1.An experimental study on human embryonic stem cell-derived hepatocytes transplantation into liver decompensated mice

Objective To study the ability of human embryonic stem cells to integrate into mouse liver and to repair chronic liver injury of the recipient.Methods On day 1,day-7 and day-15 after human embryonic stem cells were induced to differentiate into hepatocyte-like cells and were transplanted into mice with chronic liver failure,liver histopathology,liver function,liver tissue regeneration and mature hepatocytes of mice were tested respectively.Results Hepatic tissue pathology of mice significantly improved after transplantation and necrotic foci diminished,hemorrhage and congestion of hepatic cells relieved,and liver function improved.It was observed that human embryonic stem cells survived,proliferated,integrated with host liver,and differentiated into mature hepatocytes.Conclusions Human embryonic stem cells xenotransplanted into mice can participated liver tissue regeneration to some extent,and differentiated into functional liver cells.
2.Clinical observation on effects of Xuebijing injection on inflammatory mediators of patients with severe acute pancreatitis

Objective To evaluate the effects of Xuebijing injection on inflammatory mediators of patients with severe acute pancreatitis. Methods Using double blind method, 80 cases with severe acute pancreatitis were randomly divided into two groups, with 40 cases in each group. The control group was treated with conventional therapy such as: fasting, fluid replacement, acid inhibition and anti-infection, combined with the continuing injection with the growth hormone release inhibiting 6 mg, ulinastatin 200000u for 7 days. Based on the control group's treatment, the other group was added with Xuebijing injection 50 ml, twice a day for 7days. The plasma levels of prostaglandin Ⅰ2 (PGI2), thromboxane A2 (TXA2), tumor necrosis factor α (TNFα),interleukin-1,6,8 (IL-1,6,8) were compared before and after the treatment in each group and between two groups.Results Compared with the control group, Xuebijing injection group had marked effects on modulating the levels of inflammatory mediators. The levels of PGI2 and PGI2/TXA2 were increased significantly and the levels ofTXA2, TNF2, IL-1,6 and 8 were deceased(P<0.05)sharply after the treatment. Conclusion Combined with conventional western medicine therapy, Xuebijing injection has the effects of inhibiting and controlling the release of inflammatory mediators on patients with severe acute pancreatitis.
3.Postoperative complication and the risk factors of radical resection for colorectal cancer

Objective To investigate the associated risk factors of postoperative complications after rad?ical resecting of colorectal cancer. Methods The clinical data of 237 patients with colorectal cancer performed radical resection in the Second Affiliated Hospital of Kunming Medical University from January 2011 to Decem?ber 2014 were analyzed retrospectively. The incidence of postoperative complications was analyzed,and the relat?ed factors were analyzed by single factor and multi factor correlation analysis. Results The postoperative com?plications occurred in 114 cases of 237 patients( 48. 1%) . Univariate analysis showed that the age more than 70 years old,preoperative comorbidity,intraoperative blood loss,surgeon’ s experience,combined evisceration or en?larged evisceration were related to postoperative complications ( P = 0. 033, 0. 014, 0. 045, 0. 028, 0. 040 ) . Compared with the patients without complications,the postoperative anal exhaust time,length of stay was longer of the patients with complications((3. 7±1. 6) d vs. (3. 2±1. 4) d,P=0. 035;(21. 3±6. 5) d vs. (12. 1 ±2. 4) d,P=0. 001). Logistic regression analysis showed that depth of tumor invasion(P=0. 001),preoperative comorbidity including NRS>3 points (χ2 =8. 903, P=0. 003 ) , anemia and hypoproteinemia (χ2 =3. 494, P=0. 048) ,hypertension complicated with type 2 diabetes mellitus(χ2=5. 418,P=0. 026) ,not complete intesti?nal obstruction(χ2=8. 376,P=0. 003),mild and severe ventilation dysfunction(χ2=6. 331,P=0. 011) and WBC<3. 9×109(χ2=4. 256,P=0. 041) were closely related to the occurrence of complications. The rate of post?operative complication of laparoscopic group and open group had no significant difference( 44. 4%( 64/144) vs. 47. 3%(44/93),P>0. 05) . Conclusion Age>70 years old,preoperative comorbidity,intraoperative blood loss,surgeon`s experience,combined evisceration or enlarged evisceration are main risk factors for patients per?formed colorectal cancer radical resection,but laparoscopy?assisted radical resection is not the risk factor.
4.Application of Echocardiography for Occluding the Multiple Seconded Atrial Septal Defects in Relevant Patients

Objective: To evaluate the effect of echocardiography for occluding the multiple seconded atria1 septal defects ( MASD) in relevant patients.
Methods: A total of 42 MASD patients were selected by transthoracic echocardiography (TTE) or transesophageal echocardiography (TEE). The transcatheter closure was monitored by TTE and X-ray, the effect of treatment was evaluated by TTE, ECG and chest X-ray at 24 hours and 1, 3, 6 months after the procedure. There were 38 patients with 2 defects, 3 with many central small defects and 1 with 3 defects. The patients included 17 male and 25 female with the mean age of (32.6 ± 13.4) years.
Results: The size and distance of the defects were measured with 4 cardiac chambers cross sectional study. The mean diameter of bigger defect from up to down was (9-27) mm, with the mean of (20.7 ± 6.1) mm, from front to back was (8-23) mm, with the mean of (16.9 ± 5.7) mm;the mean diameter of small defect from up to down was (3-12) mm, with the mean of (6.8 ± 3.6) mm. The length of the defect interva1 was (4-18) mm, with the mean of (7.9 ± 2.6) mm. There were 4 patients with 2 defect interval at 8.7 mm who received 2 occluders without post-operative lfow shunt. The rest 38 patients received 1 Amplatzer occluder which was (14-36) mm with the mean of (25.1 ± 6.4) mm which was (6.9 ± 1.7) mm larger than echocardiographic prediction. The success rate of implantation was 100%. There were 17 (40.5%) patients with post-operative lfow shunt. The patients were followed-up for (6-36) months, with the mean of 18.7 months, there were 11 (26.2%) patients with small amount of lfow shunt at 3 months after the operation.
Conclusion: Echocardiography is important for choosing the type and size of occluder devices, in-operative monitoring and post-operative effect evaluation for treating the ASD patients.

OBJECTIVETo study the periodontal health status of Hui and Han adults in Ningxia Hui autonomous region, thus to provide scientific basis for the establishment of oral health care policies in this region.
METHODS2 628 adults in Ningxia Hui autonomous region were investigated in this study according to the criterion issued by World Health Organization on the basic methods of oral health investigation and China oral health third epidemiological survey protocol. The inspection item included gingival bleeding (bleeding on probing, BOP), dental calculus, probing depth (PD) and attachment loss (AL). SPSS 15.0 statistical software was applied to analyze the results.
RESULTS1) The positive rate of BOP sites, detection rate of calculus and PD were peaked in 36-45 and 46-59 years old, but in the > or = 60 age group rather lower. The prevalence of periodontitis and the percentage of AL > or = 4 mm sites were increased with the adults' age. 2) In addition to the percentage of AL > or = 4 mm sites, the differences of the other indicators between the genders were not statistically significant. 3) The positive rate of BOP sites, detection rate of calculus, PD, the prevalence of periodontitis and the percentage of AL > or = 4 mm sites were higher for the mountain areas than the plain areas. 4) Hui population's positive rate of BOP sites, detection rate of calculus, PD, the prevalence of periodontitis and the percentage of AL > or = 4 mm sites were significantly lower than the Han population in Ningxia.
CONCLUSIONThe periodontal health status of adults in Ningxia Hui autonomous region is associated with age, gender, nationality and region.

The formulae decoctions are commonly used in clinic and have characteristics of simple preparation and rapid effect. It was considered as a complex physical and chemical system. In this article, the physical and chemical properties and chemical components and their relationships were investigated to explore the changes of characteristics resulted from different combinations of drugs of Shaofu Zhuyu decoction. The method of partial leasts-quares regression was adopted to evaluate the correlation between physical and chemical parameters and chemical components. These researches may provide a new way to investigate the state of bioactive components, the action mechanisms, and property of treating diseases of traditional Chinese medicine decoction.

8.Bio-activity evaluation of Qinlian Siwu decoction on inhibiting mice uterine contraction in vitro and its components analysis.

OBJECTIVETo evaluate the bio-activity of Qinlian Siwu decoction on in vitro uterus contraction model and exploit the relationship between chemical components and the bio-activity.
METHODThe samples were prepared by macroporous adsorptive resins. The in vitro uterus contraction model was adopted to appraise the bio-activities of Qinlian Siwu decoction and its different separated fractions. HPLC-DAD- ESI -MS method was applied to analyze and identify the components in the fraction QL-3.
RESULTIt was found that five active fractions (QL-1, QL-3, QL-5, QL-7 and QL-11) were separated from Qinlian Siwu decoction, mainly contributed to the observed antagonismto the contraction of the mouse uterus. 28 compounds in the fraction QL-3 were identified as malic acid, gallic acid, catalpol, protocatechuic acid, aucubin, chuanxiongzine hydrochloridum, vanillic acid, caffeic acid, paeoniflorin, berberastine, albiflorin, tetrahydropalmatine, coptisine, jatrorrhizine, leonuride, worenine, ferulic acid, palmatine, berberine, scutellarin, baicalin-7-0-glucoside, baicalin, rehmannioside C, wogonoside, chrysin-7-glucuronide, ttetuin, baicalein, wogonin and oroxylin-A.
CONCLUSIONIn vitro inhibiting the contraction of the isolated mouse uterine of Qinlian Siwu decoction was mainly attributed to the fraction QL-1 and QL-3. The active fractions (QL-5, QL-7 and QL-11) were obtained from QL-3 on the macroporous adsorptive resins by the gradient elution using ethanol.

9.The concept and preliminary practice of precise flap surgery based on CTA

Objective:To introduce the concept and procedure of CTA guided precision flap surgery and report the preliminary clinical application.Methods:The theory of CTA guided precision flap surgery is: based on CTA high-throughput data, comprehensively evaluate the wound and potential donor sites, and select the best plan for wound repair. This article reviews 177 patients treated under the guidance of CTA guided precision flap surgery from August 2014 to December 2018. Among them, 131 were males and 46 were females; they were 7-72 years old, with an average age of 39.5 years. The tissue defects were: 74 cases of foot and ankle, 64 cases of wrist, 15 cases of calf, 11 cases of forearm and upper arm, 7 cases of thigh and perineum, 5 cases of face and neck, and 1 case of abdomen. The size of the defect was 2 cm×1 cm to 65 cm×50 cm. The recipient and donor sites were assessed based on CTA guided precision flap surgery, then choose the optimal flap to repair the defect. The flap types, outcomes and complications were recorded. The visual analogue scale (VAS), British Medical Research Council sensory rating scale, and revascularization assessments of digital replantation standard by Hand Surgery Branch of Chinese Medical Association was used to assess clinical results.Results:All flaps were uneventfully harvested as planned according to the CTA guided precision flap surgery concept.A total number of 181 flaps were harvested, including 125 free flaps and 56 pedicled flaps. The flap area rangedfrom 4 cm×3 cm to 40 cm×8 cm. After surgery, 165 cases were survival completely, 7 cases were total necrosis, 9 cases were partial necrosis. A total of 174 cases of flaps survived. The mean follow-up was 9.2 months (range, 6-60 months). After surgery 53 flaps were bulky, and 7 cases with other complications.At the last follow-up, only 6 patients had mild pain, the VAS score was 1-3. The sensation of the flaps recovered partially. According to the British Medical Research Council sensory rating scale: 68 cases were grade S2 and 106 cases were grade S3. According to the revascularization assessments of digital replantation standard by Hand Surgery Branch of Chinese Medical Association, 171 cases were excellent and 3 cases were good.Conclusion:The concept of precision flap surgery based on CTA can effectively guide flap surgery and achieve precise wound repair.
10.Digital reconstruction technology in assisted design of lobulated deep inferior epigastric perforator flap for reconstruction of large defect in lower extremity

Objective:To investigate the efficacy of digital reconstruction technology in assisted design of lobulated deep inferior epigastric perforator flap (DIEPF) for surgical reconstruction of large defect in lower extremity.Methods:From January 2017 to January 2022, a study was carried out to retrospectively analyse 8 patients who had massive soft tissue defects in lower extremities were admitted in the Department of Orthopaedic Surgery of the 920 Hospital of Joint Logistic Support Force of Chinese PLA. The size of defects varied from 16.0 cm×12.0 cm-28.0 cm×22.0 cm. CTA scans were performed over abdominal aorta and the arteries of lower extremities. Three-dimensional model of DIEPF and vascular pedicles were reconstructed by Mimics software. According to the shape and size of the wound, targeted perforators were determined on the 3D images, and precisely mapped in a digitised rectangular coordinate system. The lobulated flap was then digitally designed in the 3D coordinate system. Flaps were harvested according to preoperative digital designs for the reconstruction of large defects in the lower extremities. The donor site was sutured directly. The flaps and recovery of lower extremities were observed though postoperative follow-ups and were conducted through visits of outpatient clinics and distance interviews via telephone and WeChat. Recoveries of lower extremities were evaluated using Maryland ankle-foot function scoring system.Results:The 3D reconstructed models of the vessels in donor sites were successfully completed for all patients. The harvests of lobulated DIEPF were successfully guided by the digital designs. Fifteen lobes of lobulated DIEPF survived successfully in all 8 patients. All donor sites were closed in the stage-I. Necrosis occurred at the distal tip of a lobulated flap due to a local venous occlusion, and healed after debridement and re-suture. Four patients received further flap debulking surgery. Time of postoperative follow-ups had ranged 15-27 months, with an average of 20 months. At the last follow-up, all the flaps had satisfactory appearance with linear scars at the donor sites. All fractures were healed. Five patients achieved Maryland's ankle-foot function score in excellent, 2 in good, and 1 was acceptable.Conclusion:Digital reconstruction technology can accurately map the perforators and reasonably assist the design of lobulated flaps. A lobulated DIEPF offers a wider area for flap excision and allows a primary closure of the donor sites. Combination of the 2 advantages of a lobulated DIEPF can be effectively applied in reconstruction of a large-sized defect in lower extremity.
