1.Analysis and countermeasures of complications in video-assisted thoracoscopic lobectomy

Objective: To summarize the methods of preventing and managing the complications in thoracoscopic lobectomy. Methods:The participants of this study included 317 patients undergoing lobectomy with video-assisted thoracoscopic surgery in the Department of Thoracic Surgery between January 2007 and December 2012. Intra-operative complications were observed, and countermeasures were summarized. Results: Complications occurred 28 times (8.8%), including bleeding in 16 cases because of accidental vascular injury (5.0%), accidental injury/break of bronchus in two cases (0.6%), vascular stump errhysis from cutting stapler in four cases (1.3%), lung stump air leakage in three cases (0.9%), lung injury in two cases (0.6%), and diaphragmatic injury in one case (0.3%). Conversion to thoracotomy was conducted in 17 cases, with a conversion rate of 5.4%. Thoracoscopic repair operation was performed in 14 cases that exhibited bleeding, with a success rate of 70% (14/20). No mortality was reported during the operation. Conclusion:Thoracoscopic lobectomy is a highly difficult method in thoracic surgeries. The procedure requires substantial attention on the timely prevention and correct management of intra-operative complications, particularly the injury and bleeding of major vessels, to reduce the rate of conversion to thoracotomy and the incidence of post-operative complications, as well as to promote the surgery in clinics.
2.Progress in technology for glycomics

Technologies for glycomics usually involve methods for separation and purification of poly-saccharides, and separation, structure resolution, quantification, property investigation and function comment of glycan chains. Because of the different biochemical properties of glycoproteins, proteogly-cans and glycolipids, the separation and purification of polysaccharides involve corresponding fractional precipitation, boric acid affinity, titanium dioxide, affinity chromatography, size exclusion method, and gel filtration chromatography column chromatography methods. The lectins, water affinity chromatogra-phy , solid phase extraction and other technologies could be applied to the oil enrichment of high pure and specific glycan chains. The structure of glycan chains can be analyzed using lectin microarray technolo-gy, mass spectrometry, and derivatization markers of glycan chains. lsotope labelling and metabolic labeling can be used to quantify glycan chains. The glycan biological function can be better understood using glycan chain structure analysis software and database of glycan chains by bioinformatics.
3.Genetic structure and diversity of T cell receptor alpha chain genes in Hezuo minipig

Objective To analyze the complexity of molecular structure in porcine T cell receptor gene and its similarity compared to humans.Method Based on the gene of porcine T cell receptor alpha chain ( TCRα) from the Gen-Bank database, 93 swine T cell receptor alpha chain genes ( STA) were cloned by reverse transcription-polymerase chain reaction (RT-PCR) from porcine peripheral blood lymphocytes, lymph nodes and spleen.Result Sequence analysis showed that STA genes all contain a domain of variable signal peptide and V, hypervariable J and conservative C.Howev-er, nucleotide sequence of STA was not completely identical with only 68.4%to 98.7%homology among genes, and had extremely sophisticated polymorphism and diversity.This was accord with the genetic structure of TCRαchain.Molecular structure, genetic evolution and classification of these genes were carried out according to the homology of TCRαgene, which all have several sites and zones of mutation on the domain of signal peptide, FR1 and CDR1, FR2 and CDR2, FR3 and CDR3.Analysis of similarity and classification of TCRαV domain(STAV)and J domain (STAJ) of Hezuo minipig u-sing IMGT/V-QUEST tools compared with those of humans found the genetic evolution relationship that was closer, and each of TRAV and TRAJ also found to have a corresponding fragment of humans, ever in 92% of similarity of TRAV be-tween swine and humans.Conclusion Our results indicate that inbred Hezuo minipig possesses genetic diversity against complicated environment of microbes in healthy status, and Hezuo minipig is suitable as an animal model for research on human immunology and diseases.
4.Comparison of thoracic drainage by two kinds of devices after single-port video-assisted thoracoscopic surgery in treatment of primary spontaneous pneumothorax

Objective To explore the clinical values of the modified thoracic drainage devices that were applied in treatment of primary spontaneous pneumothorax patients who underwent single-port video-assisted thoracoscopic surgery. Methods Clinical data of 82 primary spontaneous pneumothorax patients who underwent single-port video-assisted thoracoscopic lung wedge resection and pleurodesis from January 1st, 2015 to August 31st, 2016 was analyzed retrospectively. These patients, according to the thoracic drainage devices, were divided into traditional group (Group A, n = 42) or modified group (Group B, n = 40). These statistical data, including duration of thoracic drainage, lengths of hospital stay, duration of using antibiotic, amounts of pleural drainage, scales of pain, and complications of the two groups of patients in postoperative stage, were compared. Results There was no significant differences (P > 0.05) in volumes of thoracic drainage while there were significant differences (P < 0.05) in duration of thoracic drainage, lengths of hospital stay, duration of using antibiotic, scales of pain, and complications in postoperative stage between the two groups. And patients in group B had shorter time of thoracic drainage, hospital stay, and using antibiotic, lower scales of pain and rates of complications when compared with group A. Conclusion There are some clinical values of application of modified thoracic drainage devices in treatment of primary spontaneous pneumothorax underwent single-port video-assisted thoracoscopic surgery, because of it could give the patients fast recovery and made the operations simpler.
5.Study on effect of fushenkeli on expression of TAK1 in human renal tubular epithelial cells and its possible mechanism.

OBJECTIVETo investigate the effects of fushenkeli on the expression of TAK1 in the proliferation of the renal tubular epithelial cells induced by TGF-beta1 and its possible mechanism.
METHODHuman renal tubular epithelial (HK-2) cells were divided into five groups:blank control group, TGF-beta1 group (5 microg x L(-1)), intervention group 1 (5 microg x L(-1) of TGF-beta1 + 100 mg x L(-1) of fushenkeli), intervention group 2 (5 microg x L(-1) of TGF-beta1 + 500 mg x L(-1) of fushenkeli) and intervention group 3 (5 microg x L(-1) of TGF-beta1 + 1 g x L(-1) of fushenkeli). HK-2 proliferation was detected by methyl thiazolyl tetrazolium (MTT) assay. Type IV collagen in the supernatants of the cultured HK-2 was detected by ELISA at 12, 24, 48 hours respectively. The protein and mRNA expressions of TAK1 was measured by Western blot and real-time quantitative PCR.
RESULT1) The cell proliferation and the expression of type IV collagen were increased compared with the control group (P<0.05, P<0.01), but they were decreased in intervention group. 2) The expressions of protein and mRNA of TAK1 in TGF-beta1 group were upregulating significantly compared with control group (P<0.01), but they were downregulating in intervention group, especially in intervention group 3.
CONCLUSIONFushenkeli could inhibits TAK1 expression induced by TGF-beta1 in the proliferation of HK-2 cell.

6.Three-dimensional reconstruction of individual hepatic veins and portal veins system in hepatectomy.

OBJECTIVETo study the imaging characteristics and variations of individual digitized hepatic vein and portal vein which were reconstructed by medical image three-dimensional visualization system (MI-3DVS), assess the value of MI-3DVS assisted hepatectomy.
METHODSFrom June 2008 to September 2012, the clinical data of 81 patients who underwent hepatectomy with the assist of MI-3DVS were retrospectively reviewed. There were 61 male and 20 female patients, and their age were 12-81 years (median 46 years). The patients with malignant tumors were in 69 cases and with benign tumors in 12 cases. The characteristics and variations of individual digitized hepatic vein and portal vein were observed.Omnidirectional rotation of the three-dimensional (3D) model to observe the distribution of intrahepatic venous system as well as the relationship between the tumor and the veins. 3D models were then simulated resection by the Freeform modeling system.
RESULTSOf all the 81 3D models of the patients, greater posterior hepatic veins appeared in 10 (12.3%) cases, segment VI hepatic vein appeared in 34 (41.9%) cases. The portal vein was separted with the left branch and the right branch in the hilar in 64 cases, the portal trunk was divided into the left branch of portal vein, the right anterior portal branches, the right posterior portal branches trifurcated in 10 cases, the branch of right anterior portal vein start from the left trunk and the branch of right posterior portal vein start from the main trunk independently in 6 cases, there was 1 case, lack of left branch of portal vein. 81 patients underwent hepatectomy with the assist of MI-3DVS, minor hepatectomy in 57 cases, major hepatectomy in 24 cases (comparatively-reduced major hepatectomy in 12 cases). R0-resection was achieved in all of the patients. Both the inflow and the outflow were maintained in the residual liver after the completion of hepatectomy. Postoperative liver failure was observed in none of the patients.
CONCLUSIONSMI-3DVS in liver resection was the best choice of surgical approach provides an intuitive basis, and it could reduce the risk of surgery to prevent postoperative hepatic failure.
