1.MR Diagnosis and Clinical Analysis of Moyamoya Disease

Objective To probe MRI diagnostic characteristics of moyamoya disease(MMD). Methods Both MRI and MRA findings in 17 cases, aged 10~49 years, with MMD were analyzed retrospectively. Axial T_1WI and T_2WI were performed for MRI, while 3D TOF was used for MRA.Gd-DTPA enhanced MRA and T_1WI were carried out in 2 and 4 patients respectively.FLAIR was performed in 5 cases.Results ( 1 ) Multiple focal infarction was noted in 5 cases , old hemorrhage in 4 cases, dilatation of unilateral ventricle with history of ventricle hemorrhage was displayed in 2 cases;(2)MRI showed signal void of the following vessels as well as other lesions:① internal carotid artery (ICA) bifurcation, anterior cerebral artery (ACA), middle cerebral artery (MCA) and posterior cerebral artery (PCA) (17/17);②collateral circulation from the skull base, PCA and ECA were demonstrated well in combination with MRA,T_1WI and T_2WI (17/17);③“Ivy sign” or leptomeningeal high signal intensity could be demonstrated on contrast-enhanced T_1WI or FLAIR images(2/17). Conclusion MRI findings can suggest the diagnosis of the MMD, and definite diagnosis can be usually made if combined with MRA.
2.The diagnosis and treatment of adrenal pheochromocytoma and adrenal ganglioneucoma

Objective To evaluate the clinical characteristics, diagnosis and treatment procedures for adrenal pheochromocytoma and adrenal ganglioneucoma.Methods Thirty-two cases of adrenal pheochromocytoma and 14 cases of adrenal ganglioneucoma surgical treated between 1999—2005 were analysed, The average of ganglioneucoma was 39 years old (16~63). tumor size ranged from 2.5 cm to 15 cm. The mean age of pheochromocytoma was 42 years old (19~74), tumor size ranged from 2cm to 16 cm. Tumor located in the adrenal area. All cases accepted surgical operation and were confirmed by pathology. Results Most cases of adrenal ganglioneucoma did not have any symptom but were founded incidentally. Differentials were obviously between ganglioneucoma and pheochromocytoma. Conclusion Both adrenal ganglioneucoma and pheochromocytoma are rare diseases.CT、urinary CA and 131I-MIBG are good methods for diagnosis.The final diagnosis depends on the pathology, surgical resection of tumor is a preferred choice.
3.Clinical Observation of Bifidobacterium Triple Viable Capsules in the Adjunctive Treatment of Hp Positive Chronic Atrophic Gastritis Complicated with Anxiety-depression

OBJECTIVE:To observe clinical efficacy and safety of Bifidobacterium triple viable capsules in the adjunctive treatment of Helicobacter pylori(Hp)positive chronic atrophic gastritis(CAG)complicated with anxiety-depression. METHODS:A total of 100 Hp positive CAG patients with anxiety-depression were divided into control group and observation group according to random number table,with 50 cases in each group. Control group was given standard triple therapy (rabeprazole+amoxicillin and clavulanate+levofloxacin). Observation group was additionally given Bifidobacterium triple viable capsules 0.42 g,tid. The treatment lasted for 14 d in both groups. Clinical efficacies,Hp eradication rates as well as HAMA and HAMD scores before and after treatment were all observed in 2 group. The occurrence of ADR was compared. RESULTS:Total response rate of observation group was 94.0%,and Hp eradication rate was 92.0%,which were significantly higher than 76.0% and 78.0% of control group, with statistical significance (P<0.05). Before treatment,there was no statistical significance in HAMA and HAMD scores in 2 groups(P>0.05). After treatment,HAMA and HAMD scores of 2 groups were decreased significantly,the observation group was significantly lower than the control group,with statistical significance (P<0.05). The incidence of ADR in observation group (4.0%)was significantly lower than control group(20.0%),with statistical significance(P<0.05). CONCLUSIONS:Adjunctive use of Bifidobacterium triple viable capsules can significantly improve Hp eradication rate,clinical symptom,anxiety and depres-sion,while reduce the incidence of ADR.
6.Effects of different doses of dexmedetomidine on atrioventricular node conduction function in healthy volunteers

Objective To evaluate the effects of different doses of dexmedetomidine on atrioventricular node (AVN) conduction function in the healthy volunteers.Methods Sixteen healthy volunteers of both sexes,aged 18-30 yr,with body mass index of 19-26 kg/m2,were included in the study.Dexmedetomidine was infused in a loading dose of 1.0 μg/kg over 10 min,followed by an infusion of 0.5 μg · kg-1 · h 1 for 50 min (Dose Ⅰ);1-2 weeks later,dexmedetomidine was infused in a loading dose of 1.5 μg/kg over 10 min,followed by an infusion of 0.75 μg · kg-1 · h-1 for 50 min (Dose Ⅱ).Before infusion of dexmedetomidine (T0) and at 15 and 35 min of infusion (T1.2),AVN Wenckebach point,AVN 2 ∶ 1 block point,AVN relative refractory period (AVNRRP),and AVN effective refractory period (AVNERP) were measured.Results AVN Wenckebach point and AVN 2 ∶ 1 block point were significantly decreased,and AVNRRP and AVNERP were significantly prolonged at T1,2 compared with those at T0 (P<0.05).Compared with Dose Ⅰ,AVN Wenckebach point at T2 and AVN 2 ∶ 1 block point at T1,2 were significantly decreased,and AVNRRP and AVNERP were significantly prolonged at T1,2 in the subjects receiving Dose Ⅱ] (P<0.05).Conclusion Dexmedetomidine can inhibit AVN conduction function in the healthy volunteers,and the inhibitory effect is enhanced with the increasing doses.
7.The correlation between the soluble triggering receptor expressed on myeloid cells-1 and the janus kinase/signal transducer and activator of transcription (JAK/STAT) pathway in rats with abdominal sepsis

Objective To investigate the correlation between the soluble triggering receptor expressed on myeloid cells-1 (sTREM-1) and the janus kinase/signal transducer and activator of transcription (JAK/STAT) pathway in rats with abdominal sepsis.Methods Using a sepsis model produced by cecal ligation and puncture (CLP),Wistar rats were randomly (random number) divided into normal control group (n =6),sham-operated group (n =24,) CLP group (n =48),AG490 (JAK2 inhibitor) treatment group (n =48) and rapamycin (RPM,STAT3 inhibitor) treatment group (n =48).At different intervals,the rats in each group were sacrificed,then the peripheral blood cells were harvested to detect the percent of CD4 + CD25 + Treg cells in CD4 + cells by flow cytometry.Meanwhile,mRNA of sTREM-1 from the tissue samples of the liver was also measured by real-time reverse transcription-polymerase chain reaction (RT-PCR).Results Compared with the normal control group and the sham-operated group,expression of sTREM-1 in the CLP group was significantly higher,and it showed a gradually increased tendency over time.At 6 and 24 hours,there were no significant differences in the expressions of sTREM-1 between the AG490 group (1.572 ± 0.051,2.063 ± 0.025,respectively) and the CLP group (1.592 ±0.036,2.082 ±0.021,respectively).But the expression of sTREM-lin the AG490 group (2.522 ± 0.083,3.153 ± 0.021,respectively) was lower than those in the CLP group (2.592 ±0.055,3.204 ±0.013,respectively) during 48 and 72 hours.In addition,there was no significant difference in the expression of sTREM-1 between the RPM group (1.581 ± 0.017) and the CLP group (1.592 ±0.036) at 6 hours.And the expression of sTREM-1in the RPM group (1.486 ±0.019,1.263 0.011,1.115 ± 0.022,respectively) was significantly lower than those in the CLP group (2.082 ± 0.021,2.592 ±0.055,3.204 ± 0.013,respectively) during 24 to 72 hours.Conclusions These data proved that blocking JAK/STAT pathway could inhibit the expression of sTREM-1 and decelarate the progress of inflammatory reaction in rats with abdominal sepsis.
8.Three-dimensional reconstruction visualization system enhances the accuracy of lower cervical pedicle screw implantation

BACKGROUND:Pedicle screw implantation is a common method to repair many kinds of diseases of the lower cervical spine. Three-dimensional (3D) reconstruction visualization system can be used in order to improve the accuracy of the implant and improve the prognosis.
OBJECTIVE:To investigate the effect of 3D reconstruction visualization system on the accuracy of cervical pedicle screw implantation.
METHODS: The clinical data of 89 patients with cervical spine dislocation, who underwent cervical pedicle screw implantation, were analyzed retrospectively. Patients were divided into control group (46 cases) and observation group (43 cases) according to the navigation method. Patients in the control group underwent C arm X ray two dimensional navigation. Patients in the observation group underwent three-dimensional reconstruction visualization system navigation. Intraoperative placement time was observed, and the accuracy of screw placement was assessed in both groups. The cases were folowed up for 12 months, and the adverse events were recorded and compared.
RESULTS AND CONCLUSION: (1) Intraoperative implantation time was shorter in the observation group than in the control group, but no significant difference was found (P> 0.05). (2) Accuracy rate: There were no three types of nail cases in the observation group, and the accuracy rate was 93% (40/43). In the control group, two cases affected three types of nailing, and the accuracy rate was 81% (37/46). The accuracy rate was significantly higher in the observation group than in the control group (P < 0.05). (3) Postoperative folow-up: None suffered from nerve tissue injury to vertebral artery, blood vessel and spinal cord. The screws of the two groups were in stable condition, without screw breakage or loosening. (4) Results suggested that in lower cervical pedicle nail implantation, 3D reconstruction navigation system can effectively improve the accuracy of screw placement, and does not increase nailing time or lead to adverse events. It is a safe and effective navigation mode.
9.Effect of insulin-like growth factor binding protein-related protein 1 in the formation and development of hepatic fibrosis

Objective To investigate the effect of insulin-like growth factor binding protein-related protein 1 ( IGFBPrP1 ) in the formation and development of hepatic fibrosis. Methods Hepatic fibrosis model of mice was made by intraperitoneal injecting with thioacetamide, then the mice were sacrificed four, five and six weeks later (10 mice were sacrificed at each time point, model groups). Mice in the control groups were treated by normal saline (6 mice were sacrificed at each time point). Collagen accumulation in liver tissues was detected by Masson stain. Distribution and dynamic expressions of IGFBPrP1, alpha-smooth muscle actin ( α-SMA ),Collagen Ⅰ , fibronectin ( FN), TGF-β1 and Smad3 in different groups were detected by immunohistochemistry.The expressions of IGFBPrP1, α-SMA and Smad3 were detected by Western blot. All data were analyzed using the analysis of variance (ANOVA), Pearson rank correlation coefficient. Results The expressions of IGFBPrP1 in the liver tissues were increased from 0.21 ±0.03 to 5.03 ±0.09, α-SMA from 0. 11 ±0.04 to 10.09 ±0. 18,Collagen Ⅰ from 0.22 ±0.01 to 11.01 ±0. 16, FN from 0.31 ±0.09 to 19.81 ±1.62, TGF-β1 from 0.49 ±0.02 to 5.97 ± 0. 19, and Smad3 from 0.22 ± 0.03 to 2.03 ± 0.07. Compared with the control groups, the expressions of IGFBPrP1, α-SMA, Collagen Ⅰ , FN, TGF-β1 and Smad3 in the model groups were significantly increased as time passed by ( F = 783. 141,998. 200,886. 715,935. 242, 931. 241,697. 118, P < 0. 05 ). During the formation of hepatic fibrosis, the expression of IGFBPrP1 was positively correlated with the expressions of α-SMA,Collagen Ⅰ , FN, TGF-β1 and Smad3 ( r = 0. 906, 0. 927, 0. 988, 0. 947, 0. 977, P < 0.05 ). The results of Western blot showed that the protein expression of IGFBPrP1 was increased from 0. 23 ± 0.01 to 0.92 ± 0.07,α-SMA from 0.36 ± 0. 02 to 1.39 ± 0.03, FN from 0.03 ± 0.00 to 0.12 ± 0.02, and Smad3 from 0.09 ± 0. 01 to 0.56 ±0.04. The protein expressions of IGFBPrP1, α-SMA, FN and Smad3 were significantly increased in the model groups when compared with the control groups (F =57. 316, 201. 214, 103. 871, 72. 966, P <0.05).During the formation of hepatic fibrosis. IGFBPrP1 was positively correlated with the expressions of α-SMA, FN and Smad3 (r = 0. 982, 0. 924, 0. 965, P < 0.05 ). Conclusions The expression of IGFBPrP1 increases as the aggravation of the fibrosis. IGFBPrP1 promotes the formation and development of hepatic fibrosis by activating hepatic stellate cells, accelerating the synthesis and secretion of Collagen Ⅰ and FN which are the principal components of extracellular matrix, and affecting the TGF-β1/Smad3 pathway.
10.Purity Determination and Uncertainty Evaluation of Ribavirin Purity Reference Material

Objective To develop the ribavirin purity certified reference material( CRM ) which has measurement traceability and high accuracy,and establish an effective method of evaluation. Methods The homogeneity and the stability were checked by differential scanning calorimetry(DSC). High performance liquid chromatography(HPLC)and DSC methods were used for purity determination of ribavirin. Results Ribavirin showed satisfactory homogeneity and stability. The certified value of ribavirin was 99. 5%with an uncertainty of 0. 4%(k=2,P=0. 95). Conclusion Ribavirin purity CRM obtained in this paper has been proven to be a national primary CRM with high accuracy and traceability,which can be used to validate analytical methods,improve the accuracy of measurement data,establish meteorological traceability of analytical results as well as control the quality of ribavirin in the pharmaceutical industry.
