1.Investigation of injury spectrum and traumatic condition spectrum of trauma of abdominal organs

Objective To discuss the injury spectrum and traumatic condition spectrum of trauma of abdominal organs in recent years. Methods Data of 247 cases with trauma on abdominal organ admitted in our hospital from 1999 to 2001 were analyzed in respects of injury spectrum, sex, age, vocation and injury causes of the cases with trauma of abdominal organs. Traumatic condition was evaluated by abridged injury score-injury severity score (AIS-ISS). Results There were 54 cases with trauma of abdominal organs in 1999, 72 in 2000 and 121 in 2001. Of all, 241 cases were cured but 6 died, with a fatality rate of 2.5%. The occurrence frequency of trauma of abdominal organs could be ranked in turn as follows: the spleen (43.2%), the kidney (18.8%), the liver (13.7%), the small intestine (6.5%),the bladder (4.8%), the pancreas (3.8%), the colon (3.8%), the stomach, the duodenum and the rectum. A total of 192 cases at age under 40 years accounted for 77.7% . Three injury causes ranked the first three places were traffic accident in 95 cases (38.5%), fall in 54 (21.9%), kick and boxing in 36 (14.6%), respectively. Of all, 168 cases had only intraabdominal injuries, of which 80 (47.6%) were with AIS=3, 32 (19.0%) with AIS=4 and 10 (6.0%) with AIS=5; and the other 79 cases (32.0%) were complicated by multiple trauma, with ISS ≥16 in 27 cases (34.2%) and ISS ≥25 in 8 (10.1%). Conclusions With the increase of vehicles and current personnel, the trauma of abdominal organs caused by traffic accident, accidental wound and conflict increases accordingly. People most apt to organ injuries are farmers, students and workers. Three organs ranked first three places are the spleen, the kidney and the liver. More than half cases are injured severely in accordance with AIS. To decrease trauma of abdominal organs, we must strengthen the education on traffic safety to raise their bewareness of injuries.

3.The gastric mucosal features of Helicobacter pylori associated gastritis evaluated by high - resolution magnifying endoscopy

Objective To investigate the gastric mucosal characteristics of Helicobacter pylori(Hp)-associated gastritis under high-resolution magnifying endoscopy. Methods Fujinon EG 485 ZH 850k CCD high-resolution electronic magnifying endoscopy was performed. Fifty patients were inspected by regular endoscopy with high-resolution magnifying function. Particular inspection was given on the collecting venules, the pit patterns of the gastric antrum and gastric body. Four pieces of biopsy specimen were taken, 1 each in antrum and body for RUT and the other two for pathologic examination as well as W-S stain. Both of the W-S stain and RUT being positive were confirmed as Hp infection. The Hp-positive patients received the triple therapy. Endoscopy with biopsies was repeated 4 weeks after the course of eradication treatment ended. The results were assessed by SPSS 8. 0 statistical package. Results Twenty-six patients were Hp-positive and 24 patients Hp-negative. In comparing with the results of W-S stain and RUT , the high-resolution magnifying endoscopy features and regular endoscopy findings had different outcomes. The sensitivity was 69. 23% Vs 34. 62% ; the accuracy 78.00% Vs 58. 00% respectively; the positive likelihood ratio and the negative likelihood ratio were 5. 54 Vs 2. 08 and 0. 35 Vs 0. 78 respectively. The Kappa value in high-resolution magnifying endoscopy was 0.563 whereas in regular endoscopy was 0. 176. Conclusions The advantage of high-resolution magnifying endoscopy relies on its ability to observe the fine gastric mucosal patterns. The gastric mucosal features of Hp-associated gastritis can be described as followings: collecting venules are irregular or disappeared; the gastric pits are enlarged or reddened, the sulciolar are deepened. Compared with regular endoscopy, the high-resolution magnifying endoscopy is better in sensitivity, accuracy and likelihood ratio. A high degree of interobserver agreement was obtained with the Kappa value. The pathologic findings get better soon after the eradication therapy, while the endoscopic changes are not prominent.
4.Observation on the esophageal varices and their collaterals with ultrasonic microprobe before and after EVL

Objective In order to evaluate the efficacy of esophageal varices ligation( EVL) , ultrasonic microprobe (UMP) was used to observe the esophageal varices and their collaterals before and after EVL. Methods Sixty patients merely with esophageal varices underwent ultrosonography via UMP before and 4,8,12 weeks after EVL and were divided into three groups:( Ⅰ )esophageal varices(EV) ( Ⅱ )esophageal and paraesophageal vein(PEV) varices( Ⅲ ) esophageal, para esophageal and perforating vein( PV) varices. Results In the following-up, the effective rate of type Ⅰ , Ⅱ , Ⅲ were 75% , 25% , 0% ; The recurrence rate 16% , 75% , and 100% respectively. PEV occurred in 12 out of 24 cases of group Ⅰ and broadened in all of the cases of group Ⅱ and Ⅲ; PV occurred in 11 out of 20 cases of group Ⅱ and all cases in groups M , and broadened in all cases of group Ⅲ. Conclusion Endoscopic ultrasonography can be served as guidance in selecting therapeutic measure in treating EV. EVL is merely suitable in treating EV; in EV patients with PEV and/or PV measures other than EVL should he selected.
5.Endoscopic features of gastric carcinoid tumors

Objective To investigate the endoscopic feat ures and endoscopical treatment of gastric carcinoid tumors.Methods The clinical and endoscopic materials of 18 patients w ith gastric carcinoid tumors was retrospectively reviewed.Results Five of the 18 patients were benign and 13 were maligna nt. Benign gastric carcinoid tumors were found by biopsy at endoscopy ,and were resected by endoscopic mucosal resection(EMR).Their endoscopic appearance was mu ltiple polypoid masses. 13 malignant patients were confirmed by surgery. Their e ndoscopic appearance was ulcerating and infiltrating lesions.Conclusion Gastroscopy remains to be valuable to set correct di agnosis of gastric carcinoid tumors. EMR may be the first choice for benign gast ric carcinoid tumors.
6.Evaluation on gastric mucosal features of intestinal metaplasia by high-resolution magnifying endos-copy

Objective To investigate the characteristic findings of intestinal metaplasia revealed by magnifying endoscopy, and clarified their relationship with histopathological features. Methods One hundred and nine patients underwent magnifying endoscopy between March 2003 and November 2003. Patients with gastric adenocarcinoma, esophogeal and/or gastric varices, history of gastric surgery were excluded. Endoscopic examination was performed with a high-resolution magnification endoscope after methylene blue (1 % ) spraying. According to the differences in color and mucosal pattern, forms of endoscopic images were defined, and biopsies were taken (n = 115). Results Six patterns of endoscopic images were defined according to the microscopic findings. Pattern-4 and -5 were related to intestinal metaplasia. Histological features were used as the key standard. Conclusion High-resolution magnifying endoscopy is useful in detecting intestinal metaplasia of gastric mucosa pits from its normal shape changed into oval shape or villous like pattern. This procedure may improve the follow-up of individuals at high-risk of gastric cancer.
7.Metallothionein involvement in myocardial protection of basic fibroblast growth factor

AIM: To observe whether metallothionein plays a role in cardiac protective effect of basic fibroblast growth factor on anoxic/reperfusion (A/R) injury in cultured cardiomyocytes and study the possible mechanism of cardiac protection by bFGF.METHODS: The present study made the model of myocyte A/R injury after having a 24 h incubation by bFGF( 10-10、10-9、10-s mol/L) and bFGF( 10-9 mol/L) + PD098059 respectively. We measured the levels of MT and MDA in myocytes, and the changes of LDH and protein in cultured medium. We also counted the number of viable cell in groups. RESULTS: The contents of myocardial MT were significantly increased after treatment by bFGF. The levels of MT in I0-l0 mol/L、10-9 mol/L and 10-8 mol/L bFGF treated groups increased 54 %、 62%、 76% respectively, compared with the A/R group, and the number of viable cell were also greatly increased, LDH and protein leakage in cultured medium and MDA contents in myocyte were dramatically decreased in bFGF treated groups. All the protection were completely disappeared with the inhibition of MT production with PD098059, theinhibitor of mitogen- activated protein kinase(MAPK). CONCLUSION: MT involves in the protection of bFGF in cultured cardiomyocytes. It might be related with activation of MAPKase.
8.Internal Thoracic Arterial Chemoembolization in the Treatment of Advanced Lung Cancer

Objective To study the curative effect of internal thoracic arterial chemoembolization in the treatment of advanced lung cancer.Methods Internal thoracic artery was confirmed to be the main supplied artery by digital substraction angiography(DSA) in 8 cases with lung cancer. The internal thoracic artery was embolized by stainless steel coil below the supplied segment in order to avoid the normal vascular territory occlusion.Then iodized oil combined with Cis-Diaminedichloroplatinum(DDP) and mitomycin(MMC) were injected into the supplied segment slowly until blood flow stopped,at last,the gelfoam sponge sheet was inserted into this supplied segment to decrease the wash of iodized oil by blood flow.Results Clinical symptoms were improved obviously in all cases after therapy.CT scans showed that iodized oil accumulated in the tumor better four weeks after the chemoembolization. Conclusion For these cases with advanced lung cancer which the blood supply mainly by internal thoracic artery,it is necessary to perform chemoembolization.

10.Clinical applications of positron emissiont omographic-CT in pulmonary lesions

Objective To assess the cli ni cal usefulness of dual-modality positron emission tomographic (PET)-computed t omographic (CT) imaging in pulmonary lesions. Methods Fo rty-nine patients with pulmonary lesions examined with ?~2 test. Results There were 50 primary lesions in 49 patients. Accuracy, sensitivity, specificity, positive predictive value (PPV), and negative predict ive value (NPV) were 90.0%, 95.2%, 62.5%, 93.0%, and 71.4%, respectively wi th PET-CT, 74.0%, 76.2%, 62.5%, 91.4%, and 33.3%, respectively with CT. There was significant differences between them (P0.05). Conclusion PET-C T has an important clinical value in differentiating benign lesion from malignan cy and staging in lung cancer.
