Sepsis is defined as an organ dysfunction caused by systemic inflammatory response induced by infection,and may eventually developed into multiple organ failure (MOF).The onset of sepsis often involves the activation of inflammatory cells,over expression of inflammatory cytokines,immune dysfunction,damage of tissue and organ,excessive apoptosis of cell.Mesenchymal stem cells (MSCs) have a wide range of biological activities,including anti-inflammatory,antibacterial,immune regulation,regeneration and repair of tissue and organ,anti-apoptosis,etc.In recent years,a large number of preclinical studies had demonstrated that MSCs has good actions on sepsis.The purpose of this review is to describe the mechanism of MSCs in the treatment of sepsis,and to point out the possible clinical challenges.
3.Correlation between TCM Syndrome Type and Gastro-intestinal Hormones Level in Patients with Irritable Bowel Syndrome

Objective To observe the change of gastro-intestinal hormone levels about syndrome type of stagnation of liver qi, deficiency of spleen and stomach, liver qi stagnation and spleen qi deficiency in patients with irritable bowel syndrome (IBS), and to probe into the correlation between TCM syndrome type and gastro-intestinal hormone levels. Methods One hundred and three cases of patients with IBS were divided into liver qi stagnation group (35 cases), spleen and stomach deficiency group (30 cases) and liver qi stagnation and spleen qi deficiency group (38 cases), with normal group (10 cases) as control. 4.Construction and Expression of Prokaryotic Expression Plasmids of Pneumocystis carinii p55 Antigen Gene Fragment

Objective To construct prokaryotic recombinant expression plasmid carrying Pneumocystis carinii Mr 55 000 antigen(p55) gene fragment and express the recombinant protein. Methods P. carinii pneumonia(PcP) rat models were established by subcutaneous injection of dexamethasone for 14 weeks. Total RNA was extracted from lung of P. carinii rat and p55 antigen gene fragment was cloned by RT-PCR,which was identified by sequencing. The 690 bp fragment was cloned to pGEX-4T-1,the recombinant plasmid was screened and identified by restriction analysis and PCR. The recombinant plasmid was finally induced with IPTG to express a new fusion protein,and the products were analyzed by SDS-PAGE and Western blot. Results A fragment of 690 bp was obtained by RT-PCR. The recombinant pGEX-4T-1/690 was constructed. SDS-PAGE revealed that the molecular weight of the recombinant protein was approximately Mr 62 000,the maximum amount of the fusion protein produced was 11.6% of the total protein. The recombinant protein can be recognized by GST antibody and by the sera from P. carinii infected rats using Western blotting. Conclusion Prokaryotic expression plasmid pGEX-4T-1/690 has been constructed and the recombinant fusion protein shows antigenicity.
5.Chronic granulomatous disease caused by compound heterozygous mutation in NCF2 gene: case report and literature review

Objective:To explore the relationship between the type of mutation and clinical features, prognosis, and clinical characteristics of chronic granulomatous disease (CGD) caused by compound heterozygous mutations in the NCF2 gene in children. Methods:The clinical data of 1 case of neonatal CGD caused by compound heterozygous mutations of NCF2 gene at Tianjin Children′s Hospital in August 2019 was analyzed, and domestic and international literatures were searched to summarize the clinical characteristics, gene mutation type and prognosis of CGD caused by NCF2 mutation. Results:The diagnosis of CGD was confirmed by the presence of compound heterozygous mutations c. 196_197insA (p.Arg66Glnfs23X) and c. 1180T>G (p.Tyr394Asp) in the NCF2 gene, accompanied with the clinical manifestations of fever, cough, multiple clumps and nodules in the chest CT at 25 days after birth, and the neutrophil respiratory burst test stimulation index(SI) 23.This new mutation was not reported in the Human Genetic Mutation Database.The child had a residual portion of nicotinamide adenine dinucleotide phosphate (NADPH) oxidase activity and was followed up until the age of 9 months with an antifungal drug without recurrent infection.A total of 101 cases of CGD patients with NCF2 gene mutation were reported in domestic and international databases.Totally, 33 cases had SI results, with 22 cases below 3, 11 cases above 3, and 8 cases of missense mutations. Conclusions:c. 196_197insA and c. 1180T>G are new mutations in NCF2 gene that can lead to CGD.CGD patients containing missense mutations in the NCF2 gene may have more residual NADPH oxidase activity.
6.Clinical analysis of retroperitoneal laparoscopic partial nephrectomy on patient with incidental renal cell carcinoma

Objective To evaluate the safety and clinical efficacy of retroperitoneal laparoscopic partial nephrectomy (LPN) in patients with incidental renal cell carcinoma.Methods Twenty-six patients with T1 incidental renal cell carcinoma treated with retroperitoneal LPN from Dec.2011 to Oct.2013 were analyzed retrospectively.The operation time,warm ischemia time,intraoperative blood loss,complications and prognosis of perioperative periods were collected.Results All 26 cases were operated successfully without conversion to open or other surgery.The operation period was 90-190 minutes.Blockage of renal artery was applied in all 26 cases,and the warm ischemia time was 15-30 minutes.The intraoperative blood loss was 100-360 ml.The drainage was removed 3-7 days postoperatively,and the hospitalization period was 7-12 days.All cases were clear cell renal cell carcinoma by pathological examinations,and showed negative surgical margins.No complications such as postoperative bleeding,inflammation and leakage of urine were occurred.All cases had normal renal function during the follow-up of 3-25 months without local recurrence or distant metastasis.Conclusion Retroperitoneal LPN for the treatment of T1 incidental renal cell carcinoma is worthy of application with the advantages of safety,minimal invasiveness,fewer complications,quick recovery and good nephron-sparing functional recovery.
7.MRI evaluation of argon-helium cryoablation therapy for the treatment of prostate cancer:an analysis of 16 cases

Objective To evaluate MRI in judging the therapeutic effect of argon-helium cryoablation therapy for prostate cancer. Methods The clinical data and imaging materials of 16 patients with prostate cancer , who had received ultrasound-guided argon-helium knife cryogenic treatment at authors ’ hospital during the period from March 2012 to Oct. 2014, were retrospectively and comprehensively analyzed. The preoperative and postoperative laboratory test results were compared with MRI findings, and the ablation effect was assessed, focusing on the surgical residue, metastasis, etc. Results One months after the surgery, MRI demonstrated that satisfactory ablation extent was obtained in all patients, the ablated tumor tissue was characterized by long-T2 signal and no obvious recurrence could be found on DWI and DCE-MRI. A slight decrease of PSA level was observed. Six months after the surgery, MRI revealed that the prostate size was significantly reduced, PSA level was markedly decreased and no obvious evidence of recurrence was observed. No severe postoperative complications, such as urethral necrosis, urethral stricture or urethro-rectal fistula, occurred. During the 6-month following-up time, one patient died from other causes. Conclusion MRI has excellent clinical application value in estimating the ablation extent and in judging postoperative recurrence or metastasis of prostate cancer after argon-helium cryoablation treatment.
8.Randomized Controlled Trial of Type A2 Distal Radius Fracture Treated with Small Splint Fixation and Plaster Fixation

Objective To compare the efficacy of small splint fixation and plaster fixation in treating distal radius fracture. Methods One hundred and four cases of type A2 distal radius fracture admitted from January of 2012 to May of 2013 were randomly divided into small splint fixation group and plaster fixation group. After reduction, patients in the two groups were fixed with small splint and plaster separately. Wrist function scores, 36-item Short Form ( SF-36) scores and roentgenographic scores were evaluated for the two groups at different time points. Results ( 1) Within 3 months of follow up, the improvement of wrist function in small splint fixation group was significantly superior to that in plaster fixation group (P<0.05), and the differences of wrist function scores were insignificant between the two groups with the prolongation of follow-up (P>0.05). (2) SF-36 scores of the two groups were gradually increased along with the follow up. After follow-up for one week, 2 weeks, one month and 3 months, the SF-36 scores in small splint fixation group were significantly higher than that in plaster fixation group ( P<0.05) , but there was no significant difference after 6 months ( P>0.05). ( 3) After follow up for one year, the excellent rate for roentgenographic scores was higher than 80% in both groups, and there was no significant difference between the two groups (P>0.05). Conclusion Both small splint fixation and plaster fixation have satisfacory efficacy in treating type A2 distal radius fracture. But during the therapeutic process, wrist function scores and life quality scores in the small splint fixation group were higher than those in the plaster fixation group.
9.The teaching practice of propagating healthy lifestyle by combining physiology with “health cornerstones”

Teaching healthy lifestyle in combination with physiology is hardly practiced in schools of medicine. Individual lifestyle factor accounts for 60% among the factors that lead to dis-eases. Physiology contains the basic theories for health knowledge. Therefore, during physiology teach-ing , healthy lifestyle was introduced by combining such contents of physiology as digestion and absorption , energy metabolism balance and emotional physiological reaction with four “health corner-stones” such as health diet, regular physical activity, psychological balance, quitting smoking and drinking less alcohol. This teaching mode has widened the teaching ideas, which has not only in-creased the students' interest and significance in physiology learning, but also promoted them to de-velop good healthy lifestyle.
10.Clinical study of treating somatoform pain disorder with the combination of electroacupuncture and duloxetine

Objective:To observe the clinical efficacy of treating somatoform pain disorder(SPD)with electroacupuncture(EA)at the Governor and Conception Vessel points plus duloxetine.Methods:Eighty-two SPD patients were randomly allocated to an observation group and a control group,with 41 cases in each group.The control group was intervened by oral administration of duloxetine hydrochloride enteric capsules at a dose of 60 mg per time once a day;based on the medication,the observation group received additional EA treatment by selecting points from the Governor and Conception Vessels.Clinical efficacy was evaluated after 8 weeks of treatments;changes in the scores of the short-form McGill pain questionnaire(SF-MPQ),self-report symptom inventory,symptom check list-90(SCL-90),Pittsburgh sleep quality index(PSQI),and generic quality of life inventory-74(GQOLI-74)were also compared.Results:After the intervention,the observation group surpassed the control group in comparing the total effective rate(P<0.05).The SF-MPQ score,SCL-90 somatization score,and PSQI score dropped notably in both groups after treatment,and the intra-group differences were statistically significant(P<0.05);the three scores were significantly lower in the observation group than in the control group(P<0.05).The GQOLI-74 score got an increase in each dimension in both groups after treatment,and the intra-group differences were also statistically significant(P<0.05);the GQOLI-74 dimension scores were all significantly higher in the observation group than in the control group(P<0.05).Conclusion:For patients with SPD,combining EA at the Governor and Conception Vessel points and duloxetine hydrochloride enteric capsules can markedly improve their clinical symptoms and quality of life.
