2.Evaluation of Tumor Invasion in Gastric Carcinoma with CT Using Water as an Oral Contrast Agent in Prone Position.

PURPOSE: To evaluate the accuracy of CT using water as an oral contrast material in a prone position in determining the depth of tumor invasion in gastric cancer patients. MATERIALS AND METHODS: Thirty three patients(19 male, 14 female) with surgically confirmed gastric cancer were studied. We performed CT in a prone position after ingestion of 1 liter of pure water. CT findings were classified into 4 groups by the morphologic appearances of infiltrates in the perigastric fat plane :normal perigastric fat(SO), fine mottled densities(S1), irregular aggregated or linear densities(S2) and direct extension and invasion of tumor into contiguous structures(S3). Also we prospectively compared the CT staging with pathologic T staging according to the TNM systems. RESULTS: The overall accuracy of CT staging in determining the pathologic T factor was 69.6%. As we regarded T1 and T2 lesions as one group on CT, the accuracy of CT staging was increased to 80.2% because of a limitation of CT for distinguishing T1 from T2 lesions. CONCLUSION: Prone position CT using water as an oral contrast agent is quite accurate in determining the T staging of gastric carcinoma.

7.Cardiorespiratory Responses of Pilots to Maximal Exercise Loading.

BACKGROUND: To provide some fundamental physiological basis for the physical training of pilots to improve orthostatic intolerance, cardiorespiratory responses to the symptom-limited maximal exercise loading were studied in pilots and non-pilots, and the results were compared. METHOD: Cardiorespiratory reponses to the symptom-limited maximal exercise loading by Bruce protocol was studied in 11 pilots and 11 matched controls (non-pilots). RESULTS: Comparisons of various data at maximal exercise in the pilots with those in the controls revealed that RR, VE/M2, VE/VO2, VE/VCO2, VT/VC and VE/MVV as well as HR, VO2, O2 pulse and AT showed no significant difference. CONCLUSION: The fact that the aerobic power in the pilots is not superior to that in the controls seems to emphasisze the necessity of aerobic endurance training along with muscular strength training to improve orthostatic tolerance of pilots flying modern high-performance aircrafts.

8.Comparison of Clinical Characteristics of Mycoplasma pneumoniae Pneumonia According to the Pleural Effusion.

PURPOSE:Mycoplasma pneumoniae (M. pneumoniae) is a major cause of respiratory infections in school-aged children. Complications of M. pneumoniae pneumonia include atelectasis, pleural effusion, empyema, pneumothorax and bronchiectasis. We evaluated the clinical characteristics of M. pneumoniae pneumonia with pleural effusion. METHODS:A total of 210 medical records of children who were admitted to the Dong-A University hospital due to M. pneumoniae pneumonia from 2000 to 2004 were retrospectively analyzed. Diagnosis of M. pneumoniae pneumonia was based on the single titer of antimycoplasmal antibody higher than 1:320. Enrolled children were divided into Group A (with pleural effusion) and Group B (without effusion). We analysed the differences between the two groups according to sex, age, onset, symptoms, clinical manifestations, laboratory findings and chest x-rays. RESULTS:There were no significant differences in age, sex and clinical manifestations between the two groups. Group A had longer fever durations (9.3+/-7.8 days vs 5.0+/-3.7 days), and a longer duration of hospitalization (10.4+/-6.3 days vs 6.9+/-6.3 days) compare to Group B. Also, compared to the Group B, Group A had higher ESR (49.6+/-32.9 mm/hr vs 28.7+/-20.4 mm/hr), CRP (23.0+/-60.4 mg/dL vs 8.7+/-30.9 mg/dL), SGOT (67+/-74.2 IU/L vs 53.6+/-60.0 IU/L), SGPT (37.4+/-18.6 IU/L vs 26.2+/-16.9 IU/L). There was no significance between antimycoplasmal antibody titer and pleural effusion. CONCLUSION:This study shows that M. pneumoniae pneumonia with pleural effusion has a longer duration of fever and hospitalization, higher ESR, CRP, SGOT, SGPT compare to the M. pneumoniae pneumonia without pleural effusion. We conclude that these findings could be used as the prognostic factors in M. pneumoniae pneumonia with pleural effusion.

10.The Shape and Size of the Frmoral Canal in the Korean for Press Fit Femoral Stem

It is very important to obtain a reliable geometric data of the femoral canal for designing femoral component in total hip arthroplasty. So authors estimated the shape and measured the size of the proximal femoral canal by using 3 dimensional images obtained from computerized tomographic data of 60 korean normal adult cadaveric femora. EXTRACT II PROGRAM VERSION 2.5 BY CORNELL UNIVERSITY at the SUN 3 WORK STATION was used for this study. The averge femoral length was 416.0±25.2mm ranging from 358.6mm to 465. 1 mm. The average femoral anteversion angle was 20.5±9.1°ranging from 2° to 36.5°. The average neck-shaft angle was 127.8±5.7°, ranging from 115.5° to 139.0°. The average length of femoral offset was 37.5±5.2mm. The average length of transverse diameter measured at the center of lesser trochanter was 29.4+3.5mm and measured at 20mm above the lesser trochanter was 47.3±4.6mm. The isthmic position was located at 109.4±13.1mm belew the center of the lesser trochanter and the average transverse diameter at the isthmic position was 12.1±2.1mm in mediolateral direction and 14.4±2.4mm in anteroposterior direction. The average of canal flare index which indicates the shape of proximal femoral canal, was 3.8±0.7; Stove-pipe shape was 8.4% normal shape was 70.0% and Champagne-flute shape was 21.6%. The press-fit hip stem design(PFC) had much more improved stem-canal contact on total and priority regions. And the other four devices had better fit proximally than distally and viceversa(proximal-distal mismatch). The improvement of fit can be achieved by considering the proximal-distal geometry of the femur more carefully and by diversifying the stem design which adapts to the anatomical variation of femur.
