Objective To observe over-time changes in rat blood flow in the skins of related meridian points during physiological status, the pathological state of ischemic myocardial injury and low or high frequency electroacupuncture intervention and explore the post effect of different frequency electroacupuncture on related meridian points after treating ischemic myocardial injury. Method Fifty male Wister rats were randomized into five groups: blank control, sham operation, model, low frequency electroacupuncture (meridian point A) and high frequency electroacupuncture (meridian point B), 10 rats each. Blood flow in the skins of bilateral points Neiguan (PC6), Ximen (PC4) and Tianquan (PC2), and non-meridian and non-acupoint control points was measured by laser speckle contrast imaging in every group immediately and at 30 and 60 min after the end of three treatments. Statistical analysis was made. Result Blood flow in the skins of bilateral points Neiguan, Ximen and Tianquan was significantly lower in the blank control group than in the model group (P<0.01,P<0.05). Blood flow in the skin of every acupoint increased in varing degrees after low or high frequency electroacupuncture treatment. Blood flow in bilateral points Neiguan, Ximen and Tianquan regions in meridian point group A was closer to that in the blank control group immediately after treatment. Blood flow in three left-side and three right-side acupoint regions in meridian point group B was closer to that in the blank control group at 30 and 60 min, respectively, after treatment. Conclusion The immediate effect of low frequency electroacupuncture on blood flow in acupoint regions is better than high frequency electroacupuncture during intervention in ischemic myocardial injury. The post effect of high frequency electroacupuncture on blood flow in acupoint regions is better than low frequency electroacupuncture during intervention in ischemic myocardial injury.

Objective To understand the barriers of surrogate decision-makers participating in the advanced care planning(ACP)for patients with advanced cancer, and to provide references for the construction and intervention of educational programs. Methods Under the guidance of ternary reciprocal determinism, qualitative research method was used to conduct semi-structured interviews with 12 surrogate decision makers of advanced cancer patients in 2 tertiary hospitals of Zhejiang Province, and the data was analyzed using targeted content analysis. Results Based on the theory, the influencing factors of participating in ACP of the surrogate decision-makers of patients with advanced cancer were summarized into 3 themes:individual level-dual restriction of subjective thought and objective ability(value restriction, cognitive understanding deviation, economic condition restriction, patient's situation control),environment level-synergistic influence of medical environment and social environment(limited medical conditions, pressure from public moral opinion, insufficient policy guarantee, lack of family consensus),behavior level-interaction of external guidance and internal feedback(lack of guidance of medical staff, experience support of medical decision-making).Conclusion Surrogate decision-makers of patients with advanced cancer face many pressures before participating in the ACP.Under the guidance of the theory of knowledge, belief and practice, medical staff can give them pre-established medical care knowledge to correct their cognitive bias.Medical staff can guide the realization of"family-centered"decision-making mode to strengthen the family support of surrogate decision-makers.Medical staff also can lead the establishment of a surrogate decision-makers support group to share decision-making experience and provide medical decision-making assistance.

Objective:To analyze the current situation of middle-level cadres in functional departments of scientific research institutes and to construct a job competency model for middle-level cadre. Methods:A cluster random sampling method was used to conduct a questionnaire survey in March 2022 on the satisfaction level of middle-level cadres in the functional departments of the Institute of Blood Transfusion of the Chinese Academy of Medical Sciences,a mid-level cadre competency questionnaire survey was conducted to collect data among functional department personnel of scientific research institutes affiliated to the Chinese Academy of Medical Sciences and Peking Union Medical College. The principal component analysis method was used to extract factors, and the maximum variance rotation method was used to conduct exploratory factor analysis to construct a competency model. Results:The Middle-level cadres in scientific research institutes had problems such as old age, traditional management style, etc. The competency model of middle-level cadres in functional departments of scientific research institutes included 3 main competencies:responsibility taking ability, communication ability and learning execution ability, and 34 competency factors. Conclusion:The competency model of middle-level cadres in functional departments of scientific research institutes can provide basis and reference for the selection and appointment, education and training, performance management and career planning of middle-level cadres in scientific research institutes.
