The dual muscle flap (Latissimus dorsi+ Serratus anterior muscle) based on the thoraco-dorsal vascular system for limb reconstruction has been infrequently reported. We have used this flap as a free flap for limb reconstruction in four patients (3 cases in lower linb, 1 in upper limb). A Latissmusi dorsi muscle or musculocutaneous flap and Serratus anterior muscle flap were used and all cases healed satisfactorily. The indications for this flap are to resurface two separate defects simultaneously or to resurface very large defects. It has the advantage of requiring anastomosis of one vascular pedicle only. This flap is particularly suitable for resurfacing defects wider than their length, in relation to the long axis of the limb.

Urinary cytology has become an essential element in the diagnosis and management of transitional cell carcinoma(TCC) of the urinary tract. It has the advantage of being noninvasive, inexpensive, and easily accessible. Besides that it can even detect malignancy when unsuspected at cystoscopy. We report a retrospective review of urine cytology in the diagnosis of 83 TCC cases that underwent 295 cytologic evaluation. All patients had biopsy-proven TCC of the bladder, ureter and renal pelvis. The overall inci- dence of the positive cytology cases was 66.2%. To define the cytologic features of tumor cells, we tried to use three cytologic gradings such as "grade 1", "grade 2", and "grade 3" according to the cytologic degree of anaplastic neoplastic cells. These cytologic gades of TCC were relatively well correlated with the histologic grade and tumor invasiveness. This result suggests that the recognition of characteristic cellular features of TCC can suspect the histologic grade and tumor stage. The false negative TCC cases were 78.9%. They showed severe inflammatory or bloody background and a few neoplastic cells. Therefore, a cautious approach for accurate interpretation, personal experience, and proper fixation and processing could expand the role of urinary cytology.

In 1971, for purpose of reduction of peak stress occuring in cement and trabecular bone of the acetabulum, Harris reported a series of metal-backed acetabular components with removable polyethylene liners that could be replaced if excessive wear occured. Modular designs of hip prostheses have become popular recently, but they introduce the risk of disassembly of the components postoperatively. And introduce of loosening and infection of femoral or acetabular components by debris of polyethylene liner. We report a case of mechanical disassembly of polyethylene liner in cementless metal backed cup, and review previously reported articles.

To evaluate antioxidative effect of vitamin E, selenium and zinc sulfate on release of oxygen free radicals from polymorphonuclear leukocytes[PMNs], I measured the amount of superoxide release from human PMNs stimulated by phorbol myristate acetate[PMA]with addition of some antioxidant and antioxidative micronutrients using superoxide-dependent cytochrome c reduction.And to determine a protective effect of them on cultured bovine retinal pigment epithelial cells [RPE]from oxygen radicals, I measured a viability of bovine RPE using MTT assay after incubation with human PMNs and PMA. Vitamin E, selenuim and zinc sulfate are utilized at different concentrations of 1, 10 and 20 microM. Vitamin E and zinc sulfate inhibited superoxide production from PMNs stimulated by PMA from 10 microM concentration gnificantly. But in case of selenium, significant antioxidative effect was not found at each concentration.The antioxidative effect on cultured bovine RPE was evaluated using MTT assay.Among antioxidant and antioxidative micronutrients tested, vitamin E and zinc sulfate inhibited free radical damage to bovine RPE, they increased cell survival rate on culture at concentration of 1, 10 and 20 microM. In contrast, selenium did not increase cell survival rate significantly. With these results, it was found that vitamin E and zinc sulfate had antioxidative effect against superoxide release from PMNs and also protective effect of bovine RPE from free radical damages.Recent studies supported that peroxide may play an important role in causing tissue damage in human and experimental models of ocular inflammation and possibly in Behcet`s disease.It was suggested that antioxidant vitamins and minerals like vitamin E or zinc sulfate might be useful for management and/or prevention of these conditions.

Migratory birds have been suspected as playing a central role in transmission of various trematodes, but few surveys have been undertaken in the Republic of Korea. In the present study, we describe new fauna of microphallid flukes in Korea. Two species of microphallids were found in the intestines of 2 migratory bird species, namely the red necked stints (Calidris ruficollis) and the terek sandpiper (Xenus cinereus), in a coastal area of Gunsan-si, Jeollabuk-do. The microphallids recovered from the latter were 451 microm in length and 217 microm in width, and the eggs were very small, 13 microm by 8 microm. They had the characteristic extracecal vitellaria intruding into the median posttesticular area, belonging to Endocotyle incana. Another microphallids were discovered from both migratory birds, 417 microm in length and 249 microm in width. The cirrus was projecting into the genital atrium in form of male papilla, and bounded by a muscular flap, termed pseudogonotyl. They were identified as Spelotrema pseudogonotyla in consideration of the distinctive male papilla and pseudogonotyl. Besides these flukes, Maritrema obstipum and Gynaecotyla squatarolae also were recovered. This paper is in fact the first report on the presence of Spelotrema and Endocotyle in Korea.

The use of resorbable implants has always been attractive to surgeons because there is no need to remove implant ai'ter fracture fixation. Other advantages include decreased load sharing, multi-taskirv ancl no metal toxicity. But the strength and stiffness of resorhable implants are less than those of metallic implants. Therefore, these implants are suitahle for fixation of particular fractwre sites such as cancellous bone and epiphyscs in which shear loads comprise the major strains. The purpose of this experimental study was to determine whether there are changes in mechanical properties and tissue reactions in the polylactic acid (PLA) rod hy surface moditication using plasma coating or hexafluoropropylene (CF3CF=CF2). PLA rods were inserted into the subcutaneous tissue of back and distal femur in rabbits. Rods in subcutaneous tissue were retrieved for material characterization and those in distal femur were ohtained for histologic observation at postoperative 2, 5, 12 and 16 weeks. The results were as follows; 1. The hydrophobicity of PLA surface was successfully ohtained by plasma coating of hexatluoropropylene gas. 2. Thcre is no significant change in tissue reaction. between controi and plasma coating PLA group. 3. The diametral strength and 3-points bending strength of plasma coating groups were higher than those of control group until postoperative 12 wks, but, diminished at postoperative 16 weeks. In conclusion, the plasma coating of PLA rod using fluorocarbon is a reasonable technique to incrcase the surface hydrophohicily and a promising method to delay the reduction of the strength of PLA rod. Further study on thicker plasma coating and Jong term effect including degradation, nsetaholism and excretion of cotated fluorocarhon will be needed.
