2.Clinical efficacy of open radical mastoidectomy combined with ear cavity angioplasty and mastoid cavity obliteration in treatment of cholesteatoma otitis media

Objective To investigate the clinical efficacy of open radical mastoidectomy (ORM) combined with ear cavity angioplasty and mastoid cavity obliteration in treatment of cholesteatoma otitis media (COM). Methods Eighty-two patients with COM were divided into 2 groups according to surgical approach: control group (41 patients undergoing simple ORM) and observation group (41 patients undergoing ORM combined with ear cavity angioplasty and mastoid cavity obliteration). The clinical efficacy and recurrence rate between the 2 groups were compared. Results The total effective rate, dry ear rate and eardrum healing survival rate in observation group were significantly higher than those in control group: 95.12% (39/41) vs. 78.05% (32/41), 97.56% (40/41) vs. 75.61% (31/41) and 90.24%(37/41) vs. 73.17% (30/41), and there were statistical differences (P<0.01). The dry ear time and epithelialization time in observation group were significantly shorter than those in control group:(31.23 ±5.69) d vs. (48.12 ± 8.97) d and (24.41±3.23) d vs. (36.24 ± 5.69) d, the postoperative pure tone audiometry (PTA) and air bone gap (ABG) in observation group were significantly lower than those in control group:(25.61 ± 5.67) dB vs. (35.41 ± 8.23) dB and (13.24 ± 3.98) dB vs. (19.02 ± 5.52) dB, and there were statistical differences (P<0.01). There was no statistical difference in the incidence of complications between 2 groups (P>0.05). The recurrence rate in observation group was significantly lower than that in control group:2.44%(1/41) vs. 14.63%(6/41), and there was statistical difference (P<0.05). Conclusions The application of ORM combined with ear cavity angioplasty and mastoid cavity obliteration in the treatment of COM has significant effect, with rapid postoperative dry ear and epithelialization, fewer complications and lower recurrence rate. It should be widely applied.
3.Analysis on the rules of prescription for esophageal cancer from famous TCM doctor Zhang Shishun by using traditional Chinese medicine inheritance support system

Objective To analyze the TCM principles of herbal prescriptions given by professor Zhang Shishun for esophageal cancer by using traditional Chinese medicine inheritance support system (V2.0). Methods The herbal prescriptions from professor Zhang were collected and used for esophageal cancer, and then were recorded into the traditional Chinese medicine inheritance support system. The date mining methods were used such as principle analysis and revised mutual information. Results A total of 166 prescriptions for esophageal cancer were collected, which involved 316 Chinese medicines. The medicines with high frequency were Ganoderma, Caulis Marsdeniae Tenacissimae, Fructus Trichosanthis, Herba Rabdosiae Rubescentis, and Agaricus Blazei Murrill. The combinations of medicines included Ganoderma and Ramulus Juglandis, Radix et Rhizoma Sophorae Tonkinensis and Caulis Marsdeniae Tenacissimae, and Herba Rabdosiae Rubescentis and Caulis Marsdeniae Tenacissimae. The prescription rules of drug combination mode were obtained. Conclusions The Professor Zhang advocated the theory of determination of treatment based on pathogenesis through differentiation of symptoms and signs, patho-examination, patho-location. He also stressed on the therapy of relieving both primary and secondary symptoms. The Ganoderma, Ramulus Juglandis and Agaricus Blazei Murril were used to strengthen the body resistance. Radix et Rhizoma Sophorae Tonkinensis, Caulis Marsdeniae Tenacissimae and Herba Rabdosiae Rubescentis were used to eliminate pathogenesis. The Endothelium Corneum Gigeriae Galli, Fructus Crataegi Praeparata, Massa Medicata Fermentata Praeparata were used to protect stomach-qi. The Fructus Trichosanthis, Bulbus Allii Macrostemonis and Folium Ginkgo wereused to dilate the esophagus. The Fructus Trichosanthis was often combined with Radix Scutellariae to serve all medicines on the esophagus.

5.Bone marrow mesenchymal stem cells combined with VEGF gene for the treatment of limb ischemia in rabbits

Objective To evaluate bone marrow mesenchymal stem cells combined with VEGF gene in the treatment of limb ischemia in rabbits.Methods The right hind limb ischemia model of New Zealand rabbit was established by superficial femoral artery excision and deep femoral artery ligation.Rabbits then were divided randomly into 4 groups: empty plasmid control group(EP group),bone marrow mesenchymal stem cells group(BMSC group),VEGF gene therapy group(VEGF group),combination bone mesenchymal stem cells and VEGF gene therapy group(BV group).There were 8 rabbits in each group.Angiogenesis was detected by arteriography on day 28 after treatment and expression of VEGF was detected by immunohistochemical staining on day 30 after treatment.Results There were no differences of collateral vessel count between the EP group,BMSC group and VEGF group.The collateral vessel count in BV group was higher than that of the other three groups.Immunohistochemistry of VEGF showed that the integrated optical density(IOD)in BMSC and VEGF groups increased significantly compared with the EP group; the IOD in BV group was the highest compared with the other three groups.Conclusions Combination bone marrow mesenchymal stem cells and VEGF gene in the treatment of limb ischemia in rabbits can obtain stable and effective expression of VEGF along with significant improvement of limb ischemia.

OBJECTIVETo develop a simple and reliable method for intensifying the hybridization signals of gene chips.
METHODSThe authors added EDTA and another FAM-labeled probe to the normal PCR products, denatured the mixture by heat, and then let the mixture hybridize with the fastened probes on the chip.
RESULTSWith the use of EDTA and another FAM-labeled probe, the hybridization signals increased by 6 times or greater.
CONCLUSIONAdding EDTA and another probe to the normal PCR products is a simple and efficient method to intensify the hybridization signal of chips.

7.Application of 3D Printing Technology-assisted Customized Prosthesis for the Reconstruction of Distal Femoral Bone Defects

Objective To explore the early clinical efficacy of 3D printing technology-assisted customized prosthesis with preservation of femoral condyles for the reconstruction of bone defects after resection of distal femoral malignancies.Methods From June 2019 to September 2021,our department determined the lesion range of 12 cases of malignant tumors in the distal femur through CT and MRI before surgery.We used computer-aided production of osteotomy guide plates and customized prostheses that preserved the femoral condyle.After the prostheses were customized,the tumor segment was removed and a reconstruction was performed by using the customized prosthesis with preservation of femoral condyles.The knee function was evaluated by the American Musculoskeletal Tumor Society(MSTS)functional evaluation criteria.Results The operative time was 180-270 min,with a mean of 200 min.The intraoperative bleeding was 300-800 ml,with a mean of 450 ml.A total of 12 cases were followed up for 12-50 months(average,21.9 months).Two patients died of lung metastasis at 12 and 16 months after surgery,one patient survived with lung metastasis at 3 months after surgery,and one patient(8.3%)developed periprosthetic infection at 1 year after surgery.In 10 patients,imaging showed that the prosthesis and plate were in a good position without signs of loosening or bone resorption,the proximal femur was fixed in place with the 3D printed collar,and continuous bone scab formation was found in 4 cases.At the final follow-up,the functional status of the affected limb was good,with the knee flexion of 105°-135°(mean,115°)and no limitation of extension.The MSTS score was 22-28 points[mean,(24.3±1.8)points].Conclusion Application of 3D printing-assisted customized prosthetic replacement technology with preservation of the femoral condyles allows for precise resection of malignant distal femoral tumors and effective anatomical reconstruction without increasing surgical trauma and risk,significantly improving postoperative limb function and potentially reducing long-term complications.
8.Effect of clemastine fumarate on TLR4/PI3K/Akt signaling pathway during hypoxia-reoxygenation in rat cardiomyocytes

Objective To evaluate the effect of clemastine fumarate on Toll-like receptor 4/phosphatidylinositol-3-kinase/serine-threonine kinase (TLR4/PI3K/Akt) signaling pathway during hypoxia-reoxygenation (H/R) in rat cardiomyocytes.Methods H9C2 cells of rats cultured in vitro were seeded in culture wells or dishes at a density of 1×105 cells/ml and divided into 3 groups (n=11 each) by using a random number table method:control group (group C),H/R group and clemastine fumarate group (CF group).Cardiomyocytes were exposed to 5% CO2-95% N2in a low-glucose DMEM medium at 37℃ for 4 h followed by 4 h reoxygenation.At 4 h of reoxygenation,the cell viability was detected by CCK-8 assay,the ultrastructure was observed with a transmission electron microscope,the expression of TLR4,PI3K,phosphorylated Akt (p-Akt) and caspase-3 was detected by Western blot,and the expression of TLR4,PI3K and caspase-3 was detected by immunofluorescence.Results Compared with group C,the cell viability was significantly decreased,the expression of TLR4 and caspase-3 was up-regulated,and the expression of PI3K and p-Akt was down-regulated in group H/R (P<0.05).Compared with group H/R,the cell viability was significantly increased,the expression of TLR4 and caspase-3 was down-regulated,the expression of PI3K and p-Akt was up-regulated (P<0.05),and the mitochondrial damage was significantly attenuated in group CF.Conclusion The mechanism by which clemastine fumarate alleviates H/R injury to rat cardiomyocytes may be related to inhibiting TLR4 expression and activating PI3K/Akt signaling pathway.
9.Research progress on prostate-specific membrane antigen ligand positron emission tomography imaging of prostate cancer.

Prostate cancer is the most common malignant tumor in male urinary system, and the morbidity and mortality rate are increasing year by year. Traditional imaging examinations have some limitations in the diagnosis of prostate cancer, and the advent of molecular imaging probes and imaging technology have provided new ideas for the integration of diagnosis and treatment of prostate cancer. In recent years, prostate-specific membrane antigen (PSMA) has attracted much attention as a target for imaging and treatment of prostate cancer. PSMA ligand positron emission tomography (PET) has important reference value in the diagnosis, initial staging, detection of biochemical recurrence and metastasis, clinical decision-making guidance and efficacy evaluation of prostate cancer. This article briefly reviews the clinical research and application progress on PSMA ligand PET imaging in prostate cancer in recent years, so as to raise the efficiency of clinical applications.

Osteoporotic thoracolumbar fracture in the elderly will seriously reduce their quality of life and life expectancy. For osteoporotic thoracolumbar fracture in the elderly, spinal reconstruction is necessary, which should comprehensively consider factors such as the physical condition, fracture type, clinical characteristics and osteoporosis degree. While there lacks relevant clinical norms or guidelines on selection of spinal reconstruction strategies. In order to standardize the concept of spinal reconstruction for osteoporotic thoracolumbar fracture in the elderly, based on the principles of scientificity, practicality and progressiveness, the authors formulated the Clinical guideline for spinal reconstruction of osteoporotic thoracolumbar fracture in elderly patients ( version 2022), in which suggestions based on evidence of evidence-based medicine were put forward upon 10 important issues related to the fracture classification, non-operative treatment strategies and surgical treatment strategies in spinal reconstruction after osteoporosis thoracolumbar fracture in the elderly, hoping to provide a reference for clinical treatment.
