1.Prevalence of Excessive Daytime Sleepiness and Associated Factors in 3085 University Teachers and Staffs in Kunming

Objective To investigate the prevalence of excessive daytime sleepiness and explore the associated factors in university teachers and staffs in Kunming.Methods Epworth sleepiness scale was used as the tool of diagnosis. We consecutively collected the data of teachers and staffs on duty in Yunnan Nationalities University,Yunnan Agricultural University,Yunnan Normal University,Kunming Medical University and Yunnan University of Finance and Economics within 8 months.Results The prevalence of excessive daytime sleepiness in university teachers and staffs was 12.45%, the prevalence of male was 16.82% and female was 7.70%, respectively. The results of Logistic regression showed that the main associated factors of excessive daytime sleepiness were male (OR=1.898),age (30 years and less was as control group,the ORs of 31-40 year group, 41-50 years group and more than 51 years group were 1.480, 2.366 and 2.719, respectively), family history of snoring (OR=3.371), consumption sedative (OR=1.815), memory failing (OR=2.044), dizzy and feeble (OR=2. 076),choking (OR=2.143),report apnea (OR=2.684),etc. Conclusions The prevalence of excessive daytime sleepiness is high in the university teachers and staffs that can partially represent the residents of Kunming city. The associated factors of excessive daytime sleepiness hint that excessive daytime sleepiness may be caused by sleep apnea/hyperpnoea syndrome in the sample population,hence the people with excessive sleepiness need further sleep monitoring .
2.Analysis on the nursing of integrated traditional Chinese and western medicine in the treatment of diabetes mellitus complicated with nephrolithiasis

Objective To study and analyze the nursing intervention of integrated traditional Chinese and western medicine in the treatment of diabetes mellitus complicated with nephrolithiasis. Methods A total of 84 patients with diabetes complicated with nephrolithiasis who were treated from July 2014 to July 2015 were selected as the main study subjects, and they were divided into study group and control group according random number table method, with 42 cases in each group. The control group was given percutaneous nephrolithotomy for western medicine alone, and was given routine nursing care. The observation group was given integrated Chinese and western Medicine and was given comprehensive nursing intervention according to the treatment methods. The changes of blood glucose, 24 h Pro, ALB and incidence rates of complications were compared between the two groups. Results The levels of FBG and 24 h urine, Pr and other indicators in the study group after nursing were significantly better than those in the control group; the incidence rate of complications was also significantly lower than that in the control group, and the difference was statistically significant(P<0. 05). Conclusion The scientific and effective nursing intervention in integrated traditional Chinese and western medicine in the treatment of diabetes complicated with nephrolithiasis can adjust the blood sugar level and reduce the occurrence of patients' complications.
3.Clinical value of high frequency ultrasounography in the evaluation of osteophytes in patients with hand osteoarthritis

Objective:To summarize the common sites of osteophytes in patients with hand osteoarthritis (OA), and analyze the correlation between the severity of osteophytes and clinical factors.Methods:One hundred and four patients with hand OA were selected and divided into three groups according to the disease duration: <1 year, 1~5 years, >5 years. The first carpometacarpal joint(CMC1), metacarpophalangeal joint(MCP), proximal interphalangeal joint (PIP) and distal interphalangeal joint (DIP) were detected by high fre- quency ultrasound. The location of osteophytes and osteophyte semi-quantitative grading scores (OSGS) were recorded. The patients age, disease duration, erythrocyte sedimentation rate (ESR), C-reactionprotein (CRP), the visual analogy score (VAS) and Australian/Canadian osteoarthrits hand index (AUSCAN) were collected. The indicators of different groups were compared, the incidence and location of osteophytes was calculated, and the correlation between osteophytes and clinical factors was analyzed. Data were analyzed by Wilcoxon rank sum test, Kruskal-Wallis test, χ2 test, Spearman correlation analysis. Results:① Osteophytes accounted for 33.56%(1 047/3 120) of the 3 120 joints in hands. There were statistically significant differences in OSGS and AUSCAN in different groups ( H=13.485, P<0.01; H=51.491, P<0.01), while no statistically difference in VAS, ESR and CRP ( H=5.808, P=0.055; H=2.878, P=0.237; H=2.319, P=0.314). ② In different joint areas of hands, PIP accounted for the largest proportion (46.54%, 484/1 040), followed by DIP (46.51%, 387/832), CMC1 (30.77%, 64/208), and MCP accounted for the smallest proportion(10.77%, 112/1 040). There were statistically significant differences in the incidence of osteophytes in different joint areas( χ2=384.194, P<0.01).③ In the interphalangeal joint areas of hands, the largest osteophytes composition ratio was MCP3 (46.43%, 52/112), PIP3 (30.58%, 148/484) and DIP2 (31.01%, 120/387), respectively. ④ OSGS were positively associated with age, disease course, VAS and AUSCAN ( r=0.370, P<0.01; r=0.382, P<0.01; r=0.215, P=0.029; r=0.390; P<0.01), there was no correlation between OSGS and ESR or CRP ( r=0.173, P=0.079; r=0.162, P=0.101). Conclusion:PIP are the most common sites of osteo-phytes in hand OA, followed by DIP. High frequency ultrasound can help the diagnosis and evaluate the severity of hand OA.
4.Preparation and Antidepressant Activity Study of β-sitosterol and Its Derivatives from the Fruit of Sorbus pohuashanensis

OBJECTIVE: To prepare β-sitosterol (shorted fo r “Sit”) and its deri vatives from the fruit of Sorbus pohuashanensis,and to investigte their antidepressant activities. METHODS :Using the fruit of S. pohuashanensis as raw material , extracted with 75% ethanol and 20%KOH solution ,Sit was obtained after extraction and crystallization. C 3 hydroxyl group of Sit was used as the structural modification site ,CH2Cl2 was used as the reaction solvent ,DMAP was used as catalyst ,EDCI was used as dehydrating agent ,4 kinds of organic acids (salicylic acid ,2-tetrahydrofuranic acid ,phenylalanine,sorbic acid )were added to make the carboxylation reaction to produce ester derivatives Sit-S ,Sit-T,Sit-P and Sit-Sr. The chemical structure of its derivatives were elucidated by IR and NMR. The tail suspension test and the forced swimming model were used to preliminarily explore the antidepressant active components of Sit and 4 kinds of derivatives ;tail suspension test and the spontaneous exercise capacity test were used to screen effective dose of the compounds with antidepressant active. The compound was used alone or in combination with agomelatin [ 40 mg/kg,5-hydroxytryptamine(5-HT)and 5-HT 2C receptor antagonist] ,haloperidol [ 0.2 mg/kg,non-selective D2 receptor antagonist] and bicuculline [ 4 mg/kg,competitive γ-aminobutyric acid(GABA)antagonist],respectively. Sixty minutes after intraperitoneal injection ,tail suspension test was performed. The levels of 5-HT,dopamine(DA)and GABA in brain tissues of mice were detected by ELISA. The blank control group was given intraperitoneal injection of 10% propylene glycol solution. RESULTS:By spectrum technology ,the corresponding ester compounds were synthesized by the reaction of Sit with four kinds of organic acids. Among Sit and its four derivatives ,the immobility time of Sit-S group was the shortest in tail suspension test and forced swimming test ,which was significantly shorter than that of blank control group (P<0.05). Screening results of Sit-S showed that the effective antidepressant dose was 4 mg/kg, * and it did not affect the spontaneous activity of mice compared with the blank control group (P>0.05). With this dose ,Sit-S could significantly shorten the immobility time of mice in tail hua1666@163.com suspension test (P<0.01),and can s ignificantly increase the ·64levels of 5-HT,DA and GABA in the brain tissue of mice (P<0.05 or P<0.01),but its effect can be reversed by agomelatine , haloperidol and bicuculline to different extent. CONCLUSIONS :Sit salicylate derivatives Sit-S exhibits good antidepressant activity,and its mechanism of action may be mediated by increasing the levels of 5-HT,DA and GABA.
