1.The effects of posterior retraction on the displacement of the maxilla.

Three-dimensional finite element model was made from adult skull to find desirable direction of retraction force to treat skeletal class II malocclusion. The retraction force of 400g was applied to the first molar. The direction of the force application was 23degrees downward, parallel, 23degrees upward and 45` upward to the occlusal plane. The stress distribution and the displacement within the maxilla were analyzed by three-dimensional finite element method. The findings obtained were as follows: 1. Maxillary first molar was displaced posteriorly and inferiorly in 23degrees downward, parallel, 23degrees upward retraction but it was displaced posteriorly and superiorly in 45degrees upward retraction. 2. ANS, A point and prosthion were moved posteriorly and inferiorly and pterygornaxillary fissure was moved posteriorly and superiorly. Clockwise rotation of maxilla occurred when retraction force was applied. 3. The degree of clockwise rotation of maxilla was greatest when the force was applied 23degrees upward to the occlusal plane and was least when the force was applied 23degrees downward to the occlusal plane. 4. Large tensile stress appeared in maxillary first molar and alveolar bone and the infraorbital region of maxilla when the force was applied 23degrees downward to the occlusal plane. Tensile stress was smaller as the direction of force move upward. 5. Large compressive stress was appeared in maxillary first molar and infraorbital region in 45degrees upward case and large compressive stress occurred in the posterior part of maxilla as the retraction force was upward.

2.Antiviral Effect of Antisense Phosphorothioate Oligonucleotides Targeted to Herpes Simplex Virus.

To search the effective antisense oligonucleotide that inhibit the growth of Herpes simplex virus type 1 (HSV-1), six kinds of phosphorothioate oligodeoxynucleotides (S-ODNs) were synthesized and the antiviral activity was assessed by measuring cytopathic effect in Vero cells infected with HSV-1. Of the three dodecamer S-ODNs, cornplernentary to the translation initiation site of IE2 (AS2) and scrambled S-ODN (AS1) showed more significant antiherpetic activity than AS4 complementary to the IE4. Accordingly, the antiviral effect of dodecamer S-ODN was not specific. In contrast to the no inhibitory effect of sense strand S-ODN of ICP8 (AS6), two S-ODNs complementary to the translation initiation site of ICP8 (AS3) and that of IE1 (AS5) showed potent antiherpetic activity assessed in vitro HSV-1 virus yield assay. Antiherpetic effect of AS3 was decreased in proportion to the addition of AS6. The synthesis of viral protein ICPS and IE1 were inhibited in AS3 and AS5 treated HSV-1 infected Vero cells, respectively. These findings suggest that antiherpetic effect of AS3 is specifically mediated by targeting ICPS. S-ODNs had no effect on Vero cell viability. The data suggest that the 19-mer S-ODNs may be effective in antiviral chemotherapy.

3.Dental Anthropologic Study on the High School Students of Che-ju island.

This dental anthropological study was performed to analyse male and female students of Che-ju high school students. Cephalometric X-rays and dental casts were obtained of 60 male and 60 female students and the Mean and Standard Deviation were obtained. There was no difference in craniofacial morphology between male and female students but the size of craniofacial skeleton of the male is bigger than that of the female. The distance from NB line to pogonion is longer in the male group, and it meant that mandibular symphysis of the male is well developed than the female. The size of teeth of the male is larger than that of the female and is significant in upper and lower canine and first molar. The size of upper arch width, intercanine width, basal arch width and lower basal arch width of the male is larger than that of the female, but arch length of the male and the female showed no significant difference.

5.Clinical Trials of Demacre on Varioys Dermatoses.

Demacre', a new topical steroid cream containing 0.025% beclomethasone dipropionate, was used in several kinds of dermatoses. Total 35 patients including ll cases of contact dermatitis, 5 of atopic dermatitis, 3 of hand eczema, 3 of seborrheic dermatitis, 2 of neurodermatitis, 2 of nummular eczema, 2 of autosensitization dermatitis, 1 of chronic eczema, 3 of psoriasis, 1 of insect bite, 1 of miliaria rubra, and 1 of intertrigo were treated by Demacre'. Among these patients, we observed excellent effects in 4 patients (11%), good effects in 22 patients (63%), and fair effects in 3 patients (9%). Namely, among these 35 patients, 26 patients (74%) revealed excellent to good therapeutic effects on their skin conditions with Demacre'. No untoward side effects were noted in all subjects treated with Demacre.

7.The Esthetic Upper Incisor Position in Korean Adult Female.

Esthetic is one of the prime goal of the orthodontic treatment. The upper incisor position in important because it is decisive in balanced smile. This study was attempted to obtain the standards and equations on the esthetic upper incisor position. Lateral cephalometric radiographs of 66 Korean young adult females who were candidates of Miss Korea Beauty Contest were traced, measured and statistically analyzed. The results were as follows ; 1. The mean and standard deviation of the measurement for upper incisor position were obtained in the Korean Beauty. 2. Among the measurements of the axis of upper incisor, 1 to SN and 1 to FH were high1y correlated with FH-MP, ANB, FH-N Pag, Y-axis, but 1 to SN (Is-Pr) to present axial inclination of labial surface of upper Incisor didn't have any correlation with the measurements for dentofacial pattern. 3. Among the measurements for the vertical position of upper incisor, 1 to stom had low correlation with to measurements for dentofacial pattern and 1 to PP(mm) was highly correlated with the measurements for facial growth direction. 4. Sixteen multiple regression equations were established to predict upper incisor position balancing with dentofacial pattern. 5. It does not have much power of persuasion to predict esthetic upper incisor position from the measurements for dentofacial pattern.

9.Pigmented Contact Dermatitis.

Pigmented contact dermatitis, firstly reported by Dr. Osmudsen in 1970, is a disease result from recurrent contact dermatitis due to hypersensitivity to cosmetic companents, which produces secondary bizarre dark brown hyperpigmentation. Histological examination of this condition reveals liquefaction degeneration of the basal cells of the epidermis and melanophages in the upper dermis. However, this entity has not been reported in the Korean literature until now. We report three cases of pigmented contact dermatitis which were diagnosed by the histological examinations and the patch tests.

10.Implementing a Sepsis Resuscitation Bundle Improved Clinical Outcome: A Before-and-After Study.

BACKGROUND: Unlike other diseases, the management of sepsis has not been fully integrated in our daily practice. The aim of this study was to determine whether repeated training could improve compliance with a 6-h resuscitation bundle in patients with severe sepsis and septic shock. METHODS: Repeated education regarding a sepsis bundle was provided to the intensive care unit and emergency department residents, nurses, and faculties in a single university hospital. The educational program was led by a multidisciplinary team. A total of 175 adult patients with severe sepsis or septic shock were identified (88 before and 87 after the educational program). Hemodynamic resuscitation bundle and timely antibiotics administration were measured for all cases and mortality at 28 days after sepsis diagnosis was evaluated. RESULTS: The compliance rate for the sepsis resuscitation bundle before the educational program was poor (0%), and repeated training improved it to 80% (p < 0.001). The 28-day mortality was significantly lower in the intervention group (16% vs. 32%, p = 0.040). Within the intervention group, patients for whom the resuscitation bundle was successfully completed had a significantly lower 28-day mortality than other patients (11% vs. 41%, p = 0.004). CONCLUSIONS: Repeated education led by a multidisciplinary team and interdisciplinary communication improved the compliance rate of the 6-h resuscitation bundle in severe sepsis and septic shock patients. Compliance with the sepsis resuscitation bundle was associated with improved 28-day mortality in the study population.
