In the treatment of DDH, generally we have used the acetabular index (AI) and the center-edge (CE) angle as diagnostic factors for hip dysplasia. However, it is sometimes difficult to mark the exact lateral edge of the acetabulum to measure these factors due to indistinct bony shadows on the image of the dysplastic hip. The purpose of this study was to evaluate the lateral edge of the acetabulum to determine the location of the most accurate marking point on the plain radiograph. Using plain radiograph, we studied 53 patients with DDH which were treated with closed reduction. 26 patients were studied using plain radiograph and another method: 16 patients with 3- DCT, 4 patients with MRI, and 6 patients with arthrograph. According to this study, the most lateral bony margin of the acetabular roof on the plain radiograph indicates the antero-lateral portion of the acetabulum. The lateral edge of the sourcil indicates the lateral margin of the centro-lateral (mid-superior) portion of the acetabulum. The marking point on the lateral end of the sourcil reflects a dysplastic condition of the acetabulum, especially in the mid-superior portion of the acetabulum.

2.A Clinical Study of the Effects of Finasteride on Androgenetic Alopecia.

BACKGROUND: The 5α-reductase inhibitor finasteride blocks the conversion of testosterone to dihydrotestosterone(DHT), the androgen responsible for androgen- etic alopecia in genetically predisposed men. OBJECTIVE: The purpose of this study was to determine the efficacy of finaster- ide treatment according to the type of androgenetic alopecia in Korea. METHODS: 196 patients with finasteride(1.25mg/day) were registered in this study and 68 patients were followed over 8 months. Efficacy was evaluated by globa1 photographs, investigator assessment of clinical change,and patients' self assessment via self administered hair growth questionnaire. RESULTS: Patients' self-assessment demonstrated that finasteride treatment slowed hair loss, increased hair growth, and improved appearance of hair. These improvements were corroborated by objective assessments after 4 months of treatment. Adverse effects such as sexual dysfunction were minimal. CONCLUSION: Oral finasteride 1.25mg/day slowed the progression of hair loss and increased hair growth in clinical tria1s of men with androgenetic alopecia over 8 months. With its generally good tolerability profile, finasteride is a new approach to the management of this condition, for which treatment options are few.

4.Tinea pedis in garbage workers.

Tinea pedis is frequently found in people with poor personal hygiene and in hot, humid environments. The authors investigated the clinical, epidemiological, and mycological characteristics of tinea pedis in 170 garbage workers(166 males and 4 females) employed in a garbage plant in Seoul, Korea. 40 were clerical workers and 130 were field workers. Tinea pedis was found in 134 with a prevalence rate of 78.8%. Of those 134, 61(45.5%) also had onychomycosis. The prevalence of tinea pedis increased with age and the period working in garbage plant. However, there was no difference in the prevalence of tinea pedis between clerical workers and field workers, mitigating against the conclusion that their tinea pedis may be of occupational origin. Clinically tinea pedis was classified into 4 types ; interdigital (41.8%), vesicular(23.1%), dry squamous(22.4%), and mixed(12.7%). KOH smear positivity and culture positivity was related to clinical types of the lesion the lowest in the interdigital type. In the interdigital type, the etiology of nonmycotic lesions could not be identified. Negative fungal cultures could have been due to secondary bacterial infections. 63 strains of dermatophytes were isolated; 54 strains of Trichophyton rubrem(85.7%), 8 strains of Trichophyton mentagrophytes(12.7%) and 1 mixed infection of T. rubrum and T. mentagrophytes. T. mentagrophytes was isolated only from the vesicular lesions. 8 strains of yeast forms, positive in KOH smears and yielding pure colonies, were also isolated, and repeated mycologic examination yielded the same results. Among them, at least 4 cases of Trichosporon beigelii, and 1 case of Candida parapsilosis were considered to be of pathogenic significance.

5.Comparative Study of Low - Strength Anthralin Therapy in Psoriasis.

We performed anthralin comparative study(0.01% vs 0.1%) to assess the effectiveness of low-strengh anthralin therapy in 34 psoriatic patients and the resu1ts can be summarized as follows. 1. In the case of 13 patients using Burdick UVB larnp, in 1 patient the effect of 0.1% anthralin was superior to that of 0.01% anthralin from the start to the end of treatment, in 5 patients(38.5%) initially the effect of 0.1% anthralin was superior but in time became equal to that of 0.01% anthralin, and in 7 patients (53.8%) the effect of either side was same throughout the courae of the treatment. In the case of 21 patients using Waldmann UVB cabinet, there were 2(9.5 %), 3(14.3%) and 16 patients(76.2%) in the order named above. 2. The side effect of 0.1% anthralin was more severe than or at least the same as that of 0.01% anthralin in every patient who complained about side effects. So low-strengh anthalin-UVB phototherapy was thought to be the effective and alternative method of treatment particularly for the purpose of lessening the side effects.

6.Experimental Reproduction of Polymorphous Light Eruption: By Potent UVA Irradiating Instrument , Metal Halide Mercury Lamp.

We report a case of experimental reproduction of the skin lesion of polymorphous light eruption by potent UVA irradiating instrument, metal halide mercury lamp. A 20-year-old female had complained skin lesion at sun-exposed area since childhood. The skin lesion was reproduced after repetitive exposure of high dose of UVA, and the histologic feaure of the lesion was consistent with polymorphous light eruption. Metal halide mercury lamp used in this patient has hight irradiance and exclusive of UVB and UVC. It can be convenient and precise one in phototest using high doses of UVA.

7.Treatment of Psoriasis by Modified Ingram Regimen.

Our study was performed to evaluate the efficacy of anthralin-UVB therapy for psoriasis by simplifying the conventional Ingram regimen, 34 patients(21 admitted, and 13 OPD patients) were included in our study and the resulti can be summarized as follows. 1. There were no cases of treatment failure. 16 patients(76. 2%) out of the 2l admitted patients and 7 patients(53.8%) out of the 13 OPD patients showed clearing. 2. In the case of the admitted patients, the mean numbers of therapy(days) of the improved patients reaching grade 4 were 12. 0(18. 1 days) in the trunk lesions and 15. 5 (22. 0 days) in the extremity lesions' and those of the cleared patients reaching grade 4 were 11. 2(16. 0 days) in the trunk lesions, and 13. 4(lg. 5 days) in the extremity lesions, but the difference was not significant statistically(pp o.ps). The difference between the therapeutic effects of the admitted and the OPD patients was significant statistically(p<0. 05). 3. There were some notable side effects such as staining, pruritus, and erythema, but there were no cases of therapy termination.

Our study was performed to evaluate the efficacy of anthralin-UVB therapy on patients with psoriasis. We studied twenty two patients suffering from plaque-type psoriasis of varying degrees of severity. The results can be summarized as follows. l. 2p patients(90.9%) out of the total 22 patients showed grade 4 improvement. The rernaining 2 patients failed to respond to treatment, 2. Arnong the improved 2Q patients, 2 patients who were treated more than 3p times for grade 4 improvement were excluded, so cleared patients were 18(81.8%). 3. Arnong the 18 cleared patients, the mean nurnber of therapy wasg. 6 for grade 3 and 12. 5 for grade 4 in the cases of trunk lesiona, and 13. 2 for grade 3 and 17. 8 for grade 4 in the cases of extrernities lesions. With above results, anth-ralin-UVB therapy was rnore effective in lesions of the trunk than in those of extremi.ties. 4. Notable side effects were pruritus in 6 patients(27.3pg), staining in 4 patients (18,2%), and 1ITltatioB in 2 patients(9.1%)

10.Treatment of Nonunion of Long Bone by the Ilizarov External Fixator.

The Ilizarov technique has been used successfully in the treatment of nonunion of long bone accompanied by infection, severe bone and soft tissue defects, and deformity. We report the results of treatment of 21 cases of nonunion using the Ilizarov technique. There were 20 males and 1 female with an average age of 34.2 years (range, 8-72 years). The average follow up period was 45 months (range, 12-74months). Infected nonunion(15/21 cases) was the most common cause of nonunion. The treatment methods include simple compression method(4 cases) and internal and external bone transport method(17 cases). Bony union was achieved in 20 cases out of 21 cases. One case is still having difficulty in achieving union. The mean time to union was 7.3 months in femurs and 6.3 months in tibias. The amount of bone defect that developed after the removal of infected bone fragments and soft tissue averaged 3.8cm (range, 2.3-9cm). The healing index, in cases of bone transport, was an average of 45 days per cm (range, 30-62 days per cm). Complications developed in 17 cases out of 21 cases. Pin site infection occurred in 5 cases, premature consolidation in ~1 case, angulation deformity in 2 cases, LOM of the knee in 2 cases, equinus deformity in 2 cases, ankle stiffness in 3 cases, and nonunion in 1 case. The Ilizarov technique is thought to be effective in the treatment of complicated nonunion combined with shortening, deformity, bone defect and infection. However, in order to reduce complications caused by the Ilizarov method, accurate surgical techniques and postoperative care are necessary.
