1.Titanium intramedullary nail coated with vancomycin-hydroxyapatite in a model of open long bone fracture with wound infection

BACKGROUND: Tibia open fracture is a common disease, and intramedullary fixation has become the first choice, but the high risk of postoperative infection limits its clinical application.OBJECTIVE: To investigate the early prevention effect of vancomycin-hydroxyapatite (VCM-HA) coated titanium intramedullary nail in a rabbit model of open femoral fracture combined with wound infection.METHODS: Titanium intramedullary nails coated with three different concentrations of VCM-HA were prepared using biomimetic deposition, and subsequently in vitro bacteriostasis experiment was performed. Forty healthy male New Zealand white rabbits were selected and the model of mid-femur open fracture with wound infection was prepared. The rabbit models were then divided into ordinary intramedullary nail, low-, middle- and high-concentration VCM-HA coated intramedullary nail groups (n=10 per group). The wound appearance of the animals was dynamically observed. The body mass, anal temperature, white blood cells and C-reactive protein contents in the venous blood were monitored. The pathological examination of the soft tissue around the fracture and injured limb X-ray were conducted regularly.RESULTS AND CONCLUSION: The prepared VCM-HA coated titanium intramedullary nail exerted obvious antibacterial effects in vitro. The anal temperature in the ordinary nail group was significantly higher than that in the other three groups at 3 days after modeling (P < 0.05). The white blood cells and C-reactive protein contents in the venous blood in the VCM-HA groups were significantly lower than those in the ordinary nail group at 3, 7, 14 and 28 days after modeling (P < 0.05). The hematoxylin-eosin staining of the peripheral tissues displayed the inflammatory response in the VCM-HA groups was milder than that in the ordinary nail group at each time point after modeling. The injured limb X-ray displayed that the callus grew slowly accompanied with sequestration in the ordinary nail group, while the callus growth in the VCM-HA groups was significantly better than that in the ordinary nail group. To conclude, the VCM-HA coated intramedullary nail can effectively reduce postoperative infection in an animal model of open femoral fracture combined with wound infection.
2.Comparison of two methods for treating adult humeral shaft fracture

Objective To evaluate the clinical effects of open reduction plate screw nail internal fixed and minimally invasive elastic intramedullary nail internal fixation combined with neck strap suspension for treating adult humeral shaft fracture . Methods A retrospective analysis was made on the clinical data of 65 patients with humeral shaft fracture in our hospital from March 2012 to March 2014 ,including 38 cases of open reduction plate screw internal fixation (plate screw nail fixation group) and 27 cases of limited open or closed reduction elastic intramedullary nail internal fixation combined with neck strap suspension (elastic intramedullary nail group) .The operation time ,intraoperative blood loss amount ,iatrogenic radial nerve injury ,occurrence rate of incision infection ,postoperative fracture reduction degree ,fracture recovery time and elbow joint function score were compared be‐tween the two groups .Results The operation time and intraoperative bleeding volume in the elastic intramedullary nail group were less than those in the plate screw nail fixation group ,the differences were statistically significant (P<0 .05);the postoperative frac‐ture healing rate had no statistically significant difference between the two groups (P>0 .05) ,the postoperative shoulder and elbow joint function scores in the plate screw nail group were significantly higher than those in the elastic intramedullary nail group ,the difference was statistically significant (P<0 .05) .No serious nerve damage ,incision infection and other postoperative complications occurred in the two groups .Conclusion Both the open reduction plate screw nail internal fixation and elastic intramedullary nail in‐ternal fixation are effective method for treating adult humeral shaft fracture ,the latter has shorter operation time ,less intraoperative bleeding ,but shoulder elbow pain and limitation of motion are more serious than the former .
3.Three-dimensional finite element analysis of stress on connector of CAD/CAM fixed bridge

BACKGROUND:Artificial teeth computer aided design and computer aided manufacture (CAD/CAM) is a high technology involving various subjects.Many CAD/CAM systems have designed.However,they have some disavantages and a high cost.Therefore,only the system with proprietary intellectual property can solve this problem.OBJECTIVE:To optimize the connector of the CAD/CAM fixed bridge.METHODS:Three-dimensional finite element models of the connector were established which was used to design different styles of connectors.Stress under different vertical loading conditions was compared.RESULTS AND CONCLUSION:The connectors with different appearances displayed varying stress.Stress on the connector with arc transition was less than connector without arc transition.Connector was supposed to design to style with arc transition.

5.Biocompatibility of Kirschner wireversus absorbable rod fixation for repairing supracondylar fracture of humerus in children

BACKGROUND:Humeral supracondylar fracture is a common fracture occurred in children. The selection of internal fixation for humeral supracondylar fracture remains controversial. OBJECTIVE:To compare the biocompatibility between internal fixation with Kirschner wire and bioabsorbable implants for humeral supracondylar fractures. METHODS: From January 2007 to January 2013, 246 cases of humeral supracondylar fractures, from Affiliated Hospital of Luzhou Medical Colege, were treated by internal fixation with Kirschner wire. Meanwhile, the studies on internal fixation for treating supracondylar fracture of humerus in children were searched. Efficacy was evaluated by preoperative and postoperative elbow range of motion and the incidence of cubitus varus, and the results were statisticaly analyzed, and compared with other therapeutic methods. RESULTS AND CONCLUSION:Al cases were folowed up for 6-36 months, averagely 18 months. According to Mayo elbow performance score, function of the elbow joint was excelent in 194 cases, and good in 48 cases, with good and excelent rate being 98.4%. Four cases suffered < 5° cubitus varus, with incidence rate of 1.63%. The internal fixation with Kirschner wire provided functional recovery of elbow joint, but the second stage operation was needed to pick out the wires. And it might be perplexed by Kirschner wire loosening or needle withdrawal, resulting in instable fixation. Bioabsorbable implants were effective in the treatment of supracondylar fracture of humerus. Bioabsorbable sticks would break down over time, without harming to human body or influencing imaging examination. Elbow function development of the epiphysis would not be affected. However, due to lack of large-sample observation, long-term effects of bioabsorbable implants for treating supracondylar fracture of humerus in children deserve further studies.
6.Relationship between systemic lupus erythematosus and hepatitis B infection and cytokine regulation

Objective To investigate the relationship between systemic lupus erythematosus (SLE) and hepatitis B virus (HBV) infection and the regulation of Th1/Th2 cytokine.Methods The HBV surface marker of 131 SLE patients and 582 age sex matched healthy subjects as the control was tested using sandwich ELISA.Four groups:patients with SLE and HBV infection (A),with SLE (B),with chronic hepatitis B infection (C) and normal controls (D) were selected for measurement of the production of IFN ? and IL 10 in serum of each group.Results None of the 131 SLE patients were positive for HBsAg,which was significantly lower than that of controls (7 7%, P 0 05).But HBeAb,HBcAb and HBsAb positive occurence was 43 8% in SLE,which was much higher than normal controls (26 1%, P 0 05).The IL 10 in SLE patients was much higher than in the normal controls ( P 0 05).Conclusion In SLE patients,the surface marker of HBV HBsAg is remarkebly lower,while the HBsAb is comparatively higher.In patients with SLE and HBV infection,their serum IL 10 and IFN ? levels are significantly different from those of SLE patients,but not correlated to those of HBV infection patients.This phenomenon may result from the interaction between Th1/Th2 cytokines.
7.Expression and significance of CD8+CD28- T cells in the peripheral blood of patients with AS

Objective To examine the expression of CD8+CD28- T cells in the peripheral blood and explore their roles in the pathogenesis of AS. Methods The expression of CD8+CD28- T cells obtained from 50 AS patients and 21 healthy controls were assayed by flow cytometry. CRP was also measured. Results The percentage of peripheral blood CD8+CD28- T cells in patients with AS was significantly increased compared to normal individuals [(18±6)% vs (14±5)%, P=0.020], while the percentage of peripheral blood CD3+,CD8+ CD28+ T cells in patients with AS was significantly decreased[ (65±9)% vs (69±8)%, P=0.039]; [ (15±5)% vs(18±4)%, P=0.038]. No difference was found in CD8+ T cells between patients with AS and normal individuals(P>0.05). Conclusion The percentage of peripheral blood CD8+CD28- T cells in patients with AS is significantly increased comparing to normal individuals. This suggests that CD8+CD28- T cells may play an important role in the pathogenesis of AS.
8.A cross-sectional study on urgency urinary incontinence among adult women in Beijing

Objective To investigate prevalence and risk factors of urgency urinary incontinence (UUI) among adult women in Beijing.Methods Multiple-stage stratified sampling was used to recruit women equal to and more than 20 years old from 48 communities ( villages and neighborhoods ) in six districts of Beijing,by household interview with a standardized questionnaire to collect information of demographic characteristics,delivery history,health conditions and UUI related clinical data.Data analysis was performed by SPSS software version 16.0.Results Totally,3058 adult women completed the survey,with an overall prevalence of UUI of 1.7 % (52/3058).Distribution of age,cultural background,family income,number of pregnancy,number of abortion,delivery history,history of chronic diseases,general surgery,pelvic surgery,abdominal pain and abdominal distension were found significantly different between UUI participants and non-UUI ones with univariate analysis ( all P < 0.05 ).Results of multivariate logistic regress analysis showed that number of pregnancy ( OR = 1.29,95% CI = 1.06-1.58 ),history of chronic diseases ( OR = 2.53,95% CI = 1.38-4.62 ) and history of abdominal pain ( OR = 2.32,95%CI= 1.10 -4.91 ) all significantly associated with UUI.Conclusions UUI associates with complicated factors and its prevention and intervention at communities is necessary targeting at related factors.
9.A study on Pertinence of pathologic and biology features in HER-2, ER and PR negative breast cancer

Objective To analyze the pathologic and biology features and prognosis of HER-2,ER and PR negative breast cancer(TNBC).Methods Clinic pathologic data of 329 breast cancer patients was retrospectively analyzed. The expression of HER-2, ER, PR and P53 was determined by immunohistochemistry. The patients were divided into three groups, including TN group [ HER-2 (-), ER (-), PR (-)], HER-2 group [HER-2(+),ER(-),PR(-)] and HR group [ER(+),PR(-) or (+),HER-2(-) or (+) ].The pathology and biology features and P53 masculine expression of the three groups were compared.The 5-year overall survival and disease free survival were analyzed by Kaplan-Meier method.Results Of the 329 patients, 20.97% (69/329) was TN group, 34.04% (112/329)was HER-2 group, and 44.98% (148/329) was HR group.The percentage of with lymph node metastasis in TN group 55.07% (38/69) was higher than that in HER-2 group 42.86% (48/112) and HR group 43.24% (64/148) (χ2 = 12.57, P < 0.05).The rates of P53 positive, operation recurrence and metastasis in TN group were 44.93%, 27.54% and 20.29%, which were higher than that in HER-2 group (20.54% ,16.07%,16.96%) and HR group (18.24%, 12.84%, 10.81%) (χ2=12.23, 8.36, P <0.05).The ratio of tumor ≥5cm, Ⅲ stages, Ⅲ grade and soakage canula cancer among three groups had statistical difference (χ2 = 7.25,8.79,9.23,8.48, P < 0.05).The 5-year overall survival in three groups were 75.36%,82.14% ,85.14% and disease free survival were 68.12% ,78.57% ,82.43% (χ2 =8.52, P <0.05).Conclusion The pathology and biology traits of TNBC were high rate of P53 (+) and recurrence and lymph node metastasis.The most important factor for poor prognosis of TNBC was the low rate of disease free survival for 5 years.
10.Prevalence of fecal incontinence among adult women in Beijing district

Objective To investigate the prevalence of fecal incontinence (FI) among adult women in Beijing area, and to analysis risk factors associated with FI. Methods A stratified multiple-cluster systemic method was used to recruit women ≥ 20 years old in 6 districts of Beijing. A self-administrated questionnaire was used to collect information about Fl and possible risk factors. Results There were three thousand and 58 women participated in the survey, the average age was (48 ± 16 ) years (range 20 -79 years). The prevalence of FI was 1.28% ( 39/3058 ), which was related to age. There were five factors entered the logistic regress model, which were included age ≥40 years old ( OR = 3.3, 95% CI: 1.7 -6. 8), urinary incontinence ( OR = 3.0, 95% CI: 1.5 - 6. 1 ), vaginal delivery ( OR = 2.4, 95% CI:1.2 -4. 9), household per capita income ≤2000 RMB per month ( OR = 3. 3, 95 % CI: 1.6 - 6. 5 ), and feeling fatigue ( OR = 3.0, 95% CI: 1.5 - 5.8). Conclusion Prevalence of FI is low among adult women in Beijing area, while risk factors associated with Fl are complicated and further studies are necessary to be conducted.
