1.Surgical Treatment for 3 Cases with Established Volkmann's Ischemic Contracture of the Upper Limb

Volkmanns ischemic contracture, despite a relatively small incidence, is one of the most devastating complications following a limb injury. The orthopedic surgeon must be equipped to recognize and effectively treat this complication. Recently the authors have managed 3 cases with V.I.C. with the following results. 1) The etiology showed CO poisoning, arterial injury, wringer injury around the elbow in each case. 2) The method of surgical treatment depended upon deformities and functional loss of individual patient. 3) The end results seemed to be proportionate to the previous deformities and functional less. 4) Early postoperative passive exercises of the digits seemed to be helpful in restoring further range of motion.

2.Vascular Injuries Associated with the Fractures About the Knee

Fractures about the knee including the distal femur and the proximal tibia not infrequently result in vaseular injuries of the popliteal artery and the distal part of the femoral artery. This is often followed by circulatory disturbance and jeopadizes the viability of the lower extremity. During the three and a half year period, from January 1973 to June 1976, 20 cases of vascular injuries complicated by fractures about the knee were treated at the Orthopedic Department of Busan National University Hospital. Review of the cases resulted in the following observations. 1. The lower extremity escaped amputation in 45% the cases (9 of the 20 cases) 2. Femoral arteriography showed that the site of injury was proximal to the Soleus insertion. in 17 cases. 3. Types of vascular injury were contusion in 9 cases, laceration in 7 and total disruption in 4. 4. 5 of the 6 extremities that were treated within 12 hours after injury survived, while only 2 out of t he 9 extremities treated after 24 hours following injury escaped amputation. 5. 6 of the 13 extermities (46%) tredted by interval fixation survived, while 1 out of 5 extremities (20%) treated by traction escaped amputation.

5.Erdheim Chester Disease (ECD): A Case Report.

Erdheim Chester disease (ECD) is very rare non-Langerhans cell histiocytosis (LCH) which occurs in the skeletal system and multiple organs. As it is progressive, sometimes it causes fatal results. However, it is often misdiagnosed as LCH or multiple bone metastasis and, thus, is very difficult to diagnose. In Korea, only 10 cases were first reported in 1999. In particular, there have been a few orthopedic approaches or reports in English-speaking literatures, and no report has been issued in Korea. The authors performed bone biopsy in patients with knee and lower extremity pain who were referred for the integrated treatment. We attempts to report this diagnosis experience with literature review.

6.A Case of Chronic Persistent Photosensitivity.

Persistent light reaction, actinic reticuloid, photosensitive eczema are three chronic photodermatoses with a feature of clinical, histological, and photobiologieal characteristics in common. These conditions were given the name of chronic persistent photosensitivity. Chronic persistent photosensitivity with the feature of photosensitive eczema developed in a 68-year-old male patient who had experienced persisited dermatitis. On phototesting the patient showed extremely severe photosensitivity to the short wave UVB. A skin biopsy showed microscopic finding of eczematous dermatitis.

The skin lesions developed about two years ago insidiously and increased size and numberes of the lesion. Her family background suggests autosomal dominant inheritance.

8.Bizarre Parosteal Osteochondromatous Proliferation: A Report of One Case

In 1983, Nora et. al. reperted 35 cases of bizarre parosteal osteochodromatous proliferations of the hands and feet. All lesions occured on proximal phalanges, metatarsals, or metacarpals. The gross appearance was typically that of a small osteochondroma. Radiologically, the proliferations lacked both central continuity of the tumor with the underlying osseus medulla and flaring of the adjacent cortices. Histologically, the lesion exhibited marked proliferative activity, irregular bohy cartilaginous interface, and enlarged, bizarre, and binucleate chondrocyte. The authors experienced a case of bizarre parosteal osteochondromatous proliferation of the right foot in a 61 year-old female which arised from the proximal phalanges of second toe, hostologic and radiologic findings were consistent with the bizarre parosteal osteochondromatous proliferation. The mass was treated by excision, and neither recurrence, nor malignant change was observed up to one year follow-up period.

9.Overgrowth of the Tibial Shft following Fracture in Children

Longitudinal growth acceleration of the femoral shaft after fracture in children has been recognized since the late nineteenth century, Orthopaedic surgeons have attempted to compensate for the leg length discrepancy by allowing union to occur with some degree of shortening, but logitudinal overgrowth following fractures of the tibial shaft in children has been rarely studied. Thirty-one children, treated for fracture of the shaft of tibia, were studied prospectively to assess the consequent increase in longitudinal growth of the tibia. The material presented in this paper is the result of the study of thirty-one children with fractured tibia who were treated in the Department of Orthopaedic Surgery of Kangnam Sacred Heart Hospital of Hallym University, since 1986 October, the age of the child varied from 3 years to 13 years, these cases have been followed from 30 months to 40 months. We have made a radiologic study of fracture of the tibial shaft with a view to analysing the results according to a number of factors, including the sex and the type, site and degree of overriding of fracture, and method of treatment, and then the degree of overgrowth was compared with unaffected side. 1. The average tibial overgrowth following tibial shaft fracture is 7mm. 2. Growth of the femur was not affected by the tibial fracture.

10.Clinical Observation of Supracondylar fracture of Humerus in Children

Supracondylar fracture of the humerus is the most common fracture of the elbow in children and it can also be one of the most difficult fractures to be treated. A number of methods to manage supracondylar fracture of the humerus in children, have been being practiced and the ideal one is generally agreed with prompt, accurate and anatomical. reduction performed with the least possible trauma, and, it will prevent most complications. It is essential to minimize additional trauma to the already traumatized joint and pericapsular tissues including blood vessles and nerves. 105 patients with supracondylar fracture of the humerus managed at the Department of Orthopedic Surgery, Busan National University from January, 1971 to June, 1976 were reviewed and the following results were obtained. 1. The age of the patients ranged from 2 years to 14 years; the average age was 7. 0 year. 70 patients were boys. 2. The left side was involved in 62 patients; The distal fragment was displaced posterioly in 102 patients and anteriorly in 3 patients The medial displacement in the posteriorly displaced distal fragment was 60 patients. 3. 20 fractures were treated by open reduction and internal fixation. The indications for open reduction were failure to obtain accurate reduction after a few attempts of closed reduction in 15, an open fracture in 2, acute vascular embarrassment in 2, and redisplacement after open reduction in 1. 4. 15 of the 20 fractures treated by open reduction were re-examine 1 to 5 years; The results were normal elbow motion in 10, loss of extension (average 24 degree) in 5, loss of flexion in 2 and neuro-vascular complication in 2.
