had sprouting side branches. At 4~6 weeks post-ligation, the lateral surface of the hepatocytes also showed some irregularity and a tortuous appearance, and numerous small sized microvillous projections were formed. The tubular structures of the proliferated SER distributed adjacent to the lateral surface of the hepatocytes, and the direct connection of a tubular structure and the cytoplasmic membrane was observed. These results suggest that the deformity and loss of microvilli of bile canaliculi reflect the disturbance of bile secretion from the hepatocytes. And prolonged obstruction of bile flow may result in bile excretion via the lateral surface of hepatocytes.
