3.Effects of the color components of light-cured composite resin before and after polymerization on degree of conversion and flexural strength.

OBJECTIVES: This study investigated the effects of the color components of light-cured composite resin before and after polymerization on degree of conversion (DC) and biaxial flexural strength (FS). MATERIALS AND METHODS: Four enamel shades (A1, A2, A3, A4) and two dentin shades (A2O, A3O) of Premisa (Kerr Co.) and Denfil (Vericom Co.) were evaluated on their CIE L*, a*, b* color components using the spectrophotometer before curing, after curing and at 7 day. The DC of same specimens were measured with Near-infrared spectrometer (Nexus, Thermo Nicolet Co.) at 2 hr after cure and at 7 day. Finally, the FS was obtained after all the other measurements were completed at 7 day. The correlations between each color component and DC and FS were evaluated. RESULTS: The light-curing of composite resin resulted in color changes of Premisa in red-blue direction and Denfil in green-blue direction. The DC and FS were affected by product, time and shade (3-way ANOVA, p < 0.05) and product and shade (2-way ANOVA, p < 0.05), respectively. Premisa only showed a significant correlation between the DC and CIE a* component - before and after polymerization (Pearson product moment correlation, p < 0.05). The FS of Premisa showed significant negative correlations with CIE a* and CIE b* components. CONCLUSIONS: The DC and FS of the light-curing composite resin were affected by the color components of the material before and after polymerization.

5.A Case of Addison' s Disease.

Addison's disease is a rare disorder resulted from a chronic deficiency of the adreanl cortical hormone. The clinical manifestations are general weakness, weight loss, hyperpigmentation, hypovolemia with hyponatremia and hyperkalemia. We report a case of Addison's disease in a 60-year-old woman who has experienced slowly progressive weakness, weight loss and generalized cutaneous pigmentation, especially sun exposed area, extensor surface and nail bed for the last, 2 years. On a hormonal assay of the adrenal glands, basal plasma cortisol level was decreased and basal plasma ACTH level was markedly elevated. A chest X-ray showed streaky tuberculous infiltration in left, upper lobe field and adrenal CT scan showed calcific densities of both adrenal glands with nodular enlargement of right adrenal gland. There was a clinical improvement with steroid replacement therapy and anti-tuber- culosis chemotherapy. A nearly normal appearance was obtained after 5 months' treatment.

6.A Retrospective Study of Choroidal Thickness in Children with Unilateral High Myopia.

PURPOSE: To compare the choroidal thickness of unaffected and affected eyes in children with unilateral high myopia when measured using enhanced depth imaging and to analyze the relationship among choroidal thickness and axial length, spherical equivalent and best corrected visual acuity (BCVA). METHODS: Twenty children with high unilateral high myopia who received optical coherence tomography from December 2012 to May 2014 were retrospectively analyzed. Choroidal thickness was measured with a caliper at 500 microm apart from fovea superiorly and inferiorly, 2,500 microm apart nasally and at 2,500 microm apart temporally at 500 microm intervals. For statistical analyses, paired t-test was used for choroidal thickness and linear regression analysis for the relationship among choroidal thickness and axial length, spherical equivalent and BCVA. RESULTS: The patients were 10.4 +/- 3.5 years of age and the average BCVA of myopic eye was 0.38 +/- 0.2. Mean spherical equivalent was -9.8 +/- 1.9D. The choroidal thickness was significantly thinner in the myopic eye (102.5 +/- 16.9 microm) than the fellow eye (282.9 +/- 14.0 microm). The thickest choroid in the myopic eye was the temporal and the thinnest was the nasal area (p = 0.008). Choroidal thickness was significantly associated with axial length and spherical equivalent but not BCVA. CONCLUSIONS: Highly myopic eyes tend to have thinner choroidal thickness than the fellow eyes and choroidal thickness tends to be thicker temporally. Therefore, several anatomical changes in unilateral highly myopic children may exist.

7.Short-term Clinical Outcomes after Transscleral Fixation Using the Intrascleral Pocket Technique: A Retrospective Cohort Study Analysis.

PURPOSE: To compare the two transscleral fixation (TSF) techniques of intrascleral pocket and conventional scleral flap with conjunctival division techniques in terms of short-term clinical effects. METHODS: This retrospective cohort study included all consecutive patients with aphakia in Gyeongsang National University Hospital in Jinju, Korea, who underwent TSF between January 2012 and December 2014. The medical records of all patients were retrospectively reviewed, and the endothelial cell count (ECC), refraction, best-corrected visual acuity (BCVA), intraocular pressure, slit lamp, and fundus examination results before and 1 day and 6 months after surgery were recorded. The postoperative complications and visual outcomes were also recorded. RESULTS: The intrascleral pocket and conventional-flap groups did not differ significantly in terms of demographics, presurgical BCVA, or ECC. However, the intrascleral pocket group had a significantly lower BCVA at 1 day and 6 months after surgery compared to the conventional-flap group. The two groups did not differ in terms of ECC 6 months after surgery. The intrascleral pocket group had no postoperative complications, but five patients in the conventional-flap group complained of irritation. In both groups, the intraocular lens was well positioned without tilting or subluxation, and astigmatism was significantly reduced at 1 day and 6 months after surgery. CONCLUSIONS: The intrascleral pocket technique of TSF does not involve conjunctival dissection and is a successful method of sulcus fixation. It stably corrects the intraocular lens and is easy to perform, which helps to reduce operation time. It also reliably yields rapid visual acuity recovery without complications.

8.Leiomyosarcoma of the penis.

Leiomyosarcoma of the penis is an extremely rare disease. We have encountered a case of leiomyosarcoma of the penis in a 45 year old man. On staging work up. local invasion to corpus cavernosum. but no evidence of lymph node metastasis was evident. After the confirmation of leiomyosarcoma by excisional biopsy, the patient underwent partial penectemy. Herein we report this case with the review of literatures.

10.Prediction of the Diagnosis of Adnexal Masses According to the Ultrasonographic Morphologic Scoring System and Doppler Velocity Wave Form.

OBJECTS: The purpose of this study was to evaluate the relative usefulness of the morphologic scoring system and spectral doppler ultrasonographic analysis in differentiating between benign and malignant adnexal masses. METHODS: All patients scanned between July 1995 and June 1998 with sonographically identified and pathologically confirmed adnexal masses formed the study group. The adnexal masses were scored as benign or malignant on the basis of their sonographic appearance. Specific categories included inner wall structure, wall thickness, septal structure, and echogenicity. Women whose morphologic findings were suspicious for malignancy underwent doppler sonography. A threshold pulsatility index of 1.0 and resistance index of 0.4 were used to differentiate benign from malignant lesions. The findings were correlated with the presence of malignancy. RESULTS: Two hundred fifty one patients formed the study group; 215 patients were benign and 36 patients were malignant pathologically. Using the morphologic scoring system, 31 of the 36 malignant masses were classified as suggestive of malignant tumor, and 147 of the 215 benign masses were classified as not suggestive of malignant tumor (sensitivity, 86%; specificity, 68%; positive predictive value, 31%; negative predictive value, 97%). Doppler velocimetry was performed on a total of 62 patients, 19 patients with malignant tumors and 43 patients with benign tumors. A mean PI value of 0.85 and RI value of 0.52 for malignant tumors and mean PI value of 2.00 and RI value of 0.75 for benign tumors was shown. The PI value was significantly lower (p 0.001) in malignant than that of in benign masses, but RI value was not significant (p 0.184). The sensitivity and specificity in the detection of malignancy combined with doppler velocimetry was 86% and 67%, respectively. CONCLUSION: Doppler velocimetry studies did not add substantially to the prediction of malignancy using the morphologic scoring system.
