1.Therapeutic effect of Weidong Kang on diabetic gastroparesis

BACKGROUND: Modern medicine has not completely clarified the pathogenesis of diabetic gastroparesis, and Weidong Kang may have certain efficacy.OBJECTIVE: To observe the clinical efficacy and safety of Weidong Kang on diabetic gastroparesis, and compare with the efficacy of Motilium.DESIGN: A case-controlled observation.SETTINGS: Department of Traditional Chinese Medicine, Affiliated Hospital of Chongqing Normal University; Chongqing Institute of Traditional Chinese Medicine.PARTICIPANTS: Sixty outpatients and inpatients with diabetic gastroparesis were selected from the Affiliated Hospital of Chongqing Normal University and Chongqing Institute of Traditional Chinese Medicine from April 2001 to December 2005. They were randomly divided into two groups: ① treatment group (n =30): 20 males and 10 females; 31-58 years of age, the disease courses ranged from 0.5 to 26 years; ② control group (n =30): 18 males and 12 females; 30-69 years of age, the disease courses ranged from 0.5 to 20 years. Informed contents were obtained from all the subjects.METHODS: In the treatment group, the patients were treated with oral Weidong Kang decoction, three times a day,30 mL per time; Weidong Kang decoction consisted of official magnolia bark, immature bitter orange, betel nut, rhubarb,Chinese thorowax root, largehead atractylodes rhizome, dendrobium stem, 100 mL/bottle, containing 100 mg crude drug,provided by the Department of Pharmacy, Chongqing Institute of Traditional Chinese Medicine). In the control group, the patients with treated with Motilium, three times a day, 10 mg per time. All the patients were treated for four weeks as a course. The scores of dyspeptic symptoms and the time of gastric semi-emptying of liquid (T/2) were observed, the efficacy, the score of quality of life (QOL) and safety were evaluated before and after treatment.MAIN OUTCOME MEASURES: Scores of dyspeptic symptoms; Time of gastric semi-emptying of liquid (T/2); Evaluation of efficacy; score of QOL; Safety.RESULTS: All the 60 patients with diabetic gastroparesis were involved in the analysis of results. ① Amelioration of dyspeptic symptoms: After treatment, the scores of dyspeptic symptoms an total score were all obviously lower than those before treatment in both groups (P < 0.01), the scores of epigastric satiety, nausea and vomiting, decreased appetite and epigastric pain in the treatment group were all obviously lower than those in the control group (P < 0.05-0.01). ② Clinical efficacy: The total effective rate in the treatment group was obviously higher than that in the control group [90.0% (27/30); 73.3% (22/30), P < 0.05]. ③ Time of gastric semi-emptying of liquid (T/2): In the treatment group,it was obviously ameliorated after treatment as compared with that before treatment (P < 0.01). ④ Amelioration of QOL by symptom frequency-36 health survey (SF-36): In the treatment group, the scores of general health, somatic pain,vigor and mental health after treatment were obviously higher than those before treatment (P < 0.05). In the control group, the scores of general health and mental health after treatment were obviously higher than those before treatment (P < 0.05). ⑤ Evaluation of safety: In the treatment, there were no obvious differences in the blood and urine routine examinations, liver function and kidney function before and after treatment (P > 0.05).CONCLUSION: Weidong Kang is an effective and safe drug for alleviating dyspeptic symptoms in patients with diabetic gastroparesis.
2.Acupuncture Treatment of 7 Cases of Tic of Limbs Caused by an Intravenous Drip of Penicillin Combined with Kfloxcin

Seven cases of tic of limbs caused by an intravenous drip of penicillin combined with kfloxcin were treated by puncturing acupoints Neiguan (PC 6),Hegu(LI 4) and Taichong (LR 3).After the symptoms were relieved,the patients were again given an intravenous drip of penicillin or kfloxcin,no side effects was found.
3.Correlation between TCM Syndromes and Gastrointestinal Dyskinesis in Patients with Chronic Obstructive Pulmonary Diseases and Cor Pulmonale

Objective To study the correlations between the TCM syndromes of gastrointestinal dyskinesis and the disorders of gastrointestinal hormone,electrogastrogram(EGG) and blood gas in patients with chronic obstructive pulmonary diseases(COPD) and cor pulmonale.Methods The 120 cases of COPD and cor pulmonale were diagnosed with TCM syndrome differentiation,and plasma motilin(MOT),vasoactive intestinal peptide(VIP),Gastrin(GAS),EGG and blood gas were anglyzed and compared with 26 healthy subjects.Results With the aggravation of COPD and cor pulmonale,the dyskinesis of stomach and intestine was getting worse,developed from Shi-excess to Xu-deficiency.In deficiency syndrome,basic electrorhythm decreased most obviously.In phlegm-damp in lung syndrome,MOT and GAS decreased most obviously.In lung-kidney deficiency syndrome and lung-spleen-kidney deficiency syndrome,VIP increased most obviously.In lung-spleen-kidney deficiency syndrome,anoxia and carbon dioxide retention were most serious but GAS increased.Conclusion All patients of COPD and cor pulmonale have dyskinesis of stomach-intestine to a certain degree,abnormal EGG and disordered gastrointestinal hormones.Severity of ventilatory disturbance,quantity of injured organs,and condition of anoxia due to gastrointestinal blood stasis are all correlated with the development of COPD and cor pulmonale.
4.Quantitative Determination of Muscone by GC-MS

Gas chromatography combined with mass spectrography (GC - MS) was used to analyze the content of Muscone, a form of natural Moschus in high purity, with N2 as the carrier gas and isopsoralen as the internal standard. Results: The linear range of Muscone was from 0.056 to 0.451 g/L, regression equation: y=0. 3454 x-0. 1076,r=0.9999,the recovery was 97.99%and its RSD was 5.0%. RSD of within - day precision was 3.85% and RSD of day - to - day was 2.20% . The contents of Muscone from three batches of natural Moschus were 1.82% (RSD= 2.50%) ,3.33%(RSD = 1.36%) and 4.40%(RSD3.85%) respectively. The method is simple, rapid, reliable, and can be used as quality control for Muscone of Moschus and its preparation.
5.GC-MS Determination of Naphtha from Acorus tatarinowii Schott. in Rat Brain

Objective: To determine the component of naphtha from Acorus tatarinowii Schott.which can pass through the blood-brain barrier. Methods: Naphtha in rat brain was analyzed by GC-MS after gastric infusion of naphtha. Results: The methylisoeugenol,elemicin, ?-asarone and ?-asarone were detected in rat brain. Conclusion: The resuscitative effect of naphtha is resulted from the comprehensive action of multiple components.
6.Studies on Chemical Constituents of Rhizoma Acori Tatarinowii Decoction by GC-MS

[Objective] To analyze the main chemical constituents of decoction and concentrated decoction of Rhizoma Acori Tatarinowii (RAT) by gas chromatography-mass spectrometry (GC-MS). [Methods] RAT was decocted and concentrated in the pottery for two times and then 6 batches of the decoction and its concentrated decoction were analyzed by GC-MS. [Results] Five kinds of components in a higher amount were found in the first and second decoction of RAT, including: ?-asarone, ?-asarone, 2, 3-dihydro-3, 5-dihydroxy-6-methyl-4H-pyran-4-one, 5-hydroxymethylfurfural and acoramone. The contents of volatile components of ?-asarone and ?-asarone were lower while those of water-soluble components higher in the concentrated decoction of RAT. [Conclusion] The therapeutic effect of RAT is the co-action of the multiple components of RAT; the effect is related not only with the volatile components but also with the water-soluble components. Therefore, more attention should be paid to the difference of components in the concentrated decoction, which is generally used in the research of new Chinese herbal medicine, and in the clinically used decoction.
7.Effects of yichan tang on rotatory behavior, substance nigra cells and neurotransmitter of rats with Parkinson disease

BACKGROUND: Parkinson disease (PD) not only influences motor function, also affects sensation and cognition. Treatment with dopamine can only control symptom and has well side effect; however, yichan tang has great effect on treating PD in clinic.OBJECTIVE: To observe the effect of yichan tang on praxiological behavior, catecholamines substance in cerebral tissue and substantia nigra nerve cells of PD rats and analyze its effective mechanism.DESIGN: Randomized control animal study and single-blind evaluation.SETTING: Department of Traditional Chinese Medicine of General Hospital of Chinese PLA.MATERIALS: A total of 120 female SD rats weighing 180-200 g were selected in this study. Seventy modeling PD rats were randomly divided into saline group (n=30) and yichan tang group (n=40) according to digital table. Another 30 non-modeling rats were regarded as normal control group. Yichan tang was made of 15 g shanyurou, 15 g shichangpu, 10 g xianlingpi, 10 g roucongrong, 15 g gouqizi, 15 g danshen and 8 g wugong,etc., and provided by Dispensary of Chinese herb of General Hospital of Chinese PLA. Drug was decocted for 30 minutes to filter the solution and then concentrated into decoction including 1.0 g/mL raw materials.METHODS: The experiment was carried out at the Animal Laboratory of General Hospital of Chinese PLA from September to December 2001. PD rat model were established with 6-hydroxy-dopa to observe behavior of rats.The administration of rats (body mass/kg) in yichan tang group was 10 times as the administration of the adults (60 kg) (5 g/kg). Rats were perfused once a day and weighted once a week. The dosage of administration was regulated as the body mass changing, and the administration lasted for 8 weeks. Rats in saline group and normal control group were perfused with the same volume of saline as the criteria of body mass. Rotatory behaviors of rats in normal group, saline group (model group before administration)and yichan tang group were observed before administration and at 5, 6, 7 and 8 weeks after administration. Changes of content of catecholamines in extracellular fluid of cerebral tissue were measured with micro-dialysis technique and high performance liquid chromatography (HPLC), meanwhile, changes of substantia nigra nerve cells on both sides of corpus striatum were observed under 20-time and 40-time optic microscopes.MAIN OUTCOME MEASURES: Effect of yichan tang on medium of cranial nerve of PD rats; changes of substantia nigra nerve cells on both sides of corpus striatum with optic microscope and electron microscope; evaluation of yichan tang.RESULTS: Among 100 rats, 6 rats in yichan tang group died because of failure perfusion, 5 in saline group and 2 in normal control group. Therefore, 87 rats were involved in the final analysis. ① At 5, 6, 7 and 8 weeks after treatment, rotatory times within 40 minutes were all shorter in yichan tang group than those in saline group [(112.34±33.36), (91.16±73.06),(72.05±20.77), (61.63±17.93), (340.90±52.97) times, P < 0.01]. ② Levels of 3,4-dihydroxybenzene acid, homovanilic acid, dopamine and 5-serotonin on the injured side were lower than those on the uninjured side (P < 0.05-0.01). After treatment, levels of mediums mentioned above were higher in yichan tang group than those in saline group (P < 0.05-0.01); however,there were no significant differences of mediums on the uninjured side in the three groups (P > 0.05). ③ Pathological observation: Amount of substantia nigra nerve cells on the injured side was more in yichan tang group than that in saline group; moreover, volume of neurons was full, structure was clear, and values of Golgi complex and mitochondria were close to the normals.CONCLUSION: Yichan tang can relieve injured degrees of substantia nigra nerve cells, facilitate the repair, increase the content of endogenetic catecholamines in cerebral tissue, and improve rotatory behavior of PD rats.
8.Pathogenic Bacteria and Drug Sensitivity of 374 Cases of Children with Staphylococcal Scalded Skin Syn-drome

Objective:To understand the pathogen bacteria detection and drug sensitivity results in the children with staphylococ -cal scalded skin syndrome to provide data for the clinical treatment .Methods: Totally 374 children with staphylococcal scalded skin syndrome treated from January 2010 to June 2015 were selected .The children's wound secretion and blood samples were collected , and bacterial culture and drug sensitivity test were carried out .Results:Totally 223 pathogenic bacteria were detected out in the wound se-cretion samples;17 cases of blood culture were positive with the positive rate of 4.55%;187 strains of staphylococcus aureus were de-tected out;64 strains of MRSA were found out with the MRSA detection rate of 34.22% (64/187).The sensitivities of MSSA and MRSA to common antibacterial drugs were different .The susceptibility rates of MSSA and MRSA to vancomycin , teicoplanin , teicopla-nin and linezolid were all 100.00%.The sensitivity rates of MRSA to penicillin , oxacillin, piperacillin, piperacillin, cefoperazone so-dium, cefazolin, cefuroxime, cefoxitin, azithromycin and clindamycin were all zero .Conclusion: The pathogenic examination of staphylococcal scalded skin syndrome is very important , and antibiotics should be used reasonably according to the results of drug sensi-tivity.
9.Effects of Volatile Oil of Fructus Evodia, Mustard Oil and Total Anthraquinone in Rheum on Percutaneous Absorption of Paeoniflorin in Vitro

Objective To investigate the effects of volatile oil of Fructus Evodia, mustar d oil and total anthraquinone in Rheum on the transcutaneous permeability of paeoniflorin. Methods Using intelligent transdermal diffusion cell and excised mice skin in vitro as transdermal barrier, the kinetics parameters such as cumulative permeation quantity, permeation rate and permeation lagged time of the three kinds of penetration enhancers on paeoniflorin were determined by HPLC in 12 hours. Results The penetration rate of volatile oil of Fructus Evodia, mustard oil and total anthraquinone in Rheum were 8.188 6, 3.411 7, 1.230 3 μg/(cm2?h), respectively, the enhancement ratios were 22.6, 9.40, 3.40, respectively, and the permeation lagged times were 0.93, 0.51, 0.83 h, respectively. Conclusion Three penetration enhancers all can enhance previously percutaneous absorption of paeoniflorin, which provides reference for the selection of the penetration enhancers of transdermal delivery.
10.Study on Quality Control Standard of Intermediate Products of Xingshen Nasal Drops by GC-MS

Objective: To establish the quality control standards of two intermediate products of Xingshen Nasal Drops:musk aromatic water and volatile oil of Acorus tatarinowii Schott so that the content of main components and composition of the different batches of ultimate products remained stable.Methods: The content limit of muskone of musk aromatic water was ascertained by GC-MS. And the characteristic finger print of volatile oil of Acorus tatarinowii Schott was established. Results: The content of muskone of musk aromatic water should be controlled at the range of 0.166~0.202mg/mL. RSD of muskone content of 3 batches of ultimate products was 4.49%, not larger than 10%. The characteristic finger point of volatile oil of Acorus tatarinowii Schott was composed of 6 main component peaks: Methyleugenol, cis-Methylisoeugenol, trans-Methylisoeugenol, Elemicin, ?-Asarone and ?-Asarone. This characteristiz finger print could be repeated when the ultimate product was determined. Conclusion: The establishment of quality control standard of intermediate product can raise the product quality of compound preparations. And GC-MS is one of effective determination methods.
