1.Clinical observation of L-Thyroxine combined with selenium yeast in treating lymphocytic thyroiditis

Objective To analyze the clinical effect of L-thyroxine combined with selenium yeast in patients with lymphocytic thyroiditis.Methods The clinical data of 96 patients with lymphocytic thyroiditis from March 2015 to September 2016 was analyzed retrospectively.The cases were divided into study group(48 cases)and control group(48 cases)according to double-blind method.The control group were treated with levothyroxine and the study group were treated with levothyroxine combined with selenium yeast.Free tetraiodothyronine(FT4),free triiodothyronine(FT3),thyrotropin(TSH),thyroglobulin antibody(TGAb),serum thyroid peroxidase antibody(TGAb)and serum thyroid peroxidase(TPOAb),the level of peripheral blood T cell subsets and the total effective rate between the two groups were compared.Results After treatment,the levels of FT4,FT3,TSH,TGAb and TPOAb in the two groups were significantly better than those before treatment(P<0.05).FT4,FT3,TSH,TGAb,TPOAb levels in the study group were significantly better than that in the control group(P<0.05).Compared with the control group,the T lymphocyte subsets of the study group after treatment were significantly better(P<0.05).Compared with the control group,the total effective rate of the study group was significantly higher(P<0.05).Conclusion The effect of L-thyroxine combined with selenium yeast in the treatment of lymphocytic thyroiditis patients is very significant.The treatment method is worthy of promotion in the clinical treatment.
2.Diagnosis of stress urinary incontinence using poster urethrovesical angel measured by ultrasonography

Objective To evaluate the clinical value of ultrasonography detecting the poster urethrovesical angle(PUVA) in diagnosis of stress urinary incontinence(SUI).Methods From Jan.2006 to Dec.2008,the PUVA in resting phase (PUVA-r) and stress phase( PUVA-s) between with SUI and 100 healthy women were measured by color Doppler ultrasound,including 57 cases in SUI degree Ⅰ,22 cases in SUI degree Ⅱ,5 cases in SUI degree Ⅲ.Results (1) The PUVA-r and PUVA-s were (130±29) and (158±36)in SUI patients,which were significantly higher (113±19)and (115±23)in control group (P<0.01).(2) Correlation analysis showed that PUVA-s was positively correlated with the severity of SUI ( P < 0.01).(3) The area under the receiver operating characteristic curves (ROC) of PUVA-s was 0.82 (95% CI:0.76-0.89) between SUI patients and healthy women.When PUVA-s ≥ 140 ° was chosed as the cut-off value,the specificity,accuracy rate and positive predictive value in diagnosis of SUI were 84%,79%,81%,which were significantly higher than those when cut-off value of PUVA-s ≥ 120(54%,68%,62%; P < 0.05 ).Conclusion The PUVA-s in SUI patients are significantly increased and is positively correlated with the severity of SUI,which are indicated that PUVA-s ≥ 140should be used as cut-off value in diagnosis of SUI by ultrasonography.
5.Effects of oral administration of type II collagen on adjuvant arthritis in rats and its mechanisms.

OBJECTIVETo investigate the effects of oral administration of type II collagen (CII) on adjuvant arthritis (AA) in rats and its mechanisms, and to compare the effects of CII with those of the Chinese traditional medicine Tripterygium Polyglycoside administered similarly.
METHODSArthritis was induced in rats by immunization using Freund's complete adjuvant (FCA). After feeding rats either soluble CII or Tripterygium Polyglycoside, changes in degree of articular swelling and articular histological findings were observed in AA rats. Some correlative immunological indexes were measured, including delayed type hypersensitivity (DTH) reaction, anti-collagen and anti-Mycobacterium tuberculosis (MT) antibody in serum, and levels of IFN-gamma and TNF-alpha in articular steep in rats.
RESULTSOral administration of CII was able to alleviate both distinctly articular and general symptoms in AA rats, suppress synovium hyperplasia and inflammatory cells infiltration in arthrosis capsule. The effects brought about by CII were stronger than those by Tripterygium Polyglycoside. Oral administration of CII inhibited antigen-specific immune response, such as DTH and antibody reaction to CII. In addition, the expression of IFN-gamma and TNF-alpha in joints were locally downregulated.
CONCLUSIONSThe therapeutic effect of oral administration of CII is obvious on adjuvant arthritis in rats. Its remedial mechanisms are likely related to the downregulation of both IFN-gamma and TNF-alpha, and the suppression of cell immunity.

6.New Quality Standard for Schisandrae Chinensis Fructus

Objective: To establish a new standard for the quality evaluation of Schisandrae chinensis Fructus. Methods: On the basis of the 2015 edition pharmacopoeia standard of Schisandrae chinensis Fructus, some testing items including the characteristic chro-matogram (HPLC) and total lignans content (UV) were established. Results: With schizandrin as the reference material, the HPLC specific chromatograms of Schisandrae chinensis Fructus including 7 characteristic peaks were established. The content of total lignans in Schisandrae chinensis Fructus was not less than 1. 8% with schizandrin as the standard. Conclusion: The proposed quality evalua-tion standard is more comprehensive and reproducible. It can provide a more scientific evaluation tool for the quality evaluation of Schisandrae chinensis Fructus and its related preparations.
7.Efficacy and safety of hydrochloridememantine or donepezil combined with risperidone in treatment of Alzheimer′s disease patients with behavioral and psychological symptoms

One hundred Alzheimer′s disease (AD) patients with behavioral and psychological symptoms of dementia (BPSD) treated with donepezil+risperidone group ( n=50, group 1) or hydrochloride memantine+risperidone group ( n=50, group 2) in geriatric departments of Hangzhou Seventh People′s Hospital were enrolled in the study. Montreal cognitive assessment scale (MoCA), positive and negative syndrome scale (PANSS), treatment emergent symptom scale (TESS) were applied for evaluation; and blood routine examination, blood biochemistry, eletrocardiogram were performed in two groups before and after treatment. After treatment, MoCA score in group 2 (16.10±3.90) was significantly higher than that in group 1 (18.14±3.71)( t=-3.99, P<0.01), and PANSS score in group 2 (86.66±6.62) was significantly lower than that in group 1 (109.50±11.51; t=12.67, P<0.01). The incidence rates of dry mouth (16%,8/50), extrapyramidal side effects (10%,5/50) and the total score of TESS (3.92±2.02) in group 2 were markedly lower than those of group 1 [44%(22/50), 36%(18/50), (12.49±1.45);χ 2=9.33,9.54, t=17.90,all P<0.01]. Meanwhile, group 2 had significantly lower influences on the levels of blood lipids and fasting blood glucose (all P<0.01). The risperidone dose used in group 2 was significantly lower than that in group 1 [(2.06±0.50) vs. (3.85±0.89)mg, t=14.40, P=0.04].The results suggest that hydrochloridememantine combined with risperidone is more effective with less side effects compared to donepezil combined with risperidone for AD patients with BPSD.

46XY simple gonadal hypoplasia, also known as Sweyer syndrome, patients often due to primary amenorrhea or pubertal secondary sex characteristics do not develop the doctor, its combined gonadal tumor is more likely, in the treatment process is often recommended prophylactic removal of gonads, postoperative hormone replacement therapy. We describe two patients diagnosed with Sweyer syndrome, one with gonadowlastoma and mature teratoma, and one with nodular Leydig cell hyperplasia and ectopic adrenal tissue, and reviews the literature.
