1.Porokeratotic Eccrine Ostial and Dermal Duct Nevus.

A 6-year-old boy with porokeratotic eccrine ostial and dermal duct nevus on the left sole is reported. The patient was born with keratotic plugs, each measuring 1-2 mm in diameter, on the left sole. Light-microscopic examination revealed epidermal invagination and cornoid lamella, which was found to be connected with hyperplastic intraepidermal and dermal eccrine duct. The present case can be histologically differentiated from nevus comedonicus and punctate porokeratotis.

3.A Study of Interleukin-2 Activity in Behçet's Syndrome.

This study was undertaken to investigate immunological mechanisms in Behçet's syndrome, a function considered important in the pathogenesis of the disease. The activity of interleukin-2 (IL-2), which is believed, to play a central role in the regulation of both cell-mediated and humoral T cell-dependent immune responses, measured in 46 patients with complete, in-complete or suspected Behçet's syndrome. The results were as follows: 1 . There was no significant difference between the average IL-2 activity of patients and control group. 2. For each clinical subtype of Behçet's syndrome, IL-2 activity was lower than the control value, but the difference was not statistically significant. 3. In mucocutaneous and ocular types, the greater the clinical symptoms, the lower the value of IL-2 activity. However, the decrease of IL-2 activity was not statistically significant. In conclusion, IL-2 activity of phytohemaggludnin (PHIS-stimulated peripheral blood lymphocytes in patients with Behçet's syndrome was not significantly decreased compared to control population.

We report a 19-year-old man with hyperkeratosis of nipple and areola which is a part of an epidermal nevus. He did not have a history of endocrinopathy or synthetic steroid therapy. The lesion, a fingertip sized verrucous dark browncolored macule on left upper back, had spread into left-anterior chest wall and had involved nipple and areola to make them hyperkerstotic and verrueous. Histologic section from the on left areola showed hyperkeratosis, papillomatosis, acanthosis and increased pigmentation in the basal cell layer. Chernical peeling with 30% trichloroacetic showed favorable response.

7.Comparison of In-Phase and Opposed-Phase FMPSPGR Images in Breath-hold T1-weighted MR Imaging of Liver.

PURPOSE: To compare the effectiveness of the in-phase(IP) sequence and the opposed-phase(OP) sequence in the detection of focal hepatic lesions in the single breath-hold hepatic MR imaging with fast gradient T1-weighted pulse sequences. MATERIALS AND METHODS: If and OP T1-weighted breath-hold imaging was performed using fast gradient echo sequences in 45 patients referred for known focal hepatic lesions, in which 78 lesions were detected. There blind readers independently reviewed the images for lesion detectability. The signal-to-noise ratio(SNR) of the liver, the lesion-to-liver contrast-to-noise ratio(CNR) and the liver-to-spleen CNR were also compared. A consensus was reached by three readers to determine which sequence is better in image quality. RESULTS: On OP images, 61(78%), 61(78%), and 63(89%) lesions were correctly identified for reader 1, 2 and 3, respectively. On IP images, 66(85%), 65(83%), and 65(93%) lesions were detected for each reader, respectively. When two image sets were combined, 71(91%), 69(88%), and 76(97%) lesions respectively were detected for each reader. In cases of hepatocellular carcinoma, liver-to-lesion CNR was greater on the OP images(p<0.05), but in other lesions significant differences was not demonstrated. Liver-to-spleen CNR was higher in OP images(p<0.1), but the SNR of the liver was higher on the If images. CONCLUSION: Use of both If and OP imaging can be helpful to avoid erroneous missing of some focal hepatic lesions.

8.Arthroscopic Labral Repair Associated with Femoroacetabular Impingement: Short Term 2-5 Years Follow-up Results.

PURPOSE: The purpose of this study is to report on the short term follow-up of patients who underwent arthroscopic acetabular labral repair for femoroacetabular impingement and acetabular labral tear. MATERIALS AND METHODS: A total of 45 consecutive patients who underwent arthroscopic acetabular labral repair after diagnosis of femoroacetabular impingement and acetabular labral tear from January 2008 to December 2010 were included in this study. Modified Harris hip score (MHHS), VAS, Hip outcomes score (HOS), and patient satisfaction were used for evaluation of the clinical results. RESULTS: Patients included 22 males and 23 females, and the mean age of the patients was 33.0(range, 16-54) years old, and the mean follow up period was 26.7(range, 24-56) months. Of the clinical results, mean VAS score was 6.4 and 2.5 points before and after surgery, respectively, and mean MHHS score improved from 59.5 points before surgery to 85.4 points after surgery. Activities of daily living and sports-related activities of HOS were 58.3% and 51.2%, respectively, before surgery, and 83.0% and 79.8% after surgery. Revision arthroscopic surgery was performed on five cases(12.1%); labro-synovial adhesion, three cases, pull-out of suture anchor, one case, heterotropic ossification, one case. CONCLUSION: Arthroscopic acetabular labral repair is considered an effective treatment for femoroacetabular impingement and accompanying acetabular labral tear.

9.Ultrasonographic Usefulness for Diagnosis of Acetabular Labral Tear.

PURPOSE: This study evaluated the usefulness of ultrasonography for a diagnosis of acetabular labral tear in femoroacetabular impingement (FAI). MATERIALS AND METHODS: From March 2010 to October 2011, an ultrasonographic examination was performed in 58 patients(58 hips: 26 men and 32 women) with acetabular labral tear. The sensitivity and positive predictive value of ultrasonography were evaluated for 58 cases with acetabular labral tear confirmed by hip arthroscopy. RESULTS: The sensitivity and positive predictive value for an acetabular labral tear using ultrasonography was 89.6% and 100%, respectively. The concordance rate of an acetabular labral tear between arthroscopy and ultrasonography was 84.6%. CONCLUSION: Hip ultrasonography can make a significant contribution to a pathologic diagnosis and isa useful diagnostic tool for acetabular labral tears.

10.A Case of Carcinosarcoma of Skin Developed on Marjolin's Ulcer.

PURPOSE: Carcinosarcomas are rare neoplasms in which both malignant epithelial and mesenchymal elements are identified. We have found only twenty one cases of primary cutaneous carcinosarcoma in the English language literature. Therefore it has been difficult to diagnosis because its unclear etiology and low frequency. METHODS: A 31-year-old young man with a protruding mass on his ankle dorsum was examined. The tumor was 7 x 6 x 3 cm sized and arose from a burn scar. The mass was wide excised and applied split thickness skin graft. Characteristic pathologic finding was a mixture of squamous cell carcinoma and malignant fibrous histiocytoma. RESULTS: Three months after the operation, the patient died of multiple metastasis to the liver, lung and finally of sepsis despite adjuvant chemotherapy and adjuvant radiotherapy. CONCLUSION: If a metastatic cutaneous carcinosarcoma is diagnosed, wide excision is required. There should be more aggressive management to minimize the risk of recurrence. Further research into the etiology and pathophysiology of the disease and a more careful differential diagnosis may allow improvement in treatment.
