1.Reoperation of congenital choledochal cyst: experience in 53 cases

ObjectiveTo analyze the causes of and principles for reoperation of congenital choledochal cyst (CCC).MethodsDuring the period of 1986 to 2000, 120 cases of CCC were admitted to our unit,53 of them underwent reoperation. There were 33/14/2/4 cases respectively in Todani Type of Ⅰ/Ⅳa/Ⅳb and Ⅴ. First operation included simple external drainage in 17 cases, choledochoduodenostomy in 8 cases, and choledochojejunostomy with Roux en Y loop in 22 cases etc.ResultsPostoperative complications after external or internal drainage were cholangitis and hepatolithiasis, and high incidence of biliary carcinoma 19% (10/53). The mode of reoperation was mainly cyst excision with Roux Y hepaticojejunostomy (46/53).Conclusions Inappropriate surgical management of CCC incurred much complications that necessitated a reoperation. Cyst excision with reconstruction of the biliary tree by a Roux Y anastomosis is recommended as the therapy of choice for reoperation for patient with CCC.
2.DNA — anti — DNA IMMUNE COMPLEXES ENHANCE IL— 6 PRODUCTION BY MOUSE MESANQIAL CELLS

Using the IL—6 dependent hybridoma cell line KD83, we investigated the relationship between KNA—anti—DNA immune complexes(DNA—anti—DNAICs)from the sera of lupus nephritis patients and interleukin 6 (IL—6) production by mouse glomerular mesangial cells cultured in vitro. The results show that DNA—anti—DNA IgM and DNA—anti—DNA IgG and DNA—anti—DNA IgM all enhance IL—6 production by mesangial cells; of those DNA—anti—DNA IC, the effect of DNA—anti—DNA IgM on IL—6 production by mesangial cells was significant. This enhanced production of IL—6 by mesangial cells was dependent on concentration of DAN—anti—DNA ICs.
3.Multivariate analysis of prognostic factors after radical resection for hepatocellular carcinoma

Objective To determine the prognostic factors after radical resection (RR) for hepatocellular carcinoma (HCC).Methods Altogether 144 patients who had undergone RR for HCC from 1988 through 1995 were included for a univariate and a Cox multivariate analysis.Nineteen factors contributing to overall survival rate (SR) and disease-free SR were analysed.Results The 5-year SR and disease-free SR (N=144) were 47.3% and 23.9%,separately.Multivariate analysis revealed that classification of RR was the signficant factor to overall SR,and presence of vessel invasion was the signficant factor to disease-free SR.The 5-year SR and disease-free SR in the patholngically RR and clinically RR groups were 60.2%,29.0% and 14.0%,0%,respectively.The 5-year disease-free SR in the group without (or with) vessel invasion was 27.8% (or 0%).Conclusions The classification of RR is the determinative prngnostie factor.Pathologically RR is the first option for patients with in- dications.It is essential to improve adjuvant therapy to decrease postoperative recurrence and metastasis rates.
4.Surgical management of congenital choledochal cyst

Objective To evaluate the long-term effect of surgical procedures for congenital choledochal cyst (CCC).Methods From 1986 to 2000, 120 cases of CCC were admitted and 73 of them underwent the primary operations in General Hospital of PLA. Three types procedures were performed,type I: external drainage of CCC in 7 cases; type II:cystojejunal Roux-en-Y anastomosis in 5 cases; type III: cyst excision with cystojejunal Roux-en-Y anastomosis or cystoduodenostomy in 57cases,and other procedures in 4 cases.Results 68 cases were followed-up for 6 months to 5 years (median 2.7 years). Three cases undergoing type I operations accepted reoperations;two cases undergoing type II operations accepted reoperations due to severe complications as cholongitis and hepatolithiasis; 57 cases treated by type III operation with the good results 88.7% and none reoperation.Conclusions External drainage is only a first-aid management on emergency basis. Internal drainage should never be done,because the effect is temporary,and severe complications result in reoperations. Cyst excision with biliary tract reconstruction is recommended as the optimal treatment of CCC.
5.A randomized, double-blind, controlled study: Ji-Tai tablet for the treatment of acute withdrawl syndrome of mild heroin dependence.

OBJECTIVE:
To investigate the efficacy and safety of Ji-Tai tablet and Ji-Tai tablet combined with buprenorphine in the treatment of patients with acute withdrawal syndrome of mild heroin dependence.
METHODS:
A total of 150 patients with mild heroin dependence were recruited, and were randomly assigned to a Ji-Tai tablet group (n=50), a Ji-Tai tablet combined with buprenorphine group (n=50) and a control group (n=50) during a 10-day clinical trial. Opiate withdrawal scale (OWS) was used to measure the severity of withdrawal symptoms. Anxiety symptoms assessments were made at 0 day (baseline), the day 5 (middle), and the day 10 (end) by the Hamilton anxiety scale (HAMA). Symptoms were assessed before and 1 h or 2 h after medication each day. The total withdrawal symptoms scores and the daily reduction rate were used to measure the effect of Ji-Tai tablet vs Ji- Tai tablet plus buprenorphine. Safety evaluation was carried out by the following measures: baseline of treatment, drug side effects after the treatment, vital signs (blood pressure, heart rate, and respiration rate), laboratory examination (routine blood and urine tests and the liver and kidney function tests), and electrocardiograms.
RESULTS:
A total of 142 mild heroin dependence patients performed the experiments (including 48 in the Ji-Tai tablet group, 48 in the Ji-Tai tablet with buprenorphine group and 46 in the control group). The scores of baseline withdrawal symptoms were 43.520±19.786, 42.640±17.648 and 47.100±24.450, respectively, with no significant differences among the 3 groups (all P>0.05 ). During the 10-day treatment, the reduction rate of acute withdrawal symptoms scores increased daily, the acute withdrawal syndrome scores and the anxiety symptoms scores declined from day 0 to day 10, there was also no significant difference among the 3 groups (all P>0.05). Ji-Tai tablet did not affect vital signs such as blood pressure, heart rate, and respiration rate.
CONCLUSION
Ji-Tai tablet or Ji-Tai tablet combined with buprenorphine had no effect on acute withdrawal symptoms of mild heroin dependence.

6.Human action and road condition recognition based on the inertial information.

Rapid and accurate recognition of human action and road condition is a foundation and precondition of implementing self-control of intelligent prosthesis. In this paper, a Gaussian mixture model and hidden Markov model are used to recognize the road condition and human motion modes based on the inertial sensor in artificial limb (lower limb). Firstly, the inertial sensor is used to collect the acceleration, angle and angular velocity signals in the direction of , and axes of lower limbs. Then we intercept the signal segment with the time window and eliminate the noise by wavelet packet transform, and the fast Fourier transform is used to extract the features of motion. Then the principal component analysis (PCA) is carried out to remove redundant information of the features. Finally, Gaussian mixture model and hidden Markov model are used to identify the human motion modes and road condition. The experimental results show that the recognition rate of routine movement (walking, running, riding, uphill, downhill, up stairs and down stairs) is 96.25%, 92.5%, 96.25%, 91.25%, 93.75%, 88.75% and 90% respectively. Compared with the support vector machine (SVM) method, the results show that the recognition rate of our proposed method is obviously higher, and it can provide a new way for the monitoring and control of the intelligent prosthesis in the future.
7.An attention-guided network for bilateral ventricular segmentation in pediatric echocardiography.

Accurate segmentation of pediatric echocardiograms is a challenging task, because significant heart-size changes with age and faster heart rate lead to more blurred boundaries on cardiac ultrasound images compared with adults. To address these problems, a dual decoder network model combining channel attention and scale attention is proposed in this paper. Firstly, an attention-guided decoder with deep supervision strategy is used to obtain attention maps for the ventricular regions. Then, the generated ventricular attention is fed back to multiple layers of the network through skip connections to adjust the feature weights generated by the encoder and highlight the left and right ventricular areas. Finally, a scale attention module and a channel attention module are utilized to enhance the edge features of the left and right ventricles. The experimental results demonstrate that the proposed method in this paper achieves an average Dice coefficient of 90.63% in acquired bilateral ventricular segmentation dataset, which is better than some conventional and state-of-the-art methods in the field of medical image segmentation. More importantly, the method has a more accurate effect in segmenting the edge of the ventricle. The results of this paper can provide a new solution for pediatric echocardiographic bilateral ventricular segmentation and subsequent auxiliary diagnosis of congenital heart disease.
