1.Adenoma of the Nipple.

Adenoma of the nipple is a benign tumor of the lactiferous ducts of the nipple which frequently causes erosion of the surface. It can clinically mmic Paget's disease and histologically be misdiagnosed as a well-differentiated adenocarcinoma. A 20-year-old woman developed an erythematous oozing patch on the right nipple two years ago, which subsequently developed iiito verrucous surfaccd papules. A biopsy specimen of the lesion showed focal acanthosis of the epidermis. The tumor was composed of fairly well-circumscribed, glandlike structures within the nipple stroma. Ductlike struciures, papillary areas, solid nests of cells and cysts lined by squarnous epithelium, could be seen, Two cell types lining the ducts were recognized; inner columnar cells and peripherally located cuboidal cells. Although simple excision of the adenoma is the treatment of choice, our case was treated successfully by electrucoagulation.

2.The Relative Incidence of Congenital anomalies of the Hand.

There has been a few articles dealing with the incidence of congenital anomalies of the hand in Korea. In this paper, we have analyzed patients who have visited the hand clinic of Seoul National University Children s Hospital from March 1993 to February 1995 to study the relative incidence of congenital anomalies of the hand. We regarded more than two visits of the same patients as one. They were categorized into two groups by our classification system. First group consisted of the patients who showed hand anomaly only. The other group consisted of the patients who had anomalies in the other sites as well as in the hand. The first group was divided into two subgroups; the patients who had single entity of hand anomaly, and those who had more than two entities of hand anomalies. From the clinical analysis based on above criteria, following results were obtained. There were 626 anomalies in 547 patients, 319 (58.3%) males and 228 (41.7%) females (1.4: 1). Unilateral involvements of hand anomaly were noted in 385 (70.3%) patients (229 in right, 156 in left) and bilateral in 162 (29.7%). As a whole, the most common anomaly was trigger thumb (16.5%) and the second was polydactyly (16.1%). Other anomalies, in order of frequency, were syndactyly, camptodactyly, hypoplasia, brachydactyly, cleft hand and so on. In the first group who had hand anomaly only, there were 407 patients who had single entity of hand anomaly. Among these, trigger thumb were 25.3%, and polydactyly 16.2%. There were 68 patients who had multiple entities of anomalies. Among these, the syndactyly was the most common anomaly (23.9%) and the polydactyly was the next (16.2%). Fifty eight (10.6%) patients had anomalies in the hand as well as in the other site. Among these, camptodactyly was the most commonly found (36.5%) and polydactyly was the next common variety (19.0%) in the hand. In the anomalies which occurred in other than hand, arthrogryposis multiplex congenita was the most common (30.0%). Syndactyly of the foot (22.9%) and polydactyly of the foot (18.6%) were the next common anomalies.

3.Effects of Three Dimensional Corrective Osteotomy in Cubitus Varus of Adults

Cubitus varus is a complex three-dimentsional deformity. In addition to deformity on varus-valgus plane, internal torsion of distal humerus is frequently combined. Also limitation of flexion-extension could be associated in some patients. Surgical correction of the cubitus varus after skeletal maturity, could have some different clinical course from those of growing children. However there have been few papers dealing with the surgical treatment of cubitus varus in adults. From May 1988 to December 1993, 23 patients who were older than 17 years of age, had correc- tive osteotomy for cubitus varus considering its three planes of the deformity at Seoul National University Hospital. To prevent serpentine deformity medial displacement of distal fragment was also combined during operation. The avereage age was 26 years(17-47), and the average follow-up after the operation was 1 year and 6 months(6 months-3 years). The presumed diagnosis of initial injury deduced from history and preoperative radiograms, was supracondylar fracture in 19 cases(86.4%). Others were lateral condylar fracture of humerus in two, and fractures associated with elbow dislocation in two. The average age at the time of initial injury was 8 years(2-14), and average delay from occurrence of deformity to the operation, were 13 years. 5.Effects of Topically Applied Autologous Serum on Aqueous Biochemistry and Endothelial Healing Following Experimental Corneal Alkali Wounds.

The effect of topically instilled autolohous serum on the healing of endothelium and change of aqueous chemical properties following alkali wounds were evaluated. New Zealand white rabbits, weighing 2.5KG, total 64(64 eyes), were used in this experiment. Standardized corneal alkali wounds were produced by applying a round filter paper of 5.5mm if diameter, soaked in 1N NaOH, to the central cornea for 60 seconds. Autologous serum in the treated group and balanced salt solution (BSS) in the control group were given 4 times per dat for 3 weeks. Endothelial defect areas and the concentrations of ascorbic acid, glucose and minerals in the aqueous humor were measured. Endothelial defect area was not significantly different between autologous serum-treated and control groups. The concentrations of ascorbic acid in the aqueous humor were aignificantly increased in the autologous serum-treated groups compared to the control groups. However, the concentrations of glucose, Na+, K+, Ca++ and Mg++ in the aqueous humor were not statistically different between two groups.

6.Effects of Topically Applied Na-Hya luronan on Stromal and Endothelial Healing in Experimental Corneal Alkali Wounds.

The effects of topical sodium-hyaluronan on the stromal and endothelial healing were examined in the repair process of experimental corneal alkali wounds. Corneal alkali wounds were produced in one eye of each rabbit by applying a 5.5 mm round filter paper soaked in 1N NaOH onto the central cornea for 60 seconds. Then the eyes were treated topically with either 1% Na-HA(the treatment group)or a phosphate buffered saline(PBS)(the con-trol group)4 times per day for 3 weeks. Endothelial wound morphometry was performed after alizarin red and trypan blue staining. The stromal healing was assessed by counting polymorphonuclear leukocytes(PMNs)and keratocytes in the central and marginal wounds areas. The stroma treated with Na-HA had less PMNs than that of the control group during the early healing period. The defect area of the endothelium was significantly smaller in the Na-HA treated group than in the control corneas. The present findings indicate that topically applied 1%Na-HA affects stromal and endothelial healing during the early repair process after corneal alkali wounds.

9.Effects of School Lunch Program on Nutritional Knowledge and Attitude, and Dietary Behavior of Korean Middle School Students.

School Lunch Program (SLP) should provide adequate and qualitative nutritional food for student's growth and proper food habit. With this fact, this study divided 524 middle school students who resided in Jeonju, Jellabukdo into two groups, SLP or non SLP according to whether they were provided SLP or not. Then this research estimate each group's nutritional knowledge, nutritional attitude, dietary behavior, balance of three meals, and health checking their clinical symptom to investigate the effects of the SLP. This research applied anthropometric data, questionnaires and food intake data using 24 hours recall method. Each group's age average was same as 14.0 years old respectively. The average score of nutritional knowledge and condition of balance of lunch were significantly higher in SLP group than those of non-SLP group, although, there were no significant differences in the scores of nutritional attitude, dietary behavior, and balance of breakfast and dinner within both groups. This research showed that SLP had positive role on nutritional knowledge and balanced lunch. However, current SLP is not sufficient to cause change the nutritional attitude and dietary behavior. To make more applicable and effective result of SLP leading nutritional attitude changes and development of appropriate dietary behavior, certain level of nutritional education to the student may be important.
