1.Changes of the cellular immune function of the elderly patients with nonsmall-cell lung cancer after chemotherapy

ObjectiveTo investigate changes of the cellular immune function in the elderly patients with nonsmall-cell lung cancer (NSCLC) after chemotherapy.Methods T-lymphocyte subsets and natural killer cell were detected in 29 elderly patients with NSCLC,20 adults with NSCLC and 22 healthy elderly,and their levels were compared between pre-chemotherapy and at the end of 2 cycles of chemotherapy in the elderly patients with NSCLC.ResultsThe levels of CD3,CD4,CD8,CD4/CD8andNK cell were (58.9±15.8),(32.3±12.7),(22.0±9.8),(1.3±0.7),(21.6± 7.7),respectively in the elderly patients with NSCLC,(65.9 ± 7.2),(38.5 ± 7.6),(23.1 ± 9.2),(1.5±0.7),(16.8±6.2),respectively in adults with NSCLC and (67.3±9.0),(39.0±7.8),(23.9±9.3),(2.0±1.6),(22.5±5.8),respectively in healthy elderly.The levels of CD3 and CD4 were decreased (t=2.109,2.159,P<0.05) and NK cell was increased (t=2.273,P<0.05) while CD8 and CD4/CD8 had no difference(t = 0.406,0.736,P> 0.05 ) in the elderly patients with NSCLC as compared with adults with NSCLC.The levels of CD3,CD4,and CD4/CD8 were lower (t = 2.234,2.200,2.016,all P< 0.05) in elderly patients with NSCLC than in healthy elderly,with no significant change in the levels of CD8 and NK cell(t= 0.700,0.474,P>0.05) between the two groups.The levels of CD3 (51.6 ±10.3)was reduced(t=2.067,P<0.05) and CD4 (31.7 ± 11.7),CD8(21.6 ± 6.5),CD4/CD8 (1.3 ± 0.7),NK cell (26.0 ±12.7)had no remarkable difference (t =0.186,0.180,0.289,1.570,all P> 0.05)after chemotherapy in elderly patients with NSCLC.ConclusionsThe cellular immune function in the elderly patients with NSCLC is lower than in adults with NSCLC and healthy elderly,and further decreases after chemotherapy.
2.The effect of transforming growth factorβreceptor typeⅠon adipocyte differentiation

Objective To investigate the effect of transforming growth factorβreceptor typeⅠ(TGFBRⅠ) on adipocyte differentiation by using a small interference RNA (siRNA). Methods The siRNA targeting TGFBRⅠwas synthesized as experimental group, and negative control siRNA was used as control group. The efficiency of TGFBRⅠdepletion and the expression levels of adipocyte-specific transcription factors CCAAT enhancer binding protein α (C/EBPα),peroxisome proliferator-activated receptor gamma (PPARγ) and adipocyte marker gene fatty acid binding protein 4 (FABP4) were detected by quantitative real-time PCR. After treating with adipocyte differentiation agents for 5 days, the cells were stained with oil red O, and the staining of adipocyte was observed and photographed by laser confocal microscope. In addition, with isopropanol extracted oil red O, optical density values of oil red O were measured at a wavelength of 520, and which were compared between groups. Results After transfection of TGFBRⅠ siRNA, gene expression levels of TGFBRⅠwere significantly reduced in ST2 cells, the number of differentiated adipocytes was significantly increased, and the mRNA levels of adipocyte specific transcription factor C/EBPαand PPARγand adipocyte marker gene FABP4 were enhanced compared with those of control group. After treating with adipocyte differentiation agents for 5 days,the number of lipid droplets of cells with transfection of TGFBRⅠsiRNA was increased than that of cells with transfection of control siRNA. The value of optical density was higher in cells with transfection of TGFBRⅠsiRNA than that of control siRNA group. Conclusion TGFBRⅠsiRNA can effectively facilitate adipocyte formation, which suggests that TGFBRⅠis an important regulator of adipogenic differentiation from progenitor cells.
3.Pathology of primary neuroectodermal tumor and its prognostic factors:A report of 35 cases

Objective To study the pathology of primary neuroectodermal tumor (PNET),and its diagnostic standards and prognostic factors.Methods Expression of CD99,FLI-1,Syn,NSE,S-100,NF,and Vim was detected in PNET tissues stained with HE and immmunohistochemistry (En Vision method).Survival rate of 33 PNET patients with complete clinical information was analyzed with COX regression analysis method.Results The positive expression rate of CD99,FLI-1,Vim,Syn,NSE and s-100 was 88.57%,51.43%,91.42%,48.57%,45.71%,and 22.86%,respectively.The sensitivity of combined CD99 and FLI-1 was 100%.However,NF was not expressed in all PNET tissues.When other factors were unchanged,no difference was found in the effect of age on the survival rate of patients.However,a significant difference was observed in the effect of PNET site and its treatment modalities on the survival rate of patients (P
4. Progress in influence of inflammatory microenvironment mediated by tumor-associated macrophages on liver cancer stem cells

The development of liver cancer is associated with the inflammation caused by hepatitis virus. Macrophages are one of the most important cells in chronic inflammation response. The activated M2-type macrophages can secrete a series of inflammatory factors to participate in the regulation of inflammatory microenvironment, and lead to the formation of liver cancer stem cells. In this review, the relationship between tumor-associated macrophages and cancer stem cells is discussed, in order to provide a new idea for the prevention and treatment of tumor.
5.Cellular immune function changes and effect of thymosin alpha-1 on the changes in elderly patients with severe pneumonia

Objective To investigate the cellular immune function changes and the effect of thymosin alpha-1 on the changes in elderly patients with severe pneumonia. Methods T cell subset and natural killer (NK) cell were detected in 66 elderly patients with severe pneumonia and 34 elderly patients with common pneumonia. The severe pneumonia patients were randomly divided into 2 groups: the treatment groups (34 cases) and the control group (32 cases). All patients received conventional therapy of pneumonia. The treatment group received 1.6 mg of thymosin alpha-1 through subcutaneous injection once a day for a week and twice a week later. Results The levels of CD3, CD4, CD8 and NK cell were lower in elderly patients with severe pneumonia than in patients with common pneumonia [(43.54%±18.97%) vs. (45.46%±10.43%), (25.43%±12.72%) vs. (38.47%±8.20%), (16.68%±9.30%) vs. (22.36%±8.06%), (13.52%±4.66%) vs. (17.87%±7.11%), t=-6.779、-5.85、-3.161、-3.285 respectively all P<0.05]. The levels of CD3, CD4, CD4/CD8 and NK cell increased significantly after treatment in treatment group [(64.22%±5.53%) vs. (61.53%±13.41%), (31.70%±4.38%) vs. (26.07%±4.31%), (1.27%±0.91%) vs. (0.97%±0.22%), (17.67%±4.56%) vs. (15.44%±3.82%), F=5.591,11.526,8.934,4.564 respectively, all P<0.05]. The duration of antibiotic injection and length of stay were lower in treatment group than in control group [(14.17±2.51) d vs. (14.42±2.79) d, (12.69±2.80) d vs. (15.04±3.58) d, t=-3.152、-2.690 respectively, all P<0.05]. Conclusions The immune function of the elderly patients with severe pneumonia is lower. Thymosin alpha-1 can improve the immune function of the elder patients with severe pneumonia and is helpful for controlling an infection.
6.pSVPoMcat modifying Schwann cell to protect injured spinal neurons in rats.

OBJECTIVETo investigate the protective effect of pSVPoMcat (myelin basic protein microgene) modifying Schwann cell on injured spinal neurons.
METHODSA model of rat spinal cord injured by hemisection was used. One hundred and twenty healthy SD rats of both sexes weighing 250-300 g were divided into three groups: Group A (n=40, treated with implantation of pSVPoMcat modifying Schwann cell), Group B (n= 40, treated with implantation of Schwann cell only) and Group C (n=400, treated with sham operation as the control). One week after operation the rat functional recovery was observed dynamically by using combined behavioral score (CBS) and cortical somatasensory evoked potentials, the spinal cord sections were stained by Nissl, acid phosphatase enzyme histochemistry and cell apoptosis was examined by methye green, terminal deoxynucleotidyl and the dUTP Nick end labeling technique. Quantitative analysis was done by computer image analysis system.
RESULTSIn Group A the injured neurons recovered well morphologically. The imaging analysis showed a result of Group A CONCLUSIONSpSVPoMcat modifying Schwann cell implantation has protective effect on injured spinal neurons and promotes recovery of injured spinal cord function in rats.

7.Moderate hypothermia therapy for patients with severe head injury

Objective: To evaluate the effect and safety of early moderate hypothermia therapy (MHT) on patients with head injury by using parallel-control test. Methods: Thirty patients with severe head injury within 6 hours after accidents were treated by MHT generally for 4-10 days. The other 30 patients with similar head injury treated routinely were enrolled for a parallel-control test. The mortality, morbidity and changes of some neuro-functional indexes as Glasgow Coma Scores, and Glasgow Outcome Scale, levels of endothelin and some other factors of neurobiochemistry in blood plasma were observed. Meanwhile, the dynamic monitoring by transcranial Doppler ultrasonography was conducted in these patients. Results: The mortality in MHT group was significantly lower than that in control group. MHT not only reduced endothelin but also increased the brain biochemical factors, which were helpful to the protection of neurons in the early brain ischemia after head injury. Conclusions: Early MHT can help reduce mortality and morbidity in patients with acute head injury.
8.Repair effect of Schwann cells modified by microgene pSVPoMcat on injured spinal cord in rats.

OBJECTIVETo observe the repair effect of Schwann cells (SCs) modified by microgene pSVPoMcat on injured spinal cord in rats.
METHODSSemi-transection injury at the level of T(8) of spinal cord was made with cutting method on 120 Sprague Dawley (SD) rats. Then 40 rats implanted with SCs modified by microgene pSVPoMcat were taken as Group A, 40 rats implanted with simple SCs as Group B and the other 40 rats were taken as the control group (Group C). The functional recovery of the rats was observed through combined behavioral score (CBS) and cortical somatosensory evoked potential (CSEP), and the expression of the glial fibrillary acidic protein (GFAP) was measured with in situ hybridization and immunocytochemistry. At 3 months after operation, the rats were examined with magnetic resonance image (MRI), and the neurofilaments (NF) of the axons were stained with immunohistochemical method.
RESULTSGFAP expression in Group A was significantly lower than that of the other 2 groups. MRI showed that the spinal signals in the injured area recovered fundamentally in Group A, didn't recover in Group B and malacia focus was found in Group C, which was same as the results of NF staining. Wave amplitudes in incubation periods in Group A and Group B tended to recover. It recovered to the normal level in Group A, which was similar to the results of CBS.
CONCLUSIONSSCs modified by microgene pSVPoMcat can inhibit GFAP expression, improve the growth of the axons and the functional recovery of neurons after spinal cord injury.

10.Investigation and analysis of professional training situation and demands for digestive endoscopy nurses

Objective To investigate the training situation and demands for professional training of digestive endoscopy nurses, and provide feasibal basis to establish professional digestive endoscopy nurse training system. Methods Using the self-designed digestive endoscopy nurses professional training demand questionnaire, 97 students were surveyed, who participated in the digestive endoscopy nurses professional training in July 2015 in Jilin province. Results 100% of digestive endoscopy nurses showed the will to participate in the digestive endoscopy specialist knowledge and skills training, and 98.97% showed the demand to accept training of new knowledge and progress in endoscopic specialized subject. Respectively 70. 1% and 91. 8% of nurses before or after entering the department received digestive endoscopy specialist knowledge training. The demand score for professional training in digestive endoscopy nurses was ( 4. 47 ± 0. 63) points, which was in the need level;thereinto, the nursing cooperation about digestive endoscope surgery was what they need most in training content(4.56±0.61). 91.8% of nurses would choose the head nurses in Endoscopic Department as the training staff and 78.4% would choose experienced nurses.Conclusions Digestive endoscopy nurses for professional training in Jilin province is in urgent need. Based on the present nursing situation of digestive endoscopy, we should set up and improve the digestive endoscopy training scheme and training content, strengthen the digestive endoscopy professional training system and nursing teachers team construction.
