1.Social psychological factors causes of acute exacerbation or re-decompensation of clinically significant chronic tinnitus.

OBJECTIVE:
To analyze social-psychological causes of acute exacerbation or re-decompensation of chronic tinnitus and provide theoretical basis for controlling and preventing tinnitus exacerbation and re-decompensation.
METHOD:
Systemic audiological tests and tinnitus handicap inventory were performed on 136 chronic tinnitus patients with acuteexacerbation or re-decompensation. For the patients with new hearing loss, a further investigation of living conditions and assessment of social support rating scale were utilized. The patients with relatively definite causes were treated accordingly.
RESULT:
(1) There were 89 patients complained of new changes of hearing, all of whom could tell the definite time point of tinnitus exacerbation, and 5 of them felt the exacerbation of hearing loss meanwhile. (2) Forty-two patients encountered adverse events on life or working, and tinnitus exacerbation occurred within several weeks to 3 months afterwards. Most of these patients could not tell the definite time point of tinnitus exacerbation or re-decompensation. Five cases of tinnitus exacerbation didn't tell any adverse events on life or working, but showed mood disorders, and the anti-anxiety treatment was effective to them. (3) Forty-seven cases without new hearing loss scored significantly lower in SSRS than healthy adults.
CONCLUSION
Emerging hearing loss is the main cause of acute exacerbation of chronic tinnitus. To find it in time and give effective treatment can save newly presented hearing loss, cure or relieve tinnitus. Adverse events in life(or working) and short of social support is another important cause of acute exacerbation of chronic tinnitus or decompensation recurrence, which suggests that social-psychological factors besides of hearing loss should be concerned in diagnosis and treatment of tinnitus.

2.Effects of propranolol on in vitro cultured hemangioma endothelial cells and their mechanisms

Objective To evaluate effects of propranolol on the proliferation and apoptosis of in vitro cultured hemangioma endothelial cells (HemEC),and to explore their molecular mechanisms.Methods Hemangioma tissues were resected from 7 children with proliferative hemangioma,and used for in vitro culture of HemEC.Meanwhile,cultured human umbilical vein endothelial cells (HUVEC) served as controls.The 2 kinds of cells were treated with propranolol at different concentrations of 0,25,50,75,100,125 and 150 μmol/L for 24,48 and 72 hours separately.Methyl thiazolyl tetrazolium (MTT) assay was performed to evaluate cellular proliferative activity,and flow cytometry to determine the apoptosis rate.Some cultured HemEC were divided into 2 groups to be treated with 100 μmol/L propranolol-containing culture medium (propranolol group) and culture medium alone (blank control group),respectively,for 18 hours.Total RNA in the 2 groups was extracted separately.Differentially expressed genes in HemEC between the above 2 groups were identified by DNA microarray technology,and verified by real-time quantitative PCR.Results The treatment with 25 μmol/L propranolol for 24 and 48 hours caused a slight proliferation of HemEC (P < 0.05).The survival rate of HemEC was decreased after the treatment with propranolol at the concentration of ≥ 100 μmol/L for more than 24 hours,while the proliferation of HUVEC was inhibited by the treatment with propranolol at the concentration of ≥ 100 μ mol/L for more than 48 hours.During 24-72 hours of treatment with 100-150 μmol/L propranolol,the survival rates of HemEC were significantly lower than those of HUVEC (P < 0.05).After the treatment with 100-150 μmol/L propranolol,the apoptosis rate of HemEC gradually increased with the increase in treatment duration and concentrations of propranolol (all P < 0.05).Compared with the blank control group,186 differentially expressed genes (> 1.5-fold changes) were screened out by DNA microarray technology,including 128 upregulated genes and 58 down-regulated genes.Real-time quantitative PCR showed that the mRNA expression of proprotein convertase subtilisin/kexin type 9 (PCSK9) and fatty acid binding protein 3 (FABP3) in the propranolol group were (9.88 ± 2.19) and (21.90 ± 8.18) times that in the blank control group respectively (t =7.028,4.427 respectively,P < 0.05).Conclusions Propranolol at high concentrations can inhibit the proliferation of HemEC and HUVEC,and its inhibitory effect on HemEC is stronger than that on HUVEC.The inhibitory effect of propranolol on HemEC may be related to the inhibition of HemEC proliferation and promotion of HemEC apoptosis.
3.Analysis of psychological status and influencing factors of medical workers amid COVID-19 pandemic; analysis of influencing factors

Objective:The study seeks to explore the factors influencing the psychological status and sleep quality of medical workers amid the ongoing COVID-19 pandemic, in order to provide data sources and theoretical basis for the development of relevant psychological intervention programs.Methods:Employing the convenience sampling method, general information questionnaire (age, gender, marital status, educational background, job status, etc.), Generalized Anxiety Disorder-7 and Patient Health Questionnaire, epidemic stress index scale, and sleep quality questionnaire were distributed to medical staff between February 18 and April 3, 2020, using the PEM mental health care platform of by ZhongShengKaiXin for medical staff issued. Descriptive, single factor, and correlation analyses, as well as multiple linear regression analysis were used to analyze the data.Results:Overall, 24, 845 questionnaires were collected from 23 provinces, of which 24, 687 were valid, with a recovery rate of 99.36%. The findings showed that the proportion of medical personnel with symptoms of anxiety and depression was 50.58% and 51.37%, respectively; 16.11% had poor or very poor anti-stress ability; and 71.78% reported poor or very poor sleep quality. There was a positive correlation between anxiety, depression, anti-stress ability, and sleep quality ( P<0.05). Anxiety was positively correlated with depression, stress tolerance, and sleep quality( r=0.787, 0.667, and 0.486, all P<0.001); depression was positively correlated with stress tolerance and sleep quality ( r=0.709 and 0.586, both P<0.001); and stress tolerance was positively correlated with sleep quality ( r=0.452, P<0.001). Multiple linear regression analysis results showed that age, gender, marital status, educational background, professional title, job status, and participation influenced the anxiety levels of medical personnel in the backdrop of the pandemic ( P<0.001). Depression levels of medical staff were influenced by gender, educational background, job position, and participation ( P<0.001), while gender, marital status, educational background, job position, and participation influenced the stress tolerance levels ( P<0.001). The sleep quality of medical workers was influenced by age, gender, job position, participation in the fight against the pandemic, and professional title ( P<0.001). Conclusions:Amid the ongoing COVID-19 pandemic, medical staff reported poor mental health status and sleep quality, which can be attributed to diverse factors. The research findings can be useful for assisting medical staff to strengthen their self-cognition, while also providing certain psychological counseling data and theoretical basis for management departments.
4.The audiograms of 462 tinnitus victims who never perceived hearing loss.

OBJECTIVE:
To investigate the risk factors of tinnitus and provide evidence for tinnitus prevention.
METHOD:
Retrospectively analyzed the audiograms and the risk factors of 462 tinnitus victims who never perceived hearing loss, find the relationship between every risk factor and tinnitus.
RESULT:
The rate of tinnitus victims with high frequency hearing loss was the 46.1%, with low frequency hearing loss was 15.8%, with single middle frequency notch was 11.04%, with an audiogram like upset-basin was 7.58%, with normal audiogram was 14.07%, with other type of audiograms was 5.41%. The age distribution in the high frequency hearing loss group was mainly between 30 and 40 years, in the upset-basin hearing loss group was mainly older than 50 years, and in the other groups, mainly between 20 and 30 years. In the high frequency hearing loss group, male to female ratio was 158/55, in the low frequency hearing loss group and single middle frequency notch group ,male to female ratio was 14/59 and 12/39 respectively. In another three groups, the male to female ratio was nearly 1. Risk factors for high frequency hearing loss group, low frequency hearing loss, single middle frequency notch group and in the upset-basin hearing loss group were noise, fatigue, long-time musical instrument contact and ageing respectively.
CONCLUSION
Among the tinnitus victims who never perceived hearing loss, 86% of them have hearing impairment in high frequency area, low frequency area or single middle frequency. The rate of high frequency hearing loss was 46.01%, the major risk factor was noise, most of victims were men. The rate of low frequency hearing loss was 15.8%, the major risk factor was fatigue and nervous, most of the victims were women. The rate of single middle frequency notch was 11.04%, the major risk factor was long-time musical instrument contact. The rate of upset-basin hearing loss was 7.58%, ageing was the major risk factor.

5.Quality Analysis of 83 Batches of Compound Jiegeng Mahuangjian Syrup(Ⅱ)from 4 Manufacturers

OBJECTIVE:To analyze the results of national evaluation inspection for 83 batches of Compound jiegeng mahuangjian syrup(Ⅱ)from 4 manufacturers,and to evaluate their quality. METHODS:Legal inspection method and exploratory research were adopted to test,analyze and evaluate sample statistically. The contents of exploratory research included HPLC method was used to determine the contents of preservative benzoic acid and ephedrine hydrochloride;antibacterial efficacy examination of formulation was studied;pH value of solution was determined;the content of sucrose was determined by polarimetry photometry;the relationship of the content of ammonium chloride with microbial contamination was studied;headspace GC method was used to determine the contents of menthol and ethanol;Platycodon grandiflorum extractum was identified and studied by TLC. RESULTS:Results of legal inspection showed that among 83 batches of sample,80 batches were qualified and 3 batches were unqualified,with qualified rate of 96.4%. Unqualified items were loading capacity,microbial limit and content ofammonium chloride,content of ammonium chloride. The results of legal test combined with exploratory research showed thatantibacterial efficacy of formulation of one manufacturer was not in accordance with the regulations;Some raw materials were notfed according to formulation, and the quality of products was not even. It is recommended to revise the quality standard:identificatied the Campanulaceae by TLC; determination the pH, preservative content, menthol content; revision ephedrinehydrochloride assay method to HPLC. CONCLUSIONS:The overall quality of Compound jiegeng mahuangjian syrup(Ⅱ)is notsatisfactory;there is a big flaw in the production process and quality control of some manufacturers;quality standards needed to beimproved.
6.Five different vascular accesses on the outcome in peripheral blood stem cells collection: a network Meta-analysis

Objective:To analyze the effect of different vascular access on the outcome in peripheral blood stem cells collection by a network Meta-analysis, and to provide a reference for clinical medical staff to select the best vascular access.Methods:A systematic search was carried out in Chinese Knowledge Infrastructure database (CNKI), Wanfang Database, VIP Database, Chinese Biomedical Literature Database, Cochrane Library, Web of Science, PubMed, Embase, from inception until April 15, 2023. Two researchers independently screened literature and extracted data. Bayesian network meta-analysis was performed using R4.2.2 and Addis-1.16.6 softwares.Results:A total 7 pieces of research were included, 5 vascular access methods were peripheral artery, peripheral vein, artery-vein, femoral vein-central venous catheter (FV-CVC), and internal jugular vein-CVC (IJV-CVC). The results showed that compared with the peripheral veins, there was no significant difference on CD34 cells between other vascular accesses in the primary outcome measure when collected peripheral blood stem cell collection. On the single blood volume treatment time, peripheral vein and IJV-CVC were statistically significant ( MD = 14.80, 95% CI 2.70-22.38, P<0.05) . The SUCRA ranking probability chart showed that on CD34 cells, FV- CVC>IJV-CVC>artery-vein>peripheral artery>peripheral vein access; for a single blood volume treatment time and monocyte number, peripheral artery was superior and the next was peripheral vein. Conclusions:Current evidence suggested that Peripheral artery may be the best vascular access in peripheral blood stem cells collection, which opens a new way to establish the vascular access for peripheral blood stem cells collection, but it needs to be confirmed by large clinic trials.
7.Protective Effect of Huangqi Baihe Granules on Acute Brain Injury in Rats with High Altitude Hypoxia Based on HIF-1α/NF-κB/NLRP3 Signal Pathway

ObjectiveTo observe the effect of Huangqi Baihe granules on the hypoxia-inducible factor 1α (HIF-1α)/nuclear factor-κB (NF-κB)/NOD-like receptor hot protein domain related protein 3 (NLRP3) signaling pathway in a rat model of high altitude hypoxia. MethodSixty male SPF SD rats were randomly divided into blank group, model group, dexamethasone group (5 mg·kg-1), and high, middle, and low-dose groups of Huangqi Baihe granules (4.1, 2.05, 1.025 g·kg-1). Among them, each Chinese medicine group was administrated orally for continuously 14 d, once a day, and the dexamethasone group was injected intraperitoneally for continuously 3 d as the positive control group. On the 15th d, the model group, dexamethasone group, and high, middle, and low dose groups of Huangqi Baihe granules were exposed to the simulated high altitude, low pressure, and low oxygen environment in the animal low-pressure simulation cabin, and the exposure lasted for 3 d. Blood was collected from the abdominal aorta and serum was separated, and the brain tissue was taken after being killed. Hematoxylin-eosin (HE) staining was used to observe the pathological changes in brain tissue. Enzyme-linked immunosorbent assay (ELISA) was used to detect the content of tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), and interleukin-1β (IL-1β) in rat serum. Western blot was used to detect HIF-1α, NLRP3, phosphorylated nuclear factor-κB (p-NF-κB), NF-κB, desquamation D (GSDMD), and cysteine aspartate-specitis protein-1(Caspase-1) in rats of each group. The mRNA expression levels of HIF-1α, NLRP3, NF-κB p65, GSDMD, and Caspase-1 were detected by real-time quantitative polymerase chain reaction (Real-time PCR). ResultThe results of HE staining showed that as compared with the normal group, the pathological sections of brain tissues in the model group showed that pyramidal cells were loosely arranged and distributed in disorder, with different sizes. Compared with the model group, the pathological changes in pyramidal cells in the dexamethasone group and high and middle-dose groups of Huangqi Baihe granules were reduced. The results of ELISA showed that as compared with the normal group, the content of TNF-α, IL-6, and IL-1β in the serum of rats in the model group was significantly higher (P<0.01). Compared with the model group, the content of TNF-α, IL-6, and IL-1β in the serum of rats in the dexamethasone group and high and middle-dose groups of Huangqi Baihe granules decreased significantly (P<0.05, P<0.01). The results of Western blot showed that as compared with the normal group, the relative protein expression levels of HIF-1α, NLRP3, p-NF-κB p65, GSDMD, and Caspase-1 in the brain tissue of the model group were significantly higher (P<0.01). As compared with the model group, the relative expressions of HIF-1α, NLRP3, p-NF-κB p65, GSDMD, and Caspase-1 in the brain tissue of rats in the dexamethasone group and the high-dose group of Huangqi Baihe granules were significantly decreased (P<0.05, P<0.01). The relative protein expression levels of HIF-1α, NLRP3, p-NF-κB p65, and Caspase-1 in the brain tissue of rats in the middle-dose group of Huangqi Baihe granules decreased significantly (P<0.01), and the relative protein expression of HIF-1α in the brain tissue of rats in the low-dose group of Huangqi Baihe granules was reduced (P<0.05). The Real-time PCR analysis showed that as compared with the normal group, the mRNA expression levels of HIF-1α, NLRP3, NF-κB p65, GSDMD, and Caspase-1 in the brain tissue of the model group were significantly increased (P<0.01). As compared with the model group, the mRNA expression levels of HIF-1α, NLRP3, NF-κB p65, GSDMD, and Caspase-1 in the brain tissue of rats in the dexamethasone group were significantly decreased (P<0.01). The mRNA expression levels of HIF-1α, NF-κB p65, GSDMD, and Caspase-1 in the brain tissue of rats in the high-dose group of Huangqi Baihe granules decreased significantly (P<0.01). The mRNA expression levels of HIF-1α, NLRP3, and Caspase-1in the brain tissue of rats in the middle-dose group of Huangqi granules decreased (P<0.05, P<0.01). ConclusionThe protective effect of Huangqi Baihe granules on acute brain injury in low-pressure hypoxic rats may be related to the HIF-1α/NF-κB/NLRP3 signaling pathway.
8.Guiqi Baizhu Prescription Combined with Oxaliplatin Protects Intestinal Barrier of Tumor-bearing Mice with Gastric Cancer by Regulation of VIP/cAMP/PKA/AQP Signaling Pathway

ObjectiveTo investigate the protective effect of Guiqi Baizhu prescription combined with oxaliplatin on the intestinal barrier of tumor-bearing mice with gastric cancer by regulating downstream aquaporin 3 (AQP3) and aquaporin 4 (AQP4) through the vasoactive intestinal peptide (VIP)/cyclic adenosine monophosphate (cAMP)/protein kinase A (PKA) signaling pathway. MethodThe gastric cancer cell lines MFC with a density of 1×107/mL were prepared into cell suspension. The tumor-bearing mouse model of gastric cancer was established by inoculating 0.2 mL cell suspension under the right axilla of mice. After successful modeling, mice were randomly divided into 5 groups, namely, model group, oxaliplatin group (10 mg·kg-1), and high, medium, and low-dose oxaliplatin + Guiqi Baizhu prescription groups (17.68, 8.84, 4.42 g·kg-1), with 10 mice in each group, and the remaining 10 mice were set as a blank group. Mice in each group were treated with Chinese medicine, oxaliplatin, or normal saline by gavage or intraperitoneal injection for 14 d. The next day after the last dose, blood was taken from the eyeball to separate serum and take colonic samples. Hematoxylin-eosin (HE) staining was used to observe the changes in tissue morphology. The content of D-lactate acid (D-LA) and diamine oxidase (DAO) in the serum was determined by enzyme-linked immunosorbent assay (ELISA). The mRNA and protein expressions of VIP, cAMP, PKA, AQP3, and AQP4 were detected by Real-time quantitative polymerase chain reaction (Real-time PCR) and Western blot, respectively. ResultCompared with the blank group, the model group showed edema in the colonic submucosa, disordered arrangement of intestinal glands in the mucosal layer, loss of goblet cells, infiltration of inflammatory cells, and villus shedding. However, there were different degrees of improvement in each administration group. As compared with the blank group, the serum levels of DAO and D-LA in the model group were significantly increased (P<0.01). As compared with the model group, the levels of DAO and D-LA in the high-dose oxaliplatin + Guiqi Baizhu prescription group and the level of D-LA in the medium-dose oxaliplatin + Guiqi Baizhu prescription group were decreased (P<0.05, P<0.01). As compared with the oxaliplatin group, the levels of D-LA in the high and medium-dose oxaliplatin + Guiqi Baizhu prescription groups were decreased (P<0.05), and the levels of DAO and D-LA in other administration groups were decreased as well, but the difference had no statistical significance. As compared with the blank group, the mRNA and protein expression levels of VIP, cAMP, PKA, AQP3, and AQP4 in the model group were significantly decreased (P<0.05, P<0.01). As compared with the model group, the mRNA and protein expression levels of VIP, cAMP, PKA, AQP3, and AQP4 in each administration group were increased, and those in the high-dose oxaliplatin + Guiqi Baizhu prescription group were significantly increased (P<0.05, P<0.01), while the protein expression level of cAMP in the medium-dose oxaliplatin + Guiqi Baizhu prescription group were increased (P<0.05). As compared with the oxaliplatin group, the protein expression levels of cAMP in the high-dose oxaliplatin + Guiqi Baizhu prescription group were increased (P<0.05), and the mRNA and protein expressions of these indexes in the other groups were also increased but the differences were not statistically significant. ConclusionGuiqi Baizhu prescription combined with oxaliplatin can regulate AQP3 and AQP4 through the VIP/cAMP/PKA signaling pathway to protect the intestinal barrier of tumor-bearing mice with gastric cancer.
